Abstract:It remains unclear whether direct vessel wall imaging can identify carotid high-risk lesions in symptomatic subjects and whether carotid plaque characteristics are more effective indicators for cerebral infarct severity than stenosis. This study sought to determine the associations of carotid plaque characteristics by MR imaging with stenosis and acute cerebral infarct (ACI) sizes on diffusion weighted imaging (DWI).One hundred and fourteen symptomatic patients underwent carotid and brain MRI. ACI volume was determined from symptomatic internal carotid artery territory on DWI images. Ipsilateral carotid plaque morphological and compositional characteristics, and stenosis were also determined. The relationships between carotid plaque characteristics, stenosis and ACIs size were then evaluated.In carotid arteries with 30-49% stenosis, 86.7% and 26.7% were found to have lipid-rich necrotic core (LRNC) and intraplaque hemorrhage, respectively. Furthermore, 45.8% of carotid arteries with 0-29% stenosis developed LRNCs. Carotid morphological measurements, such as % wall volume, and the LRNC size were significantly associated with ipsilateral ACIs volume before and after adjustment for significant demographic factors (age and LDL) or stenosis in patients with carotid plaque (all p<0.05).A substantial number of high-risk plaques characterized by vessel wall imaging exist in carotid arteries with lower grade stenosis. In addition, carotid plaque characteristics, particularly the % wall volume and LRNC size, are independently associated with cerebral infarction as measured by DWI lesions. Our findings indicate that characterizing atherosclerotic plaque by MR vessel wall imaging might be useful for stratification of plaque risk and infarction severity.
Diagnostics Accuracy of Magnetic Resonance Imaging in Detection of Atherosclerotic Plaque Characteristics in Carotid Arteries Compared to Histology: A Systematic Review

Abstract:Carotid plaque composition represents one of the main risk factors of future ischemic stroke. MRI provides excellent soft tissue contrast that can distinguish plaque characteristics. Our objective was to analyze the diagnostic accuracy of MRI imaging in the detection of carotid plaque characteristics compared to histology in patients with symptomatic and asymptomatic carotid atherosclerosis through a systematic review. After prospective registration in PROSPERO (ID CRD42022329690), Medline Ovid, Embase.com, Cochrane Library, and Web of Science Core were searched without any search limitation up to May 27, 2022 to identify eligible articles. Of the 8168 studies, 53 (37 × 1.5 T MRI, 17 × 3 T MRI) evaluated MRI accuracy in the detection of 13 specific carotid plaque characteristics in 169 comparisons. MRI demonstrated high diagnostic accuracy for detection of calcification (3 T MRI: mean sensitivity 92%/mean specificity 90%; 1.5 T MRI: mean sensitivity 81%/mean specificity 91%), fibrous cap (1.5 T: 89%/87%), unstable plaque (1.5 T: 89%/87%), intraplaque hemorrhage (1.5 T: 86%/88%), and lipid-rich necrotic core (1.5 T: 89%/79%). MRI also proved to have a high level of tissue discrimination for the carotid plaque characteristics investigated, allowing potentially for a better risk assessment and follow-up of patients who may benefit from more aggressive treatments. These results emphasize the role of MRI as the first-line imaging modality for comprehensive assessment of carotid plaque morphology, particularly for unstable plaque. Abstract:Objective:To evaluate the value of high resolution MR imaging in detecting carotid atherosclerosis,and in characterizing the morphology of individual atherosclerotic carotid plaques.Methods:High resolution carotid arteries MR imaging was performed in 19 patients and 3 healthy volunteers using 3 inch surface coil with three sequences,including DIR T1WI,FSE T2WI and 3D-TOF.Results: 15 patients with 31 changes of carotid atherosclerosis were demonstrated clearly by MRI,including 4 in the common carotid artery,carotid artery bifurcation 17,internal carotid artery 9,and external carotid artery 1.All plaques showed defects in the high intensity of vessel lumen on TOF sequence,and presented moderate,low and/or high intensity on DIR T1WI and T2WI.MRI classification included type Ⅲ 12,type Ⅳ~Ⅴ 11,type Ⅵ 2 and type Ⅶ 6.Conclusion:MRI is a powerful noninvasive imaging tool with high spatial resolution in determining atherosclerotic plaque volume and tissue components.This imaging technique is useful for treatment planning and estimation of therapeutic efficacy and outcome of the patients.[
High-resolution MR Imaging Used for Fibrous and Lipid Components of Carotid Atheromatous Plaques in Vivo

