Retrospective analysis on cryotherapy for bronchial tuberculosis combined with lymph fistula by fiberoptic bronchoscopy under general anesthesia

Abstract:Objective To observe the clinical effect of cryotherapy for bronchial tuberculosis combined with lymph fistula by fiberoptic bronchoscopy under general anesthesia.Methods From January 2015 to December 2017,20 patients with bronchial tuberculosis combined with lymph fistula were treated with cryotherapy by fiberoptic bronchoscopy under general anesthesia,including 15 patients with unilateral fistula and 5 patients with bilateral fistula.On the basis of the conventional antituberculosis chemotherapy,bronchoscopic cryotherapy and local spray of isoniazid 0.2 g were performed every two weeks.The treatment was performed for 2-9 times,with the average of 4.6 times.The oxygen saturation (SpO2),heart rate,blood pressure,body movement,nausea and choking cough were recorded during the treatment,and the clinical symptoms and bronchial status of the patients were recorded before and after cryotherapy,too.Results All of the 20 cases of bronchial tuberculosis combined with lymph fistula had stable vital signs,with no body movement,nausea or choking cough affecting the operation during the treatment.The SpO2 was kept at ＞95％.The patients woke up 3-5 minutes after operation and had no painful memory of the operation process.In the course of treatment,the bleeding volume of 4 patients was relatively high,which was controlled by spraying epinephrine (1∶10 000) and thrombin (2 000 U).No serious complications,such as respiratory,cardiac arrest,or persistent malignant arrhythmia,occurred.The clinical symptoms disappeared and fistula was healed after the treatment,and no operative death was found.The status of all the patients were stable after half a year's follow-up.Conclusion The efficacy of cryotherapy under general anesthesia for bronchial tuberculosis combined with lymph fistula is reliable.The treatment time is relatively short,the patients in the treatment process have no pain,and less complications and low risk occur.Therefore,this method is worthy of being clinically performed.
