Abstract:OBJECTIVE To investigate the infection factors and countermeasures of puerperal hospital infections.METHODS The cases of puerperal hospital infection in recent 5 years were analyzed by retrospective survey.RESULTS The rate of infection was 1.02%,the top three infections sites were respiratory system(52.79%),surgical incision(21.32%) and digestive system(8.63%).The infection rate of cesarean section was higher than delivery vagina operation.Malnutrition,prolonged labor,premature rupture of membrane,multiple vagina checks,bad economic conditions,obesity and so on were the risk factors of puerperal hospital infection.CONCLUSION There are many relevant factors of puerperal hospital infection,scientific prenatal care,postnatal recovery,aseptic operation,proper procedure of the labor,strict infection management,and reasonable use of antibiotics are the key to prevent and control the puerperal hospital infections.

Abstract:Objective: To study the impact of informed choice on prevalence of induced abortion.Methods: A questionnaire survey was conducted in 10 clinics of family planning station.The respondents were married women of reproductive age.Each woman had an unexpected pregnancy due to the failure of intrauterine device(IUD) or condom or oral contraceptive(OC).Results: The main reasons for IUD failure were the IUD translocation and expulsion,accounting for 75.5%.The majority of IUD failure occurred in the health centers of township and street.Most OC and condom failures were attributed to "typical use".Conclusion: Further strengthening the propaganda and popularizing the knowledge of emergency contraception,taking the routine counseling and follow-up seriously,and improving the technical service capabilities of the grassroots are required to improve the efficiency of contraception,and protect against unexpected pregnancies,and promote reproductive health.
Current situation of the midwifery skills on dystocia among 154 hospitals in Zhejiang Province

Abstract:Objective:To investigate and analyze the situation of midwifery skills on dystocia among employees from different hospitals and related medical staff in Zhejiang Province.Methods:Totally 1 162 in-service staff from 154 hospitals in Zhejiang Province in 2021 joined the project in 2021. Chi-squared test was used to compare the situation of midwifery skills on dystocia from different hospitals. The results were expected to improve the management capacity on dystocia for midwives in these hospitals.Results:Among the 154 hospitals, the overall conducting rates of midwifery skills were different, management of shoulder dystocia and forceps operation/fetal head absorption were most likely to be provided by hospitals [with the proportions as 54.5% (84/154), 55.2% (85/154), respectively], and other skills were less provided; and the conducting rate of three midwifery skills were higher in Level-III hospitals than those in Level-II hospitals, including forceps operation/fetal head absorption[68.0%(51/75) vs. 43.9%(29/66)], vaginal delivery of twins[29.3%(22/75) vs. 7.6%(5/66)] and abdominal version for breech presentation [36.0%(27/75) vs.12.1%(8/66)], the differences of were all significant ( P<0.05). As for midwives and obstetricians, the mastery rates of different skills varied. However, 26.7%(310/1 162) of them actually do not master any skills. Conclusions:The conducting rates of midwifery skills on dystocia were not satisfying among hospitals in Zhejiang Province, and the managing capacity for dystocia of the midwives and obstetricians needs to be improved, the measures should include reasonable resources allocation, supporting first and second-level hospitals, strengthening the standardized training and clinical practice of midwives based on an individualized strategy for midwives with different training experience.

Abstract:Because of the legal regulations restriction and the patients' awareness of rights protection,the current clinical practice for the students of obstetrics and gynecology has encountered unprecedented resistance.To ensure the quality of the medical students' clinical practice,as well as,to cultivate the ability of their clinical operation,simulation education has become an effective way to step out of this dilemma.Nevertheless,the clinical teaching model of obstetrics and gynecology used originally was not very effective.In this respect,we' d conducted a special research and improvement.After two years' teaching practice,it' s proved that the clinical skills of students of obstetrics and gynecology had been improved effectively,meanwhile,it had also laid a solid foundation for the training of qualified doctors as well as for reducing medical disputes.

Abstract:Unintended pregnancy is a global issue, with approximately 85,000,000 women around the world having unintended pregnancy annually. The contents of clinical application of women's contraceptive methods are very wide, involving multiple areas. This consensus deeply discusses the specific contraceptive needs at different statuses, combined with gynecological diseases, postabortion contraception, and postpartum family planning, ensuring the correct use of contraceptive methods under the corresponding status. The top priority of the consensus is the specific contraception consensus section for women combined with gynecological diseases because medical treatment effect as well as side effects should be weighed carefully. The consensus is to make high-efficiency and individual contraceptive strategy for different groups based on multidisciplinary (gynecology, obstetrics, and family planning) and multidimensional aspects, which can provide uniform guidance for medical and health organizations under the condition as relevant global guidance or consensus is still lacking.