Abstract:Objective:To explore MR findings of the fibrous and lipid components of carotid atheromatous plaques in vivo with high-resolution magnetic resonance imaging.Methods:37 patients with carotid atheromatous plaques revealed by color doppler ultrasound underwent high-resolution MR scanning using the protocol of time-of-flight MR angiography,DIR T1WI,PDWI and T2WI sequences.The total vessel wall area,the fibrous and lipid components and fibrous cap of carotid atheromatous plaques were determined by two observers,9 carotid endarterectomy specimens were acquired and the corresponding regions fibrous and lipid components of the pathologic specimens were compared with MRI findings.Results:52 carotid plaques were found in 37 patients,calcification was present in 40.4%(21/52),lipid core was present in 44.2%(23/52),lipid necrotic core in 6%(3/52),intraplaque hemorrhage was present in 10%(5/52),fibrous cap rupture(ulceration or fibrous cap rupture) was present in 6%(3/52).The lipid components manifested isointensity on TOF,isointensity or slight hyperintensity on T1WI and PDWI,isointensity or hypointensity on T2WI.However,the lipid necrotic core presented hyperintensity on four sequences;the fibrous components manifested isointensity or hypointensity on TOF,hyperintensity or slight hyperintensity on T1WI,isointensity or slight hyperintensity on PDWI,slight hyperintensity on T2WI.The mean fibrous cap thickness of calcified plaque was(1.1±0.4)mm,the mean fibrous cap thickness of non-calcified plaque was(0.7±0.3)mm,there was statistical significance between them.Conclusion:High resolution MRI can manifest plaque fibrous components,lipid components and the fibrous cap.

Abstract:Background and Purpose-To determine associations between stenosis, measures of plaque burden, and compositional features of carotid atherosclerosis, including high-risk features of intraplaque hemorrhage (IPH) and surface disruption.Methods-Institutional Review Board approval and informed consent for all participants were obtained before study initiation. Patients with either carotid stenosis >50% by duplex ultrasound or suspected coronary artery disease underwent multi-contrast carotid MRI at 3.0 T. For each artery, stenosis, percent wall volume (PWV=100% x wall volume/total vessel volume), and mean wall thickness (MWT) were measured. Presence or absence of a lipid-rich necrotic core, calcification, IPH, and surface disruption were recorded.Results-One hundred eighty-one patients were included in the final analysis. The area under the curve (AUC) calculated from receiver-operating-characteristics analysis found the presence of IPH was similarly classified by stenosis (AUC=0.82), PWV (AUC=0.88), and MWT (AUC=0.88). Notably, IPH was present in the lowest category of each parameter. Prevalence of IPH in arteries with 0% stenosis was 4.4%. In arteries with PWV <40%, prevalence was 3.2%; in arteries with MWT <1.0 mm, prevalence was 2.3%. Strength of classification for surface disruption was similarly classified by stenosis (AUC=0.87), PWV (AUC=0.93), and MWT (AUC=0.94).Conclusions-Measures of plaque burden do not substantially improve disease assessment compared to stenosis. The finding of IPH in all categories of stenosis and plaque burden suggests that direct characterization of plaque composition and surface status is necessary to fully discriminate disease severity. Abstract:The purpose of this study was to evaluate the differences in carotid arterial morphology and plaque composition between patients with acute coronary syndrome (ACS) and patients with stable coronary artery disease (SCAD). Twenty-eight patients (12 ACS patients and 16 SCAD patients) underwent carotid high-resolution MRI examination using a 3.0-Tesla (3.0T) MRI scanner. The indicators of carotid arterial morphology included the maximum total vessel area (Max-TVA), mean TVA, minimum lumen area (Min-LA), mean LA, maximum wall area (Max-WA), mean WA, maximum wall thickness (Max-WT), mean WT, maximum normalized wall index (Max-NWI), mean NWI, and maximum stenosis (Max-stenosis). The indicators of plaque composition included the prevalence and mean area percentage (%) of lipid-rich necrotic core (LRNC), calcification (Ca), intraplaque hemorrhage (IPH), and fibrous cap rupture (FCR). None of the indicators of carotid arterial morphology had significant differences (all P > 0.05) between the ACS and SCAD patients. The prevalence and plaque composition area percentage of LRNC, Ca, and IPH did not exhibit significant differences between the two groups. However, carotid plaques in the ACS patients presented a higher prevalence of FCR than SCAD patients (P < 0.05). This study revealed a similar carotid arterial morphology between ACS and SCAD patients. However, FCR is more common in carotid plaques with ACS than in those with SCAD. Ruptured carotid plaques may be a forewarning factor for those patients who are at high risk of ACS.

