Antibiotics susceptibility of AmpC β-lactamases hyperproducing strains in lower respiratory tract infections

Abstract:Objective To investigate antibiotic susceptibility of AmpC β-lactamases hyperproducing strains in lower respiratory tract infections. Methods AmpC β-lactamases hyperproducing strains were detected by three-dimensional extract test. Antibiotic susceptibility were determined by Kirby-Bauer disk diffusions method. Results The overall incidence rate of strains positive for hyperproducing AmpC β-lactamases was 22.5%(84/374). The susceptibility rate of AmpC hyperproducer were 0~28.6% to the second-, third-generation cephalosporins, cephamycins, monobactam and antibiotics combined with inhibitors, while 98.8%,82.1%,72.6%,63.1% to imipenem, cefepime, ciprofloxacin, amikacin in turn. The major underlying diseases were chronic obstructive pulmonary disease (COPD), malignant tumor, cerebral stroke and chronic cardiac, hepatic, renal insufficiency. All patients had a history of using β-lactam antibiotics especially on the third-generation cephalosporins. The incident rate of mixing infection with other bacteria and / or fungi was 36.9%(31/84), most of these pathogen were opportunistic bacteria. Moreover, the case mortality was 22.6%(19/84). Conclusion The patients with severe underlying diseases and using β-lactam antibiotics especially on the third-generation cephalosporins for a long time are easily suffered AmpC β- lactamases hyperproducing strains infection, and usually have mixing infection with opportunistic bacteria. The case mortality of these patients is high. Imipenem is the best choice, cefepime being second, for treating infection caused by AmpC producers.
