Abstract:病历摘要患者男,72岁,在无明显诱因下出现发热,体温在39 ℃以上,畏寒,轻微咳嗽,痰少,无咽痛流涕,无腹痛腹胀,无尿频尿急,无乏力盗汗.在当地医院查血常规示WBC 1.5×109/L,Hb 132 g/L,PLT 100×109/L;骨髓穿刺示粒系增生低下;CT示两肺支气管肺炎表现.给予青霉素类、左氧氟沙星抗炎治疗(具体不详),体温仍反复升高;后又出现全身皮疹,无瘙痒,抗过敏治疗后皮疹无消退.为进一步诊治,患者于2010年5月5日收住浙江大学医学院附属第一医院。

Abstract:An 11-year-old girl presented with frequent chest pain, fever, and a cough that she had had for more than 13 months, as well as pleural effusion and large pericardial effusion. She was misdiagnosed with tuberculosis and received anti-tuberculosis drugs for 6 months. Within the past year, she also underwent two more thoracotomies and a thoracoscopic partial pericardiectomy. The final diagnosis of pulmonary paragonimiasis was established once it was known that she had eosinophilia, always drank stream water, and tested positive for antibodies against Paragonimus. Since antiparasitic praziquantel therapy was effective, paragonimiasis should be considered as a possibility in the differential diagnosis of tuberculosis in children.

Paragonimiasis in Chinese Children: 58 Cases Analysis.

Abstract:OBJECTIVE:To analyze the clinical and radiological features of paragonimiasis in children and raise the awareness of this disease.METHODS:A total of 58 paragonimiasis patients were reviewed. They were 42 boys and 16 girls aged 2.0 to 15.3 years.FINDINGS:Among these patients, 20 were diagnosed in the recent 5 years, 46 with a history of raw water or food ingestion. Except 2 patients without any complaint, the most common features involved the systemic (41, 70.7%) and respiratory systems (43, 74.1%), followed by abdominal, cardiac and nervous systems, with rash and mass. Eosinophilia was noted in 46 (79.3%) patients, granulocytosis in 45 (77.6%), anemia in 14 (24.1%), and thrombocytopenia in 3. Imageology showed pneumonia in 26 (44.8%) patients, pleurisy in 28 (48.3%), hydropericardium in 17 (29.3%), ascites in 16 (27.6%), and celiac lymphadenitis in 13 (22.4%). Besides hepatomegaly and splenomegaly, calcification and multiple lamellar low echogenic areas in the liver were noted, each in one patient. Abnormal brain imaging was noted in 4 of 10 patients. Karyocyte hyperplasia with eosinophilia was noted in all the 19 patients who received bone marrow puncture.CONCLUSION:Paragonimiasis should be considered in the differential diagnosis of patients with multiple organs or system lesions, especially those with eosinophilia, serous cavity effusion, respiratory, cardiac, digestive system, nervous system abnormality, and/or mass. Healthy eating habit is helpful for paragonimiasis prevention.

Abstract:Objective To study the clinical characteristics and treatment of patients with severe influenza A/H1N1.Method Clinical data of 53 patients with severe influenza A/H1N1 were retrospectively analyzed.Result Severe Influenza A/H1N1 occurred in the age range of 20~40 years. 20 cases (37.74%) were accompanied by a variety of underlying diseases. 51 cases (96.23%) were accompanied by fever,the initial symptom of 48 patients (90.57%),with accompanying symptoms such as chills,cough,expectoration,fatigue,chest tightness,shortness of breath and other symptoms. Eukocyte and neutrophil levels were normal or decreased at early stage. Chest imaging examinations showed 33 cases (62.26%) of patients developed various degrees of secondary bronchitis or pneumonia. All patients,except two against-advise discharges,were discharged after treatment. Conclusion Severe influenza A/H1N1 begins abruptly. Early diagnosis and early reasonable therapy may be effetive for better prognosis.

Abstract:BackgroundAlthough people of all ages are susceptible to the novel coronavirus infection, which is presently named "Coronavirus Disease 2019" (COVID-19), there has been relatively few cases reported among children. Therefore, it is necessary to understand the clinical characteristics of COVID-19 in children and the differences from adults.Case presentationWe report one pediatric case of COVID-19. A 14-month-old boy was admitted to the hospital with a symptom of fever, and was diagnosed with a mild form of COVID-19. The child's mother and grandmother also tested positive for SARS-CoV-2 RNA. However, the lymphocyte counts were normal. The chest computed tomography (CT) revealed scattered ground glass opacities in the right lower lobe close to the pleura and resorption after the treatment. The patient continued to test positive for SARS-CoV-2 RNA in the nasopharyngeal swabs and stool at 17days after the disappearance of symptoms.ConclusionThe present pediatric case of COVID-19 was acquired through household transmission, and the symptoms were mild. Lymphocyte counts did not significantly decrease. The RNA of SARS-CoV-2 in stool and nasopharyngeal swabs remained positive for an extended period of time after the disappearance of symptoms. This suggests that attention should be given to the potential contagiousness of pediatric COVID-19 cases after clinical recovery.

