Abstract:甘草的主要成分甘草酸(glycyrrhizic acid,GCA)及其衍生物--甘草甜素、甘草次酸具有诸多治疗呼吸系统疾病的药理活性,包括治疗严重急性呼吸综合征,抗上呼吸道病毒感染、急性肺损伤、肺纤维化,调节支气管哮喘及炎症等作用.笔者认为GCA类药物在呼吸方面有巨大的发展空间,就近年来对GCA类药物治疗呼吸系统疾病的国内外药理学研究进展作一综述。

Abstract:Interstitial lung diseases associated cough is common in clinical practice.It is usually refractory, may deteriorate dyspnea and induce the complications such as spontaneous pneumothorax, syncope and chest pain, having a profound negative impact on patients′ quality of life and prognosis.Its pathogenesis involves airway inflammation, mechanical distortion of the lung, cough hypersensitivity and gastroesophageal reflux.The diagnosis requires to establish the cause-effect relationship between cough and interstitial lung diseases with the initial confirmation of interstitial lung diseases.Cough resolution depends upon the treatment of basic interstitial lung diseases, followed by the management of concomitant diseases.Antitussive drugs including neuromodulators can help to relieve cough and improve the patients′ quality of life.Individualization of the therapy is the essential strategy.

Abstract:Refractory chronic cough may be mainly caused by central hypersensitivity, leading to a new concept of diagnosis of cough hypersensitivity syndrome.The neurotransmitters including γ-aminobutyric acid receptor and N-methyl-D-aspartic acid receptor are involved in the pathogenesis of cough hypersensitivity, which is highly similar to the central sensitization of neuropathic pain.Therefore, the neuromodulator has also been used for the treatment of refractory chronic cough.Currently, the commonly used neuromodulators such as gabapentin, pregabalin, baclofen and amitriptyline have been confirmed to be effective in the treatment of refractory chronic cough.However, a considerable part of the patients with refractory chronic cough do not respond to the therapy with the neuromodulators.The adverse effects on central nervous system also limit their clinical application.Thus, to screen the patients with refractory chronic cough suitable for the treatment with neuromodulators is necessary to improve their therapeutic success rate and minimize the adverse effects, which needs further research in the future.

Abstract:噻托溴胺(tiotropium bromide)是一个新型的用于治疗慢性阻塞性肺疾病(COPD)的吸入型支气管扩张药物,通过与M3受体结合,拮抗乙酰胆碱的作用,缓解支气管平滑肌的痉挛.吸入一次疗效持续24小时以上.几个大样本的临床试验结果表明,该药物对中重度COPD具有良好的疗效,在改善呼吸困难、生活质量、减少住院次数方面要优于沙美特罗,每日吸入一次噻托溴胺对支气管的舒张作用要明显好于每日吸入四次的溴化异丙托品和每日吸入两次的沙美特罗.而且该药物能够改善通气功能和肺容积,用药的耐受性和安全性也比较好。

Abstract:嗜碱粒细胞是支气管哮喘(哮喘)炎症发生过程中重要的效应细胞之一,它通过IgE介导引起组胺、缓激肽等血管活性介质释放,导致支气管黏膜水肿、黏液分泌增加、平滑肌收缩、气道痉挛,引起哮喘急性发作,并可诱导气道嗜酸性细胞等炎性细胞浸润并引起气道高反应性,还可以通过促进辅助T细胞2类细胞因子的表达促进哮喘的发展,并引起主要的气道炎症,最终导致气道重建.因此,进一步的研究可为哮喘的发病机制和治疗提供新的视角和途径。

Abstract:Bronchial asthma (athsma) is a chronic airway inflammatory disease involving a variety of inflammatory cells and mediators,excessive Th2 polarization play key role in asthmatic airway inflammation. YKL-40 (BRP-39) is a newly discovered chitinase-like protein and participates in inflammatory reaction,tissue remodeling in various diseases. YKL-40 can increase Th2 number by promoting their activation and differentiation as well as reducing their apoptosis in asthma patients,and play an important role in chronic airway inflammation of asthma.

