Abstract:Background: Various surgical techniques to treat posterolateral knee instability have been described. To date, the recommended treatment is an anatomic form of reconstruction in which the 3 key structures of the posterolateral corner (PLC) are addressed: the popliteofibular ligament, the popliteus tendon, and the lateral collateral ligament. Purpose/Hypothesis: The purpose of this study was to identify the role of each key structure of the PLC in kinematics of the knee and to biomechanically analyze a single-graft, fibular-based reconstruction that replicates the femoral insertions of the lateral collateral ligament and popliteus to repair the PLC. The hypothesis was that knee kinematics can be reasonably restored using a single graft with a 2-strand “modified Larson” technique. Study Design: Descriptive laboratory study. Methods: Eight fresh-frozen cadaveric knees were used in this study. We conducted sequential sectioning of the popliteofibular ligament (PFL) and then subsequently the popliteal tendon (PT), the lateral collateral ligament (LCL), and the anterior cruciate ligament (ACL). We then reconstructed the ACL first and then the posterolateral corner using the modified Larson technique. A surgical navigation system was used to measure varus laxity and external rotation at 0°, 30°, 60°, and 90° with a 9.8-N·m varus stress and 5-N·m external rotation force applied to the tibia. Results: In extension, varus laxity increased only after the sectioning of the lateral collateral ligament. At 30° of flexion, external rotation in varus and translation of the lateral tibial plateau increased after the isolated popliteofibular ligament section. From 60° to 90° of flexion, translation and mobility of the lateral plateau section increased after sectioning of the PFL. After reconstruction, we observed a restoration of external varus rotation in extension and translation of the lateral tibial plateau at 90° of flexion. This technique provided kinematics similar to the normal knee. Conclusion: The PFL has a key role between 30° and 90° of flexion, and the lateral collateral ligament plays a role in extension. Reconstruction with the modified Larson technique restores these 2 complementary stabilizers of the knee. Clinical Relevance: Although there are many different techniques to reconstruct the PLC-deficient knee, this study indicates that a single-graft, fibular-based reconstruction of the LCL and PT may restore varus and external rotation laxity to the knee.

Abstract:The main purpose of our study was to evaluate the biomechanics of different posterior cruciate ligament (PCL) reconstruction techniques. Seven fresh cadaver knees were collected. A 6-DOF robot arm was used to test the biomechanical parameters, including the posterior stability, the lateral stability and the rotation stability of different PCL reconstruction techniques. Each group was tested at the knee flexion of 0, 30[Formula: see text], 60[Formula: see text], 90[Formula: see text] and 120[Formula: see text], under the following conditions respectively: a posterior force of 134[Formula: see text]N, an internal and external rotation torque of 5[Formula: see text][Formula: see text], a varus and valgus torque of 10[Formula: see text][Formula: see text], and a combination of 100[Formula: see text]N posterior force and 5[Formula: see text][Formula: see text] external rotation torque. The posterior tibia translation and the rotational angle of the 4-tunnel double-bundle PCL reconstruction group were significantly lower than that of 3-tunnel double-bundle group and the single-bundle group; the posterior tibia translation valgus–varus-angle were lower at some specified flexion angle. No statistical difference was found between the anatomic 4-tunnel bundle group and the intact knee group concerning the posterior tibia translation, the rotational angle, and the valgus–varus-angle. This study showed that the biomechanics of PCL of 4-tunnel double-bundle reconstruction was closer to the intact knees than the other two reconstruction methods.

