Clinical Significance of the Relationship Between Upper and Lower Cervical Alignments

Abstract:Objective To measure the alignments and study the relationship between the upper and lower cervical spines.Methods The lateral X rays of cervical spine were obtained to measure the relevant parameters on CR machine with the Pacscout software, including the cervical curve dimension,Oc-C_1 angle,C_1-C_2 angle,C_2-C_7 angle and C_1-C_7 angle.Cases were classified into two groups according to the cervical curve dimension.The abnormal group, whose cervical curve dimension was less than 7 mm, consisted of 109 cases (38 males and 71 females, excluding those with cervical trauma or deformity, aged 44.3±13.7 years). The normal group, with the cervical curve dimension measuring at least 7 mm, consisted of 41 cases (25 males and 16 females, excluding cases of cervical diseases,trauma or deformity, aged 35.5 ±13.3 years). Statistical analysis was performed using a statistical package SAS 6.12.Results Comparative study showed significant difference in C_2-C_7 angle and C_1-C_7 angle (P0.01), but no difference in Oc-C_1 or C_1-C_2 angle (P0.05).In the abnormal group, there was a negative linear correlation between C_1-C_2 angle and C_2-C_7 angle(r=-0.32348,P=0.0391).Conclusions The changes of cervical alignment mainly rely on the change of C_2-C_7 angle, while no significant changes are found in Oc-C_1 and C_1-C_2 angles. There is a negative linear correlation between C_1-C_2 angle and C_2-C_7 angle when the sagittal cervical curve is abnormal. C_1-C_2 angulation should be kept in consideration when performing atlantoaxial fusion.
