Abstract:Objective China is in the process to increase the supply of organs and to make their organ donation and transplantation comply with the WHO guiding principle. As the leading living related kidney transplant (LRKT) center, we adhered strictly to the government regulations and performed over 1000 cases of LRKT during last 8 years. Methods We retrospectively reviewed the medical file of 1008 LRKT from Jan 1st 2006 to Sep 30th 2013. All clinically important items were summarized. Results There were 346 male donors and 748 (74.2%) male recipients. Donor's age was 46.68±9.81y (18-66). Recipients' age was 33.11±8.85y (11-65), height 165.84±6.69cm (145-194), weight 58.71±9.97kg (28-96).The pre-transplant dialysis period was 10.14±12.21 months(0-96). HLA matching was 3.32±1.25 and PRA was 3.14±10.08%(0-99%).Mean follow up was 34.82±21.86 months (4-93). During the follow up, 27 (2.67%) cases were lost. 303 cases received induction therapy (anti-thymocyte globulin: 46, anti-CD25 monoclonal antibody: 257). The majority of patients received triple immunosuppressant (CNI: cyclosporine A(CSA)/tacrolimus (TAC), MMF, prednisone) as maintenance immunosuppression. CSA was initiated in 289 cases and TAC in 719 cases. 50 case converted CNI to sirolimus once, but only 14 cases currently maintained with sirolimus. 29 (2.9%) graft failure occurred and 8 year death-censored graft survival was 92.8%. 19 (1.9%) patients died and 8 year patients' survival was 97.3%. There was 18 (1.8%) delayed graft function and all cases recovered. Acute rejections occurred in 126 (12.5%) cases.24 (2.4%) reoperations were performed, of which 6 due to massive bleeding. There was 6 (0.6%) urinary leakage and 64 (6.3%) lymphatic leaks. There was 255 (25.3%) post-transplant infection and 13 infections resulted in death.9 (0.9%) cases developed new onset diabetes and femoral head necrosis occurred in 5 (0.5%) cases. Two to three years after transplantation, 3 (0.2%) cases detected malignancy. Conclusion The outcome of LRKT in our center is excellent. Meanwhile, as a practice ethically accepted by international transplant community, LRKT should be encouraged in China.