Abstract:Background: Accurate assessment of the vulnerability of carotid atherosclerotic plaques is crucial for stroke prevention. The three-dimensional (3D) magnetic resonance (MR) vessel wall imaging (VWI) has been increasingly employed to evaluate carotid plaques due to its extensive coverage and isotropic high spatial resolution. However, the accuracy of such techniques lacks validation by histology. Therefore, this study aims to validate the accuracy of 3D multi-contrast MR VWI with variable-flip-angle (VFA) and turbo spin echo (TSE) readout in identifying vulnerable carotid plaques, using histological analysis as a reference. Methods: Twenty-one male patients (mean age: 64.4 ± 7.2 years old) scheduled for carotid endarterectomy (CEA) were recruited in this study. All patients underwent carotid multi-contrast MR VWI, including 3D T1- and T2-weighted VFA-TSE sequences, as well as 3D time of flight (TOF) MR angiography (MRA), using a 3.0T MR system before surgery. Histological processing was performed for carotid plaque specimens. The presence or absence, along with the area measurements, of lipid-rich necrotic core (LRNC), intraplaque hemorrhage (IPH), and calcifications (CA) were independently evaluated on both MR images and histological sections. Cohen's kappa (κ) analysis was utilized to determine the agreement between 3D multi-contrast MR VWI and histology in identifying carotid plaque compositions before and after excluding compositions bellow certain size threshold. Spearman's correlation analysis was also conducted to assess the agreement in quantifying plaque compositions. Results: A total of 81 slices of MR images were successfully matched with histological sections. Moderate to almost perfect agreements were observed between 3D MR VWI and histology in the identification of LRNC (κ: 0.85 and 0.89), IPH (κ: 0.65 and 0.69), and CA (κ: 0.46 and 0.62) before and after excluding compositions smaller than 0.79 mm2. Strong to very strong correlations were found in the quantification of plaque compositions including LRNC (r=0.88), IPH (r=0.80), and CA (r=0.74) between MR imaging and histology. Conclusion: The 3D VFA-TSE multi-contrast MR VWI is capable of accurately characterizing vulnerable carotid atherosclerotic plaques.

Abstract:We compared plaque characteristics between symptomatic and asymptomatic sides in patients with bilateral carotid vulnerable plaques using magnetic resonance imaging (MRI). Participants (n = 67; mean age: 65.8 ± 7.7 years, 61 males) with bilateral carotid vulnerable plaques were included. Vulnerable plaques were characterized by intraplaque hemorrhage (IPH), large lipid-rich necrotic core (LRNC), or fibrous cap rupture (FCR) on MRI. Symptomatic vulnerable plaques showed greater plaque burden, LRNC volume (median: 221.4 vs 134.8 mm 3 , P = .003), IPH volume (median: 32.2 vs 22.5 mm 3 , P = .030), maximum percentage (Max%) LRNC (median: 51.3% vs 41.8%, P = .002), Max%IPH (median: 13.4% vs 9.5%, P = .022), cumulative slices of LRNC (median: 10 vs 8, P = .005), and more juxtaluminal IPH and/or thrombus (29.9% vs 6.0%, P = .001) and FCR (37.3% vs 16.4%, P = .007) than asymptomatic ones. After adjusting for plaque burden, differences in juxtaluminal IPH and/or thrombus (odds ratio [OR]: 5.49, 95% CI: 1.61-18.75, P = .007) and FCR (OR: 2.90, 95% CI: 1.16-7.24, P = .022) between bilateral sides remained statistically significant. For patients with bilateral carotid vulnerable plaques, symptomatic plaques had greater burden, more juxtaluminal IPH and/or thrombus, and FCR compared with asymptomatic ones. The differences in juxtaluminal IPH and/or thrombus and FCR between bilateral sides were independent of plaque burden.

