Abstract:OBJECTIVE:To evaluate the craniofacial morphology of patients with isolated cleft palate in the mixed dentition after palatoplasty.METHODS:Twenty patients (7 to 11 years old) with isolated cleft palate in the mixed dentition were selected. The control group comprised 35 age-matched non-cleft children without orthodontic treatment. Cephalometric analysis was carried out to evaluate craniofacial morphology.RESULTS:The mean values of total cranial base length, maxillary depth, mandibular effective length in cleft patients were 86.48 mm, 44.79 mm and 65.45 mm, respectively. Those in control group were 91.27 mm, 48.84 mm and 70.49 mm, respectively (P < 0.001).CONCLUSIONS:Underdeveloped maxilla and class III skeletal profile were presented in patients with isolated cleft palate.

Abstract:Objective To observe the effect of artificial bone implantation of hard cleft palate on the development of maxilla.Methods From January 1997 to December 1999,40 patients with hard cleft palate were randomly divided into two groups:control group and implantation group(n=20 each).The patients in the implantation group received an implantation of compound artiffcial bone of HA-Bone cement.All pailents had a follow-up since 16 years old.A tllree dimensional model was established with computed tomography and rapid prototype technique to analyze the maxilla in three dimension.At the same time,a dentognathic model was employed.Results There were no differences in the results between the three dimensional and dentognathic models.No difference was found in the development of maxilla in length and height between the control and implantation groups.There were marked differences in the development of maxilla in width between two groups(67.6 mm±4.3 mm vs 61.3 1131/1±4.1 mm.63.5 mln±3.9 mm vs 57.3 mm±3.1 mm,26.2 mm±1.8 mm vs 26.4 mm±1.9 mm,all P<0.05).The width of maxilla in the implantation group was markedly wider than that in the control group.Conclusions The application of three dimensional model for evaluating the development of maxilla is both straightforward and accurate.Bone implantation of hard cleft palate is an obvious boost to the development of maxilla in width.It should be included into a comprehensive orthodontic treatment for patients with hard cleft palate.

Abstract:To research the influence of palate bone implantation to the growth of maxilla in three dimensional direction.40 patients of congenital cleft palate type II with palate board damage were stochastically divide into the comparison group and bone planted group. In bone planted group, HA-Bone cement was used to repair palate tabular damage. All patients are 16 full year old, cephalometric radiograph and tooth cast model were measured and contrast analysis was applied.Planted bone group and the comparison group does not have the significance difference in both the maxilla length and maxilla hightness( P > 0.05); But there was significance difference in the width of post-part of the maxilla (P < 0.05).It could promote the growth of maxilla width in use of bone implantation, that advocated the bone repair in the sequence treatment process in cases of the hard palate bone damage.

Abstract:In this study, we examined the synthesis, and characterization of TiO2/poly(N-maleoylglycine-co-acrylic acid) (TiO2/poly(MG-co-AA)) nanocomposite. The nanocomposite was prepared by the dispersion of TiO2 nanotubes in a water solution of the polymer (3% w/w) and then it was lyophilized. The nanocomposite was characterized by FTIR and Raman spectroscopies. The incorporation of TiO2 nanotubes to the polymeric matrix was determined by transmission electron microscopy (TEM) from which TiO2 nanotubes in the inner of copolymer with diameters ranging between 90 and 100 nm were observed. The overall morphology of the previously synthetized nanotubes was determined by scanning electron microscopy (SEM). For the nanocomposite system, the morphology was studied by atomic force microscopy (AFM) from which a grain shape structure was observed. This process resulted in a new nanocompsite material with an average grain diameter estimated by SEM and AFM ranging between 210-240 nm. It was observed that the nanotubes were homogeneously dispersed within the polymeric matrix. The prepared material could be suitable in the design of electronic devices and additionally could have potential applications as biomaterial.

