Keywords: dermoscopy, histopathology, hyperkeratosis of the nipple and areola Hyperkeratosis of the nipple and areola (HNA) is a rare benign skin disease that occurs in the breast clinically characterized by hyperkeratosis of the nipples and/or areolas and dark brown pigmentation. A previous study reported greater hazard ratios in women with HNA between the ages of 20–30s. 1 It affects self-confidence in these patients and needs to be distinguished from cutaneous dirt-adherent disease, breast eczema, and acanthosis nigricans. In this report, we present a case of HNA diagnosed using dermoscopy and histopathology. A 19-year-old female was admitted to our hospital on August 3, 2017, due to bilateral nipple-areola brown patches persisting for 5 years. Initially, the color of the bilateral nipple-areola deepened gradually without a clear origin, and the patient did not seek any treatment. Three years ago, she developed dark brown papules ranging from needle-tip to grain size on the surface of the bilateral areolas, some of which merged into plaques. She experienced slight itching and was initially diagnosed with breast eczema at a local hospital. She used an external ointment (details unknown), which alleviated the itching, but did not resolve the skin lesions. Seeking further treatment, she came to our hospital outpatient department. She had no significant past medical history and was unmarried and nulliparous. Dermatological examination revealed densely distributed dark brown keratotic papules of needle-tip to grain size on the surface of bilateral nipple-areola. Dermatoglyphics were deepened and widened, and the skin lesions had a clear boundary without erythema or erosion (Figure 1A, B, and C). Figure 1 Clinical appearance of the patient on admission. Laboratory tests, including routine blood examination, routine urine and stool tests, and liver and kidney functions showed normal results. Histopathological examination of the lesion revealed hyperkeratosis, subepidermal protuberances forming a network and surrounded by collagen, hyperpigmentation of the epidermal spinous layer and basal layer, and sparse perivascular lymphocytic infiltration in the papillary layer of the dermis. Bilateral Hyperkeratosis of the Nipples and Areolae with Linear Nevus: a Rare Case Report and Review of the Literature.

Abstract:Hyperkeratosis of the nipple and areola is an uncommon dermatosis without well-defined etiology, which occurs mostly in young women and may only be a cosmetic problem. In 1938, Levy-Frankel classified this disease into three variants; type I involves hyperkeratosis with an epidermal nevus, and has rarely been reported. We report a case of a 23-year-old woman with long-term bilateral pigmentation and thickening of the nipples and areolae, accompanied with dark brown, flat lesions with a linear distribution on her left forearm. The verrucous plaques were asymptomatic and could be scratched off by the patient herself. Consequently, she did not seek medical care for more than ten years. The prevalence of this condition is likely underestimated because many affected individuals are not sufficiently motivated to seek medical attention.
An Ulcer on the Nipple

Abstract:A 39 year old woman presented with an asymptomatic ulcer on her left nipple. The ulcer had been present for five weeks. She also reported malaise. She denied any travel history in the past three months, but recalled a history of breast bites and breast-genital contact during sexual intercourse, which had occurred two weeks before the skin lesion appeared. Physical examination revealed demarcated ulceration with a border that was slightly elevated, crusted, and indurated (fig 1). Left axillary lymphadenopathy was noted. Abstract:A patient with papillary adenoma of nipple is reported.The 42-year-old female patient presented with intermit-tent and painful hemorrhagic polyrrhea for four years,accompanied with swelling,erosion and scab on the left nipple.Histopathology revealed the papillary proliferation of ductal cells surrounded by a compact layer of myoepithelial cells.Papil-lary adenoma of nipple waa diagnosed,and the patient was under follow up.
Multiple Eruptive Clear Cell Acanthoma

