Abstract:Objective To investigate the impact of left ejection fraction( LVEF) on the long-term prognosis in patients with coronary artery disease( CAD) who undergoing percutaneous coronary intervention( PCI) , and evaluate the predictive ability of LVEF combined with GRACE scores for long-term prognosis. Methods OPT-CAD registry is a prospective,large-scale,multicenter,registered clinical study. In this analysis,a total of 8347 patients undergoing PCI were selected from OPT-CAD registry,including normal LVEF group (LVEF>50%,n=7444) and reduced LVEF group(LVEF<50%,n=903). The primary endpoint was the patient-oriented composite endpoint(PoCE) 24 months after PCI including all-cause death,myocardial infarction,ischemic stroke and/or any revascularization. Secondary endpoint for PCI patients with postoperative ischemic events in the 24 months( including cardiac death,myocardial infarction and/or ischemic stroke of the composite endpoint) and independent components,all cause of death,ischemia driven reascularization and bleeding on the basis of academic research federation definition ( BARC ) major bleeding events ( BARC 3-5 ) . Results (1) Before propensity score matching:compared with the normal LVEF group,24 months PoCE events in the reduced LVEF group increased significantly,and the difference was statistically significant(P <0. 05). In the secondary endpoint study,compared with the normal LVEF group,the risk of ischemic events and all-cause death,cardiac death and myocardial infarction in the reduced LVEF group were significantly increased(P<0. 05). The stroke,ischemic driven reascularization and major bleeding events compared between the two groups,there was no statistically significant difference(P>0. 05). (2)After propensity score matching:the incidence of 24 months PoCE events in the reduced LVEF group was higher than that in the normal LVEF group(P<0. 05). In the secondary endpoint study,compared with the normal LVEF group,the risk of ischemic events,all-cause death,cardiac death and myocardial infarction 24 hours after surgery significantly increased in the reduced LVEF group(P<0. 05). There was no statistically significant difference between the two groups in the incidence of major bleeding events,stroke or ischemic driven revascularization(P>0. 05). Conclusion The CAD patients combined with reduced LVEF after PCI has a worse prognosis due to the evidently higher risk of all-cause death,cardiac death and myocardial infarction. The optimal long-standing efforts to improve outcomes in patients with reduced LVEF required more studies to investigate.