Abstract:Introduction: Our objective was to compare the craniofacial hard and soft tissue characteristics between children with operated unilateral cleft lip and palate (UCLP) and children with noncleft lip and palate (non-CLP) with a Class III skeletal relationship. Methods: The study sample consisted of 30 subjects (18 boys, 12 girls; mean age, 10.21 years) affected by UCLP and 30 non-CLP subjects (17 boys, 13 girls; mean age, 10.19 years) as the control group. All subjects were in the mixed dentition with a Class III skeletal relationship. Cone-beam computed tomography-synthesized cephalograms were traced and evaluated, and craniofacial hard and soft tissue morphologies were compared between the UCLP and non-CLP groups. Results: Maxillary length and gonial angle were 2.66 mm shorter and 3.67 degrees greater, respectively, in the UCLP group than those in the non-CLP group. The SNA and SNB angles describing the sagittal positions of the maxilla and mandible, respectively, relative to the cranial base were significantly smaller in the UCLP group (P < 0.001 and P = 0.003, respectively). However, the 2 groups had similar sagittal intermaxillary relationships with similar ANB angles (P = 0.669). In the vertical dimension, the mandibular plane angle and the growth direction vector were significantly greater in the UCLP group (P = 0.007 and P < 0.001, respectively). Lastly, the UCLP group had a more concave soft tissue profile, manifested by a reduced facial convexity angle, as well as an acute nasolabial angle and a more protruded lower lip. Conclusions: Although the 2 groups had similar sagittal intermaxillary relationships, patients in the UCLP group had more retrusive maxillary and mandibular positions relative to the cranial base and more severe vertical discrepancies. Additionally, the soft tissue profiles of patients affected by UCLP were more concave, and the compensatory adaptation was less satisfactory.

Abstract:Objective:To investigate the relationship between the structure of TMJ and the craniofacial pattern,occlusion in ClassⅡ1mandibular retrusion children. Methods:TMJ MRI,lateral cephalometric were taken respectively from 30 ClassⅡ1 patients (14 boys and 16 girls,the average age was 10.8±1.1 years). The condyle position and disc position were measured. The correlation of condyle and disc position with craniofacial pattern and occlusion were analyzed metrically and compared with pearson test.Results:There were low positive correlation between MP-SN angle and anterial space. No correlation between posterior-anterior ratio and other data were found. The posterior facial height and Dp-Cc/Cs-Cc angle had low positive correlation. The anterior facial height and Dp-Cc/Cs-Cc、Ca-Ca'、Ca-Cp showed low negative correlation,and little correlation was found between condyle position and overbite and overjet. Overbite had no correlation with disc-condyle-fossa relationship. However,The positive correlation was found between the overjet. and Ca-Dm and Dp-Cc/Cs-Cc,and Pearson coefficient is 0.420 and 0.460.Conclusion:Vertical craniofacial patter has relation to forward displacement of condyle and backward displacement of disc. A deeper overjet is related with a higher risk of abnormal disc-condyle-fossa relationship.
Three-dimensional Analysis of Naso-Maxillary Complex Symmetry in Patients with Unilateral Complete Cleft Lip and Palate

Abstract:Objective To evaluate the symmetry of naso-maxillary complex in patients with unilateral complete cleft lip and palate with no palatoplasty history before 17 years old.Methods 9 patients (4 males and 5 females) with unilateral complete cleft lip and palate (UCLP) were selected.They all underwent lip repair before 2.5 years old and had no history of palatoplasty before 17 years.They received spiral CT scan and 3D reconstruction of their naso-maxillary complex.22 variables were measured in the reconstructed images and the symmetry of the complex were analyzed.Results The distances of NC-HP and PF-MSP in cleft side were 18.01 mm and 19.72 mm,which were 13.35 mm and 17.31 mm bigger than that in the noncleft side(P＜0.05).The distances of NC-CP and U3-HP in cleft side were 59.64 mm and 39.55 mm,which were 64.13 mm and 39.70 mm shorter than that in the non-cleft side (P＜0.05).The measurements showed a collapsed and sunken nasal cavity,a wider posterior hard palate and elevated canine in cleft side.There were also deviations of the middle structures.Crista galli,nasal septum,and the mid-point of upper central incisors deviated to cleft side,while skeletal anterior nasal spine deviated to noncleft side.Conclusions Most asymmetries are found in nasal cavity and the dentoalveolar area near the cleft.No significant asymmetry is found in upper cranialfacial strutures,nor in regions not in the adjacency of the cleft.
Comparative CBCT Analysis of Maxillofacial Skeletal Structures in Patients with Unilateral Cleft Lip and Palate and Non-Cleft Individuals

