Abstract:Objective:To evaluate the application value of fetal intelligent navigation echocardiography (FINE, 5D Heart) combined with virtual intelligent sonographer assistance (VIS-Assistance?) in the diagnosis of fetal tetralogy of Fallot (TOF).Methods:From February to January 2020, 57 pregnant women who received echocardiography examination for fetuses at Sir Run Run Shaw Hospital were selected as subjects in this study. Doctor A with rich fetal echocardiography experience collected two to five spatio-temporal image correlation (STIC) volume datasets for each fetal heart. The inexperienced doctor B and the more experienced doctor C screened out the best volume dataset in each fetus, and applied 5D Heart and VIS-Assistance? for post-processing. According to different spine positions, 57 TOF fetuses were divided into three subgroups: A (spine 2-4 o'clock), B (spine 5-7 o'clock), and C (spine 8-10 o'clock). Five diagnostic views (three-vessel tracheal view, four-chamber view, left ventricular outflow view, right ventricular outflow view, and abdomen/stomach view) and 17 main diagnostic elements of TOF fetuses were scored, and in particular the following parameters were investigated: the display rates of five diagnostic views and 17 diagnostic elements in the three subgroups and the differences between subgroups; comparison of the display rates of 17 diagnostic elements twice by two doctors with different experience; and repeatability and consistency test of two different senior doctors' scoring results in five diagnostic views.Results:With the application of 5D Heart and VIS-Assistance?, there was no significant differences in the display rates of five diagnostic views and 17 diagnostic elements between the three subgroups (P＞0.05). The display rates of the diagnostic elements of two different senior doctors are as follows: in the three-vessel tracheal view, the display rates of diagnostic elements (dilated aorta, stenosis of pulmonary artery, trachea, and superior vena cava) were 94.7%-96.5%, 93.0%-98.2%, 70.2%-84.2%, and 86.0%-91.2%, respectively. In the four-chamber view, the display rates of the diagnostic elements (four chambers, atrioventricular valves, mitral valve-tricuspid valve distance, ovale foramen, cardiac cross, ventricular septum, and moderator band) were 100.0%, 100.0%, 93.0%-96.5%, 78.9%-82.5%, 94.7%-96.5%, 100.0%, and 100.0%, respectively. In the left ventricular outflow view, the display rates of diagnostic elements (ventricular septal defect, dilated aorta, and the aorta overriding ventricular septal defect) were 96.5%-100.0%, 100.0%, and 96.5%-98.2%, respectively. In the right ventricular outflow view, the display rates of stenosis of the pulmonary artery was 80.7%-91.2%. In the abdomen/stomach view, the display rate of the diagnostic elements (stomach and descending aorta) was 100.0%. There was no statistical differences for the results between two doctors with different experience, and for scores of the five diagnostic views by the same doctor 1 month before and after (P＞0.05).Conclusion:5D Heart combined with VIS-Assistance? has high display rates in various diagnostic views and diagnostic elements in fetal TOF, and has good repeatability and consistency. It has relatively less dependence on the experience of the operating physician, which may has potentials in the prenatal diagnosis of TOF. Using 5D Heart, the fetal spine position within a certain range seems not to affect the TOF image display rate.
Preliminary study of cardiac morphology and function in fetuses with tetralogy of Fallot using fetal heart quantification

