Abstract:Purpose: Determine the dosimetric quality and the planning time reduction when utilizing a template-based automated planning application. Methods: A software application integrated through the treatment planning system application programing interface, QuickPlan, was developed to facilitate automated planning using configurable templates for contouring, knowledge-based planning structure matching, field design, and algorithm settings. Validations are performed at various levels of the planning procedure and assist in the evaluation of readiness of the CT image, structure set, and plan layout for automated planning. QuickPlan is evaluated dosimetrically against 22 hippocampal-avoidance whole brain radiotherapy patients. The required times to treatment plan generation are compared for the validations set as well as 10 prospective patients whose plans have been automated by QuickPlan. Results: The generations of 22 automated treatment plans are compared against a manual replanning using an identical process, resulting in dosimetric differences of minor clinical significance. The target dose to 2% volume and homogeneity index result in significantly decreased values for automated plans, whereas other dose metric evaluations are nonsignificant. The time to generate the treatment plans is reduced for all automated plans with a median difference of 9' 50″ ± 4' 33″. Conclusions: Template-based automated planning allows for reduced treatment planning time with consistent optimization structure creation, treatment field creation, plan optimization, and dose calculation with similar dosimetric quality. This process has potential expansion to numerous disease sites.

Abstract:Independent calculations of proton therapy plans are an important quality control procedure in treatment planning. When using custom Monte Carlo (MC) models of the beamline, deploying the calculations can be laborious, time consuming, and require in‐depth knowledge of the computational environment. Abstract:Background and purpose To describe the clinical commissioning of an in‐house artificial intelligence (AI) treatment planning platform for head‐and‐neck (HN) Intensity Modulated Radiation Therapy (IMRT). Materials and methods The AI planning platform has three components: (1) a graphical user interface (GUI) is built within the framework of a commercial treatment planning system (TPS). The GUI allows AI models to run remotely on a designated workstation configured with GPU acceleration. (2) A template plan is automatically prepared involving both clinical and AI considerations, which include contour evaluation, isocenter placement, and beam/collimator jaw placement. (3) A well‐orchestrated suite of AI models predicts optimal fluence maps, which are imported into TPS for dose calculation followed by an optional automatic fine‐tuning. Six AI models provide flexible tradeoffs in parotid sparing and Planning Target Volume (PTV)‐organ‐at‐risk (OAR) preferences. Planners could examine the plan dose distribution and make further modifications as clinically needed. The performance of the AI plans was compared to the corresponding clinical plans. Results The average plan generation time including manual operations was 10–15 min per case, with each AI model prediction taking ∼1 s. The six AI plans form a wide range of tradeoff choices between left and right parotids and between PTV and OARs compared with corresponding clinical plans, which correctly reflected their tradeoff designs. Conclusion The in‐house AI IMRT treatment planning platform was developed and is available for clinical use at our institution. The process demonstrates outstanding performance and robustness of the AI platform and provides sufficient validation.

Abstract:Abstract Despite the recent advanced developments in radiation therapy planning, treatment planning for head-neck and pelvic cancers remains challenging due to large concave target volumes, multiple dose prescriptions and numerous organs at risk close to targets. Inter-institutional studies highlighted that plan quality strongly depends on planner experience and skills. Automated optimization of planning procedure may improve plan quality and best practice. We performed a comprehensive dosimetric and clinical evaluation of the Pinnacle 3 AutoPlanning engine, comparing automatically generated plans (AP) with the historically clinically accepted manually-generated ones (MP). Thirty-six patients (12 for each of the following anatomical sites: head-neck, high-risk prostate and endometrial cancer) were re-planned with the AutoPlanning engine. Planning and optimization workflow was developed to automatically generate “dual-arc” VMAT plans with simultaneously integrated boost. Various dose and dose-volume parameters were used to build three metrics able to supply a global Plan Quality Index evaluation in terms of dose conformity indexes, targets coverage and sparing of critical organs. All plans were scored in a blinded clinical evaluation by two senior radiation oncologists. Dose accuracy was validated using the PTW Octavius-4D phantom together with the 1500 2D-array. Autoplanning was able to produce high-quality clinically acceptable plans in all cases. The main benefit of Autoplanning strategy was the improvement of overall treatment quality due to significant increased dose conformity and reduction of integral dose by 6–10%, keeping similar targets coverage. Overall planning time was reduced to 60–80 minutes, about a third of time needed for manual planning. In 94% of clinical evaluations, the AP plans scored equal or better to MP plans. Despite the increased fluence modulation, dose measurements reported an optimal agreement with dose calculations with a γ-pass-rate greater than 95% for 3%(global)-2 mm criteria. Autoplanning engine is an effective device enabling the generation of VMAT high quality treatment plans according to institutional specific planning protocols.