Abstract:Objective To observe the change of metalloproteinase (MMP)-9,tissue inhibitor of metalloproteinase (TIMP)-1 and interleukin(IL)-8 between pre-and post-treatment in elderly patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD).Furthermore,the effects of corticosteroid and β agonist were evaluated.Methods Forty patients with AECOPD were selected and randomly divided into four groups with 10 patients for each.Conventional treatment,inhaled corticosteroid,β3 agonist and inhaled corticosteroid combined with β3 agonist were respectively administered to the different groups.The levels of MMP-9,TIMP-1 and IL-8 in the induced sputum and serum of the patients from each group were detected by ELISA.Results In the group treated with corticosteroid plus β agonist,the levels of MMP-9,TIMP-1 and IL 8 in sputum and serum were lower in post-treatment than in prmtreatment.The difference showed a statistically significant decrease (P ＜ 0.05).Conclusions Corticosteroid combined with β agonist may be the effective therapy to inhibit airway inflammation and remodeling in COPD.
Clinical Analysis of Efficacy and Safety of Long Term Use of Montelukast Sodium in the Treatment of Patients with Chronic Cough and Negative Bronchial Provocation Test

Abstract:Objective To evaluate the efficacy and safety of 61 cases of chronic cough with negative bronchial provocation test treated by montelukast sodium for 4-8 weeks.Methods A total of 61 cases of patients with chronic cough were selected,of which the bronchial provocation test were all negative.The use of montelukast sodium in the treatment of 4-8 weeks,the chronic cough score of patients were monitored,while liver and kidney function of patients and side effects of related drugs were tracked and analyzed.Results After 4 weeks treatment with montelukast sodium,the symptoms of 37 cases of patients were improved and the cough score was significantly lower.24 patients were used for more than 6 weeks.There were dry mouth and other adverse reactions during the treatment.No serious adverse events occurred.Conclusion For patients with chronic cough and negative bronchial provocation test,the long-term use of montelukast sodium has a good effect and high safety of treatment.

Abstract:CXCR3是CXC趋化因子亚家族的受体之一,属Th1细胞趋化因子受体.最近的研究发现,CXCR3及其配体IFN-γ诱导性肽-10(IP-10)、IFN-γ诱导性单核细胞因子(Mig)、IFN-γ诱导性T细胞α化学趋化因子(I-TAC)在支气管哮喘(哮喘)个体中表达异常,其基因多态性与患哮喘的风险性相关.CXCR3与其配体参与哮喘的免疫调节过程,与哮喘炎症细胞趋化游走、哮喘气道上皮的损伤后修复和重构等环节均有密切关系。

Abstract:哮喘是一种以慢性气道炎症和黏液高分泌为特点的疾病,但黏液的过度分泌在哮喘病理生理中的作用被相对低估了.实际上黏液栓塞对气流受限及气道高反应有相当的影响,严重关系到哮喘的发病率和死亡率.哮喘患者分泌的黏液中主要表现为MUC5AC、MUC5B和MUC2的过度表达,哮喘相关的炎性介质也参与了哮喘黏液高分泌的调控.EGFR级联反应是许多不同刺激导致黏液分泌的共同路径,这些刺激主要包括氧化应激、蛋白酶和细胞因子等.在这些领域的研究有助于我们发现新的抗黏液高分泌药物,并借此加强对哮喘的治疗,提高哮喘患者生活质量.本文从哮喘黏液高分泌特点、调控机制和药物治疗等方面对哮喘黏液高分泌进行综述。

Abstract:有关β2-肾上腺素能受体(β2-AR)基因与哮喘的相关研究大多数都集中于两个常见的编码区非同义突变,即Arg16Gly和Gln27Glu,研究的结果存在着大量的不一致性.近来对β2-AR单倍型的研究表明,β2-AR基因的多个SNP以及它们的相互作用即单倍型在哮喘发病中比这两个常见的多态性更为重要.最近三个meta-分析表明Arg16Gly以及Gln27Glu多态性与哮喘发病无关,尽管已有研究证明它们与哮喘的其它表型相关.β2-AR基因不是哮喘发生的主要危险因素,但其多态性在决定药物反应性方面很可能重要的.本文综述了近年来有关β2-AR基因多态性在哮喘研究中的作用和这些研究的局限性以及将来β2-AR基因多态性的研究方向。