Abstract:Objective:To evaluate the fetal heart shape and function in tetralogy of Fallot (TOF) by fetal heart quantitative analysis (fetal HQ).Methods:A total of 52 fetuses with TOF diagnosed by fetal echocardiography and 200 normal fetuses matched with their gestational weeks from March 2020 to March 2022 at Sir Run Run Shaw Hospital, Zhejiang University were retrospectively evaluated. The basic parameters of fetal cardiac blood vessels in the two groups were measured by fetal HQ technology and conventional M-mode ultrasound technology: aortic valve diameter(AV), pulmonary artery valve diameter(PV), main pulmonary artery diameter (MPA) and Z-score. The overall morphometric measurements including end-diastolic length diameter, transverse diameter, area, and global spherical index (GSI) of the fetal heart in the 4-chamber view(4CV), area and length of the right and left ventricles and their ratios. Measurements of left and right ventricular function included ejection fraction (EF), fraction area change rate (FAC), tricuspid annular plane systolic excursion (TAPSE), left and right ventricular global longitudinal strain (GLS), and left and right ventricular end-diastolic diameter (ED), spherical index (SI), and fractional shortening rate (FS) of 24-segments. The differences of above parameters between TOF group and control group were compared. In addition, the relationships between the absolute value of left and right ventricular GLS of TOF fetus and PV/AV, PV Z-score and MPA Z-score were analyzed. The optimal critical values of GSI, left ventricular EF and left ventricular FAC of TOF fetus were determined by ROC curve, and their corresponding sensitivity and specificity were obtained.Results:Compared with control group, there were significant differences in 4CV end-diastolic length, area, GSI, left ventricular area, left ventricular length, left ventricular EF, left ventricular FAC and left ventricular GLS in TOF group (all P<0.05). There were significant differences in ED between left ventricular 15-24 segments and right ventricular 1-21 segments (all P<0.05). There were significant differences in SI between left ventricular 1-16 segments, 21-24 segments and all segments of right ventricle (all P<0.05). The differences in FS were statistically significant (all P<0.05) when comparing all segments of the left ventricle and 1-2 segments of the right ventricular, and the remaining parameters were not statistically significant (all P>0.05). The left ventricular GLS absolute value of TOF fetuses was positively correlated with PV/AV, PV Z-score and MPA Z-score( rs=0.338, 0.441 and 0.458, all P<0.05), the right ventricular GLS absolute value of TOF fetuses was positively correlated with PV AV, PV Z-score and MPA Z-score( rs=0.418, 0.368 and 0.338, all P<0.05). The optimal critical values of GSI, left ventricular EF, and left ventricular FAC in the diagnosis of fetal TOF were 1.19, 59.05%, and 44.4%, respectively. At this time, the sensitivities of diagnosis of TOF were 0.78, 0.75, and 0.80, respectively. The specificities were 0.88, 0.88 and 0.83, respectively. The areas under ROC curve were 0.89, 0.88 and 0.89, respectively. Conclusions:Fetal HQ technology can provide a simple and reliable quantitative evaluation of fetal heart shape and function, and provide certain theoretical parameters for the study of fetal heart shape and function.
A Preliminary Study of Fetal Intelligent Navigation Echocardiography in the Prenatal Diagnosis of Conotruncal Defects

Abstract:Objective To investigate the application of fetal intelligent navigation echocardiography ( FINE ,5D Heart) in the diagnosis of conotruncal anomalies ( CTD ) . Methods For prospective study , volume transducer was used to collect the three-dimensional data of fetal heart at 20th - 30th week of gestation . The STIC volume images were processed by three independent physicians of different hierarchies via 5D Heart software afterwards ,and the quality of the STIC diagnosis as well as the consistence among the physicians were analyzed according to the gold standard of postpartum ultrasonic follow-up or surgery results . Results Twelve out of 16 CTD cases were finally included in the study ,and the volume data were analyzed . Senior physicians A could apply 5D Heart diagnosis to achieve 2D-US diagnosis efficiency and gold standard consistently( P = 0 .157 ,0 .083) ,medium and low seniority physician B and C got roughly equal score using 5D Heart diagnosis( P = 0 .705) ,but the diagnostic performance could not reach 2D-US ( P =0 .000 ,0 .000) and gold standard( P = 0 .000 ,0 .000) . Conclusions Fetal cardiac anomalies of CTD can be diagnosed correctly by an expert using 5D Heart ,which has important application prospects in remote consultation , training and scientific research and teaching of congenital heart disease of fetus .
Study on Diagnostic Performance of Fetal Intelligent Navigation Echocardiography for Congenital Heart Defect

