Abstract:Objective　Effects of various kinds of modified Pendulum Appliances were discussed after clinical application.Methods　Three kinds of modified Pendulum Appliances were introduced.All the indications,manufacturing methods,principles of clinical application,as well as advantages and disadvantages of these appliances were described.Results　①First premolar and first molar's pendulum appliances could be used for any kinds of patients who need distalization of upper molars.②Lateral incisor and first molar's Pendulum appliance could be used for the patients with crowding in anterior segments, especially in patients with labial positioned canines who need distalization of upper molars.③Lateral incisor and first molar Pendulum Appliance with screw expander can be used for the patients with crowding in anterior segments accompanied narrowed upper arch.Conclusion　Modified Pendulum appliance can be used in various kinds of patients with different situation of malocclusion. The appliances are used for distalization of upper molars effectively, together with expansion and alignment of upper arch.

Abstract:Using spring clips or caps lock instead of conventional ligation ,self-ligating bracket can offer lower sliding resistance and light force,which enables the alveolar bone to remodel efficiently,and allows shorter chairside time and less visits,making great importance in rapid tooth moving and anchorage control. However,some disadvantages have been reported too. In this article,current research and implication of self-ligating bracket are reviewed concerning developing history,structure forms,force system,and clinical practice.

Abstract:ABSTRACT A 20-year-old female patient with severe maxillary crowding and skeletal Class III malocclusion was successfully treated using an enhanced pendulum appliance. Camouflage treatment was chosen because the patient refused surgical intervention. Detailed examination and analysis revealed that labial inclination of the maxillary anterior teeth could provide the alignment space necessary after tooth extraction in the maxillary arch but it would not be beneficial esthetically and would increase the moment perpendicular to the tooth long axis as well as increase the risk of bone dehiscence. Therefore, lingual inclination of the mandibular anterior teeth was performed. To create space for aligning the maxillary arch, an enhanced pendulum appliance was utilized, in addition to premolar extraction. The enhanced pendulum appliance had one mesial and one distal helix on each spring, exerting opposing forces in the buccolingual direction and reducing the buccolingual inclination of the molars. In addition, miniscrews were integrated into the Nance arch of the enhanced pendulum appliance to provide skeletal anchorage, minimizing the reciprocal movement of anchorage teeth. This modification not only enhanced control over tooth movement but also improved treatment efficiency. During the entire treatment duration of 17 months, substantial improvement in facial and occlusal aspects were noted. Additionally, the patient retained these positive changes until the subsequent 2-year follow-up period.

Cold-formed Clasp in Combination with Casting Frame for Repair of Dentition Defect

Abstract:Objective To observe the effect of improved cold-formed clasp combined with casting frame as removable partial dentures.Methods Stainless steel round wire from forging was made flat with bending pliers to turn the system into semi-circular curved arm clasp.The modified clasp was used together with casting Co-Cr frame to repair dentition defect.The clinical effect was followed up for two years and compared with that of 96 cobalt-chromium alloy framework with the traditional cold-formed plastic denture base plate.Results The abutment tooth's condition of improved cold-formed clasp with casting frame denture was better than the denture with conventioinal clasp.Conclusion The improved cold-formed clasp combined with casting frame removable partial dentures performs very well and can be applied in clinic.
Banded versus modified appliances for anchorage during maxillary protraction

Abstract:Introduction: The aim of this study was to compare banded versus modified appliances for anchorage during maxillary protraction in Class III malocclusions. Patients and methods: The sample size consisted of 40 growing patients with Class III maxillary deficiency: 20 patients received maxillary protraction with a modified appliance and 20 patients with a banded appliance. Pre- and posttreatment cephalometric radiographs of all subjects were obtained and analyzed. The paired t‑test and Wilcoxon ranks test were used for statistical analysis. Results: The patients in the modified appliance group needed fewer appointments and shorter treatment time than those in the banded appliance group. The modified appliance was superior to the banded appliance with respect to simple structure, comfort, retention, and convenience in maintaining oral hygiene. The modified appliance was as effective as the banded appliance in correcting the Class III malocclusion. However, a greater increase was found in mandibular plane angle, anterior facial height, total facial height, mesialization of maxillary molars, and proclination of maxillary incisors in the banded appliance group compared with that in the modified appliance group (P < 0.05). Conclusions: The newly developed modified appliance may be a promising approach in treating growing Class III patients with maxillary deficiency, which could decrease treatment time, increase treatment efficiency, and reduce anchorage loss.
Clinical application of the modified impression method for the lower complete denture