Abstract:Background and purpose: Treatment planning is a time-intensive task that could be automated. We aimed to develop a "single-click" workflow, fully deployed within a commercial treatment planning system (TPS), for autoplanning prostate radiotherapy treatment plans using predictions from a deep learning model (DLM). Materials and methods: Automatically generated treatment plans were created with a single script, executed from within a commercial TPS scripting environment, that performed two stages sequentially. Initially, a 3D dose distribution was predicted with a ResUNet DLM. The DLM was trained and validated using previously treated datasets (n = 120) which used 3D contours as inputs. Following this, dose predictions were converted into treatment plans by extracting dose-volume metrics from the predictions to use as objectives for the inverse optimizer within the TPS. An independent test dataset (n = 20) was used to evaluate the similarity between automated and clinical plans. Results: For planning target volumes, the median percentage difference and interquartile range between the automatically generated plans and clinical plans were 0.4% [0.2-1.1%] for the V100%, -0.5% [(-1.0)-(-0.2)%] for D99% and -0.5% [(-1.0)-(-0.2)%] for D95%. Bladder and rectum volume-at-dose objectives agreed within -6.1% [(-12.5)-0.9%]. The conversion of the DLM prediction into a treatment plan took 15 min [13-16 min]. Conclusions: An automatic plan generation workflow that uses a DL model with scripted optimization was fully deployed in a commercial TPS. Autoplans were compared to previously treated clinical plans and were found to be non-inferior.

Abstract:A toolkit for interacting with the Elekta Monaco (Clements et al 2018) treatment planning system (TPS) has been designed without the need of a dedicated Application Programming Interface (API). It provides automatization of the radiotherapy planning procedure allowing the TPS to calculate or optimize plans during non-working hours. The software is based on an open source library that mimics human interaction with Microsoft Windows applications. The impact on the clinical workflow is important not only providing better efficiency but also increasing treatment quality. Successful inverse planning depends on the tweaking of many parameters that can be explored more exhaustively with this tool without significantly increasing planning time. Furthermore, a simple way to analyze calculated plans and the impact of the cost functions in the optimization has been implemented. The Autoflow sequence has been developed allowing fully automated planning; it works analyzing the relative impact of different cost functions in the optimization and modifying constraints accordingly. It also changes segmentation parameters to fit more complex treatments. A prostate study has been conducted, comparing automatically created plans with already treated ones. The Autoflow sequence has proved to reduce dose to organs at risk with a negligible decrease in target coverage. Implementing this tool has enabled an efficient use of the TPS, allowing research and clinical use to coexist in a friendly way. It is surely of great use in clinics with little resources. It provides consistency and efficiency throughout the treatment planning process.

