Abstract:Objective To investigate whether ABO blood groups is associated with the severity of coronary artery disease (CAD). Methods Between January 2015 and December 2017, 1425 first diagnosed CAD patients confirmed by selective coronary angiography were recruited into this cross-sectional study, and their baseline characteristics, ABO blood groups, Gensini score were collected. Multiple linear regression analysis was performed to test the association between the severity of CAD and ABO blood groups. Results The Gensini score was significantly higher in the blood group A than in the non-A groups (41.2 ± 32 vs. 38 ± 27; P = 0.026). After adjusting for age, male, smoking, family history of CAD, hypertension, diabetes mellitus and hypercholesterolemia, multivariate linear regression indicated that blood group A was associated with the severity of CAD (β = 3.298, 95% CI: 0.91–6.505, P = 0.044). In diabetes group, A blood type was also associated with increased Gensini score (P = 0.02) after adjusting for age, male, family history of CAD, hypercholesterolemia, smoking and hypertension. Conclusion In this cross-sectional study, the data indicated that blood group A was an independent risk factor of severity of CAD in Chinese population and Chinese patients with type 2 diabetes.

The right coronary artery was significantly more frequently involved in the diabetics. Duration of CAD, Homa-IR, and diabetes mellitus were the independent risk factors for CAD found in the present study, while ApoA was the protective one.

Abstract:Objective To study insulin resistance and dyslipidemia in the nondiabetic first degree relatives with type 2 diabetes.Methods A total of 55 nondiabetic first degree relatives with type 2 diabetes and 42 sex ,age and BMI matched control subjects without a family history of diabetes.All subjects underwent 75g OGTT for 0,60,90,120 minutes glucose,insulin and C peptide and baseline blood lipids determination.Results 1.INS 0,CP 0 and HOMA IR were significantly higher in relatives than those in controls.TG was higher and HDL C was lower in relatives.2.The first degree relatives were classified into three subgroups according to BMI23,23≤BMI25,BMI≥25,with BMI increasing,HOMA IR,TG,TC,glucose 60,90,120,and INS,CP 0,60,90,120 were significantly higher.3.TG was significantly correlated with INS 0,CP 0,HOMA IR.A significant correlation could be found between HOMA IR with BMI,TG,INS 0,CP 0.Conclusion Insulin resistance and dyslipidemia could appear early in normoglycemia first degree of type 2 diabetes relative.With BMI increasing,insulin resistance and dyslipidemia are more serious in the first degree relatives.

Abstract:OBJECTIVE:To explore the pathophysiologic and clinical features and investigate the roles of insulin resistance and insulin secretion in the pathogenesis of type 2 diabetes mellitus.METHODS:A total of 888 first-degree relatives without glucose intolerance history underwent an oral glucose test (OGTT) and their level of HbA1c, insulin concentration and lipid levels were determined. The homeostasis model assessment was used to estimate insulin resistance (HOMA(IR)) and beta-cell function (HOMA-beta). The ratio of incremental glucose (DeltaG30) and insulin (DeltaI30) response was used to evaluate the early insulin secretion. DeltaI30/DeltaG30/HOMAIR was used to evaluate the glucose disposition index (DI).RESULTS:In the subjects, 167 were diagnosed with diabetes, 180 with impaired glucose tolerance or/and impaired fasting glucose (impared glucose regulation), 457 with normal glucose tolerance and normal HbA1c, and 84 with normal glucose tolerance and high HbA1c. From normal glucose tolerance through impared glucose regulation to diabetes mellitus, the HOMA(IR), body mass index (BMI), waist/hip ratio (WHR) and serum triglyceride (TG) progressively increased, HOMA-beta cell, DeltaI30/DeltaG30, DI and high density liproprotein (HDL) progressively decreased. Subjects with normal glucose tolerance were divided into three tertile subgroups (1/3, 2/3 and 3/3 groups) with different area under the curve of OGTT glucose, after being adjusted by sex, age, BMI, the 3/3 group was found having higher HOMA(IR), and lower HOMA-beta, DeltaI30/DeltaG30/, and DI than the 1/3 group.CONCLUSION:Both insulin resistance and impaired beta cell function are important pathophysiologic changes contributing to the onset and development of type 2 diabetes. These changes and lipid profile have occurred before a patient is diagnosed with abnormal glucose tolerance.
THE CORONARY ANGIOGRAPHY FEATURES AND CLINICAL CHARACTERISTICS OF PATIENTS WITH CORONARY HEART DISEASE COMPLICATED TYPE 2 DIABETES MELLITUS