Abstract:Stroke is a leading cause of mortality worldwide. One of its main reasons is rupture of carotid atherosclerotic plaques. Conventional B-mode ultrasound images and Doppler/color flow measurements are mostly used to evaluate degree of carotid atherosclerotic stenosis. However, stenosis underestimates plaque vulnerability due to positive remodeling. In addition, studies have shown that traditional B-mode ultrasound has limited capability in characterizing vulnerability associated plaque compositional features, particularly intraplaque hemorrhage. In contrast, the correspondence between multi-contrast magnetic resonance imaging (MRI) features, including morphology and plaque compositions, and histology has been well established. In this study, the feasibility of B-mode image combined with strain image in composition characterization and risk assessment of carotid atherosclerotic plaques is investigated. Preliminarily in-vivo results on a small number of human subjects are initially validated by multi-contrast, high-resolution MRI, and it shows that combination of echogenicity and strain values might be feasible to evaluate the plaque vulnerability.

Abstract:Background: Magnetic resonance imaging (MRI) has the potential in assessing the inflammation of perivascular adipose tissue (PVAT) due to its excellent soft tissue contrast. However, evidence is lacking for the association between carotid PVAT measured by MRI and carotid vulnerable atherosclerotic plaques. This study aimed to investigate the association between signal intensity of PVAT and vulnerable plaques in carotid arteries using multi-contrast magnetic resonance (MR) vessel wall imaging.Methods: In this cross-sectional study, a total of 104 patients (mean age, 64.9 +/- 7.0 years; 86 men) with unilateral moderate-to-severe atherosclerotic stenosis referred to carotid endarterectomy (CEA) were recruited from April 2018 to December 2020 at Department of Neurosurgery of Peking University Third Hospital. All patients underwent multi-contrast MR vessel wall imaging including time-of-flight (ToF) MR angiography, black-blood T1-weighted (T1w) and T2-weighted (T2w) and simultaneous non-contrast angiography and intraplaque hemorrhage (IPH) imaging sequences. Patients with contraindications to endarterectomy or MRI examinations were excluded. The signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) of PVAT were measured on ToF images and vulnerable plaque characteristics including IPH, large lipid-rich necrotic core (LRNC), and fibrous cap rupture (FCR) were identified. The SNR and CNR of PVAT were compared between slices with and without vulnerable plaque features using Mann-Whitney U test and their associations were analyzed using the generalized linear mixed model (GLMM).Results: Carotid artery slices with IPH (30.93 +/- 14.56 vs. 27.34 +/- 10.02; P<0.001), FCR (30.35 +/- 13.82 vs. 27.53 +/- 10.37; P=0.006), and vulnerable plaque (29.15 +/- 12.52 vs. 27.32 +/- 10.05; P=0.016) had significantly higher value of SNR of PVAT compared to those without. After adjusting for clinical confounders, the SNR of PVAT was significantly associated with presence of IPH [odds ratio (OR) =0.627, 95% confidence interval (CI): 0.465-0.847, P-uncorr=0.002, PFDR=0.016] and vulnerable plaque (OR =0.762, 95% CI: 0.629-0.924, P-uncorr=0.006, P-FDR=0.020). However, no significant association was found between the CNR of PVAT and presence of vulnerable plaque features (all P>0.05).Conclusions: The SNR of carotid artery PVAT measured by ToF MR angiography is independently associated with vulnerable atherosclerotic plaque features, suggesting that the signal intensity of PVAT might be an effective indicator for vulnerable plaque.
Comparison of carotid atherosclerotic plaque characteristics between symptomatic patients with transient ischemic attack and stroke using high-resolution magnetic resonance imaging

Abstract:Background: This study aimed to compare the characteristics of carotid plaques between patients with transient ischemic attack (TIA) and ischemic stroke using magnetic resonance (MR) imaging. Methods: Patients with a recent ischemic stroke or TIA who exhibited atherosclerotic plaques of carotid arteries in the symptomatic sides determined by MR vessel wall imaging were recruited. The plaque morphology and compositions including intraplaque hemorrhage (IPH), lipid-rich necrotic-core (LRNC) and calcification were compared between TIA and stroke patients. Logistic regression was performed to relate the plaque characteristics to the types of ischemic events. Results: A total of 270 patients with TIA or ischemic stroke were recruited. Stroke patients had a significantly higher prevalence of diabetes (42.2% vs. 28.2%, p = 0.021), greater mean wall area (35.1 ± 10.1 mm2 vs. 32.0 ± 7.7 mm2, p = 0.004), mean wall thickness (1.3 ± 0.2 mm vs. 1.2 ± 0.2 mm, p = 0.001), maximum normalized wall index (NWI)(63.9% ± 6.0% vs. 62.2% ± 5.9%, p = 0.023) and %volume of LRNC (9.7% ± 8.2% vs. 7.4% ± 7.9%, p = 0.025) in the carotid arteries compared to those with TIA. After adjustment for clinical factors, above characteristics of carotid arteries were significantly associated with the type of ischemic events. After further adjustment for maximum NWI, this association remained statistically significant (OR, 1.41; CI, 1.01-1.96; p = 0.041). Conclusions: Ischemic stroke patients had larger plaque burden and greater proportion of LRNC in carotid plaques compared to those with TIA. This study suggests that ischemic stroke patients had more vulnerable plaques compared to those with TIA.