Abstract:Objective:To explore the sensitivity and specificity of fetal intelligent navigation echocardiography(FINE, 5D Heart) in the prenatal detection of congenital heart defect(CHD), and to compare its diagnostic performance with spatiotemporal image correlation(STIC) loop (STICloop).Methods:A total of 250 pregnant women having a singleton pregnancy in the second and third trimesters from April 2018 to May 2019 in Sir Run Run Shaw Hospital, Zhejiang University College of Medicine were enrolled in this prospective study. 2D ultrasound diagnosis has been made before acquiring STIC volume data set. After the appropriateness of each volume data set was preliminarily determined, all identifying informations were concealed and the appropriate volumes were randomly distributed to less experienced sonographer for analysis using 5D Heart software. The sensitivity and specificity, positive and negative likelihood ratios, as well as area under the receiver operating characteristic curve (AUC) of the two methods were determined respectively, and the differences between the two methods were statistically analyzed.Results:The diagnostic performance of 5D Heart for the prenatal detection of CHD was: sensitivity of 95.45% (105/110), specificity of 94.29% (132/140), positive likelihood ratio of 16.72, negative likelihood ratio of 0.05, and AUC of 0.95, respectively. Among cases with confirmed CHD, the 5D Heart diagnosis completely matched the final diagnosis in 77.27% (85/110). The diagnostic performance of STICloop for the prenatal detection of CHD was: sensitivity of 90.91% (100/110), specificity of 92.86% (130/140), positive likelihood ratio of 12.73, negative likelihood ratio of 0.10, and AUC of 0.92. The sensitivity and secificity of 5D Heart in diagnosing CHD were both higher than 90%, and it could successfully detect a broad spectrum of severe fetal CHD. Among cases with confirmed CHD, even the STICloop diagnosis completely matched the final diagnosis in 61.81%(68/110), its diagnostic accuracy was relatively lower than 5D Heart ( P=0.013). However, there were no statistical differences between the two methods in sensitivity and specificity(all P>0.05). Conclusions:The sesitivity and specifity of 5D Heart in diagnosis CHD were both higher than 90%, and it could successfully detect a broad spectrum of severe fetal CHD. STICloop could be used for preliminary screening of CHD, as it has the same high sensitivity and specificity as 5D Heart. Further examination may be recommended when fetal heart abnormalities are suspected.
New Insight from Using Spatiotemporal Image Correlation in Prenatal Screening of Fetal Conotruncal Defects

Abstract:Background: To establish the reference range of the angle between ascending aorta and main pulmonary artery of fetus in the second and third trimester using spatiotemporal image correlation (STIC), and to investigate the value of this angle in prenatal screening of conotruncal defects (CTDs). Materials and Methods: Volume images of 311 normal fetuses along with 20 fetuses with congenital heart diseases were recruited in this cross-sectional study. An offline analysis of acquired volume datasets was carried out with multiplanar mode. The angle between aorta and pulmonary artery was measured by navigating the pivot point and rotating axes and the reference range was established. The images of ascending aorta and main pulmonary artery in fetuses with congenital heart diseases were observed by rotating the axes within the normal angle reference range. Results: The angle between ascending aorta and main pulmonary artery of the normal fetus (range: 59.1˚~97.0˚, mean ± SD: 78.0˚ ± 9.7˚) was negatively correlated with gestational age (r = -0.52; p < 0.01). By rotating the normal angle range corresponding to gestational age, the fetuses with CTD could not display views of their left ventricular long axis and main pulmonary trunk correctly. Conclusion: The left ventricular long axis and main pulmonary trunk views can be displayed using STIC so that the echocardiographic protocol of the cardiovascular joint could be standardized. The reference range of the angle between ascending aorta and main pulmonary artery is clinically useful in prenatal screening of CTD and provides a reliable quantitative standard to estimate the spatial relationship of the large arteries of fetus.

Abstract:Objective To assess the feasibility of Doppler tissue imaging(TDI) evaluation of arrhythmias in the fetuses. Methods Thirty-six normal fetuses between 16 to 41 weeks of gestation were studied to identify color- TDI patterns consistent with the various phases of the cardiac cycle. The characteristics of TDI of mitral (or tricuspid) annulus spectrum of 29 fetuses with arrhythmias were compared with the results obtained with M-mode and/or diastolic mitral color Doppler spectrum in evaluating fetal arrhythmias. The ability and diagnose accuracy of TDI were further evaluated independently by two observers with experience in fetal echocardiography. The agreement between them was assessed with the Kappa statistics. Results TDI examinations including 2D-TDI, M-mode TDI and PW-TDI were acceptably reproducible in all 65 fetuses. The TDI spectrum of mitral(or tricuspid) annulus were readily analyzed and yielding diagnostic results in all 29 fetuses with arrhythmias.The ability and diagnose accuracy of DTI were evaluated independently by 2 observers indicated satisfactory agreements.Conclusions TDI evaluation of the fetal heart is feasible and reproducible, this technique may aid in the diagnosis and understanding of fetal arrhythmias.