Fully automated radiotherapy treatment planning: A scan to plan challenge

Abstract:Background and purpose: Over the past decade, tools for automation of various sub-tasks in radiotherapy planning have been introduced, such as auto-contouring and auto-planning. The purpose of this study was to benchmark what degree of automation is possible. Materials and methods: A challenge to perform automated treatment planning for prostate and prostate bed radiotherapy was set up. Participants were provided with simulation CTs and a treatment prescription and were asked to use automated tools to produce a deliverable radiotherapy treatment plan with as little human intervention as possible. Plans were scored for their adherence to the protocol when assessed using consensus expert contours. Results: Thirteen entries were received. The top submission adhered to 81.8% of the minimum objectives across all cases using the consensus contour, meeting all objectives in one of the ten cases. The same system met 89.5% of objectives when assessed with their own auto-contours, meeting all objectives in four of the ten cases. The majority of systems used in the challenge had regulatory clearance (Auto-contouring: 82.5%, Auto-planning: 77%). Despite the 'hard' rule that participants should not check or edit contours or plans, 69% reported looking at their results before submission. Conclusions: Automation of the full planning workflow from simulation CT to deliverable treatment plan is possible for prostate and prostate bed radiotherapy. While many generated plans were found to require none or minor adjustment to be regarded as clinically acceptable, the result indicated there is still a lack of trust in such systems preventing full automation.

Abstract:Background/Objectives: To evaluate an end-to-end pipeline for normo-fractionated prostate-only and whole-pelvic cancer treatments that requires minimal human input and generates a machine-deliverable plan as an output. Methods: In collaboration with TheraPanacea, a treatment planning pipeline was developed that takes as its input a planning CT with organs-at-risk (OARs) and planning target volume (PTV) contours, the targeted linac machine, and the prescription dose. The primary components are (i) dose prediction by a single deep learning model for both localizations and (ii) a direct aperture VMAT plan optimization that seeks to mimic the predicted dose. The deep learning model was trained on 238 cases, and a held-out set of 86 cases was used for model validation. An end-to-end clinical evaluation study was performed on another 40 cases (20 prostate-only, 20 whole-pelvic). First, a quantitative evaluation was performed based on dose–volume histogram (DVH) points and plan parameter metrics. Then, the plan deliverability was assessed via portal dosimetry using the global gamma index. Additionally, the reference clinical manual plans were compared with the automated plans in terms of monitor unit (MU) numbers and modulation complexity scores (MCSv). Results: The automated plans provided adequate treatment plans (or minor deviations) with respect to the dose constraints, and the quality of the plans was similar to the manual plans for both localizations. Moreover, the automated plans showed successful deliverability and passed the portal dose verification. Despite higher median total MUs, no statistically significant correlation was observed between any of the gamma criteria tested and the number of MUs or MCSv. Conclusions: This study shows the feasibility of a deep learning-based fully automated treatment planning pipeline that generates high-quality plans that are competitive with manually made plans and are clinically approved in terms of dosimetry and machine deliverability.

Automatic treatment planning for radiotherapy: a cross-modality and protocol study

Abstract:This study investigates the applicability of 3D dose predictions from a model trained on one modality to a cross-modality automated planning workflow. Additionally, we explore the impact of integrating a multi-criteria optimizer on adapting predictions to different clinical preferences. Using a previously created three-stage UNet in-house model trained on the 2020 AAPM OpenKBP challenge dataset (340 head and neck plans, planned using 9-field static IMRT), we retrospectively generated dose predictions for 20 patients. These dose predictions were used to generate deliverable IMRT, VMAT, and Tomotherapy plans using the fallback plan functionality in Raystation. The deliverable plans were evaluated against the dose predictions based on primary clinical goals. A new set of plans was also generated using MCO-based optimization with predicted dose values as constraints. The mimicking approach accurately replicated the predicted dose distributions across different modalities, with slight deviations in spinal cord and external contour maximum doses. MCO customization significantly reduced doses to OARs prioritized by our institution while maintaining target coverage. All tested plans met clinical deliverability standards, evidenced by a delivery QA gamma analysis passing rate above 98%. Our findings show that a model trained only on IMRT plans can contribute to planning across various modalities. Additionally, integrating predictions as constraints in an MCO-based workflow, rather than direct dose mimicking, enables a flexible, warm-start approach for treatment planning. Together, these approaches have the potential to significantly decrease plan turnaround time and quality variance, both at high resource medical centers that can train in-house models, and smaller centers that can adapt a model from another institution with minimal effort.

