Abstract:Purpose: Of commonly performed urological cancer procedures radical cystectomy is associated with the highest morbidity and mortality. The impact of each individual type of complication or a combination of them on various outcome measures, such as mortality, charges and length of stay, is unclear. We quantified the impact of specific post-cystectomy complications and combinations thereof in terms of mortality, charges and length of stay. Materials and methods: All 6,577 patients undergoing radical cystectomy for bladder cancer were identified from the Nationwide Inpatient Sample of the Healthcare Cost and Utilization Project (1998 to 2002). The prevalence of different International Classification of Diseases, 9th Revision, Clinical Modification coded complications following cystectomy were determined. Outcome variables of interest were in-hospital mortality, total charges and length of stay. The association between types of complications and measured outcomes were examined using univariate and multivariate regression models. The cumulative impact of multiple complications and various combinations of complications on outcomes was also examined. Results: The overall complication rate was 28.4% in 1,869 cases and the mortality rate was 2.6%. Median total charges was 41,905 dollars and median length of stay was 9 days. Overall 20.7% of patients had 1, 6.1% had 2, 1.2% had 3 and 0.42% had greater than 3 complications. At least 1 complication almost doubled the odds of mortality and increased median total charges and length of stay by 15,000 dollars and 4 days, respectively. We defined expected levels of increase in the various outcome measures with increasing numbers of complications. The combination of postoperative infection and respiratory complication had the greatest impact on mortality, while the combination of wound and urinary tract infection had the greatest impact on length of stay and total charges. Conclusions: Although most patients undergoing cystectomy are older and have multiple comorbidities, the postoperative complications with the most significant impact were those directly related to surgery (primary complications). Secondary complications (cardiac, respiratory, vascular, etc) appear to have less of an impact on most common outcome measures. Hence, the greatest gains can be achieved by limiting primary complications. These data could be used to develop benchmarks of expected levels of primary and secondary complications after cystectomy.
Survival after Radical Cystectomy for Bladder Cancer: Multicenter Comparison Between Minimally Invasive and Open Approaches

Abstract:Objective: To investigate oncological outcomes in patients with bladder cancer who underwent minimally invasive radical cystectomy (MIRC) or open radical cystectomy (ORC). Methods: We identified patients with bladder cancer who underwent radical cystectomy (RC) in 13 centers of the Chinese Bladder Cancer Consortium (CBCC). Perioperative outcomes were compared between MIRC and ORC. The influence of surgical approaches on overall survival (OS) and cancer-specific survival (CSS) in the entire study group and subgroups classified according to pathologic stage or lymph node (LN) status was assessed with the log-rank test. Multivariable Cox proportional hazard models were used to evaluate the association among OS, CSS and risk factors of interest. Results: Of 2 098 patients who underwent RC, 1 243 patients underwent MIRC (1 087 laparoscopic RC and 156 robotic-assisted RC, respectively), while 855 patients underwent ORC. No significant differences were noted in positive surgical margin rate and 90-day postoperative mortality rate. MIRC was associated with less estimated blood loss, more LN yield, higher rate of neobladder diversion, longer operative time, and longer length of hospital stay. There was no significant difference in OS and CSS according to surgical approaches (p=0.653, and 0.816, respectively). Subgroup analysis revealed that OS and CSS were not significantly different regardless of the status of extravesical involvement or LN involvement. Multivariable Cox regression analyses showed that the surgical approach was not a significant predictor of OS and CSS. Conclusions: Our study showed that MIRC was comparable to conventional ORC in terms of OS and CSS.
Postoperative complications and hospital costs following small bowel resection surgery

