Abstract:OBJECTIVE:To evaluate the craniofacial morphology of patients with isolated cleft palate in the mixed dentition after palatoplasty.METHODS:Twenty patients (7 to 11 years old) with isolated cleft palate in the mixed dentition were selected. The control group comprised 35 age-matched non-cleft children without orthodontic treatment. Cephalometric analysis was carried out to evaluate craniofacial morphology.RESULTS:The mean values of total cranial base length, maxillary depth, mandibular effective length in cleft patients were 86.48 mm, 44.79 mm and 65.45 mm, respectively. Those in control group were 91.27 mm, 48.84 mm and 70.49 mm, respectively (P < 0.001).CONCLUSIONS:Underdeveloped maxilla and class III skeletal profile were presented in patients with isolated cleft palate.
Long-term craniofacial morphology in young adults treated for a non-syndromal UCLP: A systematic review

Abstract:Minimizing mid-facial growth impairment is one of the treatment goals in cleft lip and palate surgery. As growth of the maxilla extends into young adulthood, long-term evaluation is essential to make a comprehensive assessment of a treatment protocol. There are numerous treatment approaches for cleft lip/palate surgery, and most have the characteristic distinction between either an early or a late cleft palate closure. PRISMA guidelines were applied to explore the quality of the current literature and to identify treatment factors influencing long-term cephalometric outcomes. The literature search was conducted in Pubmed, The Cochrane Library and Embase. We included studies evaluating cephalometric outcomes (SNA and ANB values on 2D cephalograms) in UCLP patients with a mean age of 16 years and older. Studies with an inadequate description of the timing of surgery were excluded. 17 studies comprising 906 patients were selected and included for critical appraisal. Treatment protocols differed considerably among the included studies and inconsistent methodology was common. Eight studies applied a one-stage procedure, 11 studies performed a two-stage reconstruction, and five studies made use of a vomer flap. Applying a multivariate model, we did not identify any treatment factors that significantly influenced growth (SNA/ANB values), except for the method of inclusion, suggesting the presence of significant selection bias within the studies. The current literature remains inadequate for evidence-based decision making and to advise parents if an early or late palate closure leads to a more favorable maxillary outgrowth. This manuscript will propose guidelines and recommended quality criteria for future studies.
Synthesis and morphological characterization of nanocomposite based on anodic TiO2 nanotubes and poly(N-maleoyl Glycine-CO-Acrylic acid)

Abstract:In this study, we examined the synthesis, and characterization of TiO2/poly(N-maleoylglycine-co-acrylic acid) (TiO2/poly(MG-co-AA)) nanocomposite. The nanocomposite was prepared by the dispersion of TiO2 nanotubes in a water solution of the polymer (3% w/w) and then it was lyophilized. The nanocomposite was characterized by FTIR and Raman spectroscopies. The incorporation of TiO2 nanotubes to the polymeric matrix was determined by transmission electron microscopy (TEM) from which TiO2 nanotubes in the inner of copolymer with diameters ranging between 90 and 100 nm were observed. The overall morphology of the previously synthetized nanotubes was determined by scanning electron microscopy (SEM). For the nanocomposite system, the morphology was studied by atomic force microscopy (AFM) from which a grain shape structure was observed. This process resulted in a new nanocompsite material with an average grain diameter estimated by SEM and AFM ranging between 210-240 nm. It was observed that the nanotubes were homogeneously dispersed within the polymeric matrix. The prepared material could be suitable in the design of electronic devices and additionally could have potential applications as biomaterial.
Occlusal Outcomes in Non-Robin Sequence Patients with Isolated Cleft Palate

