Abstract:Objective The aim was to compare the short-term (30 days after surgery) and long-term (over 6 months' follow-up) postoperative safety of off-pump and on-pump coronary artery bypass grafting (CABG) for patients with coronary heart disease (CHD).Methods Relevant randomized controlled trials (RCTs) were obtained from PubMed, Embase, Springer, Ovid, and Cochrane library up to July 2013. Odds ratios (ORs) with their 95% confidence intervals (CIs) were used to access effect sizes.Results Seven RCTs involving 9,128 patients were included. Results suggested no significant difference in all the short-term outcomes (mortality, OR = 0.90, 95% CI: 0.69 to 1.16, p = 0.41; myocardial infarction, OR = 0.95, 95% CI: 0.79 to 1.15, p = 0.61; stroke, OR = 0.78, 95% CI: 0.56 to 1.07, p = 0.12; renal failure, OR = 0.84, 95% CI: 0.60 to 1.18, p = 0.43; revascularization: OR = 1.95, 95% CI: 0.79 to 4.85, p = 0.15) and some long-term outcomes (mortality, OR = 1.02, 95% CI: 0.86 to 1.22, p = 0.81; myocardial infarction, OR = 0.86, 95% CI: 0.70 to 1.04, p = 0.12; stroke, OR = 0.89, 95% CI: 0.67 to 1.19, p = 0.44) between off-pump and on-pump CABG. However, off-pump CABG had a significantly higher revascularization rate (OR = 1.45; 95% CI: 1.02 to 2.06; p = 0.04) than on-pump CABG in long-term follow-up.Conclusion Short-term postoperative safety was similar between off-pump and on-pump CABG. A high revascularization rate was the drawback of off-pump CABG for CHD patients in long-term follow-up.

Abstract:The patients of heart diseases combined with chronic idiopathic thrombocytopenic purpura face higher bleeding risk after being performed open heart surgery on cardiopulmonary bypass(CPB). By means of preoperative adrenal corticosteroids, high-dose intravenous gamma-globulin, danazol, preoperative or simultaneous splenectomy, use of membranous oxygenator , intraoperative administration of aprotinin and/or tranexamic acid, heparin-coated CPB ducts, centrifugal pump, Cell Saver, normothermic CPB, and platelets transfusion at the termination of CPB, the risk of bleeding complications in the postoperative period will be significantly lowered.
Analysis of 272 consecutive off pump coronary artery bypass patients

Abstract:Objective:To analyze early clinical results of 272 consecutive patients undergone off pump coronary artery bypass(OPCAB) procedure.Methods:OPCAB was performed for 272 consecutive patients without conversion to cardiopulmonary bypass.The average number of distal anastomoses was 3.2±0.9,for which the grafts used were 258 left internal mammary arteries(LIMA),107 radial arteries(RA) and 258 saphenous veins.Results: The extubation time was 13±5h and the ICU stay was 52±15h.There were 190 patients blood transfusion.The operative complication was including 3 myocardial infarction,3 bleeding,1 stroke and 0 mediastinitis, 1 new renal failure and 6 life-threatening arrhythmia.30 patients used IABP.4 death occur in hospital,mortality is 1.47%.Conclusions: Our experience suggests that OPCAB procedures are feasible and safe for most patients,and suitable for a wider application in clinic.
Comparative study on the efficacy of coronary artery bypass grafting under cardiac-pulmonary bypass and non-cardiac-pulmonary bypass

Abstract:Objective:To evaluate clinic efficacy of off pump coronary artery bypass grafting (OPCABs) and conventional CABGs(CCABG).Methods:Retrospectively analysed the operative results of 158 cases of CABGs.Results:There were no significant differences in clinic characteristic between 2 groups. The average bypass grafts for every patient were 2.1 ± 0.6 in OPCAB group and 3.4 ± 0.9 in CCABG group(P 0.05 ). There were no operative death. Four case of postoperative deaths occurred in CCABG group. The rate of nerve psychiatric complications occurred was 2.6 % in OPCAB and 6.1 % CCABG group(P 0.05 ). The results of 1 13 month follow up showed that recurrent rate of angina pectoris was 4.0 % in OPCAB and 4.9 % in CCABG. There were no follow up death.Conclusion:OPCAB procedure is a practicable, safe alternative to conventional CABG. Compared with CCABG, OPCAB shows some advantages, but cannot replace CCABG.
Peri-operative Management of Off-Pump Coronary Artery Bypass Grafting