Abstract:Objective: To probe into the clinical application of the modified impression method for the lower complete denture.Method: Five patients suffered from angular cheilitis with severely atrophic mandibular alveolar ridge,whose old denture teeth were worn severely,and bad stability in the mouth.According to the lower suction denture theory and new clinical requirement by Dr.Abe Jiro from Japan,the process of the modified impression method for the lower complete denture was introduced in this study.Result: The lower denture achieved a complete sealing of denture border by the modified impression method,so that it could improve masticatory efficiency and quality of patients'life.The patient were all satisfied.Conclusion: The lower complete denture retention was improved by applying the modified impression method on the patient with severely atrophic mandibular alveolar ridge.

Abstract:OBJECTIVE:To compare the retentions of different designs of cobalt-chromium (Co-Cr), pure titanium (CP Ti), and titanium alloy (Ti-6Al-4V) removable partial denture (RPD) circumferential clasps manufactured by selective laser melting (SLM) and to analyze the stress distribution of these clasps during the removal from abutment teeth. METHODS:Clasps with clasp arm size A (1.9 mm width/1.1 mm thickness at the body and 0.8-taper) or B (1.2 times A) and 0.25 mm or 0.50 mm undercut engagement were modeled on a prepared first premolar die, named as designs A1, A2, A3, and A4, respectively. The density and elastic modulus of SLM-built Co-Cr, CP Ti, and Ti-6Al-4V were measured and given to different groups of clasps. The density, elastic modulus, and Poisson ' s ratio of enamel were given to the die. The control group was the cast Co-Cr clasp with design A1, to which the density and elastic modulus of cast Co-Cr alloy were given. The Poisson's ratio of all metals was 0.33. The initial 5 N dislodging force was applied, and the maximum displacement of the clasp along the insertion path was computed. The load was reapplied with an increment of 5 N than in the last simulation until the clasp was completely dislodged. The retentive force range of different groups of clasps was obtained. The retentive forces of the SLM-built Co-Cr, CP Ti, and Ti-6Al-4V clasps with equivalent computed retentive force range to the control group were validated through the insertion/removal experiment. The von Mises stress distributions of these three groups of SLM-built clasps under 15 N loads were analyzed. RESULTS:SLM-built Co-Cr, CP Ti, and Ti-6Al-4V clasps with designs B1 or B2, and Co-Cr clasps with design A2 had higher retentive forces than those of the control group. SLM-built CP Ti and Ti-6Al-4V clasps with design A1 had lower retentive forces than those of the control group. SLM-built Co-Cr clasp with design A1 and SLM-built CP Ti and Ti-6Al-4V clasps with design A2 had equivalent retentive forces to those of the control group. The insertion/removal experiment showed that the measured retentive forces of these three groups of SLM-built clasps were (21.57±5.41) N, (19.75±4.47) N, and (19.32±2.04) N, respectively. No statistically significant measured retentive force difference was found among these three groups of SLM-built clasps (P>0.05). The maximum von Mises stress of these three groups of SLM-built clasps exceeded their responding yield strength except for the Ti-6Al-4V one. CONCLUSION:SLM-built Co-Cr circumferential clasps had higher retention than CP Ti and Ti-6Al-4V ones with the same clasp arm size and undercut engagement. The retention of SLM-built circumferential clasps could be adjusted by changing the undercut engagement and clasp arm size. If SLM-built circumferential clasps are used in clinical practice, the Ti-6Al-4V clasp with clasp arm size A and 0.50 mm undercut engagement is recommended considering the long-term use of RPD in the patient's mouth.
Titanium clasp fabricated by selective laser melting, CNC milling, and conventional casting: a comparative in vitro study

