Abstract:Cutaneous metastases from colorectal cancer are extremely rare and generally appear several years after diagnosis or resection of the primary tumor. Although this phenomenon is uncommon, it is very important and often indicates a poor prognosis. We present a case of a 76-year-old female patient with multiple cutaneous metastatic nodules on the back, just 1 month after resection of rectal cancer. Unfortunately, the patient gave up the follow-up treatment due to her age and poor physical condition; she died 3 months later. In view of its rarity of occurrence and lack of experience in treatment, we reviewed the literature and report as follows.

Abstract:Background: An estimated 119,300 new cases of cervical cancer occur annually in China, accounting for 372,00 deaths. Cutaneous metastasis from cervical cancer is a rare event, with an incidence of 0.1-1.3% and typically a preterminal occurrence. Scalp metastasis from cervical cancer is exceptionally anecdotal, with only a dozen examples well documented. Case presentation: The patient is a 33-year-old Chinese woman who was diagnosed with International Federation of Gynecology and Obstetrics stage IVB cervical cancer in November 2021. From December 2021 to April 2022, the patient was enrolled in the clinical trial of sintilimab combined with chemotherapy and radiotherapy for treatment of stage IV cervical cancer and underwent six cycles of immunotherapy and chemotherapy (sintilimab plus paclitaxel liposome and cisplatin). Treatment was well tolerated and led to a partial response. The masses adjacent to the spine and iliac bone was largely reduced. Thus, radiotherapy of the metastatic residues was carried out and followed by radiotherapy to the primary tumor at the cervix uteri. However, by the time of the radiotherapy completion in October 2022, the patient noticed painless nodules at the left scapular region and the right hypochondrium. The following month, more nodules occurred on the scalp and trunk, including the left axilla, anterior abdomen, and left back, along with a lesion invading the sternum that caused acute bone pain. The cutaneous masses were white, discrete with a rubbery consistency, and fixed to the skin. Several nodules increased in size and eventually ulcerated. Fine‑needle aspiration cytology of the left back swellings revealed metastatic squamous cell carcinoma, P16 positive. No visceral or brain metastasis was observed at this point. Conclusions: Cervical cancer metastases to the scalp are extremely uncommon. When a scalp metastasis is present, it might be the only symptomatic sign of disease progression or widespread metastatic lesions. So far, there is no clear guideline regarding skin metastases treatment. Such skin lesions warrant a thorough radiologic and pathologic workup to form a comprehensive management plan.
A clinical and pathological analysis of 27 cases of cutaneous metastases from internal cancers

Abstract:Objective: To analyze the clinical and histopathological characteristics of cutaneous metastases of a cancer. Method: Retrospectively analyzed the clinical and pathological features, the primary sites of cancers and prognosis in 27 cases of cutaneous cancer metastases diagnosed in the department from 2006 to 2012. Results: There were 12 males and 15 females with the male to female ratio of 1:1.25. The average ages at the onset of skin lesions were aged 60 years (24-91). Nine cases (33.33%) were with single lesion and 18 cases (66.67%) with multiple lesions. The metastatic sites included the chest, abdomen, genital area, scalp, back, shoulder, and the extremities. Histopathological examination revealed that 20 cases (74.07%) were metastatic adenocarcinoma; 3 cases (11.11%) were metastatic squamous cell carcinoma; 3 cases (11.11%) were metastatic mucinous carcinoma, and 1 case (11.11%) was metastatic melanoma. Four of 16 followed up patients died, and the others continued to radiation and chemotherapy. Conclusions: With increasing incidence of cancers, cutaneous metastases increased in recent years. To raise awareness of cutaneous metastases is helpful for early diagnosis, treatment, and prolonging patients' life.
Case report: A case of advanced gastric cancer with multiple skin metastases, with significant relief from immunotherapy