Abstract:Objective To explore the characteristics of clinical manifestation and artery lesion in coronary heart disease patients combined with type 2 diabetes mellitus(T2DM). Methods 313 cases of Coronary heart disease(CHD) confirmed by coronary angiography were divided into T2DM group and NDM group according to whether or not combined with T2DM.We analyzed and compared two group in the general condition,clinical data,serum markers and coronary angiographic findings.According to coronary lesion morphology classification standards and SYNTAX score,we can qualitatively and quantitatively evaluated coronary artery lesions. Results Compared with NDM group,the ratio of hypertension,hyperglycemia and acute coronary syndrome were much more in T2DM group;Serum triglyceride(TG) increased,high density lipoprotein-cholesterol(HDL-C) decreased in T2DM group(p0.05).The proportion of multiple vessel disease,diffuse change,complicated lesions and arteriostenosis,vessel lesions of type C and SYNTAX score was significantly higher in T2DM group.There were no significant differences in ages,history of smoking,total cholesterol(TC) and low density lipoprotein cholesterol(LDL-C) in both groups. Conclusion The T2DM patients complicating CHD have more chance accompanied with risk factors such as hypertension,hyperglycemia,the proportion of ACS in DM group were higher,and have more accompanied with multiple vessel disease,diffuse change,complicated lesions and arteriostenosis.

Abstract:Abstract To investigate the relationship between diabetic foot complications (DFCs) and clinical characteristics, especially the number and types of first-degree family members with diabetes. A total of 8909 type 2 diabetes patients were enrolled. The clinical characteristics of these patients, including DFCs and family history of diabetes (FHD), were collected from medical records. Multiple regression was used to investigate the association between FHD and DFCs after adjusting for confounding factors. The patients with one and more than one first-degree family member with diabetes accounted for 18.7% and 12.8%, respectively. The proportions of the participants with a father with diabetes, a mother with diabetes, both parents with diabetes, siblings with diabetes, father and siblings with diabetes, mother and siblings with diabetes, and both parents and siblings with diabetes were 3.5%, 6.2%, 1.1%, 14.4%, 1.5%, 4%, and 0.7%, respectively. The multiple regression analysis showed that the number of family members with diabetes was positively associated with DFCs. However, among the different types of FHD, only the patients with a mother with diabetes showed a statistical association with DFCs. In addition to FHD, other factors, including gender, body mass index, platelet count, hemoglobin levels, albumin levels, high-density cholesterol levels, diabetic peripheral neuropathy, and the use of lipid-lowering agents, oral hypoglycemic agents, and insulin, were also associated with DFCs. DFCs were associated with different numbers of family members with diabetes and types of FHD. This association reveals the importance of genetic and environmental factors in DFCs and highlights the importance of adding FHD to public health strategies targeting detecting and preventing the disease.