Abstract:Objective To investigate the effect of the combining standardized patients and clinical case to train the clinical thinking of internship students in obstetrics and gynecology.Methods Using 41 undergraduate students of grade 2006,eight-year medicine education,major in clinical medicine in Peking University People's Hospital as the object of research,the students were taught by standardized patients combined with actual patients.Results After the end of clinical practice,the students carried out a comprehensive capacity assessment and clinical thinking exam and compared with the student of grade 2005.The result showed that the capacity test results of two groups were not significant differences,the students'achievement of grade 2006 (8.79 ± 0.64) was higher than the students' achievement of grade 2005 (8.43±0.98) in clinical examination of thinking (P＜0.05),the difference was statistically significance.Conclusions The methods of combining standardized patients with clinical case had a positive effect to develop students' clinical thinking.

Abstract:Objective To investigate the clinical internship status quo of eight-year medical program students in gynecology and obstetrics department of Peking University Third Hospital,and to give further targeted suggestions.Methods Questionnaire survey was used in this study.The clinical internship status quo of 35 eight-year medical program students in gynecology and obstetrics department of Peking University Third Hospital were surveyed by self-compiled questionnaires,and the related data was analyzed.Results There were 57.1％ (20/35) of students believe that there were difficulties during the internship,and the major difficulties were complicated and trivial knowledge points,relatively short time to learn regulated content,too theoretical professional knowledge to understand and patients' refusal attitude to clinical training.There were 85.7％ (30/35) of the students were willing to preview,and 94.3％ (33/35) of them were willing to review,the textbooks or recommended reading materials were students' favorite forms of preview and review.Conclusions There are still some difficulties during the internship in gynecology and obstetrics department for eight-year clinical program students,and most of them have high acceptance of acquiring knowledge in extracurricular time.In the future,we can adopt more flexible internship modules,add online courses besides and appropriately adopt simulation tools.A sequential curriculum system should be established by reasonably utilizing the extracurricular time,in order to improve the overall outcome of clinical internship in gynecology and obstetrics department.

Abstract:Background: To explore the needs of obstetricians and gynecologists (OB-GYNs) for training methods and contents, under China’s three-level diagnosis and treatment system. Methods: Epidemiological investigation was adopted, and network questionnaires were distributed in the largest academic training platform of obstetrics and gynecology in China, from April 2020 to May 2020. The investigation contents mainly included training methods and contents of continuing medical education (CME), as well as the mastery of diseases by doctors from hospitals at different levels of training. Results: The questionnaire received a total of 16,400 cumulative page views. 4458 questionnaires were collected in total, and 3954 questionnaires were included in the research. Doctors from hospitals at different levels chose the professional direction of gynecological endocrine diseases as the subject requiring the most strengthening of training, with ratios of 80.47%, 81.60% and 82.10%, respectively. Abnormal uterine bleeding (AUB) was the most desirable training content for doctors from primary, secondary, and other hospitals, while training of polycystic ovary syndrome (PCOS) was mostly needed by doctors from tertiary hospitals. According to the investigation, network education was the major training form favored by doctors, as well as a preferential way to acquire professional resources. The number of doctors selecting site meeting report only took up 4.70%. Based on the single-factor chi-square (χ2) analysis of the degree of mastery of 19 diseases, significant statistical differences were found among doctors from hospitals at each level, except for birth control (p < 0.001). After related factors were corrected, the multivariate regression analysis indicated that the degree of mastery of diseases was positively correlated to hospital levels. Conclusions: Doctors from hospitals at each level failed to effectively master gynecological endocrine diseases, and AUB, menopausal syndrome, and PCOS were confirmed as diseases for which the doctors had the greatest need of continuing education.

Abstract:Background: Medicine is to some extent an empirical science that requires repeated clinical practice. To explore the needs of obstetricians & gynecologists (Ob & Gy) in different kinds of hospitals for continuing medical education (CME) training content and forms. Method: From April 2020 to May 2020, an online questionnaire was distributed on the largest academic training platform for Ob & Gy in China, which included doctors’ mastery of diseases, training content and forms of CME. Results: A total of 4458 questionnaires were returned, of which 3954 were included in the study. There is a significant statistical difference between doctors in general hospitals, maternity specialized hospitals, or maternity and child specialized hospitals in the mastery of most common gynecological and obstetric diseases, among which, doctors in general hospitals have a better mastery (p < 0.001). Generally, doctors in different types of hospitals have a lower mastery of gynecological endocrine-related diseases; doctors in general hospitals have a better mastery of abnormal uterine bleeding (Exp 1.25, 95% Confidence Interval (CI) 1.09~1.44, p = 0.00), and doctors in specialized hospitals are better at infertility and precocious puberty/delayed pubertal development (p = 0.00). The preferred professional direction and training for doctors in different types of hospitals were to strengthen the knowledge of gynecological endocrine diseases through online teaching. Conclusions: Doctors in different types of hospitals generally have lower mastery of gynecological endocrine-related diseases, and doctors in general hospitals have a better mastery of common gynecological and obstetric diseases than those in specialized hospitals. The training content with the greatest need focuses on common gynecological endocrine-related diseases. Online teaching is the most popular educational model.

