A new method to move mandible to intercuspal position in virtual three-dimensional orthognathic surgery by integrating primary occlusion model.

Abstract:With the development of computer-assisted surgery systems, surgeons can perform dynamic cephalometry of the facial skeleton and complete the virtual orthognathic surgery process in a 3-dimensional (3D) environment. 1 For virtual 3D orthognathic surgery, acquired computed tomography (CT) images are imported into 3D image analysis software to segment the anatomic structures of interest, such as the maxilla and mandible, that will be used for virtual planning. An automatic segmentation protocol is usually preferred to segment bone based on thresholding followed by manual processing, because the maxilla and mandible are usually connected and must be separated in the temporomandibular joint, particularly on the occlusal surface. However, the occlusal surface between the maxilla and mandible is difficult to segment when craniofacial CT images are taken with the mouth closed. Thus, it has been recommended that the CT image should be taken with the mouth open to avoid damage to the tooth surfaces caused by separating the occlusal surface during segmentation, which would decrease the accuracy of the virtual occlusion splint. However, after the 3D craniofacial models are calculated using these segmented structures, a vertical closing of the lower jaw is necessary to return to the intercuspal position for visualization and accurate 3D measurement of the facial skeleton, especially for patients with facial asymmetry. However, few reports are available about how the rotational axis of the mandible can be precisely defined during virtual 3D orthognathic surgery to move the mandible to the intercuspal position, although inaccurate rotation of the mandible can profoundly affect the 3D cephalometry and orthognathic surgical outcomes. Therefore, precise movement of the mandible to the intercuspal position would result in an actual mandibular-cranial and mandibular-maxillary relationship that could improve the accuracy of 3D cephalometry and would be helpful in the diagnosis and treatment plan design. The purpose of the present technical report is to offer a new method for accurately moving the mandible from the mouth opening back to the intercuspal position by integrating the preoperative plaster models.