Abstract:Gastric cancer is the fifth leading cause of cancer-related mortality worldwide, with a low 5-year survival rate in advanced stages. Cutaneous metastasis is rare in gastric cancer, with only 0.8-1% incidence. We reported a rare case of female gastric cancer. The patient had undergone subtotal gastrectomy and chemotherapy 13 years ago, followed by a subsequent surgery of residual stomach, partial jejunum, and partial colon resection 11 years later. The pathological examination revealed poorly differentiated stomach adenocarcinoma, Lauren classification: diffuse type. The patient received 2 cycles of SOX chemotherapy. Two years later, cauliflower-like skin nodules, which were surgically excised, appeared on the back. The histopathological examination showed a spindle cell tumor; no specific anti-tumor treatment was administered. Six months later, the skin lesions increased in size and number, spreading to the neck, chest, and abdomen, presenting as erythematous patches with some cauliflower-like elevations. A skin biopsy of a 1cm0.5cm0.3cm lesion on the left abdomen was performed, and based on the immunohistochemistry, clinical history, and the possibility of metastatic or infiltrating adenocarcinoma, the gastrointestinal origin was highly suspected. Genetic testing was performed on the gastric recurrence and skin lesions, revealing 103 shared genetic variations, further suggesting the skin metastasis originated from gastric cancer. Subsequently, the patient received 10 cycles of immunotherapy combined with intravenous chemotherapy (200mg Tislelizumab and 100mg albumin-bound paclitaxel). The treatment response was evaluated as partial remission, with significant improvement in the skin lesions compared to before. This case highlights the possibility of tumor metastasis in patients with extensive skin lesions in advanced gastric cancer. Early examination, diagnosis, skin biopsy, immunohistochemistry, and genetic sequencing are recommended.

Abstract:OBJECTIVE:To investigate the clinicopathologic risk factors associated with extra-abdominal metastases in epithelial ovarian carcinoma patients and to determine prognostic factors associated with survival.METHODS:A retrospective chart review was conducted on 20 cases of epithelial ovarian carcinoma patients with extra-abdominal metastases and 645 cases of epithelial ovarian carcinoma patients without extra-abdominal metastases treated at our unit between 1993 and 2007.RESULTS:A total of 25 extra-abdominal metastatic sites were diagnosed in 20 patients. Sites of metastases were lung, n = 7; skin, n = 6; pleura, n = 5; brain, n = 3; mediastinal lymph node, n = 3; and bone, n = 1. Median interval time between diagnosis of epithelial ovarian carcinoma and documentation of metastatic disease of these 18 patients was 16 months. Median survival time after extra-abdominal metastases was 11 months. In a univariate analysis, Karnofsky performance status (KPS), clinical stage, and sensitivity of primary chemotherapy were the risk factors significantly associated with extra-abdominal metastases. These risk factors remained significant in multivariate analysis. In univariate analysis KPS, sensitivity of primary chemotherapy, metastatic site, and systemic therapy after diagnosis of extra-abdominal metastases were the factors significantly associated with survival.CONCLUSION:Some clinicopathologic risk factors may associate with extra-abdominal metastases. Prognosis after documentation of extra-abdominal metastases from epithelial ovarian carcinoma is poor. The important prognostic factors associated with survival are KPS, sensitivity of primary chemotherapy, metastatic site, and systemic therapy after diagnosis of extra-abdominal metastases.

Abstract:Background and purpose:Sweat gland carcinoma of the skin is a rare malignant neoplasm.There are some differences in diagnosis,treatment and prognosis because of its rarity.The aim of this study was to evaluate treament and prognostic factors of sweat gland carcinoma of the skin.Methods:The clinical manifestation,treament,survival rate,prognosis of 33 cases of sweat gland carcinoma of the skin were reviewed.All patients were treated in Zhengjiang Cancer Hospital from Feb.2000 to May 2012.Results:The 5-year overall survival(OS) and relapse free survival(RFS) were 70.0%,57.0%,respectively.Patients with high differentiated primary tumor had OS of 78.9% vs 69.2% in patients with low differentiatiated entity.The lymph node metastasis was 69.2% in 13 cases who had accepted lymphadenectomy.Conclusion:Sweat gland carcinoma of the skin had high rates of local recurrence and a propensity for lymph nodal metastases.Operation is the first choice of treatment,Extensiveness resection should be fully considered during operation.Chemotherapy and radiotherapy efficacy remain unclear.

