Abstract:Objective To assess the clinical performance of realtime PCR for the detection of hepatitis B virus(HBV) DNA in the patients with chronic HBV infection and to explore the relationship between the quantities of HBV DNA and the models of serologic markers and hepatic function.To reveal the internal law between the amount of HBV-DNA and the lesion degree of liver in clinical patient with viral hepatitis B,and to increase the usage efficient of clinical laboratory diagnosis information.Methods Real-time fluorescent quantitative PCR was used to measure the HBV DNA.ELISA was used to detect the HBV-SM.Hepatic function was detected by an automatic biochemistry analyzer.Results There are some intra-correlation between the amount of HBV-DNA and HBV-SM.The positive rate and the amount of HBV-DNA of HBV-SMⅠpattern are the highest and HBV-SMⅡpattern lies at second.Most patients in HBV-SMⅠpattern and Ⅱpattern have more than 104 HBV in their peripheral blood.In others patterns of HBV-SM,the amount of HBV-DNA is chiefly distributed in 104 copies/ml.The positive rates and the amount of HBV-DNA of single HBsAg positive group,HBeAg positive group,HBeAb positive group and HBcAb positive groups are higher than single HBsAb positive group and negative group.The amount of HBV-DNA is HBeAg positive groupHBcAb positive groupHBeAb positive groupHBsAg positive group.Among the person with single HBsAb positive or HBV-SM negative,the positive rate of HBV-DNA is less than 10%,but the levels of HBVDNA is below 104.There is no correlation between the amount of HBV-DNA and the total protein as well as the rate of albumin and globulin.There are an-single-cusp relationships between the amount of HBV-DNA and AST and ALT.Follow the heightening of the amount of HBV-DNA,the levels of AST and ALT show a process from low to high and from high to low again.Conclusion There are certainly relationships among HBV-DNA,HBV-SM and serum transaminase.The clinical application value of HBV-DNA quantitative analysis by real-time PCR is in infective differentiation,early infection diagnosis and low-level-infection diagnosis and help to judge the course and monitor curative effect.A conjoint use of real-time PCR and ELISA can make up for each other's deficiencies.
