Abstract:Metanephric adenoma (MA) of the kidney is an uncommon benign epithelial tumour that can present at any age. The histology of the lesion is well established, but imaging findings have been described in only a few cases. We report the CT and ultrasound appearance of MA in two females to increase the awareness of this entity among radiologists. Its recognition may facilitate nephron-sparing surgery.

Abstract:Objective To explore the clinic characteristics of metanephric adenoma (MA),in order to improve our understanding of this disease. Methods We retrospectively summarized clinical data of 8 cases of MA from January 2011 to June 2014 in our hospital.Information of all patients with MA was col-lected and analyzed including their symptoms,laboratory examinations,imaging studies,surgical proce-dures and pathological data,and related articles were retrieved to analyze the clinic characters of MA. Re-sults There were three male and five female patients and their age ranged from 2 1 to 7 1 .Three of them had flank pain and others were found asymptomatically.Seven of them were found MA unilaterally,other one patient was found bilaterally.Four patients had nephron sparing surgery or microwave ablation,other two patients had retroperitoneal laparoscopic nephrectomy and the rest two had combined kidney and viscera organ resection.Of all cases,six were pathologically confirmed as MA with the maximum diameter ranging from 1 .5 to 5 .2 cm,while other two patients were reported as MA combined with sarcomatoid carcinoma with the maximum diameter ranging from 6 .0 to 1 1 .4 cm. Conclusions MA is a rare renal tumor which is difficult to identify from renal cancers.The renal tumor in patient with polycythemia may suggest the di-agnosis of MA.After confirmation by ultrasound-guided core-needle biopsy,conservative treatment by par-tial nephrectomy can be performed as a priority,while cryotherapy and microwave ablation can also be se-lected.However,radical nephrectomy should be performed when the tumor size is greater than 6 .0 cm.

Abstract:Objective To investigate the clinical, radiological and histopathologic features of metanephric adenoma(MA). Methods 2 cases of MA were reported and literatures were reviewed. The clinical, radiological and histopathologic features, as well as diagnosis and treatment of MA were also discussed. Results MA has no typical clinical and radiological features but unique histopathologic structures. Immunohistochemistry staining contributes to a clear diagnosis. Conclusions MA is a rare benign renal tumor, and it has neither typical clinical nor radiological features. Based on its histopathologic characteristics and immunohistochemical staining, a clear diagnosis was made. Partial nephrectomy is safe and feasible for these patients, however, long-time close follow-up is necessary.

Abstract:Our objective is to summarize the characteristic imaging manifestations of metanephric adenoma (MA) and to improve the diagnosis of MA. The imaging and clinical data of two cases with pathologically confirmed MA in our hospital were analyzed, retrospectively. The literature reports and characteristic imaging manifestations of the disease were discussed. MA is typically unilateral and accompanied by polycythemia. We found that the main body of the tumor is outside the kidney outline. The tumor is round with smooth edges. CT scans show an equal or low density, and the internal density can be uniform or uneven. Some cases are accompanied by calcification and cystic degeneration. Enhanced-CT scanning shows a mild to moderate progressive enhancement. MA presents as a hypoechoic mass on ultrasound examination. Color Doppler flow imaging (CDFI) shows a minimal blood flow echo. So we can see the imaging manifestations of MA show specific characteristics. When the main body of the tumor is located outside the outline of the kidney, the mass shows a mild to moderate progressive enhancement or a delayed enhancement after contrast-enhanced scanning. When a renal tumor with polycythemia is encountered, MA should be suspected.

Abstract:Objective:To explore the clinical characteristics of the metanephric adenoma and improve awareness of the MA.Methods:1 case of Bilateral MA was reported,histopathological characteristics,clinical data and relevant literature of MA were also analyzed.Results:The tumor cells were rich,arrange densely,and consist of tubular structure of simple cuboidal epithelium.Immunohistochemistry staining: WT1,CD57 and vim were positive,the PCK,EMA,CK7,CD10,CD117 were negative,Ki67 LI: 1%.Conclusions:MA mostly is unilateral,bilateral rare.It has neither typical clinical nor radiological features,preoperative diagnosis and differentiated from malignant lesions are difficult.It required preoperative puncture biopsy or intraoperative pathological diagnosis.Partial nephrectomy is safe and feasible for these patients,however,long-time close follow-up is necessary.

