Diagnostic Value of Transbronchoscopic Biopsy Guided by Miniprobe Ulthrasonography for Pulmonary Peripheral Lesions

Abstract:Objective To evaluate the value of transbronchoscopic biopsy guided by miniprobe ulthrasonography (MPS) for pulmonary peripheral lesions .Methods From January 2017 to December 2017 ,50 patients with extrapulmonary lesions identified by computed tomography (CT) examination in Northern Jiangsu People's Hospital affiliated to Yangzhou University were enrolled .During the examination of bronchoscopy ,MPS was used to guide the biopsy of the lesions . The positive diagnosis rate ,operating time ,and incidence of adverse reactions were observed .The correlation between positive diagnosis rate and some influence factors was also observed ,including the size of the lesion ,whether there were bronchial truncation signs on the chest CT ,and whether there was bleeding when the probe was explored .Results 37 of the 50 patients had a definite diagnosis ,and the total positive diagnosis rate was 74 .0％ .Further sub-group analysis found that the positive rate was 78 .6％ (22/28) and 68 .2％ (15/22) respectively with maximum diameter of lesions ≥ 3 cm and < 3 cm ,and the positive diagnosis rate between the two groups was not statistically significant ( χ2=0 .691 , P =0 .520) .The positive rate was 89 .5 ％ (17/19) and 64 .5％(20/31) respectively with or without bronchial truncation on the chest CT ,and there was no statistically significant difference in the positive diagnosis rate between the two groups ( χ2=3 .841 , P =0 .095) .The positive rate was 95 .7％ (22/23) and 55 .6％ (15/27) with or without bleeding at the site of the lesion during probe exploration ,and the positive diagnosis rate between the two groups was statistically significant ( χ2=10 .378 , P =0 .001) .Operating time varied from 10 to 30 minutes ,with an average operating time of 17 minutes .The overall adverse reaction rate was 20 .0％and there were no serious complications .Conclusions MPS is of high value in the diagnosis of pulmonary peripheral lesions by guiding bronchoscopy ,and this technique is worth promotion .