To assess the skeletal class occlusion and lateral cephalometry in children with isolated cleft palates (non-Robin sequence) and 2. to identify associations between these findings and pre-palatoplasty cleft palate measurements.Study designRetrospective cohort study.SettingNorth American Institutional Tertiary Paediatric Center.PatientsOur cleft database was reviewed, and patients were included if they had an isolated cleft palate without a Robin Sequence diagnosis, had a Furlow palatoplasty and had available per operative cleft palate measurements and available lateral cephalogram between 6 and 8 years old. Thirty-two patients matched to inclusion criteria.InterventionFurlow's Palatoplasty.Main Outcome and MeasuresCleft size at palatoplasty, cephalometric measurements and skeletal occlusal classes were analysed. ANOVA was used to test the association between cephalometric measurements and occlusal classes. Results are presented as means with a 95% confidence interval. The association between cleft measurements and cephalometric parameters was tested with Spearman Correlation (rs).ResultsThe skeletal occlusal outcome at 7 years old for this series of patients was: Class I: 19%; Class II: 59% and Class III: 22%. No single cleft measurement at palatoplasty was predictive of the skeletal occlusal outcome. A larger hard palate cleft was associated with a shorter antero-posterior maxilla.ConclusionsThe skeletal occlusal class outcomes were similar to those found in a previous study in the literature. The occlusal prognosis appears to be better than in patients with Robin Sequence or with an associated cleft lip. No preoperative measurement was found to be associated with the occlusal outcome.

Transverse Maxillary Development in Complete Cleft Patients: Comparison of 2 Treatment Strategies by Longitudinal and Cross-Sectional Model Analysis

Abstract:Objective: This study aimed to compare differences in the maxillary transverse dimension following cleft repair using 2 protocols involving a 2-staged palatal closure with similar timing but different techniques. Design: Retrospective study. Two matched study samples, each containing 10 patients. Setting: Cleft center A is a university hospital. Cleft Center B is a general teaching hospital. Patients: Patients with cleft lip alveolus and palate (CLAP) were matched by cleft type, gender, age, and palatal morphology at the age of 1 month. Differences in the transverse dimension were measured at 4 ages in 2 matched groups (A and B from center A and B), each including 5 patients with unilateral CLAP (UCLAP) and 5 patients with bilateral CLAP (BCLAP). Interventions: Orthodontic/orthopedic expansion treatment began at 8 and 5 years of age at centers A and B, respectively. Results: The mean intercanine distance decreased by 8 mm (standard deviation [SD] = 1.6; P = .030) in group A patients with UCLAP between 1 and 5 years (SD = 1.6; P = .030) and by 6.3 mm (SD = 3.2; P = .016) in group B patients with BCLAP. The mean intermolar distance increased by 4 mm (SD = 3.4; P = .076) in patients with BCLAP. A trend toward improved canine positioning was observed in patients with BCLAP by 12 years of age, with improvement occurring between 4 and 12 years. Conclusion: The type of palatal closure (inducing scar tissue at the bony level) and the late installment of orthodontic/orthopedic expansion probably reduced the intercanine width in bilateral complete clefts.

Abstract:The products of the recG and ruvAB genes of Escherichia coli are both thought to promote branch migration of Holliday recombination intermediates by their junction specific helicase activities in homologous recombination and recombination repair. To investigate the in vivo role of the recG gene, we examined the effects of a recG null mutation on cell division and chromosome partition. After UV irradiation at a low dose (5J/m2), delta recG mutant filamentous cells with unpartitioned chromosomes. A mutation in the sfiA gene, which encodes and SOS-inducible inhibitor of septum formation, partially suppressed filamentation of recG mutant cells, but did not prevent the formation of anucleate cells. The sensitivity of UV light and the cytological phenotypes after UV irradiation of a recA recG double mutant were similar to a recA single mutant, consistent with the role of recG, which is assigned to a later stage in recombinant repair than recA. The recG ruvAB and recG ruvC double mutants were more sensitive to UV, almost as sensitive as the recA mutant and showed more extreme phenotypes concerning filamentation and chromosome nondisjunction, both after UV irradiation and without UV irradiation than either recG or ruv single mutants. The recG polA12 (Ts) mutant, which is temperature sensitive in growth, formed filamentous cells with centrally located chromosome aggregates when grown at nonpermissive temperature similar to the UV irradiated recG mutant. These results support the notion that recG is involved in processing Holliday intermediates in recombination repair in vivo. We suggest that the defect in the processing in the recG mutant results in accumulation of nonpartitioned chromosomes, which are linked by Holliday junctions.

