Total intravenous anesthesia composed of propofol, dexmedetomidine and lidocaine as an alternative for laparoscopic cholecystectomy

Abstract:Objective To explore the feasibility of total intravenous anesthesia (TIVA) composed of propofol,dexmedetomidine and lidocaine as an alternative to opioids for laparoscopic cholecystectomy.Methods Sixty patients,American Society of Anesthesiologists (ASA) Ⅰ or Ⅱ grade,scheduled forlaparoscopic cholecystectomy were randomly divided into two groups (30 cases in each group).They received either dexmedetomidine,lidocaine,and propofol infusions (group L) or opioid-based anesthesia with fentanyl,remifentanil,and propofol infusions (group R).Noninvasive blood pressure and heart rate were recorded at the time points of entering operating room (T0),before placing laryngeal mask (T1),placing laryngeal mask (T2),5 minutes after placing laryngeal mask (T3),establishing pneumoperitoneum (T4),and 5 minutes after establishing pneumoperitoneum (T5).Patients needed ephedrine when mean arterial pressure (MAP) < 60 mmHg (1 mmHg=0.133 kPa),needed nicardipine when MAP> 120 mmHg and needed atropine when heart rate< 45 beats per minute.The time of extubation and orientation,visual analogue scale (VAS) score 30 minutes after extubation,cases of receiving flurbiprofen 6 hours after surgery (VAS>5) and incidence of postoperative nausea and vomiting (PONV) were recorded.Results There were no statistical differences in MAP or heart rate between two groups at T0 (both P>0.05).MAP and heart rate in group L were significantly higher than those of group R at time points of T1 to T5 (all P<0.05).More patients used ephedrine and nicardipine in group L than in group R (both P<0.05).There was no significant difference in operation time between groups (P>0.05).The time ofextubation and orientation in group L were significantly longer than those in group R (both P<0.05).The VAS score in group L was significantly lower than that in group R at 30 minutes after extubation (P <0.05).Flurbiprofen was used in 2 patients of group L and in 9 patients of group R.PONV occurred in one patient of group L and in 7 patients of group R.There were significant differences in application of flurbiprofen and incidence of PONV (both P<0.05).Conclusion The combination of propofol,dexmedetomidine and lidocaine may be an alternative anesthesia for laparoscopic cholecystectomy It can achieve better postoperative analgesics and lower incidence of PONV compared with opioids.