Abstract:To compare angiographically-determined coronary artery disease in diabeticpatients with nondiabetics of Chinese,782 patients coming to cardiaccatheterization were reviewed retrospectively to find 42 diabetic and 740nondiabetic patients matched for sex and age.The following results wereobtained:37 of 42 diabetic patients（88.10%）had coronar artery diseasecompared to 412 of 740 nondiabetics（55.68%）（P＜0.005）.18 of 37 diabeticpatients with coronary artery disease（48.65%）had multivessel diseasecompared to 143 of 412 nondiabetics（34.71%）（p＜0.005）19 of 37 the diabeticpatients（51.35%）had diffuse coronary artery disease compared to 32 of 412nondiabetics （7.77%）（P＜0.05）21 of 37 the diabetic patients （56.76%）hadsmall vessel disease compared to 114 of 412 nondiabetics （27.67%）（P＜0.005）.We conclude that diabetic patients have more coronary artery disease thannondiabetics and diabetic patients with coronary artery disease have morediffuse coronary disease more multivessel disease and more small vesseldisease than nondiabetics. The results have been attributed to many factors oftenassociated with diabetic patients:hypcrlipidemia obesity svstemichypertension.insulin resistance.silent myocardial ischemia and so on.

Abstract:Objective To explore the clinical characteristics and coronary angiographic findings in patients with type 2 diabetes mellitus and coronary artery disease (CAD).Methods A total of 357 patients with angiographically diagnosed CAD were divided into 2 groups according to 1999 diabetes criteria of WHO:189 CAD patients with and 168 without type 2 diabetes mellitus.All patients underwent 75 g oral glucose tolerance test (OGTF).Blood glucose,insulin,HbA_(1C),serum lipid profile and 24 h urine albumin/creatinine ratio (24 h-Alb/ Cr) were determined.Insulin resistance index (HOMA-IR) and insulin secretion index (HOMA-IS) were determined by the HOMA model.The clinical features and the data from selective coronary angiographies were compared between diabetic and non-diabetic CAD patients.Results Compared to non-diabetic CAD patients, diabetic CAD patients had increased HOMA-IR and 24 h-Alb/Cr (both P<0.05),along with decreased HOMA- IS (P<0.01),and they suffered more from triple vessel lesions (P<0.05),severe coronary artery stenosis, complete occlusions and diffuse lesions (P<0.05 or P<0.01),and their cumulative scores were higher (P<0.01).Conclusion Diffuse and severe lesions were more common in the CAD patients with type 2 diabetes.

Abstract:Objective:To investigate insulin resistance and the β-cell function in nondiabetic first degree relatives of type 2 diabetes mellitus(FDR) over 5 years follow-up. Methods:Thirty-two patients with non-diabetic first degree relatives of type 2 diabetes(FDR) and 18 healthy controls(NC)were assigned and measuerd body mass index(BMI), waist hip ratio(WHR), blood pressure, serum lipids, serum glucose and fasting insulin(FINS) at baseline. Homeostasis model assessment(HOMA) was applied to assess the status of insulin resistance and β-cell function. Five years later, all subjects were reexamined. Results:HOMA-IR was significantly elevated and HOMA-β was lower in FDR group compared with baseline. During 5 years follow-up, HOMA-IR was not significantly changed between two the groups, but HOMA-β was significantly lower in FDR group than in NC group. Multiple linear regression analysis showed that FBG, FINS,HDL-C were the major risk factors affecting insulin resistance in FDR and the major risk factors affecting β-cell function was FBG and FINS. Conclusion:The nonobese first degree relatives of type 2 diabetes mellitus had insulin resistance and impaired β-cell function at prediabetic stage, and by step, β-cell function may be significantly lower in the period of developing type 2 diabetes mellitus.