Abstract:Objective: To seek the clinical and histopathological characteristics of the metanephric adenoma and the criteria for its pathological diagnosis. Methods: 3 cases were stained with HE and immunohistochemistry. Results: The clinical features of metanephric adenoma were not typical, and these tumors consist of tubular, papillary and glomeruloid-like formation. Immunohistochemically, Leu7 and vimentin were positive, CK7 was focally positive, the EMA, Cga, Syn and S-100 were negative. Conclusion: Metanephric adenoma is a rare benign tumor that originates from the remnants of early metanephric tissues and is often diagnosed as malignancy. It occurred mainly in youth and middle-aged female. It is very important to distinguish it from Wilms' tumor, renal cell carcinoma, neuroendocrine tumor, nephroblastomatosis and nephrogenic rests.

Abstract:Rationale: Hemangioblastomas in the kidney are rare. Although a few cases of renal hemangioblastoma (RH) have been reported, the content of these articles mainly focused on clinical and pathological research, with minimal descriptions of radiologic findings. Moreover, there are no descriptions of magnetic resonance imaging (MRI) with enhancement of this condition. Herein, we report 2 cases of RH with computed tomography (CT) and MRI findings. Patient concerns: Two patients presented to our institution because of dull pain in the left abdomen, and a mass in the left kidney was found by ultrasound examination in each case. The patient had no special family history. Physical examination revealed no obvious tenderness or percussion pain in the renal and ureteral walking areas, and there was no obvious mass. Routine blood and urine tests were normal, and the serum tumor markers were negative. No obvious lesions were found on imaging of the other body parts. Diagnosis: Similar radiologic findings were observed in both cases and mimicked those of cavernous hemangiomas of the liver, including peripheral nodular enhancement in the corticomedullary phase, progressive centripetal enhancement in the nephrographic and delayed phases, and occasional complete "filling in" in the delayed phase. We made a radiologic diagnosis of renal clear cell carcinoma for patient 1 and suspected renal clear cell carcinoma for patient 2, but the pathological results showed RH. Interventions: Given the suspicion of renal cell carcinoma, both patients underwent partial nephrectomy. Outcome: The recovery of the two patients was uneventful, and there was no evidence of local recurrence or metastasis many years after surgery. Lessons: RH is a rare benign tumor that can be easily misdiagnosed as clear cell carcinoma. Characteristic CT and MRI manifestations may improve preoperative diagnostic accuracy to avoid surgery or indicate nephron-sparing surgery.
Metanephric adenoma: association between the imaging features of contrast-enhanced ultrasound and clinicopathological characteristics

Abstract:Background: This study aimed to improve the understanding of metanephric adenoma (MA) by retrospective analysis of contrast-enhanced ultrasound (CEUS) findings and clinicopathological characteristics of MAs. Methods: Gray-scale ultrasound (US) and CEUS findings of 7 adult MA patients, confirmed by postoperative pathology, were summarized via collection of clinicopathological and ultrasonographic imaging data, including tumor location, size, echo intensity, color flow, presence or absence of calcification, and liquefactive necrosis, contrast-enhanced pattern, enhancement characteristics, and contrast wash-out compared with adjacent parenchyma, and the presence or absence of a pseudocapsule. Histopathological analyses, including hematoxylin and eosin (HE) and immunohistochemical (IHC) staining, were conducted with the EnVision method. Results: All 7 participants were female, aged 29-73 years (mean age, 54 years), with flank pain (3/7). All tumors were solid (7/7) with sizes of 2.0-5.0 cm (mean diameter, 3.07 cm), including 4 in the left kidney, 3 in the right kidney, 2 in the renal pelvis, and 5 in the renal parenchyma. On the gray-scale US, MA was shown as hypoechoic (4/7), slightly hyperechoic (2/7), isoechoic (1/7), and with a defined border. The morphology was regular and rounded (7/7), internal echogenicity was homogeneous (5/7), and no calcification was seen (7/7). The CEUS showed clear boundaries (7/7), homogeneous isodensity (5/7), with calcification (0/7), necrosis (2/11), heterogeneous hyperattenuation (2/7), pseudocapsule (2/7), and medullary phase fast wash-out (7/7). The surgical methods were radical nephrectomy (4/7) and partial nephrectomy (3/7). The duration of follow-up period for all participants was 3-74 months, and no local or distant recurrences were found. The IHC staining showed that most tumor cells were positive for WT1, cytokeratins AE1/AE3, vimentin, and CD57, and exhibited focal positivity for CK7, while negative for CD10, AMACR, and CK720. The proliferative index (Ki-67) was 2-3%. Conclusions: On gray-scale US, MA appears as a solid nodule with a well-defined boundary, regular morphology, and homogeneous echogenicity; CEUS shows slow progression and slightly lower homogeneous enhancement and fast wash-out in the medullary phase. These findings may provide insight into the progression of MA and aid in the development of diagnostic and therapeutic strategies.

