Abstract:Racialminorities on hemodialysis in theUnited States have lower mortality rates than nonminority populations (1). Multiple observational studies have suggested biological differences among the races to account for such discrepancies in health outcomes (2–4). InHispanic populations, this apparent survival advantage has been labeled the ‘‘Hispanic paradox’’ (5). Despite striking socioeconomic disparities compared to whites, Hispanics in the United States tend to have better health and survival outcomes (5). The ‘‘healthy migrant’’ effect has been proposed as one possible explanation of this ‘‘paradox,’’ namely the healthiest and strongest people immigrate to the United States resulting in an apparent survival advantage of Hispanic patients compared to whites (5). A role for nutritional status has also been proposed, with some investigators suggesting that higher muscle mass and serum albumin levels in Hispanics and blacks account for their better outcomes (5,6). In addition, dialysis tends to be initiated at a higher glomerular filtration rate (GFR) in black populations due at least in part to higher serum creatinine levels at any given GFR, potentially making them ‘‘healthier’’ at the time dialysis is initiated (1). Pharmacologic replacement of vitaminD and vitamin D analogs is increasing seen as more than a therapeutic tool to correct low vitaminD levels, reduced parathyroid hormone (PTH) levels and manage metabolic bone disease in hemodialysis patients (7). Vitamin D deficiency has been associated with osteomalacia, rickets, cardiovascular disease, infection, malignancy, myopathy, and death (8,9). The benefits of vitamin D supplementation are not all known, but the vitamin D receptor is present in several cell types and activation of this receptor has been shown to havewidespread effects (5,9,10). Current interest in nontraditional effects of vitamin D and related analogs was greatly stimulated by a historical cohort study published in 2003 that showed that chronic dialysis patients treated with paricalcitol had a significant survival advantage over those treated with calcitriol (8). This study was later criticized for using data from patients restricted to ‘‘for-profit’’ dialysis centers and for excluding patients who received doxercalciferol (11), but a subsequent study (12) by the same group found that chronic hemodialysis patients who received any form of injectable vitamin D had a survival advantage over similar patients who do not receive vitamin D. Mortality in paricalcitoland doxercalciferol-treated dialysis patients appears to be virtually identical (11). Most recently, in the paper prompting this Summary and Commentary, Wolf et al. (13) examined the role that vitamin D may play in the commonly reported observation that blacks and Hispanics have longer survival on dialysis than whites. They hypothesized that greater use of activated vitamin D in blacks and Hispanics might be a factor contributing to their survival advantage over white patients. In addition, blacks and Hispanics not treated with activated vitamin D were hypothesized to have worse survival outcomes than whites. This prospective cohort study collected data on 9303 incident hemodialysis patients from 1056 Fresenius Medical Care, North America dialysis units between July 1, 2004 and June 30, 2005. The cohort included 5110 whites (55%), 979 Hispanics (11%), and 3214 blacks (35%). Demographic characteristics of the three groups were well matched with the exception that blacks were younger, heavier, and more often female than whites. Hypertension was more commonly the cause of end-stage renal disease (ESRD) among blacks and less so amongHispanics, compared to whites, while diabetes was much more commonly identified as the cause of ESRD among Hispanics. Cardiovascular and other comorbid conditions were significantly less prevalent among black and Hispanic patients compared to whites. Serum creatinine and PTH levels at baseline were lowest among whites and highest among blacks, with intermediate levels among Hispanics. In a subset of patients Address correspondence to: Jeffrey S.