Abstract:PURPOSE: This study aimed to investigate the effects of selective laser melting (SLM), milling methods, and casting on the behavior of titanium clasp.METHODS: The clasp and its die simulating the molar were designed using 3D software. Clasp specimens were fabricated using SLM approaches (SLM Ti) and computerized numerical control (CNC) milling technology (Milling CPTi). Cast clasps of the same forms were also prepared as controls using titanium alloy powder (Cast Ti) and commercial pure titanium (Cast CPTi), following the conventional casting methods. The surface roughness and accuracy of clasps were analyzed. The changes in retentive force and permanent deformation were measured up to 10,000 insertion/removal cycles. One-way analysis of variance and Tukey's test or Kruskal-Wallis H test were performed for data analysis and comparisons.RESULTS: The Milling CPTi clasps had a smoother inner surface than the other groups (p<0.05). The accuracy of the inner surface showed no significant difference among the groups, whereas that of the outer surface showed significant differences (p<0.05). The SLM Ti clasp had significantly higher retentive forces than the other groups (p<0.05), but it rapidly reduced after 2000 insertion/removal cycles until the fracture of all specimens was at 4000 cycles. The Milling CPTi clasps had more permanent deformation, but the rate of reduction of retentive force was only 9.5% (at 10,000 cycles).CONCLUSIONS: Milling has the potential to replace casting for fabricating removable partial denture (RPD) titanium clasps. However, SLM should be further improved for fabricating RPD titanium clasps before clinical application.

Abstract:Objectives: To investigate the effect of different designs of movable parts and prosthetic materials on the stress distribution of supporting tissues in mandibular free end dentition defects using three-dimensional finite element analysis of digital Roach attachments. Material and methods: A 3D model of a patient with Kennedy class I mandibular edentulous conditions was generated, and twelve prosthesis models were applied, combining two designs of removable parts and six types of CAD/CAM restorative materials with different elastic modulus (conventional zirconia, ultra-translucent zirconia, Polyetheretherketone (PEEK), Lithium disilicate, Nanoceramic resin, and resin composite (Paradigm MZ100, 3 M ESPE)). The stress distribution of abutment periodontal ligament, edentulousmucosa, and junction of attachment were analyzed using finite element analysis. Results: The stress value of the buccal neck of the periodontal ligament and the maximum compressive stress of the distal periodontal ligament of the design with clasp arms were higher than those without clasp arms, while the stress on the junction of attachment and the displacement of the mucosa in the edentulous area were smaller. Restorative materials with high elastic modulus, such as conventional zirconia and ultra-translucent zirconia, are recommended to be used as the fixed part of Roach attachment. Conclusion: CAD/CAM Roach attachments with clasp arms are recommended for the protection of mucosal soft tissue. Restorative materials with high elastic modulus, such as conventional zirconia and ultra-translucent zirconia, are recommended as the fixed part of Roach attachment for patients with free end defect of mandibular dentition. Clinical significance: This study provides references for the design with clasp arms and the selection of clinical fixed-movable prosthetic materials. Comparative Static Force Analysis of Molar Distalization by A Novel Sliding Rod Microscrew Implantation Jig and Direct Traction

Abstract:Background: Tooth distalization is an effective approach for mandibular prognathism. In the current study, we sought to characterize and compare the static force distribution among the upper maxillary teeth after load application by a novel sliding rod microscrew implantation jig and by a direct traction method using a traction hook. Methods: The sliding rod microscrew implantation jig was used and the microscrew was implanted into the cancellous bone above the mucogingival junction and a traction force (F) of 1.5 N was applied. Static force at the upper canine, first and second premolar and first molar was analyzed. In addition, the entire maxillary arch was distalized by direct traction using a traction hook and the horizontal and vertical traction force on the upper incisor and canine was analyzed. Results: When the load by the jig was applied to anchorage site A, B, O, C, or D, the upper canine, the first and second premolar and first molar experienced no horizontal traction force. As the track force moved down the sliding rod, the horizontal traction force on the upper first molar showed a gradual decline. When the load was applied to anchorage site O, which is at the archwire palne, the upper second premolar and first molar experienced no vertical depressing force. In the direct traction method, the upper incisor and canine received an equivalent horizontal traction force regardless of the anchorage site. When the load was applied to anchorage site O, the distalization force was evenly distributed to the upper incisor and canine. Conclusion: Anchorage site close to the archwire plane provides maximal distalization force and minimal vertical traction force in both methods. The data provides guidance for further optimization of the jig for clinical use.
Design and clinical application of preadjusted appliance based on Chinese normal occlusion