Abstract:Objective To retrospectively analyse the clinical data of off-pump coronary artery bypass grafting surgery(OPCAB)in our department and summarize the clinical experience of peri-operative management.Methods From January 2007 to May 2008,253 patients underwent OPCAB.Seventy-six cases with left main disease(including 18 cases of single left main lesion and 58 cases of complex lesion);13 cases had single vessel disease;32 cases had double vessels disease and 190 cases had triple vessels disease.Eighteen cases received emergency OPCAB.All vessel anastomosis were performed with coronary artery stablizer and shunt through median sternotomy.Resuits Three cases died after operation(1.2%).Intra-aortic balloon pump was used in 11 cases.Mechanical ventilation time was 3-168 h[(24.1±22.3)h],and ICU stay time was 1-14 day[(2.8±2.0)d].Postoperative hospital stay time was 8-42 days[(15.6±6.3)d].Conclusion OPCAB is reliable for most bypass cases.With appropilate peri-operative management,a satisfied result will be acceptable.

Abstract:Objective To summarize the clinical experiences of 1386 patients undergoing off-pump coronary artery bypass grafting(OPCAB)during Jan 2002 to Mar 2008.Methods OPCAB was done through midsternumtomy under ordinary-temperature anesthesia after radial artery,left breast artery and vena saphena magna were taken.Patients over 70 years old accounted for 32.35%,and the oldest was 99 years old.Emergency and subemergency OPCAB was done in 111 cases,re-do CABG with off-pump technique was done in 52 patients.Results 24 cases died during perioperation,with mortality of 1.73%and all other patients discharged from the hospital.Following operation,the symptoms of angina pectoris were relieved and the life quality was much beaer than that before operation.Conclusion OPCAB is feasible for older patients,who are complicated with other chronic diseases for elder patients.Skillful technique of surgeon,right indications for OPCAB and management of peri-and postoperation are key to success of operation.
Analysis of Hemodynamic Changes in 100 Patients Undergoing Off-Pump Coronary Artery Bypass Surgery

Abstract:Objective To analyze retrospectively 100 patients who underwent off-pump coronary artery bypass grafting surgery(OPCABG)in order to summarize the hemodynamic changes.Methods One hundred patients undergoing OPCABG were collected and analyzed retrospectively.To analyze the hemodynamic changes including heart rate(HR),mean arterial pressure(MAP),central venous pressure(CVP),mean pulmonary artery pressure(MPAP),mixed venous oxygen(SVO2)and cardiac output(CO).The changes of hemodynamic were recorded at the moment of every graft anastmosised and during left circumflex coronary artery (LCX) anastmosised.Results The patients undergoing OPCABG had hemodynamic derangements during cardiac displacement: MAP and CO decreased significantly in all territories(P0.05 or P0.01),CVP increased significantly in all anastomosed distal blood vessels.That was serious when the CX or RCX was performed.Conclusion Cardiac displacement and compression induced by exposure and anastomosis of distal blood vessels during OPCABG bring usually hemodynamic changes,especially in CX and RCA territories.Those hemodynamic changes are transient and well tolerated under suitable anesthetic management.