Abstract:Background: Postoperative complications after major gastrointestinal surgery are a major contributor to hospital costs. Thus, reducing postoperative complications is a key target for cost-containment strategies. We aimed to evaluate the relationship between postoperative complications and hospital costs following small bowel resection. Methods: Postoperative complications were recorded for 284 adult patients undergoing major small bowel resection surgery between January 2013 and June 2018. Complications were defined and graded according to the Clavien-Dindo classification system. In-hospital cost of index admission was calculated using an activity-based costing methodology; it was reported in US dollars at 2019 rates. Regression modeling was used to investigate the relationships among a priori selected perioperative variables, complications, and costs. Findings: The overall complication prevalence was 81.6% (95% CI: 85.7-77.5). Most complications (69%) were minor, but 22.9% of patients developed a severe complication (Clavien-Dindo grades III or IV). The unadjusted median total hospital cost for patients with any complication was 70% higher than patients without complications (median [IQR] USD 19,659.64 [13,545.81-35,407.14] vs. 11,551.88 [8,849.46-15,329.87], P < 0.001). The development of 1, 2, 3, and ≥ 4 complications increased hospital costs by 11%, 41%, 50%, and 195%, respectively. Similarly, more severe complications incurred higher hospital costs (P < 0.001). After adjustments were made (for the Charlson Comorbidity Index, anemia, surgical urgency and technique, intraoperative fluid administration, blood transfusion, and hospital readmissions), a greater number and increased severity of complications were associated with a higher adjusted median hospital cost. Patients who experienced complications had an adjusted additional median cost of USD 4,187.10 (95% CI: 1,264.89-7,109.31, P = 0.005) compared to those without complications. Conclusions: Postoperative complications are a key target for cost-containment strategies. Our findings demonstrate a high prevalence of postoperative complications following small bowel resection surgery and quantify their associated increase in hospital costs. Identifying risk factors for potentially avoidable complications following radical cystectomy

Abstract:Purpose: Morbidity after radical cystectomy is common and associated with increased health care resource use. Accurate characterization of complications after cystectomy, associated patient specific risk factors, and perioperative processes of care are essential to directing changes in perioperative management that will reduce morbidity and improve the quality of patient care. Materials and methods: The National Surgical Quality Improvement Program (NSQIP) is a prospective quality management initiative of 123 Veterans Affairs Medical Centers nationwide. The NSQIP collects clinical information, intraoperative data and outcomes on a wide variety of surgical procedures from multiple surgical disciplines. Since 1991, 2,538 radical cystectomy procedures have been captured by the NSQIP. Modeling using logistic regression was performed to identify patient specific risk factors and perioperative process measures associated with postoperative morbidity. Results: Of the 2,538 subjects at least 1 postoperative complication developed in 774 (30.5%). The most frequent complication was ileus (10%). Several factors were associated with the development of a complication, including age, dependent functional status, preoperative dyspnea, preoperative acute renal failure, chronic steroid use, preoperative alcohol consumption, American Society of Anesthesiology score, use of general anesthetic, operative time, intraoperative blood requirement and surgeon level of training. Conclusions: Morbidity remains high after cystectomy with 30.5% of subjects experiencing at least 1 complication. Measurable patient specific risk factors and perioperative processes associated with postoperative morbidity following cystectomy are now delineated which allows for improved risk stratification, patient counseling, and the development of novel processes that may incrementally reduce risk and improve outcomes.
Post-Operative Infections among Patients Undergoing Radical Cystectomy at a Tertiary Center

Abstract:Background: Post-operative infection after radical cystectomy is a common complication. It is essential to identify modifiable risk factors that can predict post-operative infection to reduce the incidence of infection after radical cystectomy. We investigated the incidence of post-operative infection, associated pathogens, and risk factors for post-operative infection after radical cystectomy. Patients and methods: Patients who underwent radical cystectomy for bladder cancer in a single urologic department from 2014 to 2016 were included. Age, gender, length of stay, body mass index (BMI), diabetes mellitus, hypertension, pre-operative estimated glomerular filtration rate, hydronephrosis, smoking status, neoadjuvant chemotherapy, pathologic tumor stage, pathologic nodal stage, types of urinary diversion, laparoscopic approach, operative time, and receipt of peri-operative blood transfusion were assessed to determine the association of these factors with the risk of infection within 30 days of radical cystectomy. Risk factors were assessed for correlation with any kind of infection and separately for urinary tract infection (UTI), blood infection/septic shock (BI), and surgical site infection (SSI). Pathogens were identified in all cases of infection. Results: A total of 134 patients were included in the analysis. Post-operative infection was diagnosed in 10.45%. Urinary tract infection, BI, and SSI were diagnosed in 3.73%, 4.48%, and 7.46%, respectively. Co-infections and mixed infection were diagnosed in 50% of patients with infection. Enterococcus was the most common pathogen. On multivariable analysis, age, BMI ≥30 kg/m2, and laparoscopic approach were associated with the risk of infection; odds ratios (ORs) were 1.27 (95% confidence interval [CI] 1.01-1.59), 13.07 (1.39-122.88), and 0.07 (0.01-0.78), respectively. Conclusion: One-tenth of patients undergoing radical cystectomy developed an infection within 30 days of surgery. Fifty percent of patients had co-infection with UTI, SSI, and/or BI and 50% of infections were polymicrobial. Increased BMI is the strongest factor associated independently with all types of infection. Therefore, obese patients should be encouraged to lose weight pre-operatively.