Abstract:Objective: To design and develop a preadjusted appliance based on Chinese normal occlusion, and to apply it in treatment of malocclusion cases.Methods: According to the results of the research performed in Department of Orthodontics, School of Stomatology of Peking University about teeth position and morphology of 67 Chinese with normal occlusion, the preadjusted appliance with the optimal prescription for Chinese has been designed and developed (Z1 appliance). Z1 appliance has been used in treatment of malocclusion cases. The efficiency of Z1 appliance was evaluated. Results:The treatment of 30 non-extraction cases and 16 extraction cases has been completed with Z1 appliance. A quite good result of clinical application of Z1 appliance was represented. Conclusion: Z1 appliance could be used in treatment of Chinese malocclution. The bracket base for anterior teeth was too thick and need more improvement.

Abstract:Symphyseal fractures of the mandible with concomitant bilateral condylar fractures can often be a particularly challenging fracture pattern for the facial surgeon. Improper reduction of these fractures may lead to facial widening that proves difficult to correct secondarily. Here the authors describe a method to prevent this unwanted facial widening with the use of an orthopedic periarticular reduction clamp. This method allows for not only increased ease of reduction but also reduces the burden of assistance in manual reduction of the fracture.

Abstract:Purpose: To analyze the distribution of stresses for mandibular Kennedy class I removable partial overdentures submitted to different implant lengths and clasp designs. Materials and methods: Twenty-seven heat-cured acrylic resin casts with a uniform soft acrylic layer were constructed from models representing the mandibular Kennedy class I removable partial denture with the first premolars terminal abutment on both sides. The casts were grouped into a control group, group I, and group II with three casts of each for designing a different clasp on the last abutment with Rest, Proximal plate, Aker circumferential (RPA), Rest, Proximal plate, I-bar (RPI), and wrought wire (WW). All casts were submitted to vertical load for stress analysis after preparing channels at the buccal/distal surface of abutments, the crest of the ridge, and the buccal/lingual of the implant for placement of uniaxial strain gauges that can convert the electro-signals to micro-strain by using the software. Kolmogorov-Smirnov normality test, independent t-test, and one-way anlysis of Variance (ANOVA) test followed by Tukey`s Post Hoc for multiple comparisons test were used for statistical analysis. Results: The control group results revealed more significant stresses on the ridge with RPA and RPI clasps but on the abutment with WW clasp as P < 0.05. In group I, the ridge was significantly stressed more than the implant followed by the abutment when the RPA clasp was used while there was insignificance on the implant with RPA and RPI as p > 0.05. In group II, the stresses were more on the abutment with RPA, RPI, and WW clasps followed by lower on the implant and lowest on the ridge. Conclusions: Proper implant and clasp type selection are critical for stress distribution on the ridge, abutment, and implant when using removable partial overdenture. The stresses on longer implants are tolerated and more widely distributed than shorter ones.

