The clinical value of MRI in diagnosing high-grade from low-grade chondrosarcoma in long bones

Abstract:Objective To explore the image characteristics of high-grade and low-grade chondrosarcoma, and to improve the accuracy of preoperative classification diagnosis of the central chondrosarcoma in long bones. Methods Analysis of 52 cases with long bone central chondrosarcoma were confirmed by pathology. Thirty cases were divided into low-grade group and 22 cases into high-grade group. Respectively, the location of the tumor, longest diameter, bone expansion, cortical destruction, cortical thickening, periosteal reaction or active periostitis, endosteal scalloping lesion, marrow edema, soft tissue edema, soft tissue mass, cartilage collapse, entrapped fat within the tumor, calcification in the tumor, annular strengthening etc. were recorded for statistical analysis to find out the most valuable differential imaging signs and optimal diagnostic mode. Results Chondrosarcoma of the long bone most commonly occurred in the femur ( 36 cases, 63.7％ ). When the tumor was larger than 8.25 cm, the sensitivity of the diagnosis of high-grade chondrosarcoma was 82％ and the specificity was 80％. Statistical significance existed in bone expansion, cortical destruction, periosteal reaction or active periostitis, endosteal scalloping lesion, marrow edema, soft tissue edema, soft tissue mass, cartilage collapse, entrapped fat within the tumor, annular strengthening etc. ( P ) The accuracy of prediction diagnosis of high-grade chondrosarcoma was 89.4％ by analyzing periosteum reaction, soft tissue edema and cartilage collapse. Conclusions Chondrosarcoma of long bones would be of high-grade when signs of periosteal reaction and soft tissue edema are positive, while cartilage collapse negative.