Abstract:Objective To screen the risk factors for blood coagulation abnormality in patients undergoing off-pump coronary artery bypass grafting (OPCABG).Methods A total of 140 patients undergoing elective OPCABG were included in this study,and combined intravenous-inhalational anesthesia was performed during operation.The patients were divided into normal group and abnormal group according to whether or not blood coagulation abnormality developed during operation and within 48 h after operation.The data such as gender,age,body mass index,American Society of Anesthesiologists physical status,the number of operation per year for surgeons,comorbidities (hypertension and diabetes mellitus),preoperative hematocrit (Hct),left ventricular ejection fraction,arterial oxygen pressure,liver function,operation time and requirement for intraoperative continuous cardiac output monitoring,positive end expiratory pressure,tranexamic acid,ulinastatin and hydroxyethyl starch,postoperative acidosis and hypothermia were recorded.Results Blood coagulation abnormality was found in 43 patients,and the incidence was 31％.The results of logistic regression analysis showed that the number of operation per year for surgeons＜ 50,preoperative abnormal liver function,preoperative Hct＜35％,surgery time≥240 min,no use of continuous cardiac output monitoring during operation and postoperative hypothermia were risk factors for blood coagulation abnormality in patients undergoing OPCABG.Conclusion The number of operation per year for surgeons＜50,preoperative abnormal liver function,preoperative Hct ＜ 35％,operation time ≥ 240 min,no use of continuous cardiac output monitoring during operation and postoperative hypothermia are risk factors for blood coagulation abnormality in patients undergoing OPCABG.
Early outcomes and clinical experience of 1198 off-pump coronary artery bypass procedure

Abstract:Objective: To summarize and analyze the early outcomes and clinical experience in 1198 patients undergone off-pump coronary artery bypass (OPCAB). Methods: This multi-center report covers 15 different institutes in China from October 1996 to May 2002. The ratio of OPCAB to conventional coronary artery bypass grafting (cCABG) was 85.2%. The mean age was (61.6±9.0) years. 77.3% patients were male and 22.7% were female. There were 34.6% patients with myocardial infarction, 58.2% with unstable angina pectoris, 18.7% with left main lesion, 76.9% with triple-vessel disease, 1.5% was redo surgery, and 9.9% emergent or urgent surgery. The mean left ventricular ejection fraction was 0.56±0.17. Results: The number of distal anastomosis was 3.1±0.9. IMA was used in 65.6% patients and radial artery was used in 18.0%. 13.5% OPCAB was combined with TMLR. IABP was used in 5.2% patients. The hospital mortality was 1.2%. The morbidity included 0.4% peri-operative myocardial infarction, 0.3% heart failure, 1.2% fatal arrhythmia, 2.0% pulmonary complications, 1.2% neurotic disorder, 0.7% renal dysfunction, 1.0% reopen for bleeding, and 0.6% delayed wound healing. Conclusion: OPCAB surgery is a safe and feasible procedure with good early results. Its late outcomes need further follow-up. OPCAB requires certain special skills and methods in practice so a specific training course should be designed.

Abstract:Objective To investigate the efficacy of on-pump coronary artery bypass grafting (on-pump CABG) and off-pump coronary artery bypass graft(OPCABG) in treatment of coronary heart disease(CHD). Methods Among the 160 CHD pa-tients underwent CABG,who were received in the hospital from January 2012 to December 2013,80 cases adopted on-pump CABG(on-pump CABG group),and the other 80 cases employed OPCABG(OPCABG group). To compare the operative time,ICU stay time,awakening time,blood transfusion,hospitalization costs,intubation time,bypass number and complications of the two groups. Results The operative time,ICU stay time,awakening time of OPCABG group were significantly shorter than those of the on-pump CABG group,while the blood transfusion and hospitalization costs were less than those of the on-pump CABG group, and the differences had statistical significance(P<0.05);the differences of intubation time and bypass number between the two groups were not statistically significant(P>0.05);the incidence rates of myocardial infarction,respiratory failure and pulmonary infection during perioperative period of the OPCABG group were lower than those of the on-pump CABG group,and the difference had statistical significance(P<0.05). Conclusion The OPCABG for CHD has advantages of quicker recovery,better safety and lower hospitalization costs ,so it is worthy of promoting in clinic.