Abstract:Introduction Identifying patients with carotid atherosclerosis at risk for ischemic stoke would greatly benefit patient outcomes. Recently, high-resolution methods have enabled MRI to noninvasively image the fine internal structure of the carotid plaque [1]. Some carotid plaque features, such as the presence of intraplaque hemorrhage (IPH) and thin or ruptured fibrous cap on multi-contrast weighted MR imaging, have been reported to be predictors of subsequent stroke [2]. However, multi-contrast weighted MRI requires a long scan time. On the other hand, some simple morphology characteristics of carotid atherosclerosis are believed to be indicators of high risk plaque. Among those characteristics, plaque eccentricity of carotid artery has recently been found to be associated with ipsilateral cerebrovascular events in an ultrasonography study [3]. In this study, we sought to explore the contribution of plaque eccentricity to identify high-risk plaque features in carotid artery by MRI.

Abstract:Objective To study the relationship between progression and clinical characteristics of carotid plaques at different sites in elderly carotid atherosclerosis patients.Methods Fifty-one elderly carotid atherosclerosis patients who underwent twice of high resolution MRI in our hospital were included in this study.The MRI data of 75 carotid arteries,annual progression of stenosis,maximum wall thickness,wall size,lumen size at plaques in common carotid artery (CCA),carotid bifurcation (BIF) and internal carotid artery (ICA) were analyzed.Results Of the 131 plaques in 75 carotid arteries,42 were detected in ICA,54 were detected in BIF.The annual progression rates of wall size and maximum wall thickness were higher at plaques in ICA than at those in CCA and BIF (P=0.036,P=0.028).The wall size of plaques in ICA was related with age,hypertension and diabetes (r=0.39,P=0.011;r=0.37,P=0.016;r=0.31,P=0.041).Conclusion The progression of plaques in ICA is faster than that of those in CCA and BIF,and is related with the high clinical risk factors.MRI follow-up can characterize the progressive plaques in elderly carotid atherosclerosis patients.
Shape and Location of Carotid Atherosclerotic Plaque and Intraplaque Hemorrhage: A High-resolution Magnetic Resonance Imaging Study

Abstract:Aim: The present study aimed to investigate the association between shape and location of atherosclerotic plaques and intraplaque hemorrhage (IPH) in carotid arteries using magnetic resonance (MR) imaging. Methods: Overall, 114 symptomatic patients (mean age: 64.9 +/- 10.9 years; 81 males) who underwent MR imaging and had advanced carotid plaques were included in analysis. IPH presence and carotid plaque shape and location (below and above bifurcation) were evaluated. The plaque shape was defined as follows: type-I: the arc-length of plaque is greater in the upstream; type-II: the arc-length of plaque in downstream and upstream is equal; and type-Ill: the arc-length of plaque is greater in downstream. The plaque shape and location were compared between plaques with and without IPH and their associations with IPH were determined. Results: Of 181detectedplaques, 57 (31.5%) had IPH. Compared with plaques without IPH, those with IPH had higher incidence of the plaque shape of type-I (66.7% vs. 32.2%, P< 0.001), lower incidence of plaque shape of type-III (24.6% vs. 50.0%, P- 0 .001), and were more likely located above carotid bifurcation (71.9% vs. 48.4%, P=0.003). The plaque shape of type-I (OR, 4.01; 95%Cl, 1.36-11.83; P=0.012) and location above bifurcation (OR, 3.21; 95%Cl, 1.07-9.61; P=0.037) of carotid plaques were significantly associated with IPH after adjusting for confounder factors. Conclusions: Carotid plaque shape and location are significantly associated with the occurrence of IPH. Our findings could provide new insights for the pathogenesis of IPH and vulnerably plaques.
Characteristics of Symptomatic Intra- and Extracranial Atherosclerotic Plaques in Anterior Circulation Based on Multi-contrast Magnetic Resonance Vascular Wall Imaging