Evaluation of a hybrid automatic planning solution for rectal cancer

Abstract:Background: Script-based planning and knowledge-based planning are two kinds of automatic planning solutions. Hybrid automatic planning may integrate the advantages of both solutions and provide a more robust automatic planning solution in the clinic. In this study, we evaluated and compared a commercially available hybrid planning solution with manual planning and script-based planning. Methods: In total, 51 rectal cancer patients in our institution were enrolled in this study. Each patient generated 7 plans: one clinically accepted manual plan ([Formula: see text]), three script-based plans and three hybrid plans generated with the volumetric-modulated arc therapy technique and 3 different clinical goal settings: easy, moderate and hard ([Formula: see text], [Formula: see text], [Formula: see text], [Formula: see text], [Formula: see text] and [Formula: see text]). Planning goals included planning target volume (PTV) Dmax, bladder Dmean and femur head Dmean. The PTV prescription was the same (50.00 Gy) for the 3 goal settings. The hard setting required a lower PTV Dmax and stricter organ at risk (OAR) dose, while the easy setting was the opposite. Plans were compared using dose metrics and plan quality metric (PQM) scores, including bladder D15 and D50, left and right femur head D25 and D40, PTV D2, D98, CI (conformity index) and HI (homogeneity index). Results: Compared to manual planning, hybrid planning with all settings significantly reduced the OAR dose (p < 0.05, paired t-test or Wilcoxon signed rank test) for all dose-volume indices, except D25 of the left femur head. For script-based planning, [Formula: see text] significantly increased the OAR dose for the femur head and D2 and the PTV homogeneity index (p < 0.05, paired t-test or Wilcoxon signed rank test). Meanwhile, the maximum dose of the PTV was largely increased with hard script-based planning (D2 = 56.06 ± 7.57 Gy). For all three settings, the comparison of PQM between hybrid planning and script-based planning showed significant differences, except for D25 of the left femur head and PTV D2. The total PQM showed that hybrid planning could provide a better and more robust plan quality than script-based planning. Conclusions: The hybrid planning solution was manual-planning comparable for rectal cancer. Hybrid planning can provide a better and more robust plan quality than script-based planning.

Abstract:Objective.Automatic treatment planning of radiation therapy (RT) is desired to ensure plan quality, improve planning efficiency, and reduce human errors. We have proposed an Intelligent Automatic Treatment Planning framework with a virtual treatment planner (VTP), an artificial intelligence robot built using deep reinforcement learning, autonomously operating a treatment planning system (TPS). This study extends our previous successes in relatively simple prostate cancer RT planning to head-and-neck (H&N) cancer, a more challenging context even for human planners due to multiple prescription levels, proximity of targets to critical organs, and tight dosimetric constraints.Approach.We integrated VTP with a real clinical TPS to establish a fully automated planning workflow guided by VTP. This integration allowed direct model training and evaluation using the clinical TPS. We designed the VTP network structure to approach the decision-making process in RT planning in a hierarchical manner that mirrors human planners. The VTP network was trained via theQ-learning framework. To assess the effectiveness of VTP, we conducted a prospective evaluation in the 2023 Planning Challenge organized by the American Association of Medical Dosimetrists (AAMD). We extended our evaluation to include 20 clinical H&N cancer patients, comparing the plans generated by VTP against their clinical plans.Main results.In the prospective evaluation for the AAMD Planning Challenge, VTP achieved a plan score of 139.08 in the initial phase evaluating plan quality, and 15 min of planning time with the first place ranking in the adaptive phase competing for planning efficiency while meeting all plan quality requirements. For clinical cases, VTP-generated plans achieved an average VTP score of125.33±11.12, which outperformed the corresponding clinical plans with an average score of117.76±13.56.Significance.We successfully integrated VTP with the clinical TPS to achieve a fully automated treatment planning workflow. The compelling performance of VTP demonstrated its potential in automating H&N cancer RT planning.
Development and validation of an automated Tomotherapy planning method for cervical cancer