Abstract:Background: A palatal cleft can be reconstructed using various palatoplasty techniques. Many techniques use local mucoperiosteal flaps to close the hard palate cleft, without closing the underlying bone defect. The purpose of this study was to explore the possibility of spontaneous bone regeneration in the remaining bone defect following a two-stage palatoplasty. Furthermore, the effect of this bone regeneration on transverse maxillary growth was studied. Methods: A retrospective study of unilateral cleft lip and palate patients was performed. Cleft size was measured at the hard-soft palate junction on plaster models obtained during palatoplasty. Residual bony cleft was evaluated at the time of alveolar process reconstruction using CBCT images. The presence of crossbites was evaluated through clinical photographs to evaluate transverse maxillary growth. Results: Forty-six patients were included in this study. Thirteen patients (28%) presented with complete ossification of the hard palate at the age of six years. Six patients (13%) had no ossification, while twenty-seven patients (59%) demonstrated partial closure, averaging 75% of the total hard palate length. All patients with complete closure and 89% of patients with partial closure presented without lateral crossbites. Statistical analysis revealed no significant correlation between lateral crossbites and the extent of hard palate ossification (p = 0.4314). Conclusions: 87% of children presented with at least partial ossification of the hard palate after a two-stage palatoplasty, indicating the potential for spontaneous bone regeneration. Importantly, lateral crossbites demonstrated no correlation with palatal ossification (p = 0.1819), suggesting no detrimental impact of regenerated bone on transverse maxillary growth.
The single Z-plasty for cleft palate repair: a preliminary report.

Abstract:Objective: To introduce a surgical technique for the repair of cleft palate with the single and full-thickness Z-plasty method. Patients and Methods: The procedure was performed from 1999 to 2006 in 34 patients with cleft palate with a median age of 6.0 years (range, 2.0 to 21 years). In these patients, in order to push the soft palate back farther, a single and full-thickness Z-plasty, designed on the soft palate across the defect, was applied to the von Langenbeck procedure. The full thickness of the soft palate was incised after the hard palate was closed by suturing the two mucoperiosteal flaps, and two Z-plasty flaps were formed. The two flaps were then transposed and closed by interrupted suturing of three layers of the palate in proper order, from nasal mucosa, levator muscle to oral mucosa. The patients were followed for 6 months to 3 years, velopharyngeal closure was examined by nasopharyngeal fiberscope and/or x-ray radiography, and a clinical speech evaluation was performed. Results: In all patients, no problem of flap viability was encountered and all healed well. The postoperative results were satisfactory without any complications such as dehiscence, perforation, or palatal fistula. The clinical speech evaluation was satisfactory, and the effects of the operation were stable. Conclusions: The technique presented has been effective, having the advantages of palatal closure without tension and with sufficient lengthening of the soft palate.
Simultaneous Monitoring of Force, Acceleration and Electromyogram during Computer-Controlled Infusion of Atracurium in Sheep

Abstract:Five sheep were anaesthetised with sodium thiopentone, nitrous oxide and oxygen, and the lungs ventilated artificially. A computer-controlled infusion of atracurium employing feedback from isometric force measurement was set to maintain 15, 50 and 85% depth of neuromuscular block. Each was held constant for 30 minutes, while isometric force and electromyogram were measured on the left forelimb and acceleration on the right. The acceleration transducer records at the three levels of block were practically identical with those from force measurements: mean differences (SD) between simultaneous measurements being 3.6(5.4)%. EMG value was 10.1 (12.0)% different from force measurement. Correlation between simultaneous force and either acceleration or EMG was excellent (r = 0.986 and 0.938 respectively). In clinical practice all three techniques measure block comparably. In the sheep forelimb, EMG was a less reliable method of measuring block.