Abstract:Background: The aim of this study was to evaluate and compare the maxillofacial structures of individuals with unilateral cleft lip and palate (UCLP) and healthy controls using cone beam computed tomography (CBCT). Methods: The study included a total of 90 subjects, comprising 45 randomly selected individuals with UCLP (30 males and 15 females, mean age 14.69 ± 3.95 years) in the study group and 45 healthy individuals (30 males and 15 females, mean age 14.46 ± 3.65 years) in the control group. Maxillofacial measurements were taken in three different planes and categorized into five groups, namely vertical, facial, cranial, maxillary, and mandibular. In the statistical comparison between groups, the significance level was determined as p 0.05). Upper anterior face height and posterior face height in the UCLP group were found to be significantly shorter than the control group (p < 0.05). Midface width and depth were inadequate in the UCLP group (p < 0.05). Anterior and posterior cranial base lengths were significantly shorter in individuals with UCLP (p < 0.05). Nasal width and interorbital width were significantly greater in the UCLP group (p < 0.05). In addition, maxillary width, maxillary length, and mandibular width were significantly shorter in the UCLP group than in the control group (p < 0.05). Conclusions: While the control group exhibited generally longer measurements in all three dimensions compared to the study group, the skeletal structures adjacent to the cleft demonstrated the most notable developmental deficiency.

Simultaneous Monitoring of Force, Acceleration and Electromyogram during Computer-Controlled Infusion of Atracurium in Sheep

Abstract:Five sheep were anaesthetised with sodium thiopentone, nitrous oxide and oxygen, and the lungs ventilated artificially. A computer-controlled infusion of atracurium employing feedback from isometric force measurement was set to maintain 15, 50 and 85% depth of neuromuscular block. Each was held constant for 30 minutes, while isometric force and electromyogram were measured on the left forelimb and acceleration on the right. The acceleration transducer records at the three levels of block were practically identical with those from force measurements: mean differences (SD) between simultaneous measurements being 3.6(5.4)%. EMG value was 10.1 (12.0)% different from force measurement. Correlation between simultaneous force and either acceleration or EMG was excellent (r = 0.986 and 0.938 respectively). In clinical practice all three techniques measure block comparably. In the sheep forelimb, EMG was a less reliable method of measuring block.

Abstract:Nonenzymatic glycation by glucose and/or ascorbate leads to the formation of advanced glycation end products (AGEs), which are thought to be a critical element in lens protein aging and cataract formation. The relative participation of these two glycating agents was evaluated in vitro. The incubation of 100 mM [U-14C]-D-glucose and 10 mM [U-14C]-L-ascorbate with lens proteins resulted in an increasing incorporation over 3 weeks, reaching a maximum of 100 nMol mg-1 protein and 160 nMol mg-1 protein with ascorbate. Glycation was proportional to carbohydrate concentration with both reagents, however ascorbate was 18-fold more reactive with lens proteins than glucose. Protein crosslinking was not obvious with 250 mM glucose as measured by SDS-PAGE, however, ascorbate caused extensive crosslinking even at 3.0 mM. The sugar-dependent incorporation of N alpha-formyl-[U-14C]-L-lysine ([U-14C]Nfl) into proteins, gave values of 1.5 nMol mg-1 protein after 3 weeks with 100 mM glucose compared to 11 nMol mg-1 protein with 10 mM ascorbate. On a molar basis, ascorbate was 70-fold more active than glucose and 100-fold more active than fructose in the crosslinking assay. N alpha-formyl-N epsilon-fructosyllysine (1.0 mM) dissociated to cause the incorporation of 1.2 nMol of [U-14C]NfL, but 1.0 mM 3-deoxyglucosone, the putative active dissociation product of fructosyl-lysine, produced only 1.5 nMol mg-1 protein of crosslinks. The chelator, DTPA, had little or no effect on crosslinking in our assay except at the highest carbohydrate level. These data argue that glucose crosslinking can be shown in vitro with lens proteins, however, it does not proceed significantly via 3-deoxyglucosone, and does not require transition metal ion-mediated oxidation to occur. Quantitatively, however, it is almost two orders of magnitude less than the crosslinking by ascorbate oxidation products in vitro.
THE STUDY OF THE RELATIONSHIP BETWEEN CRANIAL BASE AND ANGLE CLASS II AND III MALOCCLUSION