Abstract:Some problems and countermeasures of clinical anesthesia practice for the students from non-anesthesia program were discussed in this paper.Some students from non-anesthesia program neglected clinical anesthesia practice, lacked of activeness and anesthesia procedures were careless.Their practices were hard to win patients' cooperation.All of the approaches, including to improve clinical teachers' quality, to change the methods of clinical teaching, to stimulate students' interest of learning and to help them build up the self-confidence of communicating with patients, has improved the effect of clinical anesthesia teaching for the students from non-anesthesia program.

Abstract:Objective To investigate the current situation of drug clinical trials in obstetrics and gynecology hospital,and to explore the development direction of clinical drug trial institutions under the situation of new drug clinical trial standards.Methods Through the implementation of clinical trials in recent years by gynecology and obstetrics hospital,we analyzed the characteristics and the present situation of Obstetrics and gynecology in clinical trials.Results Because of the selectivity of drug research and development,the distribution of clinical trials is unbalanced in different subjects in obstetrics and gynecology hospital.The development of the subjects is limited by the particularity of the test population and the ability of comprehensive diagnosis and treatment.Strengthening institutional management is an important factor in the development and implementation of quality assurance in drug clinical trials.Conclusion The characteristic of drug clinical trials in obstetrics and gynecology hospital is different sub-groups in one large specialty.According to the professional and organization characteristics,we should establish the corresponding management mode to protect the interests of special subjects and to ensure the qualities of clinical trials.
Analysis on the reasons for cesarean section in a second level hospital in Hubei Province

Abstract:Objective To analyze the reasons for the increasing of caesarean section rate and the rationality in the judgment of surgical indicators so as to explore effective measures to decrease caesarean section rate.Methods Medical records of 539 caesarean section cases during the period from January of 2008 to January of 2009 were reviewed and analyzed.Results Caesarean section rate was 74.7%.Social factors(39.3%),fetal distress(10.7%),caesarean section history(10.2%),macrosomic(8.0%),umbilical cord abnormalities(7.2%),relative cephalopelvic disproportion(7.1%) were the six main indications among which social factors were the main reasons for increasing of caesarean section.Conclusions The main reason for the continuous increasing of caesarean section rate was the relaxation of caesarean section indication.It is necessary to strengthen health education in maternities to guide them understand the delivery process correctly,improve the technical levels of medical staff and service quality,grasp caesarean section indications strictly,"minimize intervention and return to nature".

Survey on status of knowledge, attitude, and practice about cervical cancer prevention and control and its influencing factors among Chinese women

Abstract:Objective To describe the knowledge of cervical cancer prevention and control,the willingness and behavior of participating in cervical cancer screening programs in rural and urban areas of China,and to obtain the status,influencing factors and correlation of knowledge,attitude and practice (KAP) about cervical cancer.Methods A total of 599 women from 6 districts in 3 provinces in eastern,central and western regions of China were recruited through a multi stage stratified random sampling method.Questionnaire was used to survey women's knowledge,attitude and practice about cervical cancer prevention and control.The relevant factors and correlation between KAP were analyzed.Results The awareness rate of knowledge about prevention and control of cervical cancer was only 12.2％.Education background (secondary education OR =5.608,college degree and above OR =22.256) and region (eastern OR =2.431,western OR =2.219) were the influencing factors of knowledge awareness rate.About 71.3％ women had positive attitudes towards screening and treatment of cervical cancer.Education background (secondary education OR =2.371,college degree and above OR =3.940),nationality (OR =1.763),occupation (OR =2.208) and area (OR =0.575) were the influencing factors of attitude.About 65.6％ women had ever received cervical cancer screening,and region and family history of cervical cancer were the influencing factors.The participation rate of "Cervical cancer free screening program of China" was 35.9％.There was a significant correlation between knowledge and attitude,attitude and practice.However,there was no significant correlation between knowledge and practice.Conclusion Knowledge of cervical cancer prevention and treatment was deficient among women in China.The overall attitude was positive,but participation in screening was still insufficient.
