A New Parallel External Fixator Design for Correcting Ankle and Foot Sagittal Plane Deformities

Abstract:Ilizarov technology indicates that biological tissues can produce a certain tension under the tractive force. Such tractive force will stimulate tissue regeneration and active growth. Commonly, the external fixator is applied to make the deformed parts being fixed passively and then produces the tractive force. As the widespread application of the Ilizarov technology, many researchers across the world have studied the characteristics of the external fixators intensively. Some of them made performance improvements based on the conventional Ilizarov fixators. In addition, Taylor [1] quantified the use of external fixators by applying Stewart platform and Chasles theory to orthopedics, which have made correcting severe foot and ankle deformities much easier. The conventional Ilizarov external fixator for correcting ankle and foot deformities is shown in Fig. 1, the Taylor space frame (TSF) fixator to correct such deformities is shown in Fig. 2.Actually, the TSF fixator could be recognized as a parallel mechanism, the moving platform and the fixed platform are linked by six UCU kinematic chains. U represents the universal joint, and C represents the cylindrical pair. The TSF fixator is an advanced orthopedic modality, and it can be applied in correction of bone deformities with both three angulation deformities and three shifted deformities. However, it also means that the six degrees-of-freedom (DOF) TSF fixator requires the control of six variables. For most bone deformities, such as ankle and foot sagittal plane deformities, they have just one or two angulation deformities. Although TSF fixators can be generalized for use in correcting variable bone deformities, it does increase the complexity of correcting some common bone deformities, e.g., equines deformity.The complexity of the 6DOF TSF fixator can be described as follows:Hence, it is necessary to simplify the TSF fixators when focusing on bone deformities with one or two angulation deformities. To solve this issue, screw theory was used to obtain the new configuration of parallel external fixator with improved structure and adjustability, as it is shown in Fig. 3. It focuses on sagittal plane deformities and takes the biomechanical characteristics of ankle and foot bone into consideration.According to the feedback of clinical application of the Ilizarov external fixators, the possible structural deadlocking or collapse brings a lot troubles to the orthopedists. So, it is quite important to guarantee that the designed parallel fixators avoid any possible singular postures during work status. To solve such problem, this study conducted kinematics analyses of the proposed fixator and partook in its singularity so as to avoid possible deadlocking reflected in the clinical conditions.Because the osteotomy in the deformed position is executed before applying the external fixator, the design of the fixator should be exactly in accordance with the principle of osteotomy. The osteotomy for correcting angulation deformities can be regarded as making a segment of the corresponding bone change its angulation relative to another joints section, surrounding to a space illusion axis. This axis is the angulation correction axis (ACA). The mechanical shaft of the bone is a line, correcting to the proximal joints and the distal joint center, and the intersection of the axis of proximal and distal axis is called the center of rotation angulation (CORA). For ankle and foot sagittal plane deformities, shifted deformities should be taken into consideration when ACA is inconsistent with CORA. So, the parallel mechanism should provide 3DOF of Y-axis in translation, Z-axis in translation, and rotation along X-axis.According to screw theory, the constraint screw system of the constraint chain can be obtained. Based on reciprocal screw, the basic kinematic screw system can be obtained as shown in the following equation:(1)$P1r=(0, 0, 1; 0, 0, 0)$P2r=(0, 0, 0; 1, 0, 0)$P3r=(0, 0, 0; 0, 1, 0)It shows that the mechanism with such kinematic screw system can implement 3DOF of Y-axis and Z-axis in translation and rotation along X-axis. Then, the 3-UCU/RPR external fixator is proposed, which is shown in Fig. 3. The moving platform and the fixed platform are linked by three UCU chains and one RPR chain. R represents the revolute pair, and P represents the prismatic pair. Additionally, the DOF properties of the proposed external fixator could be certified by detailed analysis of the kinematic performance.The kinematic inverse resolution for this mechanism is indispensable because it provides theoretical guidance for correcting deformed posture as well as the identification of singularity postures. C = [0, y, z]T, it represents the coordinates of the moving platform center in the reference coordinate system.The flowchart to obtain the optimal singularity-free workspace is described in Fig. 4. The position of osteotomy, parameters of external fixator, and the angular deformity are integrated to obtain the workspace of the parallel fixator by polar coordinate fast searching method. The driven-input equation is obtained based on inverse kinematics, and singular postures are obtained by Jacobi matrix. It is worth mentioning that the algorithm to obtain the optimal singularity-free workspace not only considered the properties of the parallel mechanism but also took the process of correcting deformities into consideration.Figure 3 shows the three-dimensional diagram after assembling the bony model into the designed parallel external fixator. It can be seen that the parallel external fixator involves four chains to achieve 3DOF of a certain movement, so one chain is redundant. The selection of four chains is to increase the mechanical stiffness and stability. As illustrated in Fig. 3, the cylindrical pairs of the two UCU chains and the revolute pair of the RPR chain attached to the static platform are set as three driving inputs.Three algorithms in Fig. 4 are solved by matlab, and the optimal singularity-free workspace is obtained as shown in Fig. 5. Generally, the shifted deformities are less than 10 mm, and the angle of the angulation deformity is no more than 30 deg. The area enclosed by the white curve represents singularity-free workspace. The best position for the wire which is used to fix the bone to the external fixator is located in the center of the singularity-free workspace.In this study, a general method to obtain parallel external fixators for bony deformities is proposed. Meanwhile, it involves the method to identify the installation parameters of the external fixator, which guarantees that the fixator works inside the singularity-free workspace. The 3-UCU/RPR external fixator is used as the deformed correction devices. In this study, the designed parallel external fixator is used for deformity correction of ankle sagittal plane. It allows 3DOF of rotating around the X-axis and translation along the Y-axis and the Z-axis so as to identify a three input–output movement.