Abstract:Objective To investigate the value of diagnosis and evaluation of MRI in tetralogy of Fallot(TOF)through retrospectively analyzing the MRI manifestation of TOF.Methods We collected 15cases of TOF that proved by operation and 20normal volunteers as references.We summarized the MRI manifestation of TOF,and songht the differences between TOF group and normal group.Results Ventricle defect,right outflow tract or pulmonary stenosis,right ventricle hypertrophy and aorta overriding were revealed well through MRI.The right outflow tract diameter was(9.47±2.80)mm,narrower than normal group(17.75±2.31)mm,the thickness(11.67±2.77)mm of the free wall of the right ventricle was thicker than normal group(3.50±0.83)mm,the ratio of left ventricle wall thickness(0.86)to left ventricle wall thickness was bigger than normal group(2.87),the aorta root and ascending aorta diameter was bigger than normal group,the differences were all signifant.The average ratio of aorta overriding was about 50.47%.Conclusion Not only diagnosing TOF accurately,MRI also had the value of evaluating right outflow tract,right ventricle hypertrophy and ascending aorta.
Diagnostic Value of Parasternal Pulmonary Artery Short-Axis View for the Anomalous Origin of the Left Coronary Artery from the Pulmonary Artery

Abstract:Objective: To evaluate the diagnostic value of parasternal pulmonary artery (PA) short-axis view for the anomalous origin of left coronary artery (LCA) from the pulmonary artery by echocardiography.Methods: A total of 13 patients (3 boys) aged from 2 months to 12 years were enrolled. Transthoracic echocardiography, including cross-sectional imaging and color Doppler flow imaging, were performed, and their diagnoses were confirmed by operation.Results: Among the 13 patients, 7 had LCA originated from the left posterior wall of PA, 2 from the posterior wall, and 4 from the right posterior wall. The PA short-axis view could visualize the anomalous origin of the LCA from left posterior or posterior wall of PA clearly. The LCA and aortic wall were overlapping at 3-4 o'clock at PA short-axis view in the patients with LCA originating from the right posterior wall of PA. It was similar with the image of the LCA originating from the aorta. But the blood flow was opposite to that of LCA with normal origin.Conclusions: The parasternal PA short-axis view is a good view to visualize the anomalous origin of the LCA. Echocardiographyic Diagnosis of Tetralogy of Fallot with Absence of Left Pulmonary Artery

Abstract:Purpose: To study the value and the features of echocardiography in the diagnosis of Tetralogy of Fallot(TOF) with absence of left pulmonary artery. Materials and Methods: The echocardiographic characteristics of TOF with absence of the left pulmonary artery in five cases were compared with the results of cardiac catheterization and open heart surgery. The diagnostic features of echocardiography were summarized. Results: In addition to the common features like intracardiac malformations in TOF, the cases with absence of left pulmonary artery showed stenosis in their main pulmonary arteries. A circular shaped bifurcation of pulmonary arteries were noticed in 4 cases, and an absence of left pulmonary artery in 1. The echocardiographic diagnosis in all cases were in accordance to that of cardiac catheterization and operation. Conclusion: Echocardiography is an accurate diagnosis for TOF with absence of the left pulmonary artery, especially when a circular shaped bifurcation of pulmonary artery noted in children with TOF.
Noninvasive perioperative evaluation of right ventricular function in children with tetralogy of Fallot.

Abstract:Early and accurate noninvasive means of identifying right ventricular (RV) dysfunction in children with tetralogy of Fallot (TOF) are needed. RV function was examined using tissue Doppler imaging (TDI), strain rate (SR), and strain analysis (SA) in children before (N = 37) and after (6-12 months; N = 32) TOF repair, and in a control group of children (N = 37). Plasma concentrations of N-terminal pro-brain natriuretic peptide (NT-proBNP) and matrix metalloproteinase 9 (MMP-9) were measured. TDI, SR, and SA revealed that RV systolic and diastolic function indices were lower preoperatively in the TOF group compared with the control group, and did not improve after TOF repair. Plasma NT-proBNP concentrations were significantly higher in the TOF group pre- and postoperatively compared with the control group. In the preoperative TOF group, NT-proBNP concentration was significantly correlated with peak systolic SR and systolic strain in the mid segments of RV free wall. Plasma MMP-9 concentrations were significantly increased in the preoperative TOF group compared with the control group, and significantly correlated with plasma NT-proBNP and logNT-proBNP concentrations. RV function correlated with plasma NT-proBNP concentrations in children with TOF. Assessment of this noninvasive measure may help identify RV dysfunction in patients with TOF before they become clinically symptomatic.
Real Time Three-dimensional Echocardiographic Topologic Diagnosis in Tetralogy of Fallot