Abstract:Sir, A 54-year-old Chinese woman presented to our department with multiple itchy erythematous to brown rounded sessile nodules and irregular plaques on the perineum. No abnormal symptoms were described by the patient, except pruritus. The lesions started 7 years back and gradually increased in number and size. There were more than 20 lesions with the long axis of individual lesions varying between 5 to 50 mm. The sessile nodules were rounded, rufous or brownish red, and sharply circumscribed with irregular shape, sometimes with a peripheral collarette scaling or slightly moist surface [Figure 1]. Moreover, red pinpoint dots on a whitish background were observed under dermoscopy [Figure 2]. Clinical examination did not reveal any systemic abnormality. There was no history of trauma or insect bite. Differential diagnoses included seborrheic keratosis, amelanotic melanoma, squamous cell carcinoma, Bowen's disease, eccrine poroma, guttate psoriasis, and clear cell hidradenoma. An incisional skin biopsy from a plaque on her left inguinal region was performed.Figure 1: Multiple erythematous to brown and irregular sessile nodules on the perineumFigure 2: Dermoscopic appearance. Red pinpoint dots on a whitish backgroundHistologically, the specimen revealed a psoriasiform pattern with papillomatosis. Acanthosis, hyperkeratosis, parakeratosis, and neutrophils in the epidermis and inflammatory cell infiltration and blood vessel dilation in the upper dermis were seen [Figure 3a]. Clear cell was noted in the epidermis, and no nuclear division was recognized [Figure 3b]. As for the immunohistochemical study, those clear cells showed abundant cytoplasmic glycogen by periodic acid-Schiff staining [Figure 4a and 5a]. High molecular weight cytokeratins CK34βE12 [Figures 4c and 5b] and CK5/6 [Figure 4d] were intensely positive in almost all keratinocytes. Epithelial membrane antigen [Figure 4e] was also positive, while CK20 and carcinoembryonic antigen [Figure 4b, f and 5c] were negative in the keratinocytes. Degos et al. were first to describe CCA in 1962,[1] so this entity is also called Degos acanthoma or pale cell acanthoma.[2] Usually, CCA presents as solitary, brownish-red papule or nodule that always occurs in the lower extremity, with a diameter <20 mm.[3] It grows quite slowly and maintains the same shape and size for years. About 98% of CCA patients are beyond 40 year old and more often around 60 year.[4] Multiple CCA is a rare form of this disease. The case is presented here because of this rare form of multiple lesions on the perineum. Under dermoscopy, lesions presented red dots, due to glomeruloid vessels in a serpiginous pattern, which provided a significant clue in the diagnosis of CCA. However, it still depends on a skin biopsy and its unique histopathological features to make a certain diagnosis. As for the treatment, electrofulguration, cryotherapy, curettage, carbon dioxide laser, surgical excision, topical 5-fluorouracil, and topical calcipotriol were included.[5] The lesions of our patient are multiple; we chose microwave treatment for those giant lesions combined with the use of topical calcipotriol for those small papules.[5] Now, the patient is still in the follow-up. Abstract:A 62-year old woman presented with dark red patches in the right nipple with itch for more than 2 months. There was thick reddish-brown crust around the right nipple and the basal skin was dark-red. Histopathologic examination showed epidermal hyperkeratosis, part of the granular layer wedged, basal cell liquefaction degeneration, and lymphocytes ribbon infiltration in the superficial dermal. It was diagnosed as lichen planus. After topically application of mometasone furoate cream and 0.03% tacrolimus ointment, the skin lesion was improved.

Abstract:A case of acantholytic dyskeratotic epidermal naevus accompanied by pigmented hairy epidermal nevus is reported.The patient was a 40-year-old man.He had hairy epidermal nevus when he was 10 years old.At the age of 30,he had brownish keratotic papules on his left side of the body.The histopathology of skin lesions revealed feature of Darier's disease with papillomatosis,hyperkeratosis and focal areas of suprabasal cleft with acantholysis.

Abstract:A 9 year-old female patient presented with large areas of light brown macules on her trunk and buttocks since birth. The lesions enlarged with age and appeared in a short pants shape. There was a pigeon's egg-sized black plaque on her right buttock. There was thick hair on the surface of dark brown lesion on her dorsal trunk. Her clitoris and right labium majus were soft hypertrophy. On the right side of anus there was a walnut-sized, soft, skin-colored mass with granular surface and unclear border. The patient's growth and intelligent development were normal. The pathologic results confirmed neurofibromatosis. The final diagnosis was neurofibromatosis with congenital melanocytic nevi and pigmented hairy epidermal nevi.

Abstract:A ten-year-old girl presented with hyperpigmented patches on her trunk and extremities for ten years.Dermatologic examination revealed clear features of liner and whorled brown patches following the Blaschko lines on her trunk and extremities,without scales and atrophy.Histopathology showed mild hyperkeratosis,focal liquefaction and hyperpigmentation in epidermal basal layer,and mild perivascular infiltration of lymphocytes and melanophages.The patient was diagnosed as linear and whorled nevoid hypermelanosis.

