Abstract:Purpose: Tooth segmentation is an essential step in acquiring patient-specific dental geometries from cone-beam computed tomography (CBCT) images. Tooth segmentation from CBCT images is still a challenging task considering the comparatively low image quality caused by the limited radiation dose, as well as structural ambiguities from intercuspation and nearby alveolar bones. The goal of this paper is to present and discuss the latest accomplishments in semisupervised tooth segmentation with adaptive 3D shape constraints. Methods: The authors propose a 3D exemplar-based random walk method of tooth segmentation from CBCT images. The proposed method integrates semisupervised label propagation and regularization by 3D exemplar registration. To begin with, the pure random walk method is to get an initial segmentation of the teeth, which tends to be erroneous because of the structural ambiguity of CBCT images. And then, as an iterative refinement, the authors conduct a regularization by using 3D exemplar registration, as well as label propagation by random walks with soft constraints, to improve the tooth segmentation. In the first stage of the iteration, 3D exemplars with well-defined topologies are adapted to fit the tooth contours, which are obtained from the random walks based segmentation. The soft constraints on voxel labeling are defined by shape-based foreground dentine probability acquired by the exemplar registration, as well as the appearance-based probability from a support vector machine (SVM) classifier. In the second stage, the labels of the volume-of-interest (VOI) are updated by the random walks with soft constraints. The two stages are optimized iteratively. Instead of the one-shot label propagation in the VOI, an iterative refinement process can achieve a reliable tooth segmentation by virtue of exemplar-based random walks with adaptive soft constraints. Results: The proposed method was applied for tooth segmentation of twenty clinically captured CBCT images. Three metrics, including the Dice similarity coefficient (DSC), the Jaccard similarity coefficient (JSC), and the mean surface deviation (MSD), were used to quantitatively analyze the segmentation of anterior teeth including incisors and canines, premolars, and molars. The segmentation of the anterior teeth achieved a DSC up to 98%, a JSC of 97%, and an MSD of 0.11 mm compared with manual segmentation. For the premolars, the average values of DSC, JSC, and MSD were 98%, 96%, and 0.12 mm, respectively. The proposed method yielded a DSC of 95%, a JSC of 89%, and an MSD of 0.26 mm for molars. Aside from the interactive definition of label priors by the user, automatic tooth segmentation can be achieved in an average of 1.18 min. Conclusions: The proposed technique enables an efficient and reliable tooth segmentation from CBCT images. This study makes it clinically practical to segment teeth from CBCT images, thus facilitating pre-and interoperative uses of dental morphologies in maxillofacial and orthodontic treatments. Abstract:For patients with partial jaw defects, cysts and dental implants, doctors need to take panoramic X-ray films or manually draw dental arch lines to generate Panorama images in order to observe their complete dentition information during oral diagnosis. In order to solve the problems of additional burden for patients to take panoramic X-ray films and time-consuming issue for doctors to manually segment dental arch lines, this paper proposes an automatic panorama reconstruction method based on cone beam computerized tomography (CBCT). The V-network (VNet) is used to pre-segment the teeth and the background to generate the corresponding binary image, and then the Bezier curve is used to define the best dental arch curve to generate the oral panorama. In addition, this research also addressed the issues of mistakenly recognizing the teeth and jaws as dental arches, incomplete coverage of the dental arch area by the generated dental arch lines, and low robustness, providing intelligent methods for dental diagnosis and improve the work efficiency of doctors.

Abstract:Cone-beam dental computerized tomography has been introduced as a system geared towards maxillofacial imaging, which is gradually becoming a common tool within the dental industry. Segmentation of teeth from dental CT data is vital for a variety of computer-aided procedures such as dental implants, orthodontic planning and cosmetic surgeries. Teeth segmentations near soft tissue structures can help clinicians to assess the quality of a patient's teeth more easily. Moreover, the ability to identify particular regions surrounding teeth can help to provide better image results generated using 3D surface/volume rendering techniques. This paper, presents an experimental study for teeth segmentation employing two different thresholding techniques. The performances of these techniques are compared with each other. The experimental results show that there is a noticeable difference in segmentation quality between the two methods presented in this paper.