Abstract:Background/Objectives: Metastatic colorectal cancer remains a fatal disease, with a 5-year survival rate lower than 15%. The most common metastatic sites are the lungs and the liver, while skin metastases are very rare and often indicate a poor prognosis with a lower survival rate. Methods. Herein, we present the clinical case of a 62-year-old female patient with rectal cancer metastases to the skin of the anogenital and abdominal regions, diagnosed 2 years after completion of treatment of the underlying disease. Results: Histological examination of the skin lesions revealed adenocarcinoma, and expression of the same immunohistochemical markers was also found in the primary tumor and in the cutaneous metastases. However, next-generation sequencing demonstrated differences in the mutational profiles of the primary tumor and metastasis to the skin. Somatic mutations in the APC, TP53, and PTPN11 genes were revealed in primary rectal adenocarcinoma, but another pathogenic TP53 mutation and a frameshift variant in the DYNC1I1 gene were found in cutaneous metastases. The patient underwent several courses of FOLFOX6 chemotherapy in combination with bevacizumab, but the treatment was unsuccessful. An analysis of 50 clinical cases from the literature concerning various manifestations of cutaneous metastases of rectal cancer showed a median survival of 8.5 months from the time of detection of the skin lesions. Conclusions: In this regard, careful skin examination of patients with rectal cancer and timely detection of cutaneous metastases are essential steps in the follow-up of patients who have undergone treatment of the primary tumor.

Abstract:Cutaneous metastases of ovarian cancer are rare and often have poor prognosis. We report a case of a 62-year-old woman with recurrent low-grade serous ovarian cancer, who presented with lung, brain, and multiple skin (nasal and anterior chest wall) metastases approximately six months after the initial diagnosis. In this case, Nijmegen breakage syndrome carrier status caused by RAD50 heterozygous mutation and previous bevacizumab therapy could be the predisposing factor for cutaneous metastases. The patient was treated with local radiotherapy (nasal skin and brain, 30Gy/6f/1.2W) and three courses of chemotherapy with albumin-bound paclitaxel and carboplatin, resulting in drastic remission of the cutaneous metastases. Unfortunately, treatment interruption resulted in rapid tumor progression, followed by death. This case represents an interesting example of cutaneous metastasis of ovarian cancer with rare clinical manifestations, unique genetic mutations, and reasonable response to treatment. Chemoradiotherapy might be an appropriate option for cutaneous metastases of ovarian cancer. Nevertheless, we still hope to find out the best treatment strategy after collecting and reviewing more cases in the future.

Clinical Analysis of Primary Cervical Small Cell Cancer: Report of 5 Cases

Abstract:Objective To explore the clinical characteristics , diagnosis and therapy for primary cervical small cell carcinoma . Methods A total of 5 patients diagnosed as primary cervical small cell carcinoma between January 2006 and October 2016 in our hospital were enrolled .Clinical features , therapies and prognosis were retrospectively evaluated . Results The onset ages of these patients were 28-53 years old (median, 42 years old).All the patients suffered abnormal vaginal bleeding .Among the 5 patients, 3 patients received radical hysterectomy , excision of bilateral accessory and pelvic lymphadenectomy , 1 patient received radical hysterectomy and pelvic lymphadenectomy , and 1 patient received endometrial cancer cytoreductive surgery .All the patients received chemotherapy after surgery , and 3 patients received radiotherapy after surgery .Up to December 2016, there was no loss of follow-up cases.Three patients survived without tumor recurrence after 34, 34, and 68 months, respectively.Two patients with lymphatic metastasis died, with the survival time of 22 and 31 months, respectively. Conclusions Primary cervical small cell carcinoma has no specific clinical features , and its diagnosis is mainly based on histopathological examinations .Lymphatic metastasis can happen inⅠb2 phase patients, which is a signal for poor prognosis .It' s vital to provide early diagnosis and treatment for clinical benefit . Surgery combined with chemotherapy and radiotherapy is effective for Ⅰb1 phase patients and lymphatic-metastasis-negative Ⅰb2 phase patients .

Abstract:Objective To study the characteristics of lymph node metastases of squamous cell carcinoma of thoracic esophagus.Methods To analyse retrospectively the clinical and pathological data of 170 cases of squamous cell carcinoma of thoracic esophagus.Results Lymph node metastases was found in 79 of 170 treated patients(46.5%),30 patients were found with more than 4 involved lymph nodes(17.6%).Tumor site,tumor grade and cells differentiation were factors influencing lymph node metastases of squamous cell carcinoma of thoracic esophagus.(P<0.05).Inferior phrenic lymph nodes metastases was higher in Esophageal carcinoma of stage Ⅲ and stage Ⅳ(P<0.05),about 15.2% and 29.6%.Conclusion The lymphatic involvement is associated with location of tumor,tumor grade and cells differentiation.An additional inferior phrenic lymph nodes irradiation is required for the patients who develop advanced squamous cell carcinoma locate in low thoracic segments of oesophagus,while receive radiotherapy only.
[Analysis of the Relationship Between Clinicopathological Features and Pelvic Lymph Node Metastasis in Patients with Early Stage Squamous Cell Carcinoma of the Uterine Cervix].