Abstract:Purpose: This study aimed to develop an automated Tomotherapy (TOMO) planning method for cervical cancer treatment, and to validate its feasibility and effectiveness. Materials and methods: The study enrolled 30 cervical cancer patients treated with TOMO at our center. Utilizing scripting and Python environment within the RayStation (RaySearch Labs, Sweden) treatment planning system (TPS), we developed automated planning methods for TOMO and volumetric modulated arc therapy (VMAT) techniques. The clinical manual TOMO (M-TOMO) plans for the 30 patients were re-optimized using automated planning scripts for both TOMO and VMAT, creating automated TOMO (A-TOMO) and automated VMAT (A-VMAT) plans. We compared A-TOMO with M-TOMO and A-VMAT plans. The primary evaluated relevant dosimetric parameters and treatment plan efficiency were assessed using the two-sided Wilcoxon signed-rank test for statistical analysis, with a P-value < 0.05 indicating statistical significance. Results: A-TOMO plans maintained similar target dose uniformity compared to M-TOMO plans, with improvements in target conformity and faster dose drop-off outside the target, and demonstrated significant statistical differences (P+ < 0.01). A-TOMO plans also significantly outperformed M-TOMO plans in reducing V50Gy, V40Gy and Dmean for the bladder and rectum, as well as Dmean for the bowel bag, femoral heads, and kidneys (all P+ < 0.05). Additionally, A-TOMO plans demonstrated better consistency in plan quality. Furthermore, the quality of A-TOMO plans was comparable to or superior than A-VMAT plans. In terms of efficiency, A-TOMO significantly reduced the time required for treatment planning to approximately 20 min. Conclusion: We have successfully developed an A-TOMO planning method for cervical cancer. Compared to M-TOMO plans, A-TOMO plans improved target conformity and reduced radiation dose to OARs. Additionally, the quality of A-TOMO plans was on par with or surpasses that of A-VMAT plans. The A-TOMO planning method significantly improved the efficiency of treatment planning.

Abstract:Online evaluation can be achieved within 2 minutes for most clinical proton cases (80% in this study) with the commercial TPS. The speed can be further increased with reduced OAR mapping, fine-tuned deformable registration, optimized and reduced spot number in the initial plans, and upgraded computer server. This proposed method may optimize the efficiency and quality of care for proton radiotherapy.

Abstract:Purpose: Online adaptive proton therapy (oAPT) is essential to address interfractional anatomical changes in patients receiving pencil beam scanning proton therapy. Artificial intelligence (AI)-based autosegmentation can increase the efficiency and accuracy. Linear energy transfer (LET)-based biological effect evaluation can potentially mitigate possible adverse events caused by high LET. New spot arrangement based on the verification computed tomography (vCT) can further improve the replan quality. We propose an oAPT workflow that incorporates all these functionalities and validate its clinical implementation feasibility with patients with prostate cancer. Methods and materials: AI-based autosegmentation tool AccuContour (Manteia) was seamlessly integrated into oAPT. Initial spot arrangement tool on the vCT for reoptimization was implemented using raytracing. An LET-based biological effect evaluation tool was developed to assess the overlap region of high dose and high LET in selected organs at risk. Eleven patients with prostate cancer were retrospectively selected to verify the efficacy and efficiency of the proposed oAPT workflow. The time cost of each component in the workflow was recorded for analysis. Results: The verification plan showed significant degradation of the clinical target volume coverage and rectum and bladder sparing due to the interfractional anatomical changes. Reoptimization on the vCT resulted in great improvement of the plan quality. No overlap regions of high dose and high LET distributions were observed in bladder or rectum in replans. Three-dimensional γ analyses in patient-specific quality assurance confirmed the accuracy of the replan doses before delivery (γ passing rate, 99.57% ± 0.46%) and after delivery (98.59% ± 1.29%). The robustness of the replans passed all clinical requirements. The average time for the complete execution of the workflow was 9.12 ± 0.85 minutes, excluding manual intervention time. Conclusions: The AI-facilitated oAPT workflow demonstrated to be both efficient and effective by generating a replan that significantly improved the plan quality in prostate cancer treated with pencil beam scanning proton therapy.
Automated contouring, treatment planning, and quality assurance for VMAT craniospinal irradiation (VMAT-CSI)

