Clinical observation of Jiajian Zhigancao decoction on the cardiotoxicity induced by breast cancer chemotherapy and analysis of the protective effect on cardiac function

Abstract:Objective To observe the effect of Jiajian Zhigancao decoction on the cardiooxicity induced by breast cancer chemotherapy and to investigate the protective effect on cardiac function.Method 96 patients with breast cancer were randomized as study group and control group as per a random number table and each group was 48 cases,those who in control group was treated with TAV chemotherapy regimen(docetaxel+doxorubicin+cyclophosphamide),while the study group was additionally given Jiajian Zhigancao decoction on the basis of TAV,with 21 d as 1 cycle,and all patients were administered with 2 cycles of continuous treatment. The changes of phosphocreatine kinase (CK), creatine kinase MB isoenzyme(CK-MB),cardiac troponin I(cTnI)and left ventricular ejection fraction(LVEF),left ventricular end dia-stolic diameter(LVEDD)and left ventricular end systolic diameter(LVESD)before and after treatment were compared between the two groups,and the incidence of cardiotoxicity was compared.Result Before treatment,there were no sig-nificant differences in CK,CK-MB,cTnI,LVEF,LVEDD and LVESD between the two groups(P>0.05);after treatment, there were no obvious changes in CK,CK-MB,cTnI,LVEF,LVEDD and LVESD in study group as compared with be-fore treatment (P>0.05); in control group, the CK, CK-MB, cTnI, LVEDD and LVESD increased significantly, while LVEF decreased markedly, and the differences were statistically significant (P<0.05); the CK, CK-MB, cTnI, LVEDD and LVESD in study group were significantly lower than those in the control group,but the LVEF was significantly high-er than that in control group(P<0.01);the overall incidence of cardiotoxicity in study group was significantly lower than that of the control group(16.67% vs 37.50%),demonstrating statistically significant difference(P<0.05).Conclusion Ji-ajian Zhigancao decoction may be added during the chemotherapy for breast cancer,which is effective and may relieve the cardiotoxicity caused by chemotherapy,thus protecting cardiac function,with significant clinical value.