Abstract:We report the first detection of interstellar nitrogen sulfide (NS) in cold dark clouds. Several components of the 2 pi 1/2, J = 3/2 --> 1/2 and J = 5/2 --> 3/2 transitions were observed in TMC-1 and L134N. The inferred column density for TMC-1 is NNS approximately 8 x 10(12)cm-2 toward the NH3 peak in that cloud, and in L134N is NNS approximately 3 x 10(12)cm-2 toward the position of peak NH3 emission. These values correspond to fractional abundances relative to molecular hydrogen of fNS approximately 8 x 10(-10) for TMC-1, and fNS approximately 6 x 10(-10) for L134N. The NS emission is extended along the TMC-1 ridge and is also extended in L134N. The measured abundances are significantly higher than those predicted by some recent gas phase ion-molecule models.

Abstract:Purpose To assess in-hospital mortality and complication rates after radical cystectomy (RC) in patients with history of heart-valve replacement. Materials and methods Using the National Inpatient Sample (2000–2019), non-metastatic bladder cancer patients undergoing RC were stratified according to history of heart-valve replacement. Regression models (RM) predicted hospital outcomes. Results Of 25,535 RC patients, 250 (1.0%) harbored heart-valve replacement. Heart-valve replacement patients were older (median 74 vs. 70 years), more frequently male (87.2 vs. 80.6%), and more frequently had Charlson comorbidity index ≥3 (26.8 vs. 18.9%). In RC patients with history of heart-valve replacement vs. others, 62 vs. 2634 (24.8 vs. 10.4%) experienced cardiac complications, 28 vs. 3092 (11.2 vs. 12.2%) intraoperative complications, 11 vs. 1046 (4.4 vs. 4.1%) infections, <11 vs. 594 (<4.4 vs. 2.3%) perioperative bleeding, <11 vs. 699 (<4.4 vs. 2.8%) vascular complications, 74 vs. 6225 (29.6 vs. 24.7%) received blood transfusions, 37 vs. 3054 (14.8 vs. 12.1%) critical care therapy (CCT), and in-hospital mortality was recorded in <11 vs. 463 (<4.4 vs. 1.8%) patients. In multivariable RM, history of heart-valve replacement independently predicted cardiac complications (odds ratio 2.20, 95% confidence interval 1.62–2.99; p < 0.001). Conversely, no statically significant association was recorded between history of heart-valve replacement and length of stay, estimated hospital costs, intraoperative complications, perioperative bleeding, vascular complications, infections, blood transfusions, CCT use, and in-hospital mortality. Conclusions Radical cystectomy patients with history of heart-valve replacement exhibited a 2.2-fold higher risk of cardiac complications, but no other complications, including no significantly higher in-hospital mortality.

Abstract:Intracytoplasmic inclusion bodies with an unusual immunohistochemical profile, histologically similar to alpha 1 antitrypsin globules and megamitochondria, were seen within hepatocytes in a liver allograft biopsy. Electron microscopy revealed that these structures were erythrocytes in various stages of degradation. Their appearance following an episode of Pneumocystis carinii pneumonia suggests a possible relationship between bouts of hypoxia and erythrocyte entry into hepatocytes.
[Critical analysis on risk factors of postoperative in-hospital complications in radical cystectomy of bladder cancer]