Abstract:Objective To compare the characteristics of intra- and extracranial plaques using 3D multi-contrast magnetic resonance vessel wall imaging in patients with symptomatic intra- and extracranial atherosclerotic diseases. Methods The data of patients with symptomatic intra-and extracranial carotid artery culprit plaques identified by MR vessel wall imaging were retrospectively analyzed. The distribution, burden and components of plaques were analyzed. The plaque burden including the maximum wall thickness (Max WT), plaque length and vessel stenosis were measured. Plaque components such as lipid-rich necrotic core (LRNC), intraplaque hemorrhage (IPH) and calcification on MRI were evaluated. The imaging characteristics of plaques between intracranial and extracranial arteries were compared. Results In total, 45 patients were recruited and 203 atherosclerotic plaques were detected, including 156 extracranial plaques (76.8%) and 47 intracranial plaques (23.2%). Max WT [(3.2±1.0) mm vs (1.9±0.4) mm, P<0.001] and plaque length [10.7 (7.5-13.7) mm vs 4.3 (2.6-6.3) mm, P<0.001] of extracranial plaques were significantly larger than that of intracranial plaques. Stenosis degree [28.0 (20.0-38.5)% vs 18.8 (10.8-30.5)%, P<0.001] of intracranial plaques were significantly larger than that of extracranial plaques. Compared with intracranial plaques, extracranial plaques showed more higher rate of LRNC (66.0% vs 21.3%, P<0.001) and lower rate of IPH (13.5% vs 27.7%, P=0.027). Conclusions For patients with atherosclerotic culprit plaques and symptomatic intracranial and extracranial artery diseases, extracranial plaques commonly have greater Max WT, higher rate of LRNC, less severe stenosis and lower rate of IPH than intracranial plaques.

Abstract:Objective To evaluate the application value of high resolution MRI in the detection of carotid arteriosclerosis and in characterizing the morphology of individual atherosclerotic carotid plaques in old patients. Methods Carotid arteries MRI was performed in 16 elderly patients. DIR-T1WI,FSE-T2WI and 3D-TOF sequence were used in the MRI measurement for all patients. Results Of 16 patients, 13 showed changes of carotid arteriosclerosis on 29 locations,including 4 in the common carotid artery, 17 in carotid artery bifurcation, 7 in internal carotid artery, and 1 in external carotid artery. The MRI classification included 12 cases of type Ⅲ,9 of type Ⅳ-Ⅴ,3 of type Ⅵ and 5 of type Ⅶ. All plaques showed defects in the high intensity of vessel lumen on TOF sequence,and presented moderate,low and(or) high intensity on DIR-T1WI and T2WI. Conclusions MRI is a powerful noninvasive imaging tool with high spatial resolution in determining atherosclerotic plaque volume and tissue components. This imaging technique is useful in establishing treatment plan and estimating the therapeutic efficacy and outcome of the patients.
Identifying vulnerable plaques: A 3D carotid plaque radiomics model based on HRMRI

Abstract:Background: Identification of vulnerable carotid plaque is important for the treatment and prevention of stroke. In previous studies, plaque vulnerability was assessed qualitatively. We aimed to develop a 3D carotid plaque radiomics model based on high-resolution magnetic resonance imaging (HRMRI) to quantitatively identify vulnerable plaques. Methods: Ninety patients with carotid atherosclerosis who underwent HRMRI were randomized into training and test cohorts. Using the radiological characteristics of carotid plaques, a traditional model was constructed. A 3D carotid plaque radiomics model was constructed using the radiomics features of 3D T1-SPACE and its contrast-enhanced sequences. A combined model was constructed using radiological and radiomics characteristics. Nomogram was generated based on the combined models, and ROC curves were utilized to assess the performance of each model. Results: 48 patients (53.33%) were symptomatic and 42 (46.67%) were asymptomatic. The traditional model was constructed using intraplaque hemorrhage, plaque enhancement, wall remodeling pattern, and lumen stenosis, and it provided an area under the curve (AUC) of 0.816 vs. 0.778 in the training and testing sets. In the two cohorts, the 3D carotid plaque radiomics model and the combined model had an AUC of 0.915 vs. 0.835 and 0.957 vs. 0.864, respectively. In the training set, both the radiomics model and the combination model outperformed the traditional model, but there was no significant difference between the radiomics model and the combined model. Conclusions: HRMRI-based 3D carotid radiomics models can improve the precision of detecting vulnerable carotid plaques, consequently improving risk classification and clinical decision-making in patients with carotid stenosis.

