Evaluating the histological characteristics of plaques in carotid artery by acoustic densitometry in patients with acute coronary syndrome

Abstract:Aim: To assess the characteristics of carotid atherosclerosis plaques in patients with acute coronary syndrome (ACS) by means of acoustic densitometry. Methods: (1)From March to September in 2003, 35 ACS patients treated at the Qilu Hospital of Shandong University were selected as ACS group, while 27 patients with stable angina pectoris (SAP) were taken as SAP group. They matched each other well in sex and age. (2)Instrument: Dutch Philips SONOS-5500 color Doppler ultrasound diagnostic apparatus, and the frequency of high-frequency detector in peripheral vessels was 4-10 MHz.(3)The thickening of artery tunica intima and the locus, size and echogenicity of the plaques were observed with ultrasound in AD status. The parameters of instrument: depth of collecting image 4 cm; overall gain (signal magnifying) 50 dB; Both lateral gain and time gain were compensated at 50 lines. Two-dimensional image was obtained at suitable angle of detectors, and continuous samplings were conducted to record on optical disks. According to the different two-dimensional images of atherosclerosis, the plaques were typed, and the area percentage of low echo in plaques was also calculated. Results: According to the intention-treatment analysis, 35 ACS patients and 16 SAP patients were all involved in the result analysis.(1)Comparison of average image intensity (AII) values in the ACS patients with different types of plaques: The AII values were (39.05±7.09) dB in normal tunica intima, (33.5±4.6) dB in fatty plaque, (44.38±3.14) dB in fibrous plaque, and (56.77±4.94) dB in calcify plaque.(2)The ratio of AII and adventitia (AIIc%) in ACS patients with different plaques were: (64.87±10.02)% in normal plaque, (61.80±4.38)% in fatty plaque, (78.36±7.86)% in fibrous plaque and (110.23±8.76)% in calcify plaque. The paired comparison demonstrated the significant differences (P < 0.05). The AIIc% was markedly lower in ACS patients than in SAP patients, with the significant difference [(71.18±12.53)%, (102.65±14.02)%, P <0.001].(3)Comparisons of the atherosclerosis incidence and the area percentage of low echo in plaques in two groups: The atherosclerosis incidence was 74.29% in ACS group, and peaked in fatty plaque (47.46%), whereas 88.9% in SAP group, and peaked in fibrous plaque (40%). Additionally, the area percentage of low echo in plaques was obviously higher in ACS group than in SAP group [(65.76±11.38)%, (21.65±8.44)%, P < 0.001]. Conclusion: Acoustic densitometry technique can evaluate the characteristics of plaques, and the carotid atherosclerosis plaques are associated with coronary atherosclerosis.