One hundred and sixty-eight digitized dental models of cheiloplasty and one-stage palatoplasty (G1) and cheiloplasty and two-stage palatoplasty (G2) were evaluated at preoperative time 1 (T1), preoperative time 2 (T2), and postoperative (T3). The following surface distances were evaluated: across surface distance; cleft widths anterior (P-P′) and posterior (U-U′) cleft widths, intercanine width (C-C′), and intertuberosity width (T-T′); smallest (P′-T′) and largest (P-T) segment lengths; and smallest (C′-D′) and largest (C-D) segment cleft depths. In G1, P-P′, U-U′, and C-C′ reduced at T2, unlike P′-T′ (P<0.05). P-T and C′-D′ distances increased at T3 (P<0.05), while C-D increased at all stages (P<0.001). In G2, U-U′ and C-C′ reduced at T2 (P<0.05), while P′-T′, P-T, C′-D′, and C-D′ increased at T3 (P<0.001). In an intergroup analysis of growth rate, G2 showed higher growth percentages compared to G1, in which C′-D′ was significant (P=0.038). Furthermore, C′-D′ presented a coefficient of determination of 0.076 (P=0.039). In conclusion, dental arch development is influenced by the rehabilitation protocol. However, in the sample evaluated, the comparison suggested that individuals whose palate was operated on in two stages had the most favorable palatal growth.

Abstract:Secondary deformities to the midfacial area in adolescence and adulthood are common sequelae after primary cleft lip and palate repair and lead to significant functional, aesthetic and psychosocial challenges. Treatment for patients with normal occlusion or treated malocclusion and remaining midfacial hypoplasia includes autologous and alloplastic augmentation of the maxilla. Because of the great variation of deformities, accurate evaluation is essential for appropriate treatment planning. To provide insights in the altered midfacial anatomy in cleft patients, a retrospective computed tomography morphologic analysis of the anterior projection of the maxilla in the paranasal region, soft tissue thickness, and relation of soft tissue to bony landmarks was performed in adolescent, unilateral, non-syndromic complete cleft and palate patients. The anterior projection of the maxilla was very variable with the cleft side bone surface being more anterior relative to the non-cleft side in some patients. Soft tissue thickness was not significantly different between cleft and non-cleft side. The lateral distance from the most anterior point of the maxilla to subalare was significantly smaller on the cleft side as compared with the non-cleft side. The authors' study demonstrates that in cleft lip and palate patients midfacial anatomy with regards to maxilla projection at the pyriform aperture is very variable making accurate evaluation of the bone morphology and an individualized treatment plan a prerequisite for successful correction of nasomaxillary deformities.

Abstract:Maxillary hypoplasia is a craniofacial characteristic in patients with isolated cleft palate (ICP). For maxillary hypoplasia with dental arch length deformity and severe crowding, premaxillary distraction osteogenesis can correct maxillary hypoplasia and increase the dental arch length to eliminate crowding. This case report describes the interdisciplinary treatment of a 20-year-old female with ICP with severe maxillary hypoplasia, mandibular prognathism and asymmetry, severe maxillary crowding, missing teeth, midline discrepancy, and anterior crossbite. An interdisciplinary treatment plan that included comprehensive orthodontic treatment, premaxillary distraction osteogenesis, mandibular orthognathic surgery, implant surgery, and endodontic and periodontal treatment was implemented to correct craniofacial deformity and malocclusion. A harmonious facial profile with satisfactory occlusion was obtained at the end of the interdisciplinary treatment. At 1-year follow-up, the treatment outcomes were stable. This case report aimed to demonstrate that interdisciplinary collaboration and excellent patient compliance are essential for patients with ICP to achieve functional occlusion and harmonious facial esthetics.
Catch-Up Growth Pattern in Cleft Palate: A Longitudinal Study from Infancy to Adolescence

