Imaging Findings of Primary Chondrosarcoma Correlated with Pathology Findings

Abstract:Objective:To study the CT and MRI features (without and with contrast enhancement) of primary chondrosarcoma and correlated with their pathology basis.Methods:27 patients with surgery and pathology proved primary chondrosarcoma underwent radiography,spiral CT with and without contrast administration and MRI were performed before surgery.The MRI sequences included SE T1WI,FSE T2WI and SE T1WI after contrast enhancement.The imaging materials were reviewed and analyzed retrospectively and correlated with surgical and pathological results.Results:Of the 27 patients,the pathology classification included conventional chondrosarcoma (n=22),mesenchymal chondrosarcoma (n=3) and dedifferentiated chondrosarcoma (n=2).The location of tumor included pelvis (n=11),long bone (n=10),rib (n=2),sternum,vertebral body,soft tissue and retroperitoneum (n=1 each).The diameter of the tumour ranged from 4.5~23.0cm,with 2 cases 5.0cm,17 cases 5.0~10.0cm and 8 cases 10.0cm.The CT value was similar to or lower than that of muscle.Mild expansive or osteolytic bone destruction was showed.Tumors had scallop-like contour (n=19),there were intra-tumoral calcification (n=21),and periosteal reaction (n=3).Tumors appeared iso-to hypo-intense on MR T1WI and marked hyper-intense on T2WI with ring-and arc-like hypo-intense septa and nodule within the hyper-intense tumor mass.13 patients had enhanced CT and MRI,,continuous and progress enhancement could be assessed in the intra-tumoral septa,yet no enhancement was seen in the intra-tumoral lobules.Conclusion:Mass with scallop margin,ring and arc-like calcifications,marked hyper-intense on T2WI with ring-and arc-shape hypo-intensities within were the imaging characteristics of various types of primary chondrosarcoma,which are important criterias in the diagnosis of the disease.
