Anesthetic experience of interventional treatment in De Bakey III type of aortic dissecting aneurysm

Abstract:Objective To evaluate the anesthetic management and the points forattention in the interventional treatment of De Bakey type III aortic dissection. Methods: 25 in-patients with De Bakey type III aortic dissection underwent interventional treatment in endotracheal intubation general anesthesia (Group A) or local anesthesia and intravenous intensified anesthesia (Group 13). Subjects were continuous monitored of their electrocardiogram, mean arterial pressure(MAP), pulse oxygen saturation (SpO2) and end-tidal carbon dioxide partial pressure (PetCO2). Controlled hypotension was used in operation to maintain normal blood pressure. The systolic blood pressure decreased to 70 similar to 90 mmHg when the stent was released. The anesthesia machine closed to simulate breath holding in group A. All patients were treated in Cardiac Care Unit after the operation. Results Group A showed satisfactory anesthesia, and the respiratory and circulatory system were stable. The saturation of blood oxygen is markedly higher in group A comparable to group B. Conclusion The keys to a successful operation include a combination of understanding to the pathogenetic condition and the surgical characteristic and procedure, preoperation preparations, suitable operative method and the opportunity, the exact intro- and post-operation management. Endotracheal intubation general anesthesia is more suitable.