Abstract:Purpose: Create a comprehensive automated solution for pediatric and adult VMAT-CSI including contouring, planning, and plan check to reduce planning time and improve plan quality. Methods: Seventy-seven previously treated CSI patients (age, 2-67 years) were used for creation of an auto-contouring model to segment 25 organs at risk (OARs). The auto-contoured OARs were evaluated using the Dice Similarity Coefficient (DSC), 95% Hausdorff Distance (HD95), and a qualitative ranking by one physician and one physicist (scale: 1-acceptable, 2-minor edits, 3-major edits). The auto-planning script was developed using the Varian Eclipse Scripting API and tested with 20 patients previously treated with either low-dose VMAT-CSI (12 Gy) or high-dose VMAT-CSI (36 Gy + 18 Gy boost). Clinically relevant metrics, planning time, and blinded physician review were used to evaluate significance of differences between the auto and manual plans. Finally, the plan preparation for treatment and plan check processes were automated to improve efficiency and safety of VMAT-CSI. Results: The auto-contours achieved an average DSC of 0.71 ± 0.15, HD95 of 4.81 ± 4.68, and reviewers' ranking of 1.22 ± 0.39, indicating close to "acceptable-as-is" contours. Compared to the manual CSI plans, the auto-plans for both dose regimens achieved statistically significant reductions in body V50% and D mean for parotids, submandibular, and thyroid glands. The variance in the dosimetric parameters decreased for the auto-plans as compared to the manual plans indicating better plan consistency. From the blinded review, the auto-plans were marked as equivalent or superior to the manual-plans 88.3% of the time. The required time for the auto-contouring and planning was consistently between 1-2 hours compared to an estimated 5-6 hours for manual contouring and planning. Conclusions: Reductions in contouring and planning time without sacrificing plan quality were obtained using the developed auto-planning process. The auto-planning scripts and documentation will be made freely available to other institutions and clinics.

Abstract:High dose rate (HDR) brachytherapy treatment planning is conventionally performed manually and/or with aids of preplanned templates. In general, the standard of care would be elevated by conducting an automated process to improve treatment planning efficiency, eliminate human error, and reduce plan quality variations. Thus, our group is developing AutoBrachy, an automated HDR brachytherapy planning suite of modules used to augment a clinical treatment planning system. This paper describes our proof-of-concept module for vaginal cylinder HDR planning that has been fully developed. After a patient CT scan is acquired, the cylinder applicator is automatically segmented using image-processing techniques. The target CTV is generated based on physician-specified treatment depth and length. Locations of the dose calculation point, apex point and vaginal surface point, as well as the central applicator channel coordinates, and the corresponding dwell positions are determined according to their geometric relationship with the applicator and written to a structure file. Dwell times are computed through iterative quadratic optimization techniques. The planning information is then transferred to the treatment planning system through a DICOM-RT interface. The entire process was tested for nine patients. The AutoBrachy cylindrical applicator module was able to generate treatment plans for these cases with clinical grade quality. Computation times varied between 1 and 3 min on an Intel Xeon CPU E3-1226 v3 processor. All geometric components in the automated treatment plans were generated accurately. The applicator channel tip positions agreed with the manually identified positions with submillimeter deviations and the channel orientations between the plans agreed within less than 1 degree. The automatically generated plans obtained clinically acceptable quality.

