Clinical Study of Laparoscopic-Assisted Transanal Endorectal Pull-Throughs with Short Muscle Cuff for Classic Hirschsprung’s Disease

Abstract:Objective Muscle cuff of traditional Soave pull-through procedure for Hirschsprung's disease (HSCR) is correlated with complications including constipation,enterocolitis and soiling.We intended to explore the surgical outcomes of laparoscopic-assisted transanal endorectal pull-through with a short muscle cuff.Methods A total of 112 HSCR boys were recruited from January 2014 to June 2016.The mean age was (14.5 ± 8.8) (6-72) months.Sixty-four of them underwent laparoscopic-assisted transanal endorectal pull-throughs with muscle cuffs of a traditionallength (8-10 cm) and 48 procedures with shorter muscle cuffs (2.5-3.5 cm).Laparoscopic rectal mobilization was performed until the level of levator ani.Endorectal dissections started at the same level at 0.5 cm above dentate line until 3 to 4 crn above the dentate line.After reaching the previously dissected intra-abdominal level,peritoneal cavity was entered.Other parts of the procedures were the same.Hip rinsed with 2.5% warm saline was given from 3 days after surgery and for 3 months after surgery.Anal dialation lasted for 6 months.Results The median operative duration was 142(125-160) in short-cuff group versus 132(120-150) min in long-cuff group.Median postoperative hospital stay was 10 days in both groups.There were no mortality occurred during perioperative period.During 1-year follow-up,the rate of enterocolitis was 2.1% (1/48) in short-cuff group versus 7.8% (5/64) in long-cuff group.Constipation occurred in 1 patient in short-cuff group and 4 in long-cuff group.Soiling rate was lower in short-cuff group (4/48 vs.7/64).The short-cuff group also had significantly lower rates of endorectal infection and anastomic stricture.Conclusions Recommended for laparoscopic-assisted transanal endorectal pull-throughs,short-cuff improves functional outcomes including lower rates of enterocolitis,constipation and soilings.And endorectal infection and anastomic strictures are also infrequent.
