Combined modality therapy strategies for colon cancer liver metastasis

Abstract:Colorectal cancer ( colorectal cancer ,CRC) is one of the world's commonest gastrointestinal cancer,about 50%of patients develop liver metastases eventually .For patients with liver metastases ,if liver metastases are not treated,the median survival time would be 6.9 months,while the 5-year survival rate would be 0.If the liver metastases are treated with radical surgery ,the median survival time would be 35 months,while 5-year survival rate would be 30-50%. Surgical resection is still the preferred treatment for patients with liver metastases .For patients with liver metastases , if there are no intestinal obstruction , perforation,bleeding or other symptoms,the author tends to perform simultaneous resection of the primary tumor and metastases, surgery combined with ultrasound , microwave, RFA, or other new technologies to improve the resection rate and reduce the relapse .But only a small number of patients ( 10%-15%) can be treated surgically .For unresectable liver metastases ,we need the multi-disciplinary group of experts to make detailed,individualized and comprehensive treatment ,which includes neoadjuvant chemotherapy ,combining targeted drug therapy , portal vein embolization applications , topical treatment , radiotherapy and so on , to make some unresectable lesions resectable .As cancer is a systemic disease ,It is difficult to be cured with one single treatment we need the cooperation of multidisciplinary to to find the best treatment for a certain period of the disease .Meanwhile Follow-up, timely detection of recurrence and metastasis,and further treatment plans for long-term survival and improved quality of life of patients can not be ignored .