Abstract:We propose an oAPT workflow that incorporates all these functionalities and validate its clinical implementation feasibility with prostate patients. AI-based auto-segmentation tool AccuContourTM (Manteia, Xiamen, China) was seamlessly integrated into oAPT. Initial spot arrangement tool on the vCT for re-optimization was implemented using raytracing. An LET-based biological effect evaluation tool was developed to assess the overlap region of high dose and high LET in selected OARs. Eleven prostate cancer patients were retrospectively selected to verify the efficacy and efficiency of the proposed oAPT workflow. The time cost of each component in the workflow was recorded for analysis. The verification plan showed significant degradation of the CTV coverage and rectum and bladder sparing due to the interfractional anatomical changes. Re-optimization on the vCT resulted in great improvement of the plan quality. No overlap regions of high dose and high LET distributions were observed in bladder or rectum in re-plans. 3D Gamma analyses in PSQA confirmed the accuracy of the re-plan doses before delivery (Gamma passing rate = 99.57%), and after delivery (98.59%). The robustness of the re-plans passed all clinical requirements. The average time for the complete execution of the workflow was 9.12minutes, excluding manual intervention time. The AI-facilitated oAPT workflow was demonstrated to be both efficient and effective by generating a re-plan that significantly improved the plan quality in prostate cancer treated with PBSPT.

Clinical utility of automatic treatment planning for proton therapy of head-and-neck cancer patients using JulianA

Abstract:Background: Automatic treatment planning promises many benefits for both research and clinical environments. For clinics, autoplanning promises to reduce planning time and achieve more comparable treatment plans and thereby reduce inter-planner variability. Further, it can assist clinicians in quality assurance by providing a minimum plan quality standard. Finally, autoplanning is an essential part of patient selection, which is crucial for the advancement of proton therapy itself. Methods: A retrospective planning study using a cohort of 17 head-and-neck cancer patients treated at our institute. The clinically accepted plans created by dosimetrists (d-plans) were compared to automatically generated JulianA plans (j-plans). Both methods used the same beam arrangement. The plans were analysed by two expert reviewers without knowing how each plan was created. They assessed the plan quality and stated a preference. Results: All of the j-plans were deemed rather or clearly acceptable, resulting in a higher acceptability than the d-plans. The j-plan was considered superior in 14 (82.4%) cases, of equal quality for 1 (5.9%) and inferior to the d-plan for only 2 (11.8%) of the cases. The reviewers concluded that JulianA achieves more conformal dose distributions for the 15 (88.2%) cases where the j-plans were at least as good as the d-plans. Conclusions: The results show that the JulianA is ready to be used as a clinical quality assurance tool and research platform at our institute. While these results are encouraging, further research is needed to reduce the number of spots further and introduce robustness considerations into the optimisation algorithm in order to employ it on a daily basis for patient treatment.

Automatic Planning for Nasopharyngeal Carcinoma Based on Progressive Optimization in RayStation Treatment Planning System

Abstract:Objective: To evaluate and quantify the planning performance of automatic planning (AP) with manual planning (MP) for nasopharyngeal carcinoma in the RayStation treatment planning system (TPS). Methods: A progressive and effective design method for AP of nasopharyngeal carcinoma was realized through automated scripts in this study. A total of 30 patients with nasopharyngeal carcinoma with initial treatment was enrolled. The target coverage, conformity index (CI), homogeneity index (HI), organs at risk sparing, and the efficiency of design and execution were compared between automatic and manual volumetric modulated arc therapy (VMAT) plans. Results: The results of the 2 design methods met the clinical dose requirement. The differences in D95 between the 2 groups in PTV1 and PTV2 showed statistical significance, and the MPs are higher than APs, but the difference in absolute dose was only 0.21% and 0.16%. The results showed that the conformity index of planning target volumes (PTV1, PTV2, PTVnd and PGTVnx+rpn [PGTVnx and PGTVrpn]), homogeneity index of PGTVnx+rpn, and HI of PTVnd in APs are better than that in MPs. For organs at risk, the APs are lower than the MPs, and the difference was statistically significant ( P < .05). The manual operation time in APs was 83.21% less than that in MPs, and the computer processing time was 34.22% more. Conclusion: IronPython language designed by RayStation TPS has clinical application value in the design of automatic radiotherapy plan for nasopharyngeal carcinoma. The dose distribution of tumor target and organs at risk in the APs was similar or better than those in the MPs. The time of manual operation in the plan design showed a sharp reduction, thus significantly improving the work efficiency in clinical application.