Abstract:Background: The prevalence of diabetes has risen fast with a considerable weighted prevalence of undiagnosed diabetes or uncontrolled diabetes. Then it becomes more necessary to timely screen out and monitor high-risk populations who are likely to be ignored during the COVID-19 pandemic. To classify and find the common risks of undiagnosed diabetes and uncontrolled diabetes, it's beneficial to put specific risk control measures into effect for comprehensive primary care. Especially, there is a need for accurate yet accessible prediction models. Objective: Based on a cross-sectional study and secondary analysis on the health examination held in Changchun City (2016), we aimed to evaluate the factors associated with hyperglycemia, analyze the management status of T2DM, and determine the best cutoff value of incidence of diabetes in the first-degree relatives to suggest the necessity of early diagnosis of diabetes after first screening. Results: A total of 5658 volunteers were analyzed. Prevalence of T2DM and impaired fasting glucose were 8.4% (n=477) and 11.5% (n=648), respectively. There were 925 participants (16.3%) with a family history of T2DM in their first-degree relatives. Multivariable analysis demonstrated that family history was associated with hyperglycemia. Among the 477 patients with T2DM, 40.9% had not been previously diagnosed. The predictive equation was calculated with the following logistic regression parameters with 0.71 (95% CI: 0.67–0.76) of the area under the ROC curve, 64.0% of sensitivity and 29% of specificity ( P < 0.001): P = \frac{1}{1 + e^{-z}}, where z = -3.08 + [0.89 (Family history-group) + 0.69 (age-group)+ 0.25 (BMI-group)]. Positive family history was associated with the diagnosis of T2DM, but not glucose level in the diagnosed patients. The best cutoff value of incidence of diabetes in the first-degree relatives was 9.55% ( P < 0.001). Conclusions: Family history of diabetes was independently associated with glucose dysfunction. Classification by the first-degree relatives with diabetes is prominent for targeting high-risk population. Meanwhile, positive family history of diabetes was associated with diabetes being diagnosed rather than the glycemic control in patients who had been diagnosed. It's necessary to emphasize the linkage between early diagnosis and positive family history for high proportions of undiagnosed T2DM.

Abstract:BackgroundPrevious observational studies have shown an association between smoking and coronary artery disease (CAD) in patients with diabetes. Whether this association reflects causality remains unestablished. This study aimed to explore the causal effect of smoking on CAD in patients with diabetes. MethodsGenetic signatures for smoking were extracted from a large genome-wide association study (GWAS), consisted of up to 1.2 million participants. Four smoking phenotypes were included: smoking initiation, cigarettes per day, age at initiation of regular smoking, and smoking cessation. Genetic associations with CAD in patients with diabetes were extracted from another GWAS, which included 15,666 participants (3,968 CAD cases and 11,696 controls). The analyses were performed using the univariable and multivariable Mendelian randomization (MR) method. ResultsMR analysis revealed that smoking initiation was positively related to CAD risk in patients with diabetes (OR = 1.322, 95% CI = 1.114 - 1.568, P = 0.001), but this association was attenuated when adjusted for cardiovascular risk factors (OR = 1.212, 95% CI = 1.008 - 1.457, P = 0.041). Age at initiation of regular smoking was negatively related to CAD in patients with diabetes (OR = 0.214, 95% CI = 0.070 - 0.656, P = 0.007), but this association became insignificant when adjusted for cardiovascular risk factors. ConclusionsThis study supported the effect of smoking initiation on the risk of CAD in patients with diabetes.

Abstract:Background: Coronary artery disease (CAD) is accelerated in subjects with type 2 diabetes mellitus (T2D). Methods: To test whether this reflects differential genetic influences on CAD risk in subjects with T2D, we performed a systematic assessment of genetic overlap between CAD and T2D in 66 643 subjects (27 708 with CAD and 24 259 with T2D). Variants showing apparent association with CAD in stratified analyses or evidence of interaction were evaluated in a further 117 787 subjects (16 694 with CAD and 11 537 with T2D). Results: None of the previously characterized CAD loci was found to have specific effects on CAD in T2D individuals, and a genome-wide interaction analysis found no new variants for CAD that could be considered T2D specific. When we considered the overall genetic correlations between CAD and its risk factors, we found no substantial differences in these relationships by T2D background. Conclusions: This study found no evidence that the genetic architecture of CAD differs in those with T2D compared with those without T2D.