Abstract:Keratosis pilaris (KP) is a common inherited disorder characterized by small folliculocentric keratotic papules that may have surrounding erythema, which gives the skin a stippled appearance resembling gooseflesh. The extensor surfaces of the upper arms, thighs, and buttocks commonly are affected, but a generalized presentation may occur. We report the case of a 29-year-old woman with unilateral generalized KP in the second month of her second pregnancy. Both a genetic mutation and pregnancy-induced hormonal changes played possible roles in the development and progress of unilateral generalized KP in this patient.

Abstract:A 35‐year‐old woman presented with a nodular growth on her right buttock. She had more than a 20‐year history of multiple, asymptomatic, smooth, skin‐colored papules involving her right buttock. The lesions had increased in number over time and had become yellowish or pale red with coalescing nodules exhibiting irregular surfaces. There was no general malaise. Ten years earlier, a diagnosis of connective tissue nevus was made on the basis of a biopsy obtained at a local hospital, but the patient declined treatment at that time. During the past 10 years, the lesions had continued to develop. The medical and social history was otherwise unremarkable, and a general physical examination was negative except for the skin lesions. Cutaneous examination showed soft, yellowish or pale red to violaceous papules and nodules of various sizes with wrinkled surfaces, coalescing into plaques. The lesions were aggregated on her right buttock and extended onto the thigh. The biggest nodule, with a coarse and dry surface, was 40 mm in diameter (Fig. 1). Histopathologic examination of the largest nodule revealed hyperkeratosis and epidermal hyperplasia with elongated rete ridges. There were thin‐walled, ectatic papillary dermal vessels engorged with red blood cells and masses of mature adipose cells throughout the dermis (Fig. 2) (Tianwen G, Jianfang S. Non‐epidermal related. Porokeratoma: a Case Report

Abstract:A case of porokeratoma is reported.A 35-year-old man presented with asymptomatic verrucous nodules on both buttocks for more than 1 year.Skin examination at the first visit revealed two pea-sized verrucous nodules on bilateral buttocks.The lesions recurred at the primary site 3 months after surgical resection.At the second visit,there were 4 verrucous nodules,including 1 on the left buttock and 3 on the right buttock.Histopathological examination showed a well-defined lesion characterized by acanthosis and compact,thick orthokeratosis with seveal broad cornoid lamellae.A diagnosis of porokeratoma was made.The lesion was resected again followed by oral acitretin treatment.No recurrence was observed during 15 months of follow up.

Abstract:A case of porokeratosis presented as eruptive pruritic papular lesions is reported. The patient was a 50-year-old man. He complained of pruritic and hyperkeratotic papules distributed over his buttocks for 7 years. The lesions aggravated 1 year ago, and the lower limbs were involved,with severe pruritus especially when sweating. Histopathological examination revealed the characteristic comoid lamellae. He was treated with topical keratolytics and oral acitretin, but the lesions showed no evident improvement.
A case of Micaceous and Keratotic Pseudoepitheliomatous Balanitis

Abstract:A 56-year-old Chinese male patient had presented with 4-year history of recurrent erosion followed a partial circumcision because of phimosis and unknown lump, an eruption on his glans penis developed slowly. Physical examination showed hyperkeratotic plaque with white micaceous scale. Histological examination revealed hyperkeratosis, Parakeratosis and pseudoepitheliomatous hyperplasia with no cytological atypia,and a mild focal superficial inflammatory infiltrate. The case was finally diagnosed as micaceous and keratotic pseudoepitheliomatous balanitis (MKPEB). The lesion has been improved with topical tretinoin and imiquimod, with no evidence of aggravation at 2-year follow-up.