Abstract:Objective: To evaluate the risk factors associated with the in-hospital complications of radical cystectomy. Methods: The records of 128 consecutive patients who underwent radical cystectomy from November 2006 to October 2009 were reviewed. Preoperative, intraoperative risk variables were defined based on POSSUM score and APACHE II score. ASA score and m-ACCI score were used to evaluate the co-mobidities. All variables were analyzed to evaluate the risk factors associated with in-hospital complications of radical cystectomy. Results: In the 128 patients undergoing radical cystectomy, 58 patients (45.31%) had complications, with 51 (39.84%) minor complicatons and 7 (5.47%) major complications. Wound dehiscence or unhealing (22.66%), infection (20.31%) and ileus (17.19%) were the most common complications. The risk factors related to the in-hospital complications of radical cystectomy are age > or = 65 yr. (P < 0.001,OR = 1.067), volume of intraoperative blood transfusion > or = 600 mL (P = 0.023, OR = 1.001), m-ACCI score (P < 0.001, OR = 1.583), ASA score > or = III (P = 0.003, OR = 2.411), partial cystectomy history (P = 0.025 OR = 2.444), intraoperative heart rate > 120 beats/min (P < 0.001, OR = 11.861). Conclusions: Wound complication, postoperative infection and ileus are the most common complications of radical cystectomy. Age, volume of intraoperative blood transfusion, m-ACCI score, ASA score, partial cystectomy history and intraoperative heart rate are significant risk factors.

Abstract:Consumers' perceptions of the color and texture of 8 commercial vanilla dairy desserts were studied and related to color and rheological measurements. First, the 8 desserts were evaluated by a group of consumers by means of the Free Choice Profile. For both color and texture, a 2-dimensional solution was chosen, with dimension 1 highly related to yellow color intensity in the case of color and to thickness in the case of texture. Second, mechanical spectra, flow behavior, and instrumental color were determined. All the samples showed a time-dependent and shear-thinning flow and a mechanical spectrum typical of a weak gel. Differences were found in the flow index, in the apparent viscosity at 10 s(-1), and in the values of the storage modulus, the loss modulus, the loss angle tangent, and the complex viscosity at 1 Hz, as well as in the color parameters. Finally, sensory and instrumental relationships were investigated by a generalized Procrustes analysis. For both color and texture, a 3-dimensional solution explained a high percentage of the total variance (>80%). In these particular samples, the instrumental color parameters provided more accurate information on consumers' color perceptions than was provided by the rheological parameters of consumers' perceptions of texture.
Annual trends of cystectomy complications: A contemporary analysis of the NSQIP database

Abstract:Introduction: Despite significant morbidity, radical cystectomy (RC) is standard of care for muscle invasive bladder cancer, certain high-risk nonmuscle invasive tumors and after failure of intravesical or trimodal therapy. Modern efforts have hastened the recovery after this surgery without impact on overall complication rates. Our primary aim was to examine changes in complication rates of RC over time. Methods: The National Surgical Quality Improvement Program database included 11,351 RC from 2006 to 2018 for nondisseminated bladder cancer. Baseline characteristics and complication rates were studied across time periods: 2006 to 2011, 2012 to 2014, and 2015 to 2018. Thirty-day complications, readmissions, and mortality were identified. Results: Overall complication rates decreased over time (56.5%, 57.4%, 50.6%, P < 0.01). Infectious complications were stable, including UTIs (10.1%, 8.8%, 8.3% respectively, P = 0.11) and sepsis (10.4%, 8.8%, 8.7% respectively, P = 0.20). On multivariable analysis, ASA≥3 (OR 1.399, 95% CI 1.279-1.530) was associated with increased complications, while procedures in 2015 to 2018 (OR 0.825, 95% CI 0.722-0.942), laparoscopic/robotic approach (OR 0.555, 95%CI 0.494-0.622), and ileal conduit (OR 0.796, 95% CI 0.719-0.882) were associated with decreased complication rates. Other outcomes of interest included mean length of stay (LOS), which decreased over time (10.5, 9.8, 8.6 days, respectively, P < 0.01) and readmission (20.0%, 21.3%, 21.0%, respectively, P = 0.84) and mortality rates were stable (2.7%, 1.7%, 2.0%, respectively, P = 0.13). Conclusion: Decreased early complications and LOS after RC over time may reflect beneficial effects of recent advances in bladder cancer treatment such as enhanced recovery after surgery protocols and minimally invasive techniques. Further opportunities to improve long term outcomes, readmissions and infection rates are needed.
[Analysis of Perioperative Complications of Laparoscopic Radical Cystectomy for Bladder Cancer].

