Exposure of recurrent laryngeal nerve and prevention of RLN injury during operation in patients with thyroid cancer

Abstract:Background and purpose:Recurrent laryngeal nerve (RLN) injury is one of the most serious complications of thyroid surgery, especially in operations for patients with thyroid carcinoma. The RLN injury and prevention have been highlighted for thyroid surgery. The main purpose of this article was to discuss the exposure of RLN and prevention of RLN injury in patients with thyroid cancer. Methods:We retrospectively analyzed 282 cases with thyroid cancer treated during January 2002 to July 2006. Results:Of all the 282 cases, RLN injury were observed in 4 patients with preoperative injury, 9 involved in cancer during operation, respectively. 505 branches of 282 cases (505/564, 89.5%) could be found in the area of 2-3 cm outside below the arch of the cricoid cartilage near sulcus trachea-oesophagus. There was no exposure in whole range of recurrent laryngeal nerves in all cases before cervical lymph node dissection. There was no treatment related mortality, hematoma occurred in 1 patient, with chyle leak in another 1 patient. No patient experienced preoperative hoarseness. After surgery, transitory hoarseness was observed in 9 patients, permanent hoarseness in 2 cases, unilateral RLN resection performed with, laryngectomy and tracheotomy in 1 case with cancer involving RLN and larynx, nerve decompression and anastomosis in 1 case. Conclusion:RLN is easy to be exposed in the area of 2-3 cm outside below the arch of the cricoid cartilage near sulcus trachea-oesophagus. If cervical lymph node dissection is necessary, the whole range of recurrent laryneal nerve should be exposed from oesophagus groove below cricoid cartilage after thyroid excision which would prevent the RLN injury effectively. Once RLN injury occurs, repair should be performed as soon as possible.