Abstract:Objective:To analyze the clinical and CT manifestations of metanephric adenoma in chidren.Methods:Two cases of metanephric adenoma proved pathologically in children were reported,and the clinical and imaging manifestations of the other 10 cases from the literatures were reviewed together.Results:More patients with metanephric adenoma could have polycythemia at presentation.US image depicted a well circumscribed hyperechoic mass;Unenhanced CT showed a higher attenuation mass with calcification,some cases displayed a cystic mass with a single solid mural nodule.On contrast enhanced CT, the degree of parenchymal enhancement varied.In our study,one showed polycystic mass with lots of fat attenuation.Athough the tumor is benign,metastasis may be occur.Conclusion:Imaging manifestations of metanephric adenoma varied from each other,imaging findings combined with clinical manifestations would be helpful to the diagnosis.But the final diagnosis of the tumors must be determined by histological examinations.

Abstract:Renal metanephric adenoma (MA) is an extremely rare benign tumor, especially in children under the age of 12 years old. In this article, we report a case of MA that was incidentally discovered in a 2-year-old girl by ultrasound. Magnetic resonance scan revealed a mass of right kidney that was hypointense and hyperintense on T1- and T2-weighted images, respectively. Laparoscopic nephron-sparing surgery was carried out to remove the lesion. Histopathologic examination of the specimen revealed MA with clear surgical margins. Immunohistochemically, the tumor cells showed focal reactivity for CD56, pan-cytokeratin (AE1/AE3), and epithelial membrane antigen, and negative reactivity for Wilms' tumor 1 gene product. Meanwhile, the tumor stroma was diffusely positive for vimentin. We also review the literature regarding the clinical, imaging, and histopathologic features of this lesion in the pediatric population.

Abstract:Patients with metanephric adenoma have a good prognosis after undergoing total nephrectomy or local resection with kidney preservation. Accurate diagnosing is important for guiding clinical treatment. Only few previous case reports have been found focusing on the imaging findings of metanephric adenoma.To evaluate the multislice computed tomography (MSCT) imaging characteristics of metanephric adenoma.The imaging findings in eight patients with metanephric adenoma were studied retrospectively. MSCT was undertaken to investigate tumor location, size, attenuation, cystic or solid appearance, calciﬁcation, capsule sign, and enhancement pattern.Tumors (mean diameter, 3.3 ± 1.0 cm) were solitary (8/8), solid (7/8) with cystic components (2/8), no calcifications (7/8), had a poorly-deﬁned margin (8/8), were centered in the renal medulla (7/8), compressed the renal pelvis (3/8), and none had retroperitoneal lymph node metastasis. The attenuation of metanephric adenoma tumors was less or equal compared to the renal cortex or medulla on unenhanced CT (30.6 ± 2.6 vs. 36.3 ± 4.6 vs. 33.2 ± 3.9, P > 0.05), while tumor enhancement after administration of an contrast agent was lower than that of normal renal cortex and medulla during all phases (P < 0.05).Metanephric adenoma tends to be a solitary, poorly defined margin, isodense or hypodense mass arising from the renal medulla with enhancement less than the cortex and medulla during all phases.

Abstract:Objective:To improve the understanding of primary ureter carcinoma and to avoid misdiagnosis.Methods: Clinical dada of three patients with primary ureter carcinoma were analyzed retrospectively.Results: three patient had been misdiagnosed as PUJ obstruct,urethrophyma and kindney tuberculosis.Conclusion: The incidence of primary ureter carcinoma was lower .The early stage of primary ureter carcinoma has no clinical features except hematuria. It could be easily misdiagnosed .The improvement of the understanging of early primary ureter carcinoma and careful examinations include B-ulrtasound,IVP and uroscopic were the keys to correct diagnosis and early treatment.