Abstract:Objective: To characterize the patterns of somatic catch-up growth from infancy to adolescence in patients with cleft palate (CP). Study design: We assessed 474 nonsyndromic patients with isolated cleft palate (n = 69) and unilateral and bilateral cleft lip and palate (n = 271; n = 134) who underwent palatoplasty between 1988 and 2017 and had longitudinal physical growth data at birth (T0), cheiloplasty (T1), palatoplasty (T2), childhood (T3), and adolescence (T4). The z scores of weight (ZWT), height (ZHT), and body mass index (ZBMI) were compared among the CP types (isolated cleft palate, unilateral cleft lip and palate, and bilateral cleft lip and palate) and time points (T1, T2, T3, and T4). Subgroup analyses were performed to investigate the growth of patients with malnourishment (z score < -1) at T1 or T2. A generalized linear model was used to investigate the effects of gestational age and cardiac anomalies on the longitudinal changes in ZHT and ZBMI. Results: Regardless of the time point, the overall ZHT, ZWT, and ZBMI approximated 0 in all CP types, indicating few differences from the mean values of noncleft children. Significant catch-up growth occurred in ZHT and ZWT from T1 to T4 for all CP types (all P < .05). Despite the recovery of ZHT and ZBMI in most patients with malnourishment, these values remain relatively low until adolescence. Patients who were born at preterm stage or had surgically repaired cardiac anomalies grew well. Conclusions: Even in infants with CP and malnutrition, preterm birth, or cardiac anomalies, rapid catch-up growth can occur prior to palatoplasty with the help of comprehensive cleft care.
The Square Flap Method for Cleft Palate Repair

Abstract:Objective: To introduce a new surgical technique for repair of cleft palate using the square flap method. Design and Setting: A retrospective analysis of prospectively collected data. Patients and Methods: The procedure was performed from 1995 to 2004 in 21 males and 16 females with cleft palates of different types; the patients had a median age of 6.0 years and an average age of 9.4 years (range from 22 months to 23 years). In these patients, the square flap method, consisting of one rhombic flap and four triangular flaps, designed on the soft palate across the defect, was applied to the von Langenbeck procedure. After incisions, the flaps were rotated and advanced, and each flap was inserted into the opposite side and then sutured. The patients were followed from 6 months to 2 years, the velopharyngeal closure was examined by nasopharyngeal fiberscope and/ or x-ray radiography, and a clinical speech evaluation was performed. Results: In all cases, no problem of flap viability was encountered and all healed well. The postoperative results were satisfactory without any complications such as dehiscence, perforation, palatal fistula, or functional disturbance. The velopharyngeal closure and clinical speech evaluation were satisfactory, and the effects of the operation were stable. Conclusions: The technique presented has been effective, with the advantages of palatal closure without tension, good muscular reconstruction, and sufficient lengthening of the soft palate.
The Influence of Lip Repair on the Growth of Craniofacial Structures in Bilateral Cleft Lip and Palate Patients With Unoperated Palate From Childhood to Adulthood

Abstract:Purpose: Surgical treatment of complete bilateral cleft lip and palate (BCLP) is the most difficult procedure among common cleft lip and palate. This study was to research the long-term effect of lip surgery by localizing the craniofacial growth pattern of BCLP patients in the Han people in western China compared with normal healthy people from childhood to adulthood. Patients and methods: A cross-sectional study included a sample of 43 BCLP patients who have received lip repair and a control group of 55 normal healthy people with Angle Class I occlusion at the similar age, gender and ethnicity, and all participants were obtained from the Department of Oral and Maxillofacial Surgery, Sichuan University. Sixteen linear and 14 angular bony landmarks were identified on lateral cephalometric radiographs at the average age of 5.6-year-old, 10.7-year-old, and 24.2-year-old. The measurements were used to represent the growth changes of cranial base, bony nasopharynx, maxillary, mandibular and maxilla-mandibular relationship. Between-group differences were assessed by using the Independent-Sample t test. Result: Lip repair has a slow and long-term effect on the craniofacial morphology. The tension from repaired orbicularis oris muscle would cause a decrease in maxillary sagittal length and the retroposition of maxilla combined with the mandibular functional displacement, which would cause a positive change in the postoperative facial morphology of BCLP patients in the long run. Conclusion: BCLP patients who have undergone lip repair at an early age have the potential to develop better facial convexity.