Abstract:Objective To study the perioperative risk factors for emergency coronary artery bypass grafting(ECABG).Methods Clinical data about 49patients who underwent ECABG in our hospital were retrospectively analyzed.Results Of the 49patients who underwent ECABG,9received extracorporeal parallel circulation-supported CABG and 40underwent non-extracorporeal parallel circulation-supported CABG.Among the 25patients with the IABP implantation,the IABP was implanted in 18,5,and 2before operation,during operation,and after operation,respectively.Mitral valve surgery was performed for 8patients.Three patients died during perioperation with a death rate of 6.1%.Conclusion ECABG is an effective procedure for severe acute myocardial ischemia depending on a multi-science heart center with sufficient techniques and experiences.

Abstract:Objective To compare the outcomes of off-pump versus on-pump CABG.Methods From 2002 to 2008,CABG was performed in 105 patients aged 80 years and over,including 45 without cardiopulmonary bypass (CPB) or off-pump (OP) CABG (OPCAB) and 60 with CPB (onpump CABG).The outcomes were compared between two groups.Results The mean ICU stay was (37.1±30.3) h in OPCAB group and (60.6±58.2) h in on-pump CABG group (P＜0.01).Average ventilator-assisted time was (10.8±9.7) h for OPCAB group versus (22.3±35.7) h for onpump CABG group (P＜0.01).Postoperative atrial fibrillation occurred in 31.1％ of OPCABG patients and 41.7 ％ of on-pump CABG patients (P＜0.01).The mortality rate was 5.0％ in OPCABG group versus 8.3％ in on-pump CABG group (P＜0.05).Conclusions OPCABG is a safe and efficient method of myocardial revascularization in the elderly patients with lower morbidity and complications.

Abstract:Background Although coronary artery bypass grafting (CABG) is performed via three different techniques, conventional, on-pump beating heart CABG (ONBHCAB), or off-pump CABG (OPCAB), data are limited to compare ONBHCAB with OPCAB. Methods We retrospectively investigated the postoperative cardiac biomarkers, creatine kinase-MB (CK-MB), and troponin I (cTnI), and early and late outcomes in 806 patients undergoing isolated ONBHCAB or OPCAB between February 2008 and September 2022. To eliminate the bias between different groups, propensity score matching was conducted to validate the findings. Results After matching, the number of each study group totaled 270 patients. In both complete and matched cohorts, early outcomes, including morbidities and mortalities, were similar. However, cTnI and CK-MB levels were significantly higher after ONBHCAB than after OPCAB with median peak cTnI of 9.85 versus 4.60 ng/mL and median peak CK-MB of 48.45 versus 17.10 ng/mL in the matched cohort, which were quite low, below the threshold for values defining perioperative myocardial infarction. At follow-up of 73 ± 45 months, the overall actuarial survival rates were similar between the ONBHCAB and OPCAB patients (86 vs. 87% at 5 years and 64 vs. 68% at 10 years, respectively, in the matched cohort). Conclusion ONBHCAB may be a comparable alternative to OPCAB with similar early and late outcomes, despite higher elevation of postoperative cardiac biomarkers. ONBHCAB provides more efficient hemodynamic support, providing a better surgical visual field, than OPCAB while reducing the risk of incomplete revascularization. Abstract:Objective We reviewed the data of off-pump coronary artery bypass grafting(OPCAB) and conventional coronary artery bypass grafting(cCABG) with extracorporeal circulation,to compare the outcomes of these two kinds of operation.Methods 1268 patients underwent coronary artery bypass grafting were divided into 2 groups.722 patients received cCABG were in one group as CABG group.546 patients received OPCAB were in the other group as OPCAB group.Results Average volume of blood transfusion in CABG group was (1194±1405) nd. 73 (10.1%) patients had complications.23 (3.2%) patients died.Average volume of blood transfusion in OPCAB group was (876±1486) ml.44 (8.1%) patients had complications.10 (1.8%) patients died.Statistical significance of the difference between 2 groups was obtained(P<0.05).We followed-up 441 patients(61%) in CABG group for (8.3±2.9) months and 371 patients(68%) in OPCAB group for (7.9±3.1) months.After 6 months the patients in CABG group had angina in grade 1.4±0.6(CSS),the patients in OPCAB group had angina in grade 1.6±0.8 (CSS).Conclusion OPCAB has less injury,less complication,less death than cCABG in recent outcomes.There is no significant difference in midterm outcomes between OPCAB and cCABG.
Off-pump coronary artery bypass grafting in high-risk patients