Abstract:Objective To investigate the clinical and pathological characteristics of metanephric adenoma, and to improve the diagnosis and treatment of metanephric adenoma. Methods Clinical data of one patient with renal adenoma who was admitted to our hospital in the last year were retrospectively analyzed and the literature was reviewed. Results Surgical procedure was performed for right renal tumor in the patient 2 weeks after successful completion of the left ureteroscopic lithotripsy. Intraoperative frozen-section biopsy indicated the features of renal hamartomas and nephron sparing surgery was performed. Postoperative pathological findings indicated that tumor cells were characterized by cubic and quasi-circular shape, quasi-circular nuclei, less cytoplasm, and acidophilic staining properties. Metanephric adenoma was considered through immunohistochemical staining. The patient was discharged from the hospital 8 days after operation. After the 17 months of follow-up, the patients had disease-free survival without recurrence and metastasis. Conclusion Metanephric adenoma is easily misdiagnosed as renal cell carcinoma. The clinical and pathological findings contribute to the clinical and differential diagnosis of metanephric adenoma. Nephron sparingsurgery is often performed for the treatment of metanephric adenoma. But due to its biological behavior and the uncertainty of cell origin, careful follow-up should be carried out in patients with metanephric adenoma.
Clinical Features Analysis of Metanephric Adenoma: a Series of 16 Cases

Abstract:Objective To study the clinical characteristics,image findings,therapeutic method and prognosis of metanephric adenoma. Method The clinical characteristic,image findings,operation methods and prognosis of 16 metanephric adenoma patients treated at Department of Urology,Peking University First Hospital from January 2004 to March 2016 were analyzed retrospectively.Results There were 6 male and 10 female patients in the study. The mean age of patients was 33.7 years(ranging from 14 to 83 years). Two patients came to the hospital because of fever,while other 14 patients had no symptoms and found renal tumor by medical examination. One case was found polythemia vera and another 1 case showed mild anemia. Serum creatine of all the cases were in normal range. The tumor of 11 cases were at left side and 5 cases were at right. All patients took urinary tract ultrasound. Fifteen patients took CT examination. Among them, 14 cases were solid mass and 1 case was cystosolid.CT value was(41± 4)HU. CT scan showed that the tumor was slight enhanced and CT value increased to(77±9)HU. Six patients took MRI examination. The MRI showed high or low signal of T1WI or T2WI scans.Tumor size was(4.7±3.9)cm(ranging from 1.7 to 17.5 cm). All 16 patients took operation and 11 of them took laparoscopic surgery while the other 5 cases took open surgery. Eleven cases took partial nephrectomy, 4 cases took nephrectomy and 1 case took nephroureterectomy. The surgical procedures were all successful and no complications occured during perioperative period. All cases were all confirmed metanephric adenoma by postoperative pathology and surgery cut edge were all negative. Immunohistochemical study showed that the positive rate of Vimentin, CD57, AE1/AE3, WT1, CK7 and AMACR respectively were 16/16, 15/16, 12/16, 10/16, 3/16 and 2/16. The median follow-up time of 16 cases was 44 months(ranging from 8 to 125 months)and none had recurrence or metastasis.One case died 125 months after surgery because of advanced age(83 years old). Conclusions Metanephric adenoma is difficult to be diagnosed relying on clinical characteristics and image features. Pathology can help confirm the diagnosis. Partial nephrectomy is the first choice for operation and can achieve good prognosis. But it still needs a regular follow-up.

Abstract:Objective To analyze the clinical manifestations and the imaging mainly CT of metanephric adenoma in chidren. In our new cases, one showed a large polycystic mass abounded with fat-attenuation material. Conclusion Manifestations of metanephric adenoma varied from each other, we should analyze the clinical manifestations as well as the characteristics of imagings and it would help for diagnosis. But the diagnosis of the tumors must be based on histological examinations.

Abstract:Objective To improve the diagnostic accuracy of primary renal rare benign tumors by exploring and analyzing the CT/MRI and clinical pathologic features.Methods 9 patients with primary renal rare benign tumors pathologically proven lesions after operation who had CT or MRI exams with contrast were enrolled in our hospital.The radiological and clinical pathological features of all tumors were analyzed respectively.Results The cases were the mixed epithelial and stromal tumor of kidney (MESTK),cystic nephroma(CN),renal leiomyoma(RL)and renal oncocytoma(RO)respectively.Location:6 cases were in left kidney and 3 in right kidney.Size:long diameter 2.5-8.9 cm,mean 5.7 cm;short diameter 2.5-8.4 cm,mean 4.9 cm.The tumor shape included oval(n=7) and irregular(n=2).9 cases protruded from the renal surface.MESTK showed polycystic lesions on preconstrast,various degrees of cystic wall and septum enhancement were detected on enhancement scans.CN showed cystic lesions and calcification on preconstrast, slight septum enhancement were detected on enhancement scans.RL showed heterogeneous density or signal with patchy necrosis and clear border on precontrast,the obvious enhancement was found on cortex and medullary phase and slight washout enhancement was detected on excretory phase.RO showed heterogeneous hypointense on T1 WI,hyperintense on T2 WI and mixed isodensity and hypodensity on CT precontrast.Slight delayed enhancement was found in central scar and washout enhancement was detected in parenchyma on three phases.Conclusion There are some special CT/MRI characteristics for primary renal rare benign tumors,which could improve their diagnosis and differential diagnosis combined with the clinical pathological features.

