Status of Tobacco Control in Gestational Households and the Influencing Factors

Abstract:Background Women of childbearing age in China are suffering from serious harm of passive smoking due to the high smoking rate of males.As the two -child policy was released in China , tobacco control survey in gestational households has increasingly more significance.Objective To investigate the smoking status and cessation of members in gestational households and analyze the influence of cognition about the harm of smoking on tobacco control behaviour , in order to provide new ideas for tobacco control strategies in gestational households.Methods Enrolled pregnant women who received antenatal examination , pregnant and lying -in women who were hospitalized and their spouses in Shanghai Jiading Maternal and Child Care Service Centre from July to October 2014.Self-designed questionnaires were used to conduct survey.Pregnant women and their spouses were asked to finish two versions of questionnaires respectively , by which basic information and their cognition on tobacco control were investigated.Results A total of 942 questionnaires were distributed , and 926 valid questionnaires were recovered, with an effective returning rate of 98.3%.The average age of enrolled pregnant women was (26.5 ±4.0) years old, and the average age of their spouses was (28.1 ±4.5) years old.The positive smoking rate of pregnant women was 2.8% (26/926) before pregnancy and 1.4% (13/926) during pregnancy, and the difference was significant (χ2 =291.71, P<0.05) with a tobacco control rate of 50.0% ( 13/26 ) .The positive smoking rate of the spouses was 30.7% ( 284/926 ) before pregnancy and 27.6% ( 256/926 ) during pregnancy , and the difference was significant (χ2 =680.24, P <0.05 ) with a tobacco control rate of 9.9% (28/284) .The positive smoking rate of the inmates of pregnant women was 18.7% (76/407) before pregnancy and 16.0% (65/407) during pregnancy, and the difference was significant (χ2 =269.77, P<0.05) with a tobacco control rate of 14.5% ( 11/76 ) .The smoking rate of all respondent households was 34.2% ( 317/926 ) before pregnancy and 30.1% ( 279/926 ) during pregnancy , and the difference was significant (χ2 =672.83, P <0.05 ) with a tobacco control rate of 12.0% ( 38/317 ) .The tobacco control rate of smoking households was 63.1% ( 200/317 ) , and the tobacco control rate among all households was 86.7% (803/926) .Single-factor analysis showed that influencing factors for the tobacco control rate of gestational households included the educational level of pregnant women , the educational level of spouses , domestic monthly income per capita , living with minors younger than 10 years old or not , nuclear family or not , the knowledge of pregnant women about passive smoking , the awareness of the harm of passive smoking to pregnant women and fetuses by pregnant women, the knowledge of pregnant women about the influence of passive smoking on health , the attitude of pregnant women toward face-to-face smokers , the perception of smoking spouses on smoking and the knowledge of smoking spouses about the harm of passive smoking to pregnant women and fetuses ( P<0.05 ) .Multivariate Logistic regression analysis showed that the rate of domestic tobacco control had regression relation with the educational level of spouses , living with minors younger than 10 years old or not , nuclear family or not , the attitude of pregnant women towards face -to-face smokers and the knowledge of pregnant women about the influence of passive smoking on health ( P <0.05 ) .Conclusion The tobacco control rate in gestational households is relatively high , while the tobacco control rate of smoking family members of pregnant women is low.The cognition of pregnant and lying -in women have significant influence on domestic tobacco control , thus improving the pregnant women′s cognition on tobacco control may help increase the tobacco control rate of gestational households .