Abstract:Objective To evaluate the safety,effect,and operation method selection of off-pump coronary artery bypass(OPCAB) in high-risk patients.Methods Clinical records of patients undergoing coronary artery bypass surgery at this department between June 2002 to December 2005 were reviewed.There were 113 patients(OPCAB Group) with a higher(>5) EuroSCORE(European system for cardiac operative risk evaluation) risk of death. Another group of 76 high-risk patients(EuroSCORE > 5) receiving cardiopulmonary coronary artery bypass grafting(CCAB) during the same period was reviewed as control group(CCAB Group).The preoperative risk factors,operation parameters,and postoperative outcomes between the two groups were compared for statistical analysis.Results The EuroSCORE value of the OPCAB Group and the CCAB Group was 9.0±2.6 and 8.3±2.1,respectively(t=2.016,P=0.045). The number of grafts in the OPCAB Group was less than that in the CCAB Group(3.2±0.9 vs 3.8±0.9;t=-4.267,P=0.000).The postoperative levels of creatine kinase(CK) and creatine kinase myocardial band(CK-MB) were significantly lower in the OPCAB Group [570(61～8246) U/L and 20(1～282) U/L] than the CCAB Group [870(246～8155) U/L and 55(12～300) U/L](Z=-3.648 and-5.767,P=0.000).The blood transfusion in the OPCAB Group was significantly less than that in the CCAB Group(775±693 ml vs 1088±712 ml;t=-2.887,P=0.004).There was no statistical difference in postoperative complications between the two groups.The mortality is 3.5%(4/113) in the OPCAB Group and 6.6%(5/76) in the CCAB Group,without statistical difference(χ2=0.925,P=0.336).Conclusions OPCAB is safe and effective in high risk patients.Different operation methods should be used for patients with different risks.
On-pump beating-heart coronary artery bypass: a propensity matched analysis

Abstract:Background: It remains unclear how cardioplegic arrest affects surgical results after coronary artery bypass grafting surgery (CABG). This study compares early outcomes after on-pump beating-heart CABG and conventional CABG. Methods: From 2002 to 2005, 114 patients underwent on-pump beating-heart CABG. Multivariate logistic regression revealed five characteristics according to which technique is liable to be used: history of cerebral infarction, urgent or emergent operation, lower ejection fraction, preoperative creatine kinase, and lower number of diseased vessels. The early clinical outcome for these patients was compared against 114 conventional CABG patients, matched using a propensity score constructed with these five significant variables and with two nonsignificant variables: history of diabetes mellitus and hypertension. Results: On-pump beating-heart CABG significantly reduced the duration of operation and cardiopulmonary bypass, total blood loss, and peak creatine kinase (p < 0.05). The number of patients requiring additional intra-aortic balloon pump support was significantly lower in the on-pump beating-heart CABG group (2 versus 13, p < 0.01). No patients required percutaneous cardiopulmonary support after on-pump beating-heart CABG, whereas 4 patients needed it after conventional CABG. Complete revascularization was significantly lower (42.1% versus 77.2%, p < 0.0001), but in-hospital mortality was less in the on-pump beating-heart CABG group (2.6% versus 9.6%, p < 0.05). No significant difference was found in morbidity including stroke, renal failure, mediastinitis, and prolonged ventilation. Conclusions: On-pump beating-heart CABG can be performed safely, including on high-risk patients. Use of cardiopulmonary bypass and the elimination of cardioplegic arrest may be of most benefit to hemodynamically unstable patients.
Analysis of Conventional Off-pump Coronary Artery Bypass Graftings