Abstract:Stroke is the third leading cause of death in the United States. The degree of luminal narrowing evaluated by angiography is the standard for assessing the risk of stroke in patients with carotid atherosclerosis and for determining the need for surgical intervention. However, multiple studies have shown that clinical events arise not from the degree of stenosis but from the morphologic characteristics and plaque composition. This is borne out by the difference in absolute risk reduction between symptomatic and asymptomatic patients who receive surgical carotid endarterectomy (CEA). Future clinical practice may diagnose patients with high-risk atherosclerosis, based on plaque characteristics and morphology rather than the degree of stenosis alone. Carotid MRI is a noninvasive imaging method that can provide information on atherosclerotic plaque morphology, composition, and progression or regression. This chapter describes the current capabilities of MRI for visualizing carotid atherosclerosis, including MRI protocols to appropriately evaluate carotid plaque, the image features of carotid arteries, and the future direction of carotid MR imaging and how it can be better used for the management of patients with subclinical atherosclerosis, resulting in a higher quality of life.
Correlation of Atherosclerotic Plaque Compositions in Coronary and Carotid Arteries

Abstract:Introduction: Atherosclerosis has been shown to be a systematic disease which often involves multiple vascular beds, such as coronary and carotid arteries . The size of carotid lipid-rich necrotic core (LRNC) and presence of intraplaque hemorrhage (IPH) were demonstrated to be high risk features due to their correlations with neurovascular events . For coronary atherosclerosis, previous studies suggest that coronary mixed type of atherosclerotic plaques may be vulnerable lesions prone to developing cardiac events . These studies indicate that the assessment of plaque compositional features in both coronary and carotid arteries is necessary. Recently, a number of studies demonstrated a significant correlation between coronary and carotid atherosclerosis . However, the association of coronary and carotid plaque compositions is unknown. Purpose: This study sought to determine the correlation of atherosclerotic plaque compositions between coronary and carotid arteries using CT and MRI. Methods: One hundred and twenty-three subjects (mean age 58 years, 96 males) with suspected coronary artery disease underwent coronary CT angiography (CTA) on a Siemens Dual Source CT and carotid MR imaging on a GE 3.0T MR scanner within 2 weeks interval. Coronary CTA imaging parameters were as follows: 64× 0.6 mm, 0.33s/rot, 120 kV, 350 mAs, pitch 0.20-0.28, reconstructed image slice thickness 0.75mm with 0.5mm slice spacing on 60-80% RR interval. Contrast media (Iomeprol 370mg I/ml, Bracco) was injected intravenously with bolus of 70ml at 5 ml/s. A standardized multi-contrast protocol [6] was used to perform carotid MR imaging with the following parameters: T1w, quadruple inversion-recovery, TR/TE 800/8.8ms; Double IR, TR/TE 3000/13.1ms for PDw and 3000/56.9 ms for T2w; and 3D TOF: TR/TE 29/2.1ms, flip angle 20°. MR images were acquired with 2 mm slice thickness, field of view of 14 cm, and matrix size 256×256. Coronary plaques were classified into three types: non-calcified, calcified, and mixed type plaques . The presence /absence of three types of plaques and plaque scores (plaque number) for three types of plaques were evaluated for each subject. Presence/absence and the volume of calcification (CA), LRNC and IPH were indentified and measured by two trained reviewers with consensus blinded to coronary CT images using a custom designed software (CASCADE , Seattle, WA, USA). The three types of coronary plaque scores in patients with and without carotid CA, LRNC, and IPH were compared. The Volumes of carotid CA, LRNC, and IPH in subjects with and without coronary non-calcified, calcified, and mixed type plaques were also compared. ROC analysis was used to determine the AUC of coronary plaque scores for classifying presence of carotid CA, LRNC, and IPH. In addition, the AUC of carotid CA, LRNC, and IPH volume for classifying coronary different type of plaque were also analyzed. Results: Subjects with carotid CA or LRNC showed significantly larger coronary three types of plaque scores than those without carotid plaque compositions (Table 1).