Abstract:Objective:To investigate the relationship between cranial base structure and maxillofacial morphology of malocclusion.Methods:Sixty-four patients aged from 12 to 14,were divided into 2 groups,namely Angle Class Ⅱ and Ⅲ.All the cephalographs were traced,then digitized and statisticed using a computer.Results:The cranial base angle of class Ⅱ was significantly larger than that of class Ⅲ,and was negatively correlated with articulare angle,gonion angle,and positively correlated with SN-palatal angle.Anterior cranial base length was positively correlated with facial length.Posterior cranial base length was postitvely correlated with mandibular body length,facial length,upper facial length,palatal breadth,and negatively correlated with lower facial length.Conclusion:The structure of cranial base is in close relation with the maxilloficial morphology.The larger the cranial base angle,the closer class Ⅱ face pattern is.Conversely the smaller the cranial base angle,the closer class Ⅲ face pattern is.Posterior cranial base has more effects on maxillofacial morphology than anterior cranial base.

Abstract:OBJECTIVE:To evaluate the inconformity between soft tissue defect and bony defect by observing the cleft extent of palate with complete secondary palate bony cleft in incomplete cleft palate patient.METHODS:The patients with incomplete cleft palate treated in Hospital of Stomatology Peking University from July 2012 to June 2013 were reviewed, of which 75 cases with complete secondary palate bony cleft were selected in this study. The CT scan and intraoral photograph were taken before operation. The patients were classified as four types according to the extent of soft tissue defect. Type 1: soft tissue defect reached incisive foremen region, Type 2 was hard and soft cleft palate, Type 3 soft cleft palate and Type 4 submucous cleft palate. Type 1 was defined as conformity group (CG). The other three types were defined as inconformity group (ICG) and divided into three subgroups (ICG-I), (ICG-II) and (ICG-III).RESULTS:Fifty-seven patients were in ICG group, and the rate of inconformity was 76% (57/75). The percentage of ICG-I, ICG-II and ICG-III was 47% (27/57), 23% (13/57) and 30% (17/57), respevtively.CONCLUSIONS:There are different types of soft tissue deformity with complete secondary palate bony cleft. The inconformity between soft tissue and hard tissue defect exits in 3/4 of isolated cleft palate patients.
Preliminary comparison of three-dimensional reconstructed palatal morphology in subjects with different sagittal and vertical patterns

Abstract:Background The aim of this study was to assess the difference of palatal morphology in different vertical patterns between skeletal Class I subjects and skeletal Class II subjects with retrusive mandible. Methods Seventy-six skeletal Class II subjects with retrusive mandible (38 females, 38 males) and 85 skeletal Class I subjects (45 females, 40 males) were collected retrospectively and divided into hyperdivergent, normodivergent and hypodivergent groups. CBCT images of these subjects were reoriented by Dolphin 3D Imaging software. Three-dimensional (3D) maxilla was segmented by ProPlan software before using Geomagic Studio software to reconstruct 3D palatal morphology. Deviation patterns on 3D colored map analysis was performed to compare the difference of 3D palatal morphology between different groups. Results 3D colored map analysis showed that male’s palate was higher and wider than that of female in the posterior part, regardless of different sagittal and vertical patterns. In skeletal Class II subjects with retrusive mandible, males with hyperdivergent and normodivergent showed higher and narrower in the posterior part of palate, while females with hyperdivergent and normodivergent had a higher but no obviously narrow palate compared with the hypodivergent subjects. Skeletal Class II subjects with retrusive mandible showed flatter and narrower in the posterior part of palate than that of skeletal Class I subjects. Conclusions Sagittal and vertical patterns have great influence on the palatal morphology and as the vertical dimension increased, the palate tended to be higher and narrower.

