Prognostic value of N-terminal B-type natriuretic peptide in patients with chronic heart failure

Abstract:Objective This study was prospectively designed to investigate the prognostic factors for chronic heart failure (CHF) and examine the prognostic ability of N-terminal B-type natriuretic peptide (NT-proBNP) in patients with CHF.Methods We prospectively studies 97 consecutive patients hospitalized for CHF.Serum concentration of cTnT, cTnI and NT-proBNP as well as left ventricular ejection fraction (LVEF) on admission were measured.Results During a median follow-up period of 423 (43 ~ 505) days, the endpoint of recurrence for cardiac events was reached in 23 (23.7 %) patients.Patients with endpoint events were older (83 vs.73,P=0.000), in a higher functional class (3 vs.2,P=0.002) , had higher levels of NT-proBNP (973 pg/ml vs.212.35 pg/ml,P=0.002), cTnT(0 vs.0,P=0.011)and cTnI(0.04 vs.0.02,P=0.038) compared with those without endpoint events.On a Cox proportional hazards analysis, including these biochemical markers, age, functional class and LVEF, age and NT-proBNP were found to be the independent predictors of cardiac events.OR were 1.17(95%CI:1.09~1.24,P<0.001)and 3.17 (95%CI:1.76~5.69,P<0.001), respectively.NT-proBNP>302.1 pg/ml on admission were associated with increased cardiac events (HR=4.63,95%CI:1.80~9.46,P=0.000 8) and age > 74 years were associated with increased cardiac events too(HR = 5.40,95%CI:2.16~11.52,P=0.0002).NYHA classification, LVEF, cTnT?cTnI and CK-MB were not independently predictive of cardiac events.Conclusions Measurement of NT-proBNP in patients with CHF can help to identify patients at higher risk for cardiac events .