Abstract:The invention discloses a method for performing dentition segmentation on a cone beam CT image (CBCT). The method for performing dentition segmentation on a cone beam CT image comprises steps of defining an image structure according to an interest body image area in the cone beam CT image, obtaining a complete dentition through segmenting the CBCT image on the basis of a semi-supervised random walk algorithm and a soft constraint which is defined and registered by a three-dimensional deformable model, using the three-dimensional deformable model to introduce a soft constraint of a body image for processing noise in body image segmentation based on semi-supervised mark diffusion, adopting an iteration correction method to perform iteration solving problems on mark diffusion under the soft constrain and fitting on a surface voxel set by the three-dimensional model. The method for performing dentition segmentation on the cone beam CT image can effectively eliminate a segmentation error, improves dentition segmentation obtained through single time mark diffusion, improves segmentation accuracy and meets an accuracy requirement for clinic stomatology.

Abstract:Segmenting 3D teeth models from cone beam computed tomography images is significantly important for orthodontics. In this paper, we propose a new approach based on a regional energy function to solve the fuzziness problem of adjacent teeth crowns and root-jaw issues. The initial curve is first calculated based on the endodontic seeds and gradients. Then, the moving range of the initial curve is restricted by the intensity distribu-tion. Within this moving range, we propose a regional energy function based on intensity, gradient and smooth-ness. By optimizing the function, the final curve of the slice can be generated, and is applied as the initial curve of next slice. The final tooth model can be reconstructed by Marching Cubes algorithm. The experimental results on CBCT datasets demonstrate that our method is accurate and efficient for the fuzziness problem in orthodontic treatment.

Abstract:The panoramic and lateral radiographs are commonly used in orthodontic dentistry to acquire patient-specific tooth morphology for diagnosing and treatment planning. Considering the variational dentition configurations and image blurs caused by device-specific artefacts and structure overlapping, the robust tooth segmentation and 3D reconstruction from radiographs remain a challenging issue. We propose a deformable exemplar-based conditional random fields (CRF) model for tooth segmentation and 3D shape estimation from the panoramic and lateral radiographs. The shared tooth foreground in the lateral and the panoramic radiographs are utilized for consistent labeling. The 3D deformable exemplars are introduced to provide a regularization of tooth contours to improve the tooth parsing in noisy and ambiguous radiographs. We introduce an alternating optimization scheme to solve the discrete superpixel labels and the continuous deformation of the 3D exemplars simultaneously. Extensive experiments on clinically obtained radiographs demonstrate that the proposed approach is effective and efficient for both tooth segmentation and 3D shape estimation from the panoramic and lateral radiographs.

Abstract:Background: Accurate segmentation of individual tooth and their substructures including enamel, pulp, and dentin from cone-beam computed tomography (CBCT) images is essential for dental diagnosis and treatment planning in digital dentistry. Existing methods for tooth segmentation based on CBCT images have achieved substantial progress; however, techniques for further segmentation into substructures are yet to be developed. Purpose: We aim to propose a novel three-stage progressive deep-learning-based framework for automatically segmenting 3D tooth from CBCT images, focusing on finer substructures, i.e., enamel, pulp, and dentin. Methods: In this paper, we first detect each tooth using its centroid by a clustering scheme , which efficiently determines each tooth detection by applying learned displacement vectors from the foreground tooth region. Next, guided by the detected centroid, each tooth proposal, combined with the corresponding tooth map, is processed through our tooth segmentation network. We also present an attention-based hybrid feature fusion mechanism, which provides intricate details of the tooth boundary while maintaining the global tooth shape, thereby enhancing the segmentation process. Additionally, we utilize the skeleton of the tooth as a guide for subsequent substructure segmentation. Results: Our algorithm is extensively evaluated on a collected dataset of 314 patients, and the extensive comparison and ablation studies demonstrate superior segmentation results of our approach. Conclusions: Our proposed method can automatically segment tooth and finer substructures from CBCT images, underlining its potential applicability for clinical diagnosis and surgical treatment.