Abstract:The problems caused by proximal contact loss(PCL)of dental implants have been a mainstream research topic in recent years,and scholars are unanimously committed to analyzing their causes and related factors,aiming to identify solutions to the problems related to PCL.The effects of the anterior component of force(ACF),the lifelong re-molding of the adult craniofacial jaw and alveolar socket,and the osseointegration characteristics of dental implants are the main causes of PCL.On the one hand,the closing movement of the mandible causes the ACF of the tooth to move through the posterior molar cusp.Moreover,drifting between the upper and lower posterior teeth and mandibular anteri-or teeth can cause the anterior teeth of the upper and lower jaws to be displaced labially.On the other hand,reconstruc-tion of the jaw,alveolar socket and tooth root,the forward horizontal force of the masticatory muscles,the dynamic com-ponent of the jaw and the forward force generated by the oblique plane of the tooth cusp can cause the natural tooth to experience near-middle drift.Additionally,natural teeth can shift horizontally and vertically and rotate to accommodate remodeling of the stomatognathic system and maintain oral function.Nevertheless,the lack of a natural periodontal mem-brane during implant osseointegration,the lack of a physiological basis for near-medium drift,the small average degree of vertical motion and the integrated silence of dental implants without the overall drift characteristics of natural teeth increases the probability of PCL.The high incidence of PCL is clearly associated with the duration of prosthesis delivery and the mesial position;but it is also affected by the magnitude of the bite force,occlusion,the adjacent teeth,restora-tion design,implant location,jaw,and patient age and sex.PCL has shown a significant correlation with food impaction,but not a one-to-one correspondence,and did not meet the necessary and sufficient conditions.PCL is also associated with peri-implant lesions as well as dental caries.PCL prevention included informed consent,regular examinations,se-lection of retention options,point of contact enhancement,occlusal splints,and the application of multipurpose digital crowns.Management of the PCL includes adjacent contact point additions,orthodontic traction,and occlusal adjust-ment.Existing methods can solve the problem of food impaction in the short term with comprehensive intervention to seek stable,long-term effects.Symmetric and balanced considerations will expand the treatment of issues caused by PCL.
Effects Of A Modified Activator: Activator Combined With J Hook Headgear In The Treatment Of Hyperdivergent Class Ii Malocclusion During The Late Mixed Teeth Period

Abstract:The aim of this study was to compare the effect of a combination of activator and J hook headgear to that of the headgear activator. The traditional activator was modified to combine with a J hook headgear which had two hooks mesial to the upper canine for extraoral traction instead of the labial arch. In addition, there is an optional screw in the middle of the palatal parts to widen the arch in narrow arch cases. Sixty eight cases (34 boys and 34 girls, with an average age of 10.2 years old) were recruited. They were divided into two groups (Group MA: modified activator and J hook headgear group, Group HA: headgear activator combination group). Lateral standardized cephalograms were taken before and after the treatment. The cephalograms were measured by the same doctor with the Dolphin software. There was no significant difference in the changes of SNA, U1-NA, U1-NA (mm), U1-SN, and U6-PP between the two groups. However, the changes in U1-PP showed that the MA group intrudes the upper frontal teeth while the HA group does not (P<0.001). Regarding the mandible, there were no significant differences in SNB and L1-MP (degrees); however, L1 MP (mm), NP_FH (degrees) and LL-Eline (mm) showed significant differences (P<0.001). The MA group intruded the frontal teeth and rotated the mandible anticlockwise, which was also confirmed by the changes of the OP-SN (degrees), MP-SN (degrees), and np-fh (degrees) (P<0.001). Compared to the headgear activator, the modified activator can level the frontal teeth, decrease the angle of the MP SN and MP FH, and rotate the mandible counterclockwise, which could achieve a better chin and lip outline.

Abstract:BACKGROUND: The cycle fatigue of clasps casted by different alloys may occur when the denture is token off and worn in clinical practice. OBJECTIVE: To summarize the research conditions on the causes, effecting factors and test methods of cycle fatigue of casting clasp. METHODS: A computer-based online search of papers published between January 1996 to January 2011 was performed in Medline database, CNKI database and Wanfang database using the key words of "casting clasp, cycle fatigue, effecting factors, test methods" in English and in Chinese. RESULTS AND CONCLUSION: Currently, there are many casting clasp made of different alloy materials used in clinical work and satisfactory clinical effect have been already acquired. For instants, the cobalt-chromium alloy, titanium alloy and the high-cobalt-chromium-molybdenum alloy which is gradually used in clinical work. In the use of removable partial denture, the phenomenon of cycle fatigue of the casting clasp will come out because of access repeatedly to the undercut abutment area. There are many studies on the causes, effecting factors and test methods of cycle fatigue of casting clasp. But how to decrease the cycle fatigue effectively is still unclear.

