Prognostic Value of QTc Interval and Plasma Level of Brain Natriuretic Peptide in Patients with Moderate to Severe Heart Failure

Abstract:Objective To analyze the prognostic impact of QTc interval duration and plasma level of brain natriureticpeptide (BNP) in patients with moderate to severe congestive heart failure. Methods QTc intervals and plasma levels of BNP were measured in 300 patients with heart failure (NYHA function class III~IV) at the time of evaluation. Those patients were followed over a period of 6 months for death end points. Results During 6 month follow-up, 63 patients died, including 30 sudden cardiac deaths, 27 pump failure deaths and 6 noncardiac deaths. Prolonged QTc intervals and elevated plasma BNP levels were significantly greater in sudden cardiac deaths and pump failure deaths than those survivors (P0.05). BNP level was significantly higher in pump failure deaths than that in sudden cardiac deaths (P0.05). Univariate and multivariate analysis showed the prolonged QTc interval(440 ms)and elevated BNP level (330 pg/mL) were the two independent predictors of all-cause death and pump-failure death in patients with moderate to severe congestive heart failure (P0.001); meanwhile, prolonged QTc interval was also an independent predictor of sudden cardiac death (P0.001). Conclusion Prolonged QTc interval and elevated BNP level are the two independent predictors of the adverse outcome in patients with moderate to severe heart failure.