Abstract:OBJECTIVE:To evaluate operation process and perioperative complications of patients who underwent laparoscopic radical cystectomy (LRC).METHODS:The clinical data of 49 cases of LRC from October 2004 to June 2010 were reviewed retrospectively. Perioperative complications and mortality were analyzed, and so were the operative time, blood loss and postoperative hospital stay. Perioperative complications were defined as any adverse event within 30 days of surgery. All complications were graded according to an established five-grade modification of the original Clavien system.RESULTS:The mean operation time was 418 minutes, the mean blood loss was 514 mL, the transfusion rate was 36.7%, the mean transfusion volume was 578 mL, and the average postoperative hospital stay was 20 d. For urinary diversion, ileal conduits were constructed in 27 patients (55.1%), ileal neobladders in 16 patients (32.7%), and ureterocutaneostomies in 6 patients (12.2%). A total of 17 patients (34.7%) developed at least one perioperative complication. Complications of grades 1-2 occurred in 12 patients (24.5%), which included subileus, urinary tract infections, deep venous thrombosis of the lower limbs, pneumonia, etc. Complications of grades 3-5 occurred in 5 patients(10.2%), and one patient died of pulmonary embolism. Ileal neobladders and ileal conduits were similar at the operation time, blood loss, transfusion rates, postoperative hospital stay and morbidity of perioperative complications.CONCLUSION:Morbidity of perioperative complications following LRC was still high. The most frequent complications were subileus and urinary tract infections. The surgery should be performed on selected patients, and measures need to be taken to prevent these complications. Compared with ileal conduits, ileal neobladders did not increase the operation time, blood loss and morbidity.

Abstract:Introduction: Radical cystectomy (RC) remains a surgery with important morbidity despite technical advances. Our aim was to determine the impact on outcomes and costs of robot-assisted radical cystectomy (RARC) with full intracorporeal diversion. Material and methods: We retrospectively included 196 consecutive patients undergone RC for bladder cancer between 2017 and 2022. Comparisons were done between the open radical cystectomy (ORC; n = 166) and RARC with full intracorporeal diversion (n = 30) in the overall cohort and after matched pair analysis. Results: More neobladders were performed in the RARC group (40% vs 18.7%, p = 0.011). Peri-operative parameters continuously improved over time in the RARC cohort despite an increased proportion of elderly patients with higher comorbidity index. RARC patients had lower prolonged stay (33.3% vs 68.3%, p = 0.002), lower grade 1 complication rates (26.7% vs 53.3%, p = 0.016) and blood loss (185 vs 611 ml, p <0.001) than ORC patients. RARC was an independent favorable predictor for prolonged stay (OR 0.199) and complication (OR 0.334). Cost balance favored ORC, with an increase of hospitalization cost at 816 euros for RARC. Conclusions: After matching, RARC with full intracorporeal diversion was associated with improved outcomes and a moderated increase of post-operative costs mainly due to the use of robotic devices.

Abstract:Objective: To examine the impact of increased compliance to contemporary perioperative care measures, as outlined by enhanced recover after surgery (ERAS) guidelines, among patients undergoing radical cystectomy (RC). Patients and methods: From the National Surgical Quality Improvement Program database we captured patients undergoing RC between 2019 and 2021. We identified five perioperative care measures: regional anaesthesia block, thromboembolism prophylaxis, ≤24 h perioperative antibiotic administration, absence of bowel preparation, and early oral diet. We stratified patients by the number of measures utilised (one to five). Statistical endpoints included 30-day complications, hospital length of stay (LOS), readmissions, and optimal RC outcome. Optimal RC outcome was defined as absence of any postoperative complication, re-operation, prolonged LOS (75th percentile, 8 days) with no readmission. Multivariable regressions with Bonferroni correction were performed to assess the association between use of contemporary perioperative care measures and outcomes. Results: Of the 3702 patients who underwent RC, 73 (2%), 417 (11%), 1010 (27%), 1454 (39%), and 748 (20%) received one, two, three, four, and five interventions, respectively. On multivariable analysis, increased perioperative care measures were associated with lower odds of any complication (odds ratio [OR] 0.66, 99% confidence interval [CI] 0.6-0.73), and shorter LOS (β -0.82, 99% CI -0.99 to -0.65). Furthermore, patients with increased compliance to contemporary care measures had increased odds of an optimal outcome (OR 1.38, 99% CI 1.26-1.51). Conclusions: Among the measures we assessed, greater adherence yielded improved postoperative outcomes among patients undergoing RC. Our work supports the efficacy of ERAS protocols in reducing the morbidity associated with RC.

