Analysis of Pathogen Distribution and Resistance Phenotype in Bile Culture after Liver Transplantation

Abstract:Objective To analyze the pathogen distribution and resistance phenotype in bile specimens after liver transplantation and provide a reference for the clinical diagnosis,prevention and treatment of biliary infection after liver transplantation.Method From January 2009 to September 2013,a total of 1950 bile specimens from liver transplant recipients were cultured in parallel with pathogen susceptibility testing.A comprehensive analysis of pathogens with susceptibility testing was undertaken in positive culture results.Result 1348 specimens in 1950 cases of bile cultures showed positive for pathogens,with the positive rate being 69.13%.1436 strains of pathogens were detected,including 777 strains of Gram-negative bacteria,mainly Escherichia coli (254 strains,17.69%) and Klebsiella pneumoniae (133 strains,9.26%); 607 strains of Gram-positive bacteria,predominantly Enterococcus faecium (162 strains,11.28 %) and Staphylococcus epidermidis (131 strains,9.12 %),followed by Enterococcus faecalis (109 strains,7.59%).Besides,52 strains of fungus were also found,41 of which were Candida albicans,and the other four were Candida tropicalis,Candida lusitaniae,Candida glabrata and Trichosporon asahii,with the quantity of 6,3,1,1 respectively.Strong multi-drug resistance was observed in main pathgens,except fungi.Conclusion The positive rate for pathogens in bile culture specimens after liver transplantation was high.The drug resistance for bacteria was strong and wide,and that for fungi was not obvious.
