Privention of Preoperative Complications in Thyoid Operation

Abstract:Objective To discuss the peroperative complications in thyroidectomy as well as their associated factors in order to reduce their incidence. Methods Clinical data of 350 cases of thyroid nodal underwent surgical treatment from Jan. 1997 to Dec. 2004 were retrospectively analyzed. Incidence of the chief surgical complications and their associated clinical factors were analyzed and surgical techniques were detailed. Results Postoperative paralysis of the recurrent laryngeal nerve(RLN) happened in 14 cases out of all 350 cases. Incidence of postoperative paralysis of the RLN was more frequent in RLN exposed group (7.3%) than in RLN non-exposed group(1.5%),P=0.011, and more frequent in malignant group (9.4%) than in benign group(3.0%),P=0.045. Postoperative bleeding happened in 7 cases, more frequent in the group with preoperative complications(7.4%) than in the group whthout peroperatvie complications(0.7%),P=0.004. Transient hypopathyroidism happened in 4 cases, all of which underwent bilateral subtotal thyroidectomy. Conclusions Exposing of the RLN facilitates reducing incidence of postoperative paralysis of RLN. Blood pressure,blood sugar and serum level of T3,T4 should be well controlled before operation. Subtly dissection of the dorsal capsule of the thypoid and preservation of the suspected parathyroid tissue should be emphasized in bilateral thyroidectomy.