Abstract:Objective To explore the diagnostic values of transthoracic real-time three-dimensional echocardiography (RT-3DE)and 3D topologic methods in the diagnosis of children tetralogy of Fallot (TOF).Methods Fifty-nine children with TOF were diagnosed on RT-3 DE using topologic method.The results were compared with two-dimensional echocardiography (2 DE)and surgical operations(golden standard).Results Ventricular-arteries,upward valvular view,right side and left side of interventricular septum(IVS)3D views could be obtained through topologic four-chamber 3D views,which were intuitive views for heart segmental analysis.Compared with surgical operations,the accuracy of RT-3DE and 2DE were closely consistent(P>0.05).Conclusions TOF can be accurately diagnosed through four-chamber 3D view combined with pulmonary artery longitudinal axis 3D veiw.RT-3DE with topologic method can give more special informations and it is more valuable in the diagnosis of TOF than that of 2DE.
Detection of anomalous origin of left coronary artery from pulmonary artery in children with 64-slice computed tomography

Abstract:Objective To explore the value of 64-slice computed tomography (CT) coronary artery angiography in detecting anomalous origin of left coronary artery from the pulmonary artery (ALCAPA) in children.Methods Twelve children who were confirmed with ALCAPA in surgical treatment and underwent electrocardiograph (ECG)-gated 64-slice CT coronary artery angiography were enrolled in this retrospective analysis.They were 4 boys and 8 girls with a median age of 7 months (ranging from 1 month to 9 years) and a median weight of 7 kg (ranging from 5 to 28 kg).The children were give sedatives forscanning.The scanning were performed with 80 kV tube voltage,100-130 mA tube current,0.625 mm slice thickness,0.35 s rotation time and 0.24 pitch,without controlling the heart rate.The examination ranged from the aortic arch to cardiac apex.Two pediatric radiologists with more than 5 years of experience in cardiac image independently assessed the multi-slice spiral CT (MSCT) image at AW 4.3workstation.Results ALCAPA was detected by MSCT in all the children.During the MSCT,the effective radiation dose was 1.62-2.15 mSv and heart rate was 100-150 per minute.The best reconstructed phase was between 35％ and 50％ of the R-R interval.The left coronary artery originated from the left wall of the pulmonary artery in 3 patients,from the right wall of the pulmonary artery in 3,from the posterior-left sinuses of the pulmonary artery in 3 and from the posterior wall of the pulmonary artery in 3.The right coronary artery originated from the right coronary sinus in all patients.CT coronary angiography also showed enlargement of the left heart in all patients.The diagnosis of ALCAPA was confirmed by surgery in all patients.Conclusion The 64-slice CT coronary artery angiography can reveal the location of the ectopic ostium and the course of left coronary artery in children with ALCAPA.
CT Quantification of Ventricular Volumetric Parameters Based on Semiautomatic 3D Threshold-Based Segmentation in Porcine Heart and Children with Tetralogy of Fallot: Accuracy and Feasibility