Abstract:Sir: Since the proposal of the term “porokeratotic eccrine ostial and dermal duct nevus” by Abell and Read in 1980, fewer than 30 cases have been published. The typical presentation has been congenital, asymptomatic papules and plaques on one or more of the extremities, rarely involving the trunk, face, or scalp.1,2 Lesions follow Blaschko’s lines with variable extent of involvement. The Blaschkoid configuration reflects clinical mosaicism attributed to a novel postzygotic mutation, chromosomal alterations during embryogenesis or, less frequently, functional mosaicism of an X-linked defect. Kroumpouzos et al. reported a similar pattern of clinical presentation in a rare case of systemized porokeratotic eccrine and hair follicle nevus.3 The histologic hallmark of porokeratotic eccrine ostial and dermal duct nevus is cornoid lamellae packed within epidermal invaginations overlying eccrine units. This has been attributed to either a hyperplastic acrosyringium with relative sparing of the glandular portion or a hyperplastic epidermal invagination associated with an eccrine-like duct.4 Although it has been speculated that an abnormal clone of epidermal cells at the base of the parakeratotic columns may be prone to malignant degeneration, we found no documentation of associated malignancy. We present a case of congenital porokeratotic eccrine ostial and dermal duct nevus in a 39-year-old woman whose ulcerated lesions within the porokeratotic eccrine ostial and dermal duct nevus revealed squamous cell carcinoma. The patient was born with diffuse, whirled hyperpigmentation of her trunk and extremities. Since the age of 6, these lesions had undergone intermittent pruritic blistering and gradually evolved into painful, nonhealing wounds. Two years before her current presentation, she was evaluated for worsening symptoms. Biopsy specimens obtained at that time were benign, revealing columns of cornified parakeratotic cells overlying epidermal invaginations and adnexal ostia. When referred for surgical evaluation, hyperpigmented patches and plaques were noted in a Blaschkoid arrangement throughout the trunk and upper and lower extremities (Fig. 1). The distribution pattern of the nevus had not changed since birth. Both discrete and clustered superficial ulcerations were noted within these lesions.Fig. 1.: Blaschkoid distribution of lesions extending inferiorly to include the posterior aspect of the right leg and plantar surface of the right foot. Multiple oozing ulcers are present.Over an 11-month period, she underwent staged excision of all ulcerated lesions, followed by either primary closure or split-thickness skin grafts. A total of 36 specimens were sent for pathologic examination. Thirty-one (86 percent) of these revealed squamous cell carcinoma adjacent to the cornoid lamella (Fig. 2). Seven of these 31 specimens were described as invasive squamous cell carcinoma, whereas the rest were labeled as either superficial or multifocal squamous cell carcinoma in situ.Fig. 2.: Cornoid lamellae adjacent to squamous cell carcinoma. Note the angled column of parakeratosis, diminished granular layer, and dyskeratosis (hematoxylin and eosin; original magnification, ×200).A case of diffuse, painful, nonhealing ulcers revealing squamous cell carcinoma within congenital porokeratotic eccrine ostial and dermal duct nevus lesions is presented. This is an unusual finding in what is otherwise known as a benign clinical entity that rarely requires surgical therapy. One may consider malignant transformation of an abnormal clone of epidermal cells at the base of the parakeratotic columns. Alternatively, malignant transformation has been documented in rare cases of linear epidermal nevus—an entity that bears close resemblance to porokeratotic eccrine ostial and dermal duct nevus clinically. However, the distinction between linear epidermal nevus with parakeratotic features and porokeratotic eccrine ostial and dermal duct nevus has not been adequately established in the current literature.5 Further investigation into the genetic and molecular specifics of porokeratotic eccrine ostial and dermal duct nevus pathogenesis is required. Abstract:Objective To report a group of cases characterized by pigmented symmetrical acrokeratoderma.Methods Sixteen patients characterized by pigmented syrnmetrical acrokeratoderma were collected from outpatients from May 1998 to September 2007 in the Department of Dermatology,Second Affiliated Hospital,Sun Yat-Sen University.General information on the sex,age,profession,family history of patients were documented.All patients underwent clinical and histopathological examinations.and were followed up for the assessment of treatment efficiency.Results Among these patients,14 were males and 2 were females,ranging in age from 9 to 54 years,with duration of disease varying from 2 months to 8 years(mean course 2.0±1.5 years).Characteristic manifestations were rough,scaling,brownish aubum or black maculopapular eruptions with mild pruritus.The lesions were symmetrically distributed on palms,dorsal digits and wrists of 16(100%)patients,elbows and knees of 2(12.5%)patients,and ankles of 3(18.8%)patients.Histopathologic examination revealed epidermal hyperkeratosis,acanthosis and papillomatous hyperplasia as well as dermal infiltration with a few lymphocytes.No heredity tendency was observed in any of the patients.The patients showed a poor response to,or experienced a relapse after treatment with oral tretinoins,nicotinamide,vitamin B6 as well as topical tretinoin ointment,glucocorticoid ointment,and so on.Condusion These cases may represent a new entity clinically characterized by pigmented symmetrical acrokeratoderma,and pathologically characterized by epidermal hyperkeratosis,acanthosis and papillomatous hyperplasia with unknown etiology.
A Case of Nipple Adenoma Detected by Sonography