Abstract:OBJECTIVE-To evaluate coronary artery stenosis in early diabetes or prediabetes asymptomatic of myocardial ischemia in community-dwelling Chinese adults. RESEARCH DESIGN AND METHODS-Age- and sex-matched participants with normal glucose regulation (NGR), prediabetes, or diabetes diagnosed within 5 years, asymptomatic of coronary artery disease (CAD), were randomly selected from a community-dwelling Chinese population aged 40-60 years. Dual-source computed tomography coronary angiography was used to evaluate the existence and extent of coronary stenosis, which was considered significant if >50% narrowing of vessel lumen was detected. RESULTS-After excluding uninterpretable segments attributable to motion artifacts, a total of 135 participants with NGR, 132 with prediabetes, and 134 with diabetes participated in data analysis. Significant coronary stenosis was detected in 10 (7.4%), 10 (7.6%), and 22 (16.4%) individuals with NGR, prediabetes, and diabetes, respectively (P for trend = 0.029). Diabetes, rather than prediabetes, was associated with a significant 2.34-fold elevated risk [odds ratio (OR) 2.34 (95% CI 1.01-5.43); P = 0.047] of significant coronary stenosis as compared with that associated with NGR. Levels of glucose evaluation were independently and significantly associated with risks of significant coronary stenosis in diabetes. Each 1-SD increase in fasting plasma glucose, 2-h postload plasma glucose, and HbA(1c), conveyed 2.11-fold, 1.73-fold, and 1.81-fold higher risks of significant coronary stenosis, respectively, after adjustment for other conventional cardiovascular risk factors. CONCLUSIONS-Using a noninvasive CAD diagnostic modality such as dual-source computed tomography coronary angiography, we detected a markedly elevated risk of significant coronary stenosis with early diabetes in asymptomatic Chinese adults.

Abstract:Objective To investigate the clinical characteristics in outpatients with type 2 diabetes mellitus and coronary heart disease (CHD).Methods Retrospectively analyzed the clinical data of 528 outpatients with type 2 diabetes mellitus enrolled from January to December in 2007 in outpatients.Patients were divided into two groups according to CHD history:CHD group (n =81,40 males and 41 females) and non-CHD group (n =447,249 males and 198 females).The clinical characteristics of the patients were analyzed.Logistic regression analysis was applied to explore independent correlation factor for CHD in patients with type 2 diabetes.Results Compared with patients in non-CHD group,patients in CHD group were older((60 ± 12) and (69 ± 8) years,respectively,t =-8.64,P ＜ 0.05),with longer duration of diabetes ((9 ± 7) and (12 ± 7) years,respectively,t =-4.44,P ＜ 0.05),higher glycated hemoglobin A1c (HbA1c)(6.8％ ± 1.1％ and 7.4％ ±1.2％,respectively,t =-3.38,P＜0.05),and higher systolic blood pressure ((130 ± 15) and (134 ± 15) mm Hg(1 mm Hg =0.133 kPa),respectively,t =-2.26,P ＜ 0.05) ; And the patients in CHD group had higher incidence of hypertension,dyslipidemia,stroke and positive family history of CHD(x2 =14.29,9.47,25.01,7.56,all P ＜ 0.05).The standardreaching rate of blood pressure and blood lipid in the two groups were similar; but the rate of HbAlc ＜ 7.0％was much lower in CHD group than that in non-CHD group(48.1％ and 68.2％,respectively,x2 =12.18,P ＜ 0.05).Logistic regression analysis showed that age,duration of diabetes,HbA1c,stroke,systolic pressure and the use of statins were correlation factors for CHD (OR =1.08,1.11,1.47,2.72,1.03 and 2.41,respectively,all P ＜ 0.05).Conclusions Outpatients with diabetes and CHD are concomitant with multiple risk factors,for these patients,more attention should be paid to the management of blood glucose,blood pressure,blood lipid and the use of statins.

