Abstract:Impulsive-compulsive behaviors (ICBs), one of the non-motor symptoms of Parkinson′s disease (PD), may be attributed to dopaminergic dysfunction and dopamine replacement therapy. At present, understanding of ICBs, mechanisms and exploration of treatment largely depend on neuroimaging research. Despite contradictions among the findings, most evidence indicated that PD patients with ICBs showed decreased thickness in frontal and temporal cortex and the volume of subcortical nuclei when compared to those without ICBs. And the occurrence of ICBs is associated with relatively preserved white-matter integrity in the brain regions involved in reward-related behaviors. This article will give an overview of the magnetic resonance research of PD related ICBs including cortical thickness, diffusion tensor imaging, resting state and task state functional magnetic resonance imaging analysis, which lays a foundation for further imaging studies, revealing the pathogenesis and exploring the potential treatment options.

Abstract:Parkinson's disease(PD) results mainly from progressive degeneration of dopaminergic neurons in the substantia nigra,and the nigrostriatal dysfunction triggers functional and structural change within the human motor system through the neural network of the brain.As a functional imaging technique,functional magnetic resonance imaging can be applied to detect these subtle changes in early PD and even subclinical disease in genetic forms of PD.These modalities hold promise to study the preclinical phase of PD and may provide assistance for the early diagnosis.

Abstract:Parkinson's disease (PD) is a common neurodegenerative disorder. It has a delitescent onset and a slow progress. The clinical manifestations of PD in patients are highly heterogeneous. Thus, PD diagnosis process is complex and mainly depends on the professional knowledge and experience of the physician. Magnetic resonance imaging (MRI) could detect the small changes in the brain of PD patients, and quantitative analysis of brain MRI may improve the clinical diagnosis efficiency. However, due to the complexity of clinical courses in PD and the high dimensionality in multimodal MRI data, traditional mathematical analysis could not effectively extract the huge information in them. Up to now, the accuracy of PD diagnosis in large sample size is still unsatisfying. As artificial intelligence (AI) is becoming more mature, varieties of statistical models and machine learning (ML) algorithms have been used for quantitative imaging data analysis to explore a diagnostic result. This review aims to state an overview of existing research recently that used statistical ML/AI methods to perform quantitative analysis of MR image data for the study of PD diagnosis. First we review the recent research in three subareas: diagnosis, differential diagnosis, and subtyping of PD. Then we described the overall workflow from MR image to classification result. Finally, we summarized a critical assessment of the current research and provide some recommendations for likely future research developments and trends.

Abstract:Background: Parkinson's disease (PD) is a neurodegenerative disease with high incidence rate. Resting state functional magnetic resonance imaging (rs-fMRI), as a widely used method for studying neurodegenerative diseases, has not yet been combined with two important indicators, amplitude low-frequency fluctuation (ALFF) and cerebral blood flow (CBF), for standardized analysis of PD. Methods: In this study, we used seed-based d-mapping and permutation of subject images (SDM-PSI) software to investigate the changes in ALFF and CBF of PD patients. After obtaining the regions of PD with changes in ALFF or CBF, we conducted a multimodal analysis to identify brain regions where ALFF and CBF changed together or could not synchronize. Results: The final study included 31 eligible trials with 37 data sets. The main analysis results showed that the ALFF of the left striatum and left anterior thalamic projection decreased in PD patients, while the CBF of the right superior frontal gyrus decreased. However, the results of multimodal analysis suggested that there were no statistically significant brain regions. In addition, the decrease of ALFF in the left striatum and the decrease of CBF in the right superior frontal gyrus was correlated with the decrease in clinical cognitive scores. Conclusion: PD patients had a series of spontaneous brain activity abnormalities, mainly involving brain regions related to the striatum-thalamic-cortex circuit, and related to the clinical manifestations of PD. Abstract:AIM: To explore the characteristics of motor evoked potential(MEP) and cortical excitability in Parkinson's disease by transcranial magnetic stimulation(TMS).METHODS: 20 normal volunteers were enrolled as control(group A),with the average age of(62.3±16.7) year old ranged from 52 to 79 years including 12 males and 8 females.MEP were assessed and compared separately between left and right hemispheres.21 patients with Parkinson's disease(PD) in asymmetry symptoms were performed in this study(group B),with the average age of(63.3±16.7) year old ranged from 46 to 80 years including 12 males and 9 females.UPDRS motor scores(UPDRSⅢ) were assessed before the check of cortical excitability(MEP).Motor evoked potentials(MEP) was assessed and compared separately on the minor and the serious bodyside,compared to control group.RESULTS: The indexes of MEP from both sides of normal control had no significant diversity.Amp was increased and both rest motor threshold(RMT) and cortical latency(CL) were also decreased significantly in PD patient compared to the normal control.On the serious side of PD,RMT decreased significantly compared to the minor side.Amp,CL and central motor conduct time(CMCT) changed without significant diversity between minor and serious bodyside.CONCLUSION: The motor cortical excitability is increased in PD patients.The excitability is higher with the more serious symptom side.The character of MEP in PD patients may be helpful to make a diagnosis and evaluate the severity of symptoms.
Clinical and MRI analysis of progressive supranuclear palsy：a report of 12 cases