Abstract:Objective To investigate the characteristics of cranioficial bone structure on anteroposterior cephalograms in skeletal Class Ⅲ patients.Methods Posteroanterior cephalograms were taken for Fouty-One patients with skeletal class Ⅲ malocclusion.All posteroanterior cephalograms were hand-traced.Eight linear measurements and 12 ordinates of certain points on dentoalveolar structures,jaws and other craniofacial regions were abtained using a digital vernier caliper.One sample t-test was performed to determine the difference of the measurements between samples and subjects with normal occlusion. Results Compared with normal values,the sample of this study showed a wider mid-face width and a narrower lower face width(P0.01].The ramus height and the distance from the most superior points of condyles to Me point were larger than norms.The ordinates of point Sc,Cdl,Me and CMDI were different significantly from normal values.Conclusion There were significant differences in crainoficial width proportion and ordinates of mandible and lower teeth between the sample and the subjects with normal occlusion.
A longitudinal study of vertical growth of craniofacial complex in female children with normal occlusion from 7 to 12 years old

Abstract:To find the laws of vertical growth of craniofacial complex in children with mixed dentition.19 female children were annually observed with cephalometric film in 4 successive years, of whom 10 started at 7 years while the others at age of 9. The two groups were longitudinally studied. All data were analyzed by SPSS 8.0 software.In the observation period, the height of most craniofacial regions continually grew. In anterior face, the middle facial height increased most (increased 4.7 mm from 7 to 10 years old, P < 0.01; increased 3.9 mm from 10 to 12 years old, P < 0.01), while the lower facial height nearly unchanged (increased 1.3 mm from 7 to 10 years old, P > 0.05; increased 1.4 mm from 10 to 12 years old, P > 0.05). Some other regions had their own growing characters.Once the occlusal relationships of permanent incisors are established, it is difficult to control the anterior lower facial height with orthodontic treatments.
Transverse Maxillary Development in Complete Cleft Patients: Comparison of 2 Treatment Strategies by Longitudinal and Cross-Sectional Model Analysis

Abstract:Objective: This study aimed to compare differences in the maxillary transverse dimension following cleft repair using 2 protocols involving a 2-staged palatal closure with similar timing but different techniques. Design: Retrospective study. Two matched study samples, each containing 10 patients. Setting: Cleft center A is a university hospital. Cleft Center B is a general teaching hospital. Patients: Patients with cleft lip alveolus and palate (CLAP) were matched by cleft type, gender, age, and palatal morphology at the age of 1 month. Differences in the transverse dimension were measured at 4 ages in 2 matched groups (A and B from center A and B), each including 5 patients with unilateral CLAP (UCLAP) and 5 patients with bilateral CLAP (BCLAP). Interventions: Orthodontic/orthopedic expansion treatment began at 8 and 5 years of age at centers A and B, respectively. Results: The mean intercanine distance decreased by 8 mm (standard deviation [SD] = 1.6; P = .030) in group A patients with UCLAP between 1 and 5 years (SD = 1.6; P = .030) and by 6.3 mm (SD = 3.2; P = .016) in group B patients with BCLAP. The mean intermolar distance increased by 4 mm (SD = 3.4; P = .076) in patients with BCLAP. A trend toward improved canine positioning was observed in patients with BCLAP by 12 years of age, with improvement occurring between 4 and 12 years. Conclusion: The type of palatal closure (inducing scar tissue at the bony level) and the late installment of orthodontic/orthopedic expansion probably reduced the intercanine width in bilateral complete clefts.
Evaluation of the Bony Nasal Septum Development of Cleft Lip and Palate Patients Using Three-Dimensional Computed Tomography Measurement

Abstract:Objective: To evaluate the development of bony nasal septum of cleft lip and palate patients. Methods: This retrospective study included 157 patients aged over 14 years with cleft lip and palate (CLP) and 54 normal people. The three-dimensional computed tomography reconstruction images of the posterior and inferior part of the bony nasal septum were measured. We evaluated the development of the bony nasal septum. Results: There were 39 patients with cleft lip only (CLO), 38 patients with cleft palate only (CPO), 80 patients with CLP. The L1, L2, and S of CLO and/or CPO patients were smaller than the normal control group. The L1 and S of cleft palate group and CLP group were smaller than cleft lip group, there was no significant difference for L2 between the three groups. Compared with the postoperative cleft palate group, the S of preoperative group was significantly bigger, the L2 was bigger without significant difference; but compared with the normal group, L1 and S of preoperative group was significantly smaller, L2 was not significantly different. Conclusions: The patients with cleft palate only were most affected. There was not only decrease of the fusion of maxilla and vomer, but also growth inhibition of the sutura between vomer and nasal septum cartilages, vertical plate of ethmoid bone in patients with cleft lip and palate. The adverse effects of the cleft palate operation to bony nasal septal development mainly lie in anterior inferior direction.