Abstract:BackgroundTo investigate the accuracy and feasibility of CT in quantification of ventricular volume based on semiautomatic three-dimensional (3D) threshold-based segmentation in porcine heart and children with tetralogy of Fallot (TOF).MethodsEight porcine hearts were used in the study. The atria were resected and both ventricles of the eight porcine hearts were filled with solidifiable silica gel and performed CT scanning. The water displacement volume of silica gel casting mould was referred as gold standard of ventricular volume. Results of left and right ventricular volumes measured by CT were compared with reference standard. Twenty-three children diagnosed with TOF were retrospectively included. The ventricular volumetric parameters were assessed by cardiac CT before and 6 months after surgery.ResultsLeft ventricular and right ventricular volumes of porcine hearts measured by CT were highly correlated to casting mould (r=0.845, p=0.008; r=0.933, p=0.001), and there were no statistically significant differences (t=−1.059, p=0.325; t=−1.121, p=0.299). In children with TOF, right ventricular end-systole volumes 6 months after operation were higher than that before surgery, 21.93±4.44 vs 19.80±4.52 mL/m2, p=0.001. Right ventricular ejection fractions 6 months after surgery were lower compared with that before surgery 59.79%±4.26% vs 63.05%±5.04%, p=0.000.ConclusionsCT is able to accurately assess ventricular volumetric parameters based on semiautomatic 3D threshold-based segmentation. Both of the right and left ventricular volumetric parameters could be evaluated by CT in children with TOF.

Abstract:Objective:To evaluate the diagnostic value of anomalous origin of the left coronary artery from the pulmonary artery(ALCAPA) by doppler echocardiography,to analyze misdiagnosis as well as to improve the echocardiographic diagnosis.Methods:Doppler echocardiography was performed and this imaging was compared with angiocardiographic findings,or with MRI findings in 17 patients.Results:Eleven cases(64.7%) were diagnosed correctly,6 cases(35.3%) were misdiagnosed.In the 6 misdiagnosis cases,3 cases were misdiagnosed as dilated cardiomyopathy,2 cases as coronary pulmonary fistula,1 case as mitral regurgitation.Conclusion:ALCAPA could be diagnosed accurately by doppler echocardiography.We should pay more attention when dialated left ventricle combined with intercoronary collaterals within the ventricular septum.Angiography or MRI should be perfomed in case in which the left coronary artery is not clearly demonstrated by echocardiography.

Abstract:PURPOSE:To review the imaging characteristics and evaluate the diagnostic value of echocardiography for diagnosing anomalous origin of the right coronary artery from the pulmonary artery (ARCAPA).METHODS:We retrospectively reviewed the echocardiographic records and compared these images with operative findings in six children with ARCAPA.RESULTS:ARCAPA was characterized by dilation of the left coronary artery, inappropriate origin of the right coronary artery from the pulmonary artery, and collaterals within the interventricular septum. The associated malformations included atrial septal defect in four cases. Four of six cases were diagnosed correctly, while the remaining two cases were misdiagnosed: one of fistula between the pulmonary artery and the left coronary artery and another of ARCAPA diagnosed intraoperatively in one case of atrial septal defect.CONCLUSION:ARCAPA can be diagnosed by echocardiography but is prone to misdiagnosis. This malformation should be distinguished from coronary artery fistula.

Abstract:Objective To evaluate the diagnostic value of prenatal echocardiography in fetus with tetralogy of Fallot(TOF),and to analyze the reason of misdiagnosed in order to improve the echocardiographic diagnosis.Methods A retrospective echocardiographic review of 36 fetuses with TOF confirmed by prenatal magnetic resonance imaging (MRI) or postnatal echocardiography.Results Among 36 cases of TOF fetuses,31 cases (86.1 ％) were diagnosed correctly by echocardiography,and in the remaining 5 cases,4 cases(11.1％) were misdiagnosed [2 cases misdiagnosed as ventricular septal defects (VSD),2 cases misdiagnosed as double outlet of right ventricles],and 1 case (2.8％) was missed diagnoses at 20 weeks gestation and confirmed as TOF at 36 weeks gestation based on the echocardiographic findings of malalignment type VSD,overriding aorta,pulmonary stenosis,and right ventricular hypertrophy.Malalignment type VSD,overriding aorta,and pulmonary stenosis were observed in 31 cases with TOF diagnosed by prenatal echocardiography,right ventricular hypertrophy was observed only in 6 cases.The most common associated anomalies were tricuspid regurgitation in 13 cases(36.1％),aortic insufficient in 6 cases(16.7％),absent pulmonary valve in 4 cases(11.1％),right-side aortic arch in 4 cases(11.1％),persistent left superior vena cava in 3 cases(8.3％),and complete atrioventricular septal defect in 1 case (2.8％).Conclusions Combining visualization of TOF in four-chamber view,outflow tract view,and three vessels view,TOF can be accurately diagnosed by prenatal echocardiography.