Abstract:Objective To investigate the clinical and imaging features of progressive supranuclear paralysis (PSP). Methods 12 cases of PSP patients diagnosed clinical y were retrospectively analyzed based on clinical and imaging data. Re-sults The main symptoms of 12 PSP patients were bradykinesia, postural instability, pseudobulbar palsy and vertical ophthal-moplegia performance. Partial patients showed cognitive dysfunction and urinary disorders. The initial symptom of most patients was bradykinesia. Different levels of brain atrophy were observed in the cranial MRI of 12 patients, especial y in the frontal lobe and midbrain. The change of "hummingbird sign" and "morning glory sign"was observed in some of patients. Conclusion Bradykinesia was the most common initial symptom for PSP patients. Postural instability and easy to fal were the basic clinical manifestations. Vertical ophthalmoplegia indicated the greatest value of diagnosis Brain MRI assissed the PSP diagnosis.

Abstract:Hemiparkinsonism duration in patients with Parkinson's disease (PD) is a key time window to study early pathology of PD. We aimed to comprehensively explore the alterations of deformation and structural network in PD patients with hemiparkinsonism, which could potentially disclose the early biomarker for PD. Thirty-one PD patients with hemiparkinsonism and 37 age- and gender- matched normal controls were included in the present study. First of all, we normalized the left hemisphere of structural images as the contralateral side to the affected limbs. Deformation-based morphometry (DBM) was conducted to evaluate the brain atrophy and/or enlargement. structural networks were constructed by thresholding gray matter volume correlation matrices of 116 regions and analyzed using graph theoretical approaches (e.g., small-worldness, global, and nodal measures). Significantly decreased deformation values were observed in the temporoparietal regions like bilateral middle temporal gyri, ipsilateral precuneus and contralateral Rolandic operculum extending to supramarginal and postcentral gyri. Lower deformation values in contralateral middle temporal gyrus were negatively correlated with higher motor impairment which was dominated by akinesia/rigidity. Moreover, nodal reorganization of structural network mainly located in frontal, temporal, subcortex and cerebellum was bilaterally explored in PD patients with hemiparkinsonism. Increased nodal properties could be commonly observed in frontal lobes. Disruption of subcortex including basal ganglia and amygdala was detected by nodal local efficiency and nodal clustering coefficient. Twelve hubs, mainly from paralimbic-limbic and heteromodal networks, were disrupted and, alternatively, 14 hubs, most of which were located in frontal lobes, were additionally detected in PD patients with hemiparkinsonism. In conclusion, during hemiparkinsonism period, mild brain atrophy in the temporoparietal regions and widespread reorganization of structural network, e.g., enhanced frontal function and disruption of basal ganglia nodes, occurred in both hemispheres. With our data, we can also argue that MTG contralateral to the affected limbs (expressing clinically verified brain atrophy) might be a potential living biomarker to monitor disease progression. Therefore, the combination of DBM and structural network analyses can provide a comprehensive and sensitive evaluation for potential pathogenesis of early PD patients with hemiparkinsonism.

Abstract:Parkinson's disease (PD) pathology may damage emotion circuit and cause depression. We investigated whether the neural basis of depressive symptoms varies at different PD stages. Seventy-six healthy controls (HC) and 98 PD patients (divided into early and middle stage groups) underwent brain magnetic resonance imaging (MRI) and general neuropsychological tests. Voxel-based morphometry and tract-based analysis were used to study the association between brain structural alterations and the Hamilton Depression Scale 17 Item (HAMD-17) scores in different groups. Comparing with HC group, PD patients showed widespread brain alterations in both gray and white matter. The HAMD-17 scores were positively correlated with GM volume in the right pre-central gyrus of early PD patients. In the middle stage group, HAMD-17 scores were positively correlated with GM volume in midbrain and right superior temporal gyrus, and negatively associated with GM volume in left anterior cingulate and superior frontal gyrus. In white matter analysis, The HAMD-17 scores were positively correlated with fractional anisotropy value of the bilateral inferior fronto-occipital fasciculus in the early stage group, but not the middle stage group. We concluded that the neural basis of depressive symptoms might be distinct in different stages of PD, implying the need for differential treatments.
Wider and Faster Degeneration of White Matter in Parkinson’s Disease with Possible REM Sleep Behaviour Disorder