Abstract:Objective To analyze the clinical features of type 2 diabetes mellitus(DM)patients complicated with coronary heart disease(CHD).Methods 336 DM patients undergoing coronary angiography were divided into two groups:DM with CHD(n=267)and DM without CHD(n=69).General conditions,lipid profiles,glycemic control,other serum biochemical parameters and inflammatory parameters were detected and compared.And then,logistic regression analysis was used to find the independent risk factors for CHD in DM patients.Results There were several differences between DM with and without CHD in parameters including DM duration(P=0.038),smoking history(P=0.000),W/H ratio(P=0.000),HDL-C(P=0.006),FPG(P=0.000),HbA1c(P=0.000),CREA(P=0.006),hs-CRP(P=0.011),WBC(P=0.000)and NEU%(P=0.000).But only 4 parameters,including DM duration(OR=0.925),smoking history(OR=3.576),HDL-C(OR=2.839)and FPG(OR=0.819)were independent risk factors.Conclusion The DM patients who have long DM duration,smoking history,poor glycemic control and reduction of HDL-C are inclined to suffer CHD.

Abstract:Evaluation of people at increased risk like first degree relatives of type 2 diabetes mellitus (FDRDM) may be useful to reduce the risk of disease progression, development, early intervention, and to take precautionary measures. By considering the multifactorial pathophysiological changes of D.M., we have examined the body fat distribution, cardiorespiratory fitness, and lipid profile of FDRDM. Similar age, height, waist-hip ratio (WHR) in both groups, significantly higher body mass index (BMI) in FDRDM, was observed in our study. Percentage body fat and blood glucose levels in fasting were elevated considerably, and 12 min walk distance was low in FDRDM. Visceral fat was slightly high, but it was not statistically significant. In FDRDM, High-density lipoproteins (HDL) were less but not statistically significant. Significantly higher levels of Total cholesterol (T.C.), triglycerides (TGL), low-density lipoproteins (LDL), and very-low-density lipoproteins (VLDL) were seen high in FDRDM when compared to controls. Higher body fat percentage reduced cardiorespiratory function and abnormal lipid profile in FDRDM may lead to the development of severe cardiovascular events and necessitates lifestyle modification at early phases of disease development.
[The Features of Coronary Angiography in Coronary Heart Disease with Non-Insulin-dependent Diabetes Mellitus].

Abstract:OBJECTIVE:To study the clinical and angiographical characteristics of coronary heart disease (CHD) in CHD patients with non-insulin-dependent diabetes mellitus (NIDDM).METHODS:The study was carried out by case control analysis of the clinical manifestations and the data from cine selective coronary angiography.RESULTS:CHD patients with NIDDM tended to have multiple cardiovascular risk factors. It is likely that NIDDM contributed more to dyslipidemia in the female than in the male. Unstable angina pectoris was found more frequently in CHD patients with NIDDM than in those without. The female CHD patients with NIDDM were more frequently afflicted with myocardial infarction. Moreover, CHD patients with NIDDM had much greater extent of coronary vessel involvement and more diffuse vascular lesion.CONCLUSION:The clinical and angiographical findings are quite different in CHD patients with and without NIDDM. The lesions in the coronary arteries tend to be more severe and diffuse in patients with NIDDM.
Constitution and Character Analysis of Families of Type 2 Diabetes Mellitus

Abstract:Objective:To constitute and analyse the type 2 diabetic families for analysis of genetic and environmental risk factors on study type 2 diabetes.Methods:183 probands with family history of diabetes were diagnosed.The survey of diabetes was performed in relatives of probands.All subjects who had not been diagnosed were valued by oral glucose tolerate test(OGTT).Type 1 diabetes pedigrees were excluded with immunologic test.Results:The prevalence rates of type 2 diabetes in familial group was 56.5%,and was significantly higher than that in general population.The prevalence rate of diabetes was significantly increased as the subjects after 40 years old.Among affected parents,diabetic mothers were much more than diabetic fathers(45.9% and 26.7%),P0.01.Conclusion:Familial type 2 diabetes have familial aggregation.The relatives in multiple familial type 2 diabetes pedigree are high risk population to type 2 diabetes.The crowd is suitable for study the etiology of type 2 diabetes.