Abstract:There was a 16-year-old adolescent girl complaining of intermittent convulsions in 12 hours.Forty days before her admission,she fell from a building and had mental and behavioral disorders since then;she wound her left foot and got infected.After her admission,she had a fever,hypertension (blood pressure up to 25.3/16.7 kPa),and elevation of C-reactive protein (CRP).At the beginning of admission,there was a suspect that she had brain abscess because of her fever,elevated CRP and an infectious wound in left foot,and the high blood pressure might be attributed to the intracranial hypertension.But soon,it was found that the hypertension could not be explained by intracranial hypertension because she had no other signs of high intracranial pressure.Therefore,the hypertensive encephalopathy might be resulted in convulsions.The head magnetic resonance imaging diffusion weighted imaging displayed high signal mainly in the head including occipital,frontal and parietal lobes.These images were in accordance with posterior reversible encephalopathy and supported the diagnosis of hypertensive encephalopathy.Further examination for the cause of hypertension showed that her left kidney was small and functioned poorly,and the left renal artery was malformed.After antihypertensive and symptomatic treatment,the girl's condition was significantly improved.Her blood pressure became stable (11.3-15.9 kPa / 8.7-11.3 kPa).The lesions on head magnetic resonance imaging were significantly reduced.Conclusions:More attention should be paid on basal physical examination (such as measurement of blood pressure),and multidisciplinary collaboration should be emphasized and developed.

Abstract:Objective To examine the association of characteristics of patients visiting fever clinic with different air quality index in Beijing.Methods Six days with highest ambient air quality index (AQI) (polluted weather group) and 6 days with lowest AQI (clear weather group) during February 1 st to March 31th 2014 were selected as study period.The clinical data of patients,who visited in fever clinic of Peking University Third Hospital during the selected 12 days,were retrospectively analyzed.Results There were 417 patients in clean weather group and 481 in polluted weather group.There were no significant differences in duration of fever [(2.30 ± 2.51) d vs.(2.18 ± 2.58) d,x2 =0.701,P =0.484] and highest body temperature [(38.60 ± 0.65) ℃ vs.(38.57 ± 0.66) ℃,x2 =0.545,P =0.586] between two groups.The symptoms of cough,sore throat in pollution group were higher than those in clear group (52.39％ vs.45.56％,x2 =4.166,P =0.041;50.52％ vs.42.21％,x2 =6.203,P =0.013,respectively).The proportion of cases diagnosed as influenza-like illness in polluted weather was higher than that in clear weather group (53.85％ vs.46.76％,x2 =4.484,P =0.034).Conclusion The results indicate that the symptoms cough and sore throat and influenza-like illness are increased in polluted weather in Beiiing.
Clinical analysis of 179 patients of community-acquired pneumonia in out-patients

Abstract:Objective To analyse the clinical features of community-acquired pneumonia(CAP)in out-patients.Methods A retrospective study was performed on 179 consecutive patients with CAP in fever clinic of Xuanwu Hospital during two months.Results Most of CAP patients were young(aged 14 to 30 years,accounting for 58.1%)and had no-existing diseases(72.6%).Within 2 to 4 days after onset of fever,50%~80% of patients were diagnosed as CAP.About 1/3 of patients had no respiratory symptoms except for fever.Cough and sputum production was seen in 52.5% of patients.WBC was normal in 63.1%.Unilateral pneumonia often occurred(accounting for 82.1%).After treatment with antibiotics,fever in 80%(42/52)of cases disappeared within 3 days in follow-up patients.Conclusion In fever clinic,most patients are young and have no-existing diseases.They can be identified as CAP quickly and have good outcomes.

Abstract:The baby was admitted for intermittent seizures 10 minutes after birth.She was full-term infant girl,with body mass 2800 g,delivered with the help of forceps for fetal distress and 7 hours premature rupture of membrane,without asphyxia history.Seizures appeared 10 minutes after birth and continued until the baby was sent to NICU.Blood glucose,electrolytes,and metabolic screening test of blood and urine were all normal.The brain MRI showed HIE signs.Seizure was under control after phenobarbital and midazolam were given.At the age of 90 hours,the baby had lethargy,elevated WBC count and C reactive protein (CRP) were noted.Feces test showed increased WBC.Positive blood culture,extended-spectrum beta-lactamase (-),e.Coli,supported diagnosis of sepsis.Although aggressive antibiotics,meropenem was given,the baby's CRP and WBC in stool remained unchanged.Long lasting low to moderate fever,vomiting,gastric retention and failure to thrive persisted.Allergy was ruled out.Inflammatory bowel disease was considered and confirmed through colonoscopy and local biopasy.It is suggested that such a disease might progress rapidly in neonates,so intensive monitoring is quite necessary.When dealing with complicated clinical cases,the physician should have a clear and open mind,focus on the core problem,while searching for multiple explanation to other diseases.Multidisciplinary collaboration was important in coping with the disease.