Abstract:In the field of dental imaging, precise segmentation of individual teeth from 3D Cone Beam Computed Tomography (CBCT) images remains a challenging task. This paper proposes a novel coarse-to-fine segmentation framework specifically optimized for individual tooth segmentation in CBCT imagery. At its core, in the coarse stage, the framework employs an embedding grouping network that ensures accurate localization and establishes a strong tooth-specific prior. To address the intricacies of tooth morphology, in the fine stage, our architecture designs a Tripartite Fusion Module and a Decouple Body and Edge Module. These modules work in concert to refine the segmentation process, thereby improving the granularity of individual tooth delineation. We have tested our framework on a real-world dataset, and the results demonstrate improvement over existing methods.

Abstract:Accurate and automatic segmentation of three-dimensional (3D) individual teeth from cone-beam computerized tomography (CBCT) images is a challenging problem because of the difficulty in separating an individual tooth from adjacent teeth and its surrounding alveolar bone. Thus, this paper proposes a fully automated method of identifying and segmenting 3D individual teeth from dental CBCT images. The proposed method addresses the aforementioned difficulty by developing a deep learning-based hierarchical multi-step model. First, it automatically generates upper and lower jaws panoramic images to overcome the computational complexity caused by high-dimensional data and the curse of dimensionality associated with limited training dataset. The obtained 2D panoramic images are then used to identify 2D individual teeth and capture loose- and tight- regions of interest (ROIs) of 3D individual teeth. Finally, accurate 3D individual tooth segmentation is achieved using both loose and tight ROIs. Experimental results showed that the proposed method achieved an F1-score of 93.35% for tooth identification and a Dice similarity coefficient of 94.79% for individual 3D tooth segmentation. The results demonstrate that the proposed method provides an effective clinical and practical framework for digital dentistry.

Abstract:Cone beam computed tomography (CBCT) is a common way of diagnosing dental related diseases. Accurate segmentation of 3D tooth is of importance for the treatment. Although deep learning based methods have achieved convincing results in medical image processing, they need a large of annotated data for network training, making it very time-consuming in data collection and annotation. Besides, domain shift widely existing in the distribution of data acquired by different devices impacts severely the model generalization. To resolve the problem, we propose a multi-stage framework for 3D tooth segmentation in dental CBCT, which achieves the third place in the "Semi-supervised Teeth Segmentation" 3D (STS-3D) challenge. The experiments on validation set compared with other semi-supervised segmentation methods further indicate the validity of our approach.

Abstract:Accurate tooth volume segmentation is a prerequisite for computer-aided dental analysis. Deep learning-based tooth segmentation methods have achieved satisfying performances but require a large quantity of tooth data with ground truth. The dental data publicly available is limited meaning the existing methods can not be reproduced, evaluated and applied in clinical practice. In this paper, we establish a 3D dental CBCT dataset CTooth+, with 22 fully annotated volumes and 146 unlabeled volumes. We further evaluate several state-of-the-art tooth volume segmentation strategies based on fully-supervised learning, semi-supervised learning and active learning, and define the performance principles. This work provides a new benchmark for the tooth volume segmentation task, and the experiment can serve as the baseline for future AI-based dental imaging research and clinical application development.

Abstract:Objective: To propose a novel neural network to achieve tooth instance segmentation and recognition based on cone-beam computed tomography (CBCT) voxel data. Methods: The proposed methods included three different convolutional neural network models. The architecture was based on the Resnet module and built according to the structure of "Encoder-Decoder" and U-Net. The CBCT image was de-sampled and a fixed-size region of interest (ROI) containing all the teeth was determined. ROI would first through a two-branch "encoder and decoder" structure of the network, the network could predict each voxel in the input data of the spatial embedding. The post-processing algorithm would cluster the prediction results of the relevant spatial location information according to the two-branch network to realize the tooth instance segmentation. The tooth position identification was realized by another U-Net model based on the multi-classification segmentation task. According to the predicted results of the network, the post-processing algorithm would classify the tooth position according to the voting results of each tooth instance segmentation. At the original spatial resolution, a U-Net network model for the fine-tooth segmentation was trained using the region corresponding to each tooth as the input. According to the results of instance segmentation and tooth position identification, the model would process the correspon-ding positions on the high-resolution CBCT images to obtain the high-resolution tooth segmentation results. In this study, CBCT data of 59 cases with simple crown prostheses and implants were collected for manual labeling as the database, and statistical indicators were evaluated for the prediction results of the algorithm. To assess the performance of tooth segmentation and classification, instance Dice similarity coefficient (IDSC) and the average Dice similarity coefficient (ADSC) were calculated. Results: The experimental results showed that the IDSC was 89.35%, and the ADSC was 84. 74%. After eliminating the data with prostheses artifacts, the database of 43 samples was generated, and the performance of the training network was better, with 90.34% for IDSC and 87.88% for ADSC. The framework achieved excellent performance on tooth segmentation and identification. Voxels near intercuspation surfaces and fuzzy boundaries could be separated into correct instances by this framework. Conclusions: The results show that this method can not only successfully achieve 3D tooth instance segmentation but also identify all teeth notation numbers accurately, which has clinical practicability.

