Clinical effect of total hip arthroplasty on female patients with original urinary incontinence

Abstract:Objective To explore the effect of total hip arthroplasty ( THA) on female patients with original urinary incontinence ( UI) .Methods 88 female patients with femoral head necrosis were scheduled for elective primary THA in The First Affiliated Hospital of Jinan University from September 2012 to June 2014 were chosen . The International Consultation on Incontinence Questionnaire-Short Form (ICI-Q-SF) was used to prospectively evaluate UI symptoms in eighty -eight female patients before operation and six months after operation .All patients allocated into UI group or non -UI group by evaluation of ICI -Q-SF.The hip function was assessed with Harris hip score before operation and six months after operation .Effects and clinical relationship were assessed between the UI and improvement of hip function .Results A total of 37 patients reported UI before THA .6 months after surgery, symptoms of UI were improved in 26 patients, symptoms unchanged in 10 patients and worsened in 1 patient.There was no significant difference between UI group and non -UI group (P>0.05).In the UI group, ICI-Q-SF scores decreased obviously 6 months after operation .Conclusion There is a relationship between pelvic floor structure of hip and urinary incontinence .Female patients with femoral head necrosis improved hip function by THA , which can make an improvement for urinary incontinence .It needn’t to take urinary incontinence surgery immediately for female patients with hip dysfunction .Clinical observation of urinary incontinence symptom should be taken after THA .Mid-urethral suspension is still necessary for urinary incontinence if patient without symptom improvement .