Abstract:Background In Parkinson's disease (PD), rapid eye movement (REM) sleep behaviour disorder (RBD) signifies a poorer prognosis, yet its impact on white matter (WM) degeneration remains unclear. The study examined the effect of RBD on WM alterations in PD progression. Methods The study included 45 PD patients with possible RBD (PD-pRBD), 38 PD patients without possible RBD (PD-npRBD), and 79 healthy controls (HC). All patients underwent clinical assessments and diffusion MRI scans at least once a year for up to 4 visits. 79 HC underwent the same protocol at baseline. WM metrics, including fractional anisotropy (FA), mean diffusivity (MD), radial diffusivity (RD), and axial diffusivity (AD), were calculated using tract-based spatial statistics. Linear mixed-effects models were conducted to examine the changes in clinical features and WM fibers. Results At baseline, PD-npRBD showed increased RD in several regions, predominantly in bilateral uncinate fasciculus (UF) and inferior longitudinal fasciculus (ILF), compared to HC (PFDR<0.05). During follow-up, PD-npRBD had further FA decrease in left UF and ILF (PFDR<0.05). PD-pRBD showed reduced FA in several regions relative to HC at baseline (PFDR<0.05), and faster FA decline in left UF and ILF than PD-npRBD during follow-up, with more extensive FA decrease in other regions such as anterior thalamic radiation and inferior fronto-occipital fasciculus (PFDR<0.05). Moreover, increased RD in the left corticospinal tract correlated with motor symptoms (p=0.045) in PD-pRBD. Conclusions PD patients with pRBD demonstrated more extensive WM degeneration and accelerated degeneration in the left ILF and UF during the disease course. However, due to the lack of PSG verification, these results should be interpreted cautiously while directly relating to RBD. These findings provide new insights into the neural structural basis associated with the potential impact of RBD on PD progression.

Abstract:Parkinson's disease (PD) is a common disorder, and the diagnosis of Parkinson's disease is clinical and relies on the presence of characteristic motor symptoms. The accuracy of the clinical diagnosis of PD is still limited. Functional neuroimaging using SPECT technique is helpful in patients with first signs of parkinsonism. The changes detected may reflect the disease process itself and/or compensatory responses to the disease, or they may arise in association with disease- and/or treatment-related complications. This paper addresses the value of SPECT in early differential diagnosis of PD and its potential as a sensitive tool to assess the pathophysiology and progression, as well as the therapeutic efficacy of PD.

Abstract:Parkinson’s disease (PD) exhibits heterogeneity in terms of symptoms and prognosis, likely due to diverse neuroanatomical alterations. This study employs a contrastive deep learning approach to analyze Magnetic Resonance Imaging (MRI) data from 932 PD patients and 366 controls, aiming to disentangle PD-specific neuroanatomical alterations. The results reveal that these neuroanatomical alterations in PD are correlated with individual differences in dopamine transporter binding deficit, neurodegeneration biomarkers, and clinical severity and progression. The correlation with clinical severity is verified in an external cohort. Notably, certain proteins in the cerebrospinal fluid are strongly associated with PD-specific features, particularly those involved in the immune function. The most notable neuroanatomical alterations are observed in both subcortical and temporal regions. Our findings provide deeper insights into the patterns of brain atrophy in PD and potential underlying molecular mechanisms, paving the way for earlier patient stratification and the development of treatments to slow down neurodegeneration. A contrastive deep learning analysis of MRI data in this study revealed PD-specific brain alterations, primarily in subcortical and temporal regions, which correlate with biomarkers, disease severity, and immunerelated cerebrospinal fluid proteins.

Abstract:Parkinson’s disease (PD), characterized with bradykinesia, static tremor, rigidity and disturbances in balance, is the second most common neurodegenerative disorder. Along with the largely aging population in the world, the incidence is increasing year by year, which imposes the negative impacts on patients, their families and the whole society. Traditional Chinese medicine (TCM) has a good prospect for the prevention and cure of PD due to its advantages of less side effects and multi-targets effects. At present, the pathogenesis of PD is not yet fully discovered. This paper elaborate the mechanism of TCM underlying the prevention and treatment of PD with regards to the inhibition of oxidative stress, the regulation of mitochondrial dysfunction, the reduction of toxic excitatory amino acids (EAA), the inhibition of neuroinflammation, the inhibition of neuronal apoptosis, and the inhibition of abnormal protein aggregation.