Abstract:目的提高对肾脏、前列腺恶性纤维组织细胞瘤(malignant fibrous histiocytoma,MFH)的认识.方法 MFH患者3例,均为男性.2例原发性肾脏MFH者年龄为69、73岁,临床表现为低热或腰部疼痛、消瘦,B超及CT示肾脏实性占位性病变,分别为8.0 cm×7.0 cm、7.4 cm×6.6 cm.1例原发性前列腺MFH患者年龄39岁,临床表现为排便困难,B超及CT示前列腺内囊实性占位性病变18.1 cm×14.5 cm.结果 3例分别行根治性左肾切除术、左肾切除加局部浸润组织切除、膀胱前列腺切除术加直肠切除术加乙状结肠造瘘术.首次手术均完整切除肿瘤.光镜下见肿瘤细胞常排列成席纹状,形态多样,主要由梭形的纤维母细胞样细胞和圆形、卵圆形组织细胞样细胞组成,并有不等量的多形性巨细胞及炎细胞.免疫组化染色示vimentin、α1-AAT、α1-ACT、CD68(+),EMA、Desmin、S-100(一).2例肾脏MFH患者分别于首次术后16、21个月因纵隔广泛转移或局部复发伴腹腔转移死亡,前列腺MFH患者术后3个月因肿瘤肺转移死亡.结论原发性肾脏和前列腺MFH是一类罕见的、高度恶性的肿瘤,预后差,确诊需依赖组织病理学及免疫组织化学检查,早期诊断及原发病灶的彻底切除是治疗的关键。

Abstract:To investigate the clinical and pathological features of metanephric adenoma (MA) and the clinical outcome after retroperitoneal laparoscopic nephron-sparing surgery.

Abstract:The recent recognition of a variety of pediatric renal tumors of different biologic behavior places an ever-increasing demand on the surgical pathologist for an accurate diagnosis. Although metanephric adenofibroma is one of the rarest benign renal tumors, the clinical importance of correctly diagnosing it cannot be overemphasized because it can potentially be mistaken as Wilms' tumor. We describe the clinical, radiologic, and pathologic features of a case of metanephric adenofibroma and discuss its differential diagnosis. The neoplasm was composed of two discrete components: a major fibroblastic element and a minor immature epithelial element. The latter formed a small nodule beneath the renal capsule, which could barely be detected by magnetic resonance imaging. This subcapsular nodule, however, was slightly soft and tan and was distinctly different from the white, whorled cut surface of the main tumor. It was formed by closely packed small immature epithelial cells in a slightly edematous background, which was histologically identical to metanephric adenoma and closely resembled epithelial Wilms' tumor. Unlike Wilms' tumor, however, the epithelial cells were very bland with no mitoses. The main bulk of the tumor was formed by spindle fibroblastic cells that were cytologically similar to the spindle cells in congenital mesoblastic nephroma. The tumor, however, was well demarcated without the irregular infiltrating edges of congenital mesoblastic nephroma. In contrast to the randomly distributed epithelial element throughout the stromal component in previous reported cases of metanephric adenofibroma, our finding of the exceedingly small and discrete epithelial component expands the known histologic spectrum of the disease. In addition, the presence of such minute epithelial nodule underscores the importance of diligent pathologic examination and careful sampling of tissue for histologic examination.

Abstract:We reported a case of metanephric adenofibroma in a 10-year-old boy to describe the clinical, radiologic, and pathologic features and discuss its treatment and differential diagnosis. Nephrectomy was performed for the patient; final histopathologic evaluation was that of a metanephric adenofibroma. Epithelial and stromal elements were both positive for WT-1, Vimentin, PAX2, and the epithelial tumor cells were also positive for S100, AE1/AE3, PAX8, CK8/18, EMA and a few cells were positive for CK7. Larger vessel wall components were positive for SMA, Des, caldesmon while capillary components were positive for CD10, CD31, and CD34. CA-9, α-inhibin and CD-56 were negative in the neoplasm. The Ki-67 labeling index was <1%. Metanephric adenofibroma is a rare benign renal tumor; the diagnosis of it relies on pathology and immunohistochemistry. As its rarity, there is no standard treatment for this disease. The majority of patients underwent nephrectomy and had good prognosis, as it is a benign neoplasm.