Abstract:High-grade knee posterolateral corner (PLC) injuries are potentially devastating and often associated with high-energy mechanisms. Failure of PLC injury diagnosis or treatment can lead to residual instability after combined cruciate ligament reconstruction because of the increased risk of graft failure, and varus malalignment may lead to early osteoarthritis and meniscal injuries. PLC reconstruction has consistently shown superiority over PLC repair. Biomechanical studies have compared reconstruction techniques, specifically evaluating rotational and varus laxity. Some studies have demonstrated no difference between techniques, whereas other studies have reported improved stability with techniques that include a separate tibial tunnel for reconstruction of the popliteus tendon and popliteofibular ligament. Yet many have suggested that there is less technical difficulty with techniques that do not use a tibial tunnel, and this may be an important consideration in certain settings. Recent reviews showing no differences in clinical outcomes when comparing techniques for PLC reconstruction are based on heterogeneous, low level of evidence, high-risk-of-bias literature. It is well recognized that PLC injuries are heterogeneous, with approximately three quarters occurring in combination with anterior and/or posterior cruciate ligament tears. Further, laxity patterns vary for these injuries including high-grade posterior laxity and knee hyperextension as well as proximal tibial-fibular joint laxity, and these findings may necessitate use of an anatomic (separate tibial tunnel) PLC reconstruction technique. Reassuringly, both techniques show low complication and failure rates.

ObjectivesTo compare anatomic versus non-anatomic-posterolateral-corner (PLC) reconstructions with autografts for grade 3 posterolateral corner knee lesions combined with single bundle posterior cruciate ligament (PCL) reconstruction.MethodsAn outcome comparative study of chronic PCL lesions submitted to single bundle PCL reconstruction combined with a grade 3 PLC injury from Hughston and Fanelli classifications treated either by anatomic or non-anatomic PLC-reconstruction with autografts with at least 24 months of follow-up. Outcomes: Lysholm, IKDC, Tegner, side-to-side-difference (SSD) from posterior and varus stress radiographs, IKDC objective scores for posterior drawer, varus stress, dial test and recurvatum. Student's t- and two-proportion Z-test were used. Power for sample size was 88.2%.ResultsThirty-nine patients were evaluated: 18 in the Anatomic-PLC-group and 21 in the Non-anatomic-PLC-group. Mean follow-up: 28.3 months (range 24 to 59 months). Preoperatively, no significant differences were observed between the two groups in terms of age; time from injury until surgery; Lysholm; subjective IKDC; posterior, varus, external rotation or recurvatum laxity; stress radiographs; incidence of associated ligament and meniscus injuries. Postoperatively, no significant differences were observed for Lysholm (p=0.189), subjective IKDC (p=0.261), Tegner (p=0.385), mean improvement in Lysholm and IKDC scores (p=0.144, p=0.354, respectively), posterior tibial translation on stress radiographs [postoperative SSD (p=0.205) and percentage of patients with unacceptable (≥8mm) asymmetry (p=0.912)]; and complications. Significant differences favoring Anatomic-PLC-group were observed for Posterior-Drawer-Test (0.83±0.51 vs 1.1±0.43,p=0.046); Varus-Stress-Gapping at 30o (0.5±0.78 vs 1.14±0.85, p=0.01); Tibial-External-Rotation both 30o (33.3% vs 80.9%, p=0.002) and 90o (16.6% vs 47.6%, p=0.041); Asymmetrical-Recurvatum (5.5% vs 47.6%, p=0.00362); SSD-Lateral-Gapping-Varus-Stress-Radiographs (1.8±2.1 vs 3.35±2.28, p=0.028). Failure (presence of any of the following: Posterior Drawer ≥ 2+, Varus Stress ≥ 2+, SSD ≥ 8mm on Posterior-Radiograph or ≥ 4mm on Varus-Stress-Radiograph): 5% Anatomic versus 19% Non-anatomic, no statistical difference, p=0.103.ConclusionAnatomic posterolateral corner reconstruction with autografts provided superior stability compared to non-anatomic reconstruction in the setting of combined grade 3 posterolateral corner injuries and single bundle posterior cruciate ligament reconstruction.

