Retrospective analysis of 65 cases with craniofacial cutaneous squamous cell carcinoma

Abstract:Objective To summarize the clinical manifestation ofcraniofacial cutaneous squamous cell carcinoma (cSCC) and to explore prognosis and prevention principles.Methods From August 2012 to February 2016,the clinical data of cSCC diagnosis by histopathology was retrospectively analyzed.Results There were 65 patients in our study,including 58 patients (89.2%) over 50 years old.Twenty-eight cases occurred on the head and 37 cases occurred on the face.There were 36 cases with primary benign skin tumor,24 cases with primary chronic ulcer and 5 cases with skin lesions.The incubation period from primary disease to cancer was 3 to 40 years,with an average of 16.2 ± 5.6 years.Ten patients suffered from partial nodular transfer,one patient suffered from intracranial metastasis and one patient suffered from pulmonary metastasis.Fifty cases without metastasis underwent radical excision surgery,8 cases with partial nodular transfer but no distant metastasis underwent radical excision surgery combined with local lymph node dissection.One case with intracranial metastasis and one case with pulmonary metastasis underwent wound management plus local radio therapy.By means of postoperative histopathology,58 cases were diagnosed with well-differentiated cSCC,and 7 cases with moderately-or poorly-differentiated cSCC.Fifty-seven patients received a good prognosis,and there were 8 cases of recurrence.Conclusion Craniofacial cSCC has a long incubation period.It is crucial to prevention to deal with primary disease in a timely manner.In craniofacial cSCC cases that received a good prognosis,radical excision surgery was the first treatment choice.