Abstract:OBJECTIVE:To evaluate clinical and pathologic factors associated with pelvic lymph node metastasis in patients with early-stage squamous cell carcinoma of the uterine cervix.METHODS:From February 2004 to January 2007, 135 patients with stage Ib-IIa cervical squamous cell carcinoma in Women's Hospital, School of Medicine, Zhejiang University, were retrospectively studied. The relationship between pelvic lymph node metastasis and age, clinical stage, tumor size, grade of differentiation, depth of muscular invasion, lymphatic vascular space invasion, pretreatment level of serum squamous cell carcinoma antigen, pretreatment plasma level of fibrinogen, pretreatment levels of hemoglobin and platelet were evaluated by univariate and multivariate analyses.RESULTS:Totally 3996 lymph nodes were dissected in 135 patients, with an average of 29.6 lymph nodes in each patient. 12.6% of the patients (17/135) had metastasized pelvic lymph nodes. Univariate analysis indicated that tumor size (P = 0.003), depth of muscular invasion (P = 0.004), vascular space invasion (P < 0.01), pretreatment levels of platelet (P = 0.006) and fibrinogen (P < 0.01) were significantly related to pelvic lymph node metastasis. Multivariate logistic regression analysis showed that lymphatic vascular space invasion (OR: 3.674, 95% CI: 1.825 - 7.393, P < 0.01) and pretreatment plasma level of fibrinogen (OR: 4.568, 95% CI: 1.779 - 11.725, P = 0.002) were significantly related to pelvic lymph node metastasis in patients with early-stage squamous cell carcinoma of the uterine cervix.CONCLUSION:In early-stage cervical squamous cell carcinoma, lymphatic vascular space invasion and higher pretreatment plasma levels of fibrinogen are risk factors of pelvic lymph node metastasis.
Case report: Advanced breast cancer with scalp metastases: a report of two cases

Abstract:Background: Breast cancer, identified as the most prevalent cancer worldwide, presents considerable difficulties in advanced stages, especially when involving metastatic spread. Scalp metastasis from breast cancer represents a rare and insufficiently explored occurrence. This paper seeks to illuminate this uncommon manifestation by presenting two cases of scalp metastatic breast cancer in Chinese women. Case report: Case 1: A 45-year-old Chinese woman with a history of invasive ductal carcinoma presented with a scalp lesion indicative of recurrence. Concurrently, she was diagnosed with bone metastases and recurrence at the original site. Despite undergoing various treatments, including chemotherapy and hormonal therapy, her condition worsened, ultimately leading to her passing. Case 2: A 40-year-old Chinese woman was initially diagnosed with bilateral breast invasive mucinous carcinoma presenting with bilateral breast masses and a scalp lesion. She also had multiple bone metastases. Following chemotherapy and hormonal therapy, her disease stabilized. Conclusion: These cases of scalp metastatic breast cancer underscore the complexities involved in managing advanced stages of the disease, especially with rare metastatic manifestations. They highlight the importance of comprehensive diagnostic methods, encompassing full-body skin evaluations, and draw attention to the socioeconomic challenges faced in cancer treatment. These findings point to the necessity for more targeted research on uncommon metastatic forms in breast cancer aiming to enhance patient outcomes and refine management approaches.

Abstract:Objective To investigate the clinical features of skin cancer.Methods Clinical data of skin cancer and precancerous skin lesions confirmed pathologically from 2005 to 2008 in Peking University First Hospital were retrospectively analyzed by using statistical methods.Results A total of 632 cases of skin cancer and precancerous skin lesions were studied.The most common skin cancer was basal cell carcinoma and squamous cell carcinoma (invasive and in situ) which accounted for 29.3%and 24.2%,respectively.The average age at onset was older than 60 years in 55.4%of the patients,between 35 and 59 years in 34.3%,younger than 35 years in 10.3%.The concordance between clinical and pathological diagnosis reached nearly 90.O%for Paget's disease,70.0% for other common skin cancer and precancerous skin lesions.Conclusions Skin cancer and precancerous skin lesions have a predilection for scalp and face.Patients aged from 35 to 59 years account for a significant proportion not only in cutaneous lymphoma but also in melanoma and epithelium-derived nonmelanoma skin cancer.
Skin metastasis from ovarian cancer with somatic BRCA1 mutation: A case report and literature review