Abstract:The comparison between traditional Chinese medicine Jinzhen Oral Liquid (JZOL) and western medicine in treating children with acute bronchitis (AB) showed encouraging outcomes. This trial evaluated the efficacy and safety of the JZOL for improving cough and expectoration in children with AB. 480 children were randomly assigned to take JZOL or Ambroxol Hydrochloride and Clenbuterol Hydrochloride Oral Solution for 7 days. The primary outcome was time-to-cough resolution. The median time-to-cough resolution in both groups was 5.0 days and the antitussive onset median time was only 1 day. This head to head randomized controlled trial showed that JZOL was not inferior to cough suppressant and phlegm resolving western medicine in treating cough and sputum and could comprehensively treat respiratory and systemic discomfort symptoms. Combined with clinical trials, the mechanism of JZOL against AB was uncovered by network target analysis, it was found that the pathways in TRP channels like IL-1β/IL1R/TRPV1/TRPA1, NGF/TrkA/TRPV1/TRPA1 and PGE2/EP/PKA/TRPV1/TRPA1 might play important roles. Animal experiments further confirmed that inflammation and immune regulatory effect of JZOL in the treatment of AB were of vital importance and TRP channels was the key mechanism of action.

Abstract:Chronic obstructive pulmonary disease(COPD) is a progressive lung dysfunction(disease) caused by long-term inhalation of toxic particles, especially smoking. The continued exposure to harmful substances triggers an abnormal inflammatory response, which causes permanent damage to the respiratory system, ultimately leading to irreversible pathological changes. Lung macrophages(LMs) are key innate immune effectors involved in the recognition, phagocytosis, and clearance of pathogens, as well as in the processing of inhaled hazardous particulate matter(e. g., cigarette smoke and particulate matter). LMs are polarized toward the M1 or M2 phenotype in response to the activation of inflammatory mediators to exert pro-/anti-inflammatory effects, respectively, thus being involved in the pulmonary parenchymal damage(emphysema) and repair(airway remodeling) throughout the process of COPD.In addition, they are responsible for phagocytosis and clearance of apoptotic or necrotic tissue cells, which helps to maintain the stability of the microenvironment in the lungs of COPD patients. Modern studies have revealed that macrophage polarization plays a pivotal role in the pathogenesis and development of COPD and is considered a potential target for treating COPD because of its ability to reduce airway inflammation, inhibit tissue remodeling, and combat oxidative stress. In recent years, traditional Chinese medicine(TCM) and its active ingredients have become a hot area in the treatment of COPD by targeting the balance of M1/M2 macrophage polarization. TCM and its active ingredients can intervene in the inflammatory response to promote the repair of the lung tissue in the patients with COPD. This paper reviews the research achievements of TCM and its active ingredients in this field in recent years,aiming to provide a scientific basis and strong support for the precise diagnosis and treatment of COPD.

Abstract:孟鲁司特是1998年初问世的一种新型预防和治疗慢性支气管哮喘(哮喘)的药物,它是特异性阻断白三烯D4(lukotriene D4,LTD4)与半胱氨酰白三烯1受体结合的高亲和性拮抗剂(LRA).其副作用一般来说都很轻微,但现在不断有一些哮喘患者在服用孟鲁司特后出现了变应性肉芽肿血管炎(Churg-Strauss syndrome,CSS)的表现,并进一步得到确定诊断.孟鲁司特的使用与CSS的发生究竟有无相关性,本文将就此问题综合发表文献进行述评,以期引起临床医生的重视。

Abstract:Most of the drugs that are currently used to treat chronic obstructive pulmonary disease(COPD) interact with receptors expressed by cells in the airways. This makes inhaled delivery efficientbecause it reaches the key target cells and has a low risk of systemic side effects. Both β2-agonists andanticholinergics target autonomic receptors on airway smooth muscle of large and small airways,to developthe effect on bronchodilation. They also interact with receptors expressed on other important target cells,toshow anti-inflammatory effect. Glucocorticoid receptors (GR) are widely distributed so that they are bestgiven by inhalation to interact with intracellular GR in the respiratory tract and to avoid side effects fromactivation of GR in extrapulmonary tissues. There are many other receptor targets that have now beenidentified as these may be future targets for developing novel therapies for COPD.

Abstract:目的探讨β2-肾上腺素能受体(β2- AR)基因16、27位点多态性与支气管哮喘的相关性.方法采用等位基因特异性聚合酶链反应方法,对74例支气管哮喘者进行β2-AR基因多态性分析,并与43例肺炎患者、39名健康者对照.结果①两对照组β2-AR基因16、27位点多态性分布无差异性;②支气管哮喘组β2-AR基因16位点分布频率:精氨酸/甘氨酸占47.2%,甘氨酸/甘氨酸占35.2%,与两对照组比较(p 0.05).结论支气管哮喘的发生与β2-AR基因16位点多态性相关联,与27位点多态性不相关。