In this study, the authors used anterior maxillary segmental distraction to correct these deformities. The effectiveness, advantages and disadvantages of this method were discussed.
Fatigue Behavior of Endodontically Treated Maxillary Premolars with MOD Defects under Different Minimally Invasive Restorations

Abstract:OBJECTIVES:To analyze the stress distribution and subsequent fracture resistance of human maxillary premolars with mesial-occlusal-distal (MOD) defects restored with different minimally invasive restorations.MATERIALS AND METHODS:Seventy non-carious human maxillary premolars were selected and divided into seven groups (n = 10). Ten teeth without further preparation served as control. The remaining teeth were endodontically treated and received three restorative patterns: inlays without cusp coverage (I), onlays with palatal coverage (O), overlays with both buccal and palatal coverage (Ov). Lithium disilicate glass ceramics (EM) and machinable composite resin (LU) were used for restoration. Specimens were tested under cycling loading with tongue direction of 45° for 1.2 × 106 cycles at a 50-N load and 2.0-Hz frequency. The survival time and two fracture mode classifications were assessed. Three-dimensional models of each group were designed. The magnitude and pattern of stresses were analyzed under the same condition of the in vitro test using finite element stress analysis.RESULTS:Although the overlay model pattern produced more favorable stress distribution, three restorative patterns restored with the same material had no difference in survival curves (P > 0.05). Only the survival curve of the EM-Ov group had no statistical difference with that of the control group (P > 0.05). EM groups presented mainly interface adhesive failure, while LU groups were mainly material cohesive failure.CONCLUSION:For the endodontically treated maxillary premolars with MOD defect, the lithium disilicate glass ceramic overlay pattern can reach the best restorative effect.CLINICAL RELEVANCE:Comparing with restorative pattern, restorative material had a greater influence on the minimally invasive restoration of posterior teeth.
[A clinical analysis of screw-retained implant-supported casted integrated abutment crowns in the molar region with limited interocclusal space]

Abstract:Purpose: To observe the clinical effects of screw-retained implant-supported casted abutment integrated crowns (IACs) in the molar region with limited interocclusal space. Methods: This study involved 507 implants in 376 patients with limited interocclusal space in the molar region. All implants were inserted to the jaw by standard one-stage protocol. Screw-retained IACs were used as the final prosthesis. With 6 to 24 months follow-up, the clinical effects of screw-retained IACs were recorded and analyzed using SPSS 17.0 software package. Results: During 6 to 24 months of follow-up, the implant survival rate was 99.61%. 37 patients had ceramic fracture which mainly happened in the group with 3-4 mm interocclusal space. In each group, porcelain fused to metal (PFM) prosthesis had ceramic fracture more easily than PFM prosthesis without porcelain on occlusal surface(P<0.05). As the interocclusal space became smaller, the probability of collapsing porcelain increased(P<0.05).13 patients had screw loosening. 13 patients suffered from gingival swelling and bleeding, they were given periodontal treatment and oral hygiene instruction. Patients were satisfied with the restoration results. Conclusions: Screw-retained IACs can be used to repair missing teeth in the molar region with limited interocclusal space.

Abstract:Objective: To improve retainer's aesthetics and alleviate buccal retainer's peculiar sense for patients with removable partial denture.Methods: Casting clasp between teeth(CCBT) is set up between buccal and lingual embrasure of two abutment teeth.Results: The retainer's metal is hardly revealed,with nicer aesthetics,small volume and comfortable sense.After being observed in 34 patients for 1-2 years,the CCBT retains perfect and the abutment teeth don't demonstrate removal.Conclusion: The CCBT is apparently better than the normal casting extra-coronal retainer within RPD,both in aesthetics,manufacture and comfort.