Abstract:Abstract Digital dentistry plays a pivotal role in dental healthcare. A critical step in many digital dental systems is to accurately delineate individual teeth and the gingiva in the three-dimension (3D) intraoral scanned (IOS) mesh data. However, previous state-of-the-art methods are either time-consuming or error-prone, hence hinder their clinical applicability. In this paper, we present an accurate, efficient, and fully-automated deep learning model, trained on a dataset of 4,000 IOS data annotated by experienced human experts. On a hold-out dataset of 200 scans, our model achieves a per-face accuracy, average-area accuracy and area under the receiver operating characteristic curve (AUC) of 96.94%, 98.26%, and 0.9991, respectively, significantly outperforming the state-of-the-art baseline. In addition, our model only takes about 24 seconds to generate segmentation outputs, as compared to over 5 minutes by the baseline and 15 minutes by human experts. A clinical performance test of 500 patients with malocclusion or/and abnormal teeth shows that 96.9% of the segmentations are satisfactory for clinical applications, 2.9% automatically trigger alarms for human improvement, and only 0.2% of them need rework. Our research demonstrates the potential for deep learning to improve the efficacy and efficiency of dental treatment and digital dentistry.

Abstract:Cone-beam computed tomography (CBCT) is a valuable imaging method in dental diagnostics that provides information not available in traditional 2D imaging. However, interpretation of CBCT images is a time-consuming process that requires a physician to work with complicated software. In this work we propose an automated pipeline composed of several deep convolutional neural networks and algorithmic heuristics. Our task is two-fold: a) find locations of each present tooth inside a 3D image volume, and b) detect several common tooth conditions in each tooth. The proposed system achieves 96.3\% accuracy in tooth localization and an average of 0.94 AUROC for 6 common tooth conditions.

Abstract:ABSTRACT Many scenarios in dental clinical diagnosis and treatment require the segmentation and identification of a specific tooth or the entire dentition in cone‐beam computed tomography (CBCT) images. However, traditional segmentation methods struggle to ensure accuracy. In recent years, there has been significant progress in segmentation algorithms based on deep learning, garnering considerable attention. Inspired by models from present neuro networks such as UCTransNet and DC‐Unet, this study proposes an MRCM‐UCTransNet for accurate three‐dimensional tooth segmentation from cone‐beam CT images. To enhance feature extraction while preserving the multi‐head attention mechanism, a multi‐scale residual convolution module (MRCM) is integrated into the UCTransNet architecture. This modification addresses the limitations of traditional segmentation methods and aims to improve accuracy in tooth segmentation from CBCT images. Comparative experiments indicate that, in the situation with a specific image size and small data volume, the proposed method exhibits certain advantages in segmentation accuracy and precision. Compared to traditional Unet approaches, MRCM‐UCTransNet's dice accuracy is improved by 7%, while its sensitivity is improved by about 10%. These findings highlight the efficacy of the proposed approach, particularly in scenarios with specific image size constraints and limited data availability. The proposed MRCM‐UCTransNet algorithm integrates the latest architectural advancements in the Unet model which achieves effective segmentation of six types of teeth within the tooth. It was proved to be efficient for image segmentation on small datasets, requiring less training time and fewer parameters.