Abstract:Skin metastasis from ovarian cancer is rare, and its prognosis is poor. Effective therapeutic strategies are currently lacking, but the combination of various treatment methods shrink the tumor and relieve symptoms. The present study reports a rare case of advanced ovarian cancer with skin metastases and intestinal wall thickening, along with a BRCA1 DNA repair associated (BRCA1) mutation. After standard first‐line treatment and non‐standard second‐line treatment, the patient developed skin metastases. The patient's skin itching, pain and lesions were completely relieved after administering bevacizumab in combination with paclitaxel and carboplatin. After 4 months, skin metastases recurred along with anal distension during maintenance treatment with oral poly(ADP ribose) polymerase (PARP) inhibitors. The patient was treated again with bevacizumab combined with docetaxel, and the anal distension was significantly relieved. Angiogenesis therapy combined with chemotherapy is effective, but that the disease‐free survival time is short, and PARP inhibitor maintenance effect is limited even in cases with a BRCA1 gene mutation.

Abstract:Lung cancer is a common malignant tumor in clinic. Morbidity and mortality are the first place in cancer, which seriously threaten the health of human beings. Due to the lack of effective and specific screening methods, most patients were found already at an advanced stage, and often combined with bone, brain, liver, adrenal and other parts of the metastasis. Symptoms of patient resulted from the primary tumor, metastatic spread, or paraneoplastic syndrome commonly cause the medical attention. For the syndrome related to the distant metastatic spread as the first manifestation, diagnosis would be a challenge. Retinal metastasis is very rare as the first symptom of squamous cell carcinoma of the lung. This paper reviewed a case, in which a patient developed the retinal metastasis as the inital presentation. After systemic evaluation, our patient was managed in multidisciplinary treatments, including surgery and chemotherapy. Now, the patient has a good short-term prognosis and is still in fellow-up condition. Later, we summarized the characteristics of the diagnosis and treatment of this case and provided experience for clinicians. At the same time, reviewing the relevant literature on tumor metastasis provides a window for our understanding of the rare manifestations of lung cancer.

Abstract:Renal cell carcinoma is the most common type of primary renal cancer, and clear cell carcinoma is the most common subtype, accounting for approximately 70% of all adult renal cell carcinoma cases. At the time of diagnosis, many patients already have metastatic manifestations. Cutaneous metastasis of renal clear cell carcinoma is very rare and usually represents a poor prognosis, mostly affecting the head and neck. In this paper, we report a case of renal clear cell carcinoma with multiple cutaneous metastases, including a chest wall mass for more than 10 years and an abdominal wall mass for 1 year. A 69-year-old man with a history of diabetes mellitus was admitted to the hospital for examination of a right chest and abdominal wall mass and peripheral pain, and ultrasonography suggested a solid space-occupying lesion in the left kidney, which was considered malignant, and a solid mass in the right chest and abdominal wall, which was considered metastatic. A subsequent abdominal CT scan showed malignant tumors in the left kidney and adrenal region, and multiple metastatic tumors in the liver, pancreas, right thoracoabdominal wall, and the abdomen. To clarify the nature of the pathology, an ultrasound-guided puncture of the right abdominal wall mass was performed, and the pathological diagnosis was clear cell carcinoma, with immunohistochemistry suggesting a renal clear cell carcinoma origin. The patient died within 6 months.

Abstract:OBJECTIVETo study the clinicopathological features of primary squamous cell carcinoma (SCC) of breast and the diagnosis and treatment of the disease.METHODSThe clinical, operative and pathological data from 7 cases of SCC of breast were retrospectively analysed.RESULTSFive patients complained of painless mass and two of them were accompanied with local pain. All patients were diagnosed as having SCC by fine-needle aspiration before operation. Two patients underwent modified radical mastectomy, and the others received radical mastectomy. Pathological examination showed primary squamous cell carcinoma of the breast with metastasis to axlliary lymph nodes in one patient. Follow-up of four patients revealed that two patients are living well and two patients died. Three patients lost to follow up.CONCLUSIONThe diagnosis of primary squamous cell carcinoma of the breast is dependent on pathological results. Radical mastectomy including modified radical mastectomy is most effective in the treatment of mammary SCC. Postoperative chemotherapy and radiation are necessary auxiliary therapy.