Abstract:ObjectiveThe aim of this study was to explore the factors affecting the major complications and the impacts of preoperative renal function on the incidence of complications in radical cystectomy procedures.MethodsA retrospective review of 705 patients who received radical cystectomy between 2006 and 2021 was conducted. The 90-day complications of patients after a radical cystectomy were reported and the Clavien–Dindo classification (CDC) was used for grading complications. The clinical characteristics and preoperative outcomes were compared among patients with different preoperative renal functions. A logistic regression analysis of all patients was used to identify the risk factors associated with the major complications. Spearman’s correlation analysis was used to examine the relationship between the classification of renal insufficiency and the CDC. In order to reduce the selection bias, one-to-one propensity score matching was performed, and the comparison of complications after matching was carried out for the sensitivity analysis.ResultsWithin 90 days post-surgery, 71% of patients experienced complications, with 4.8% of them being major. Patients with preoperative renal insufficiency had a higher CDC and had a higher rate of major complications (16.7% vs 3.7%, p < 0.001). There was a linear relationship between preoperative serum creatinine and complications. Spearman’s correlation analysis showed a slightly positive correlation between the classification of renal insufficiency and the CDC (r=0.094, p = 0.013). Preoperative renal insufficiency was a risk factor for major complications (OR = 6.805 [95%CI: 2.706-17.112]; p < 0.001). After matching, the patients in the preoperative renal insufficiency group had a higher CDC and a higher incidence of major complications (16.9% vs 1.7%, p = 0.004).ConclusionsIn our cohort, patients with preoperative renal insufficiency exhibited a higher incidence of complications following a radical cystectomy, and renal insufficiency was a significant risk factor for major complications.

Abstract:OBJECTIVE To determine whether robotic-assisted radical cystectomy (RARC) with intracorporeal urinary diversion (ICUD) compared to open radical cystectomy (ORC) or RARC with extracorporeal urinary diversion (ECUD) would result in a decreased rate of surgical site complications. RARC has been shown to be non-inferior to ORC. Both RARC and ORC are complicated by a high rate of perioperative morbidity, including wound-related complications, which may be decreased by a robotic approach with intracorporeal diversion. METHODS A retrospective review of our bladder cancer database for patients undergoing radical cystectomy from 2013-2021. Patients were stratified by surgical technique as RARC with ICUD vs ORC vs RARC with ECUD. Surgical site complications were measured at both 30- and 90-day intervals. RESULTS Of the 269 patients, 127 (47.2%) had RARC with ICUD, 118 (43.7%) had ORC, and 24 (8.9%) had RARC with ECUD (mean ages 71.0, 69.5, and 67.5, respectively). A comparison of the 3 groups demonstrated statistical significance at both the 30-day ( P <.001) and 90-day ( P <.001) timeframes for total surgical site complications, with RARC with ICUD having the fewest amount of patients experiencing a surgical site complication (0.8%) followed by ORC (25.4%) and RARC with ECUD (29.2%). CONCLUSION Overall, we observed lower surgical site complication rates among patients undergoing RARC with ICUD compared to patients who underwent ORC or RARC with ECUD. This study suggests that decreased surgical site complications may be one benefit of the minimally invasive approach, particularly in patients at high risk for surgical site complications after radical cystectomy.

Complications of radical cystectomy for carcinoma of the bladder

Abstract:A total of 165 consecutive patients underwent radical cystectomy for bladder cancer, 19 patients without preoperative radiation therapy, 109 patients after planned preoperative radiation therapy and 37 patients after failure of definitive radiation therapy (salvage cystectomy). The operative mortality was 2.4 per cent of the entire group: less than 1 per cent of the 128 patients undergoing cystectomy as primary treatment and 8.1 per cent of the 37 patients undergoing salvage cystectomy. Early complications occurred in 28 per cent of the 165 patients and included wound infeciton, urine leak, ureterointestinal obstruction, small bowel obstruction, intestinal leak, rectal fistual and/or medical complications. Postoperative hospitalization for those patients suffering any complication averaged 27 days compared to the average hospitalization of 12.7 days when no complications occurred. Selective use of the initial perineal approach in male patients who had received more than 6,000 rad and of staged cystectomy after urinary diversion in some high risk patiens, associated with salvage cystectomy, can reduce the postoperative mortality and morbidity significantly. The addition of a meticulous pelvic iliac lymph node dissection did not increase the operative mortality or morbidity compared to simpler procedures. Results of this study support the early aggressive use of single stage radical cystectomy with pelvic iliac lymph node dissection and urinary diversion in the management of patients with bladder cancer.