Abstract:When the potent immunosuppressive sesquiterpene ovalicin is added to lymphocyte cultures one first observes a preferential inhibition of uridine incorporation into rRNA. The uptake of the nucleoside, its conversion into the triphosphate or the polymerizing activity itself are not affected. A longer period of incubation with the drug results in a marked decrease in the number of ribosomes, with a concomitant reduction of the rate of leucine incorporation into all cellular proteins. After extended periods of time, the incorporation of thymidine into DNA in stimulated lymphocytes as well as in SV49.1 lymphoma cells is inhibited by 1 nM ovalicin or less, although part of the incorporation seems to be resistant to the drug even at much higher concentrations. A similar affect is observed with 3T6 mouse fibroblasts or HeLa cells. Here, however, a much longer incubation with the drug is required. This observation explains the selective effect of ovalicin on lymphocytes observed in vivo.
The Effect of Palatoplasty Timing on Maxillary Development

Abstract:Objective To investigate the effect of different palatoplasty timing on maxillary development by a retrospective study.Methods 101 UCLP consecutive cases who were older than 10 years were followed up,which were divided into three groups according to the age of operation,first group:0～3.00 years old,second group:3.01～6.00 years old and third group:6.01 years old above.Lateral cephalogram were taken for each patient,and cephalometric analysis were used for assessing maxillary growth.Results The results of cephalometric show that there was no significant difference among three groups for all variable(∠SNA、∠SNB、∠ANB、ANS-Ptm、N-ANS'/N-Me) except Ba-A variable,significant difference existed between 1st and 3rd groups for Ba-A variable justly.However,the mean value of ∠SNA,∠ANB and Ba-A increased along with increase of operation age.Conclusion The maxillary development was better along with increase of operation age.Not only the maxillary position,but also the maxillary length will be hindered by the cleft palate surgery.
Facial morphology analysis of children with non-syndromic cleft lip and palate in a local population

Abstract:Objectives: To study the facial morphology in children with non-syndromic cleft lip and palate by applying numerical facial analysis on photographs for planning and evaluating treatment outcomes. Methods: This descriptive study was conducted from March 2020 to July 2020 in the Department of Oral Pathology, University of Health Sciences and Cleft Lip and Palate Hospital, Lahore Pakistan. A total of 104 patients of both genders with an age range from three months to thirteen years were included. Photographs of the participants were taken to measure facial anthropometrical landmarks including facial height, nose width, mouth width and inter canthal distance. The association between facial measurements with gender and phenotype and across age groups were computed keeping the confidence level at 95%. Results: Mean age of the children was 72.43±44.2 months with slight male predominance. Thirty-one percent presented with bilateral cleft lip and palate followed by unilateral cleft lip and plate. Total mean facial height, nose width and mouth width were found to be 143.46±21.52mm, 32.24±5.03mm and 33.71±4.38mm respectively. Intercanthal distance was measured to be 31.04±5.99mm. Statistically significant association was observed between gender and facial height, nose width, mouth width and Intercanthal distance. Conclusion: Facial anthropometric measures done on frontal photographs can be used to identify the facial landmarks in children with non-syndromic cleft lip and palate in low resource stings that may help surgeons in getting better aesthetic outcomes.
