Early-term complications of radical cystectomy and management:report of 55 cases

Abstract:Objective To investigate and analyze early-term complications of radical cystecto-my and its prevention and management. Methods From January 2010 to December 2013, 55 pa-tients received open radical cystectomy in our department. All medical data were reviewed, early-term complications of radical cystectomy and its treatment were collected. Definition of early-term was a month after operation. Risk factor analysis of complications was carried out using statistical method. Results 55 operations were completed suscessfully. This group included 45 men and 10 women with years between 39 and 82y, mean (64±1 1)y. The average total operation time (include time of urinary diversions)was (35 1 ±82)min. The mean intraoperative blood transfusion volume (not containing plasma)was (550±450)ml. 22 (40. 0%)patients had 26 complications. Major com-plications occurred in 14 (25. 0%)cases, minor complications in 12 (21. 0%)cases. Ileus or intesti-nal obstruction, infection of incision, intraoperative bleeding was the most common complication. The mean postoperative hospital stay time was (22±12)d. Logistic regression analysis showed that the co-morbidities (P =0. 031)is an independent risk factor for predicting the complications. Con-clusions Early-term complication rate of radical cystectomy is higher. Ileus or intestinal obstruc-tion, infection of incision, intraoperative bleeding was the most common complication. Co-morbidi-ties is an independent risk factor for predicting the complications. Prevention and management of these complications of cystectomy is particularly important in clinical practice.