Abstract:Accurate segmentation of oral surgery-related tissues from cone beam computed tomography (CBCT) images can significantly accelerate treatment planning and improve surgical accuracy. In this paper, we propose a fully automated tissue segmentation system for dental implant surgery. Specifically, we propose an image preprocessing method based on data distribution histograms, which can adaptively process CBCT images with different parameters. Based on this, we use the bone segmentation network to obtain the segmentation results of alveolar bone, teeth, and maxillary sinus. We use the tooth and mandibular regions as the ROI regions of tooth segmentation and mandibular nerve tube segmentation to achieve the corresponding tasks. The tooth segmentation results can obtain the order information of the dentition. The corresponding experimental results show that our method can achieve higher segmentation accuracy and efficiency compared to existing methods. Its average Dice scores on the tooth, alveolar bone, maxillary sinus, and mandibular canal segmentation tasks were 96.5%, 95.4%, 93.6%, and 94.8%, respectively. These results demonstrate that it can accelerate the development of digital dentistry.

Abstract:Accurate identification, localization, and segregation of teeth from Cone Beam Computed Tomography (CBCT) images are essential for analyzing dental pathologies. Modeling an individual tooth can be challenging and intricate to accomplish, especially when fillings and other restorations introduce artifacts. This paper proposes a method for automatically detecting, identifying, and extracting teeth from CBCT images. Our approach involves dividing the three-dimensional images into axial slices for image detection. Teeth are pinpointed and labeled using a single-stage object detector. Subsequently, bounding boxes are delineated and identified to create three-dimensional representations of each tooth. The proposed solution has been successfully integrated into the dental analysis tool Dentomo.

Abstract:Computer-aided design (CAD) tools are increasingly popular in modern dental practice, particularly for treatment planning or comprehensive prognosis evaluation. In particular, the 2D panoramic X-ray image efficiently detects invisible caries, impacted teeth and supernumerary teeth in children, while the 3D dental cone beam computed tomography (CBCT) is widely used in orthodontics and endodontics due to its low radiation dose. However, there is no open-access 2D public dataset for children's teeth and no open 3D dental CBCT dataset, which limits the development of automatic algorithms for segmenting teeth and analyzing diseases. The Semi-supervised Teeth Segmentation (STS) Challenge, a pioneering event in tooth segmentation, was held as a part of the MICCAI 2023 ToothFairy Workshop on the Alibaba Tianchi platform. This challenge aims to investigate effective semi-supervised tooth segmentation algorithms to advance the field of dentistry. In this challenge, we provide two modalities including the 2D panoramic X-ray images and the 3D CBCT tooth volumes. In Task 1, the goal was to segment tooth regions in panoramic X-ray images of both adult and pediatric teeth. Task 2 involved segmenting tooth sections using CBCT volumes. Limited labelled images with mostly unlabelled ones were provided in this challenge prompt using semi-supervised algorithms for training. In the preliminary round, the challenge received registration and result submission by 434 teams, with 64 advancing to the final round. This paper summarizes the diverse methods employed by the top-ranking teams in the STS MICCAI 2023 Challenge.

Abstract:In digital dentistry, cone-beam computed tomography (CBCT) can provide complete 3D tooth models, yet suffers from a long concern of requiring excessive radiation dose and higher expense. Therefore, 3D tooth model reconstruction from 2D panoramic X-ray image is more cost-effective, and has attracted great interest in clinical applications. In this paper, we propose a novel dual-space framework, namely DTR-Net, to reconstruct 3D tooth model from 2D panoramic X-ray images in both image and geometric spaces. Specifically, in the image space, we apply a 2D-to-3D generative model to recover intensities of CBCT image, guided by a task-oriented tooth segmentation network in a collaborative training manner. Meanwhile, in the geometric space, we benefit from an implicit function network in the continuous space, learning using points to capture complicated tooth shapes with geometric properties. Experimental results demonstrate that our proposed DTR-Net achieves state-of-the-art performance both quantitatively and qualitatively in 3D tooth model reconstruction, indicating its potential application in dental practice.