Abstract:Cognitive impairment (CI) ranging from mild cognitive impairment (MCI) to dementia is a common and disturbing complication in patients with Parkinson’s disease (PD). Numerous studies have focused on neuropathological mechanisms underlying CI in PD, along with the identification of specific biomarkers for CI. Magnetic resonance imaging (MRI), a promising method, has been adopted to examine the changes in the brain and identify the candidate biomarkers associated with CI. In this review, we have summarized the potential biomarkers for CI in PD which have been identified through multi-modal MRI studies. Structural MRI technology is widely used in biomarker research. Specific patterns of gray matter atrophy are promising predictors of the evolution of CI in patients with PD. Moreover, other MRI techniques, such as MRI related to small-vessel disease, neuromelanin-sensitive MRI, quantitative susceptibility mapping, MR diffusion imaging, MRI related to cerebrovascular abnormality, resting-state functional MRI, and proton magnetic resonance spectroscopy, can provide imaging features with a good degree of prediction for CI. In the future, novel combined biomarkers should be developed using the recognized analysis tools and predictive algorithms in both cross-sectional and longitudinal studies.

Study of the effect of ischemic brain lesions on motor impairment in Parkinson's disease

Abstract:Objective: To investigate the effect of ischemic brain lesions on motor impairment in Parkinson's disease. Methods: strict criteria for PD was used,including no history of acute stroke and transient ischemic attack(TIA),absence of abnormal cerebral CT finding. Three years later, patients were performed cerebral MRI scans and motor impairment assessment by Unified Parkinson's Disease Rating Scale (UPDRS). Results: The incidence of silent cerebral infarction (SCI) and white matter lesions(WMLs) in the PD patients with hypertension and diabetes is significantly higher than the patients without these accompanying disorders (P<0. 05),and the motor impairment in patients with SCI and WMLs is remarkably severer than the patients without SCI and WMLs (P<0. 01). Conclusions: Aggressive prvention and treatment of ischemic cerebral vascular disease may contribute in controlling,symptom severity in PD.

Abstract:Introduction: Numerous studies have been carried out to explore the potential association between neurologic deficits and variable clinical manifestations of Parkinson's disease (PD). The aim of our study was to investigate the association between cognitive performance and motor dysfunction in Chinese patients with PD.Methods: Data from 96 patients with PD were obtained from the Parkinson's disease patient cohort database of Huashan Hospital. All participants underwent a comprehensive neuropsychological evaluation to assess cognitive status, that included scoring on the Mini-mental state examination (MMSE), followed by more detailed cognitive assessment on five main cognitive domains (verbal memory, nonverbal memory, visuospatial function, language and attention/executive function). Correlations between cognitive and motor scores were investigated after controlling for age, disease duration, education, and gender.Results: We report a significant correlation between subdomains of cognitive impairment and motor dysfunction using analyses of the multiple linear regression. Notably, executive function and attention was significantly associated with bradykinesia and rigidity, while visuospatial function was associated with bradykinesia and tremor.Conclusions: The association between motor dysfunction and cognitive decline in PD might highlight deficits represented by a shared neurochemical pathway.
Changes of brain structure and structural covariance networks in Parkinson's disease associated cognitive impairment

Abstract:Background: Cognitive impairment (CI) is common in Parkinson's disease (PD). Multiple brain regions and their interactions are involved in PD associated CI. Magnetic resonance imaging (MRI) technology is a non-invasive method in investigating brain structure and inter-regional connections. In this study, by comparing cortical thickness, subcortical volume, and brain network topology properties in PD patients with and without CI, we aimed to understand the changes of brain structure and structural covariance network properties in PD associated CI. Methods: A total of 18 PD patients with CI and 33 PD patients without CI were recruited. Movement Disorder Society Unified Parkinson's Disease Rating Scale, Hoehn and Yahr stage, Mini Mental State Examination Scale, Non-motor Symptom Rating Scale, Hamilton Anxiety Scale, and Hamilton Depression Scale were assessed. All participants underwent structural 3T MRI. Cortical thickness, subcortical volume, global and nodal network topology properties were measured. Results: Compared with PD patients without CI, the volumes of white matter, thalamus and hippocampus were lower in PD patients with CI. And decreased whole-brain local efficiency is associated with CI in PD patients. While the cortical thickness and nodal network topology properties were comparable between PD patients with and without CI. Conclusion: Our findings support the alterations of brain structure and disruption of structural covariance network are involved in PD associated CI, providing a new insight into the association between graph properties and PD associated CI.
