Bacterial spectrum characteristics and antibiotic resistance patterns of posttraumatic chronic osteomyelitis: a multi-center retrospective study

Abstract:Objective To characterize the bacterial spectrum and antibiotic resistance of posttraumatic chronic osteomyelitis in 4 medical institutions.Methods The clinical data of the patients who had been admitted into the 4 hospitals for posttraunatic chronic osteomyelitis from January 2012 to September 2015 were retrospectively analyzed.The tissue of visible or suspicious bone infection had been subjected to bacterial cuhure and drug sensitive test in surgery.The tissue specimens were divided into a G group and a G + group.Results A total of 29 species of 334 strains of pathogenic bacteria were isolated.Of then,19 species of 145 strains were Gram-negative bacteria while 10 species of 189 strains Gram-positive ones.The most common bacterial pathogens isolated in G group were Pseudomonas aeruginosa (39 strains;26.90%) and Escherichia coli (26 strains;17.93%),the most effective antibiotic for both of which was Meropenem.The most common bacterium Staphylococcus aureu (110 strains) was found in 58.20% of the patients in G+ group,followed by Staphylococcus epidermidis (45 strains;23.80%).A high percentage of G+ bacteria were susceptible to vancomycin.Conclusions Pseudomonas aeruginosa and Staphylococcus aureu were the most common bacterial pathogens isolated from the patients with posttraumatic chronic osteomyelits in this setting.There were more species of Gram-negative bacteria than those of Gram-positive ones.A drug sensitive test was very essential to rational administration of antibiotics.