Abstract:Objectives To investigate the diagnosis and treatment of anomalous origin of left coronary artery from the pulmonary artery.Methods Electrocardiogram,echocardiography,cardiac angiography and surgical treatment were reviewed in 16 patients with anomalous origin of left coronary artery from the pulmonary artery.Results(1)Electrocardiogram:T wave reversed in leadsⅠ,Ⅱ,aVL,V5,V6 and ST segment depressed in 9 patients,6 patients had abnormal Q wave.T wave reversed in leadsⅠ,Ⅱ,aVL,V5 and V6 were found in 4 patients.(2)Echocardiography:16 patients were diagnosed by echocardiography.Two patients accompanied with atrial septal defect(ASD).Left ventricular ejection fraction(LVEF)declined in 6 patients.(3)Fifteen patients underwent cardiac angiography.Two patients accompanied with ASD.One patient accompanied with patent ductus arteriosus.(4)Surgical treatment:All patients underwent surgical treatment.Two patients were dead.Conclusions Characteristic EKG is helpful in the diagnosis of anomalous origin of left coronary artery from the pulmonary artery.Echocardiography has important values in the diagnosis of this disease.Combined with cardiac angiography,the diagnosis can be made more accurate.When the diagnosis is established,the surgery should be performed as early as possible.
A Pilot Study of a Cardiovascular Virtual Endoscopy System Based on Multi-Detector Computed Tomography in Diagnosing Tetralogy of Fallot in Pediatric Patients.

Abstract:The present study aimed to investigate the capabilities of the cardiovascular virtual endoscopy (VE) system in diagnosing tetralogy of Fallot (TOF) and performing measurements. A total of 37 patients underwent two-dimensional echocardiography (2-DE) and multi-detector computed tomography (MDCT) examinations. The obtained MDCT images were applied to a cardiovascular VE system. Diagnostic time by VE was first studied and compared with MDCT. Subsequently, with surgical findings as the ground truth, the capabilities of VE, 2-DE and MDCT in diagnosing TOF and its complications were investigated. Additionally, measurements on aorta overriding ratio and diameters for the left pulmonary artery, right pulmonary artery and right ventricular outflow tract by 2-DE and VE were analyzed. Diagnostic time by VE was significantly shorter than MDCT (188±42 vs. 303±42 sec, respectively; P<0.0001). VE, MDCT and 2-DE demonstrated comparable diagnostic rates of TOF (35/37 vs. 34/37 vs. 32/37, respectively; P>0.05). Similar findings were demonstrated in diagnosing complications of the muscular ventricular septal defects, patent ductus arteriosus, vagus subclavian artery, right arch, double superior vena cava and pulmonary artery. Furthermore, in diagnosing the atrial septal defect, 2-DE outperformed MDCT and VE (accuracy, 100 vs. 81 vs. 73%, respectively; all P<0.05). In performing relevant measurements, VE outperformed MDCT and 2-DE, particularly in accessing aorta overriding ratios with no intra-operator difference (P=0.3770) and high consistency (r=0.916). In conclusion, cardiovascular VE was demonstrated to have acceptable accuracy in diagnosing TOF, and possess advantages in shortening the diagnostic time and in performing measurements.

Abstract:INTRODUCTION To explore the cytological characteristics of tetralogy of Fallot (TOF), we collected samples and studied the differences in cytological classification between normal fetal hearts and fetuses with TOF, and then we searched for possible differential genes of disease markers through single-cell sequencing analysis. METHODS In this study, we analyzed the right ventricle of a TOF and a healthy human fetal heart sample by single-cell sequencing. Utilizing Cellranger to perform data quality control filtering, comparison, quantification, and identification of recovered cells on the raw data, ultimately obtaining gene expression matrices for each cell. Subsequently, Seurat was used for further cell filtration, standardization, cell subgroup classification, differential expression gene analysis of each subgroup, and Marker gene screening. RESULTS Bioinformatic analysis identified 9979 and 15224 cells derived from the healthy and disease samples, respectively, with an average read depth of 25000/cell. The cardiomyocyte cell populations derived from the abnormal samples identified by the first-level graph-based analysis were separated into six distinct cell clusters. CONCLUSIONS Our study reveals some information on TOF in a fetus, which can provide a new reference for early detection and treatment of TOF by comparing it with normal heart cells.