Abstract:Objective To evaluate the clinical value and significance of off-pump coronary artery bypass grafting. Methods From June 2000 to December 2005, 21 patients underwent off-pump coronary artery bypass grafting. Surgical techniques, per-operative treatments were analyzed in this study. Results No off-pump was changed to on-pump, no postoperative myocardial infarction happened and mortality is zero in this group. All cases recovered smoothly. Patients stayed in hospital for 12～21days(average 14.53 days). Conclusion Off-pump coronary bypass grafting is an alternative treatment of coronary disease, its result is satisfactory. But the longterm effects need further observation.

Abstract:To improve operative results of coronary artery bypass grafting (CABG), off-pump CABG (OPCAB) was performed as standard procedure in 220 cases at Toho University Omori Medical Center between 1999 and 2008. In the early group, OPCAB was performed in 94 cases, minimally invasive direct coronary artery bypass (MIDCAB) in 5 cases (off-pump rate of was 81.1%), on-pump beating coronary artery bypass (CAB) in 4 cases, and on-pump arrest CAB in 19 cases. In the late group, OPCAB was done in 79 cases (80.6%), on-pump beating CAB in 8 cases, and on-pump arrest CAB in 11 cases. One patient died because of multiple organ failure after OPCAB for acute coronary syndrome (ACS) in the early group. There was no surgical death in the late group. The overall surgical mortality rate was 0.45%. In the late group, 69 PAS x Port systems were used in 53 cases for proximal anastomosis of the saphenous vein graft (SVG). The patency rate was 97.8% which was confirmed by CAG and or 3-dimensional computed tomography (3D-CT) between 10 days to 23 months after operation. OPCAB could be performed for over 80% of CABG candidates with very low mortality. To improve clinical results, on-pump beating CAB and on-pump arrest CAB should be selected in cases of poor cardiac performance or ischemic condition at the pre- and during operation. The use of proximal anastomosis device reduces the stress of surgeon and the time of anastomosis, those benefits make the excellent postoperative course with reduced postoperative cerebral infarction.

Abstract:OBJECTIVE To explore the clinical value of off-pump coronary artery bypass grafting by small incision at the left chest, and develop a better surgical regimen for coronary heart disease patients. PATIENTS AND METHODS 201 coronary heart disease patients who need coronary artery bypass grafting were required and randomly divided into 2 groups including a control group and an observation group. There were 107 cases in the control group who received coronary bypass grafting by extracorporeal circulation; there were 103 cases in the observation group who received off-pump coronary bypass grafting by small incision at the left chest. The duration of the mechanism ventilation, length of stay in ICU, hospitalization time, postoperative drainage volume, and the occurrence rate of complications were recorded and compared. RESULTS The duration of mechanism ventilation, length of stay in ICU, hospitalization time and postoperative drainage volume in the control group were (19.21 ± 1.33) hours, (5.08 ± 0.57) days, (21.20 ± 2.34) days and (997.68 ± 96.35) mL, which were (7.73 ± 0.74) hours, (2.83 ± 0.16) days, (15.67 ± 1.18) days and (901.53 ± 89.32) mL in the observation group respectively, with statistical difference between the two groups (p<0.05). The occurrence rates of renal insufficiency and arrhythmia were both 6.54% and 0.97% in the control group and the observation group, respectively. The occurrence rates of postoperative renal insufficiency and arrhythmia in the observation group were both significantly lower than those in the control group, with statistical significance analysis (p < 0.05). Postoperative low cardiac output, second thoracotomy, cerebrovascular disease, pulmonary infection, perioperative cardiac infarction and mortality did not display a significant difference between the two groups (p > 0.05). CONCLUSIONS Off-pump coronary artery bypass grafting by small incision at the left chest is a surgical method with less injury and fast recovery, which can be used as the preferred therapeutical method for the coronary heart disease patients who need coronary artery bypass grafting.