Abstract:To the Editor: A 28-year-old woman complained of a lesion with blood discharging on her right nipple. She noticed a nodule on her right nipple, with blood discharging and crust for 2 months, and the symptom of blood discharging happened frequently for almost a month. Physical examination revealed a nodule 10-mm in diameter, with an opening in the center, erosion and crusting in places, and no lesion in areola [Figure 1a]. Breast palpation failed to reveal any mass, and axillary lymph node examination was negative.Figure 1: Clinical picture, sonography, and histopathology of the lesions. Color Doppler study revealed relatively increased blood flow signal in the nodule [Figure 1c]. Histopathology of the lesions showed a well-fined tumor composed of glandular formation in the whole dermis; the glands were lined with apocrine metaplastic epithelium formed by cuboidal cells with basophilic nuclear and eosinophilic cytoplasm. The basal cells were made up of myoepithelial cells. Apocrine secretion could be found in glands [Figure 1d and 1e]. Immunohistochemistry revealed that cuboidal cells were CK5/6(+), myoepithelial cells were p63(+), and tumor cells were Ki-67 (5%+), ER(+), and PR(part+). Nipple adenoma was diagnosed. The lesion was resected, and the patient is being followed up. Nipple adenoma is a rare and benign breast tumor of the lactiferous ducts that develops in the superficial portion of the nipple. It usually happens among woman with an average age of 40 years. In early stage, the lesion is asymptomatic, and hence it is easily overlooked by patients. With the development of the disease, the presenting signs and symptoms can progress to nipple erosion, crusting, pain, itchiness and bloody discharge, making it indistinguishable from the clinical manifestation of Paget's disease. At a later stage, the nipple has an indurated nodule which makes it indistinguish from other dermatologic or mammary diseases. The course of disease is rather long, from several months to several years. In our case, noninvasive sonography was first used and it revealed a well-defined hypoechoic nodule in the superficial part of nipple with posterior echo enhancement, and Doppler sonography revealed that the entities were hypervascular, which helped to diagnose nipple adenoma, to confirm the size and depth, and to differentiate from Paget's disease in which ultrasound image showed an irregular, ill-defined, and hypoechoic lesion.[12] Histological examination is fundamental for the diagnosis. There are three main histological phenotypes of nipple adenoma: (1) Adenosis type: the lesion is in the dermis, has a relatively clear margin, and usually lacks hyperkeratosis in the epidermis. The hyperplasia glandular ducts have two types of cells: glandular epithelium and myoepithelium. (2) Epithelial hyperplasia type: significant florid hyperplasia is present in the epithelia of the collecting ducts and hyperplasia glandular ducts. This hyperplasia is often solid and may have a complex papillary form. It can be accompanied by atypical hyperplasia, necrosis, or mitotic figures. (3) Pseudo-infiltrating type: the lesion has significant pseudo-infiltrative differences from ordinary tissue that are usually located within the lesion or in peripheral areas. These three histological changes exist primarily in a mixed form within the same tumor.[3] Immunohistochemistry test can also be useful in establishing a differential diagnosis with malignant diseases. In nipple adenoma, the glandular epithelium was diffusely positive for 34betaE12, patchily positive for CK5/6, and negative for p53 and c-erbB-2. The myoepithelium, positive for p63, smooth muscle actin, and calponin, was well preserved and outlining the duct.[4] This helps in differential diagnosis with malignant diseases which were positive for c-erbB-2, p53, and lack of complete myoepithelium. The treatment of nipple adenoma is a simple resection of the tumor with local anesthetic. If the tumor is completely resected, recurrence rarely happens.[3] Financial support and sponsorship Nil.