Abstract:目的探讨膝后外侧角(PLC)的结构组成、解剖学和生物力学特点.方法 15具30例膝关节尸体标本,解剖观察PLC的位置、组成和形态学特点.在选择性离断PLC部分结构的不同工况下,观察膝内翻加载移位和胫骨上端的后外侧旋转加载移位.结果 (1)PLC位于膝关节后外侧区域,邻近上胫腓联合处,由膝外侧副韧带、豆腓韧带、弓状韧带、肌腱、腓韧带、后外侧关节囊等结构组成.(2)PLC切断后,膝内翻移位增加274%～280%,胫骨上端后外侧旋转移位增大210%～305%.其中膝外侧副韧带离断后膝内翻移位增加比例达78%～87%;而离断腓韧带后胫骨上端后外侧旋转移位增幅比例达81%～91%.结论 (1)膝外侧副韧带和腓韧带是膝后外侧角中主要的稳定结构;(2)膝后外侧角具有明显对抗膝内翻不稳和膝后外侧旋转不稳定的作用。
[Anatomical Reconstruction of Posterolateral Complex in Treatment of Multi-Ligament Injury of Knees].

Abstract:OBJECTIVE:To evaluate the short-term effectiveness after static anatomical reconstruction of posterolateral complex (PLC) in the treatment of traumatic multi-ligament injury of the knee.METHODS:Between June 2007 and July 2011, 23 cases of multi-ligament injury of the knee were treated. There were 15 males and 8 females with an average age of 41 years (range, 19-56 years). The injury was caused by traffic accident in 9 cases, sprain in 7 cases, bruise in 3 cases, and falling from height in 4 cases. The time between injury and operation was 13-78 days (mean, 32 days). The results of posterior drawer test and Lachman test were positive, and all cases complicated by varus and external rotation instability. The Lysholm score of the knee was 43.4 +/- 5.7. According to International Knee Documentation Committee (IKDC) scoring, all were rated as grade D. According to Fanelli typing, all were classified as type C. The X-ray films showed that load-induced posterior motion of the knee was (13.3 +/- 4.2) mm; the lateral joint space was (15.1 +/- 2.4) mm. Anterior cruciate ligament/posterior cruciate ligament and PLC were reconstructed simultaneously with auto-semitendinosus, gracilis tendon, and allogeneic tendon.RESULTS:All incisions healed by first intention, and no complication occurred. All patients were followed up 12-56 months (mean, 28 months). At last follow-up, the results of posterior drawer test and Lachman test were negative; 3 cases had varus instability, and 2 cases had external rotation instability. The Lysholm score of the knee was 85.6 +/- 16.7, showing significant difference when compared with preoperative score (t=-11.469, P=0.000). According to IKDC scoring, 7 cases were rated as grade A, 12 as grade B, and 4 as grade C; significant difference was found when compared with preoperative value (Z=4.285, P=0.000). The load-induced posterior motion of the knee was (5.1 +/- 4.4) mm, the lateral joint space was (3.2 +/- 2.8) mm, showing significant differences when compared with preoperative ones (P < 0.05).CONCLUSION:In the treatment of traumatic multi-ligament injury of the knee, the anatomical reconstruction of the PLC using auto-semitendinosus, gracilis tendon, or allogeneic tendon can obtain good short-term effectiveness.

Abstract:Posterolateral corner (PLC) structures of the knee joint comprise complex anatomical soft tissues that support static and dynamic functional movements of the knee. Most previous studies analyzed posterolateral stability in vitro under static loading conditions. This study aimed to evaluate the contributions of the lateral (fibular) collateral ligament (LCL), popliteofibular ligament (PFL), and popliteus tendon (PT) to cruciate ligament forces under simulated dynamic loading conditions by using selective individual resection. We combined medical imaging and motion capture of healthy subjects (four males and one female) to develop subject-specific knee models that simulated the 12 degrees of freedom of tibiofemoral and patellofemoral joint behaviors. These computational models were validated by comparing electromyographic (EMG) data with muscle activation data and were based on previous experimental studies. A rigid multi-body dynamics simulation using a lower extremity musculoskeletal model was performed to incorporate intact and selective resection of ligaments, based on a novel force-dependent kinematics method, during gait (walking) and squatting. Deficiency of the PLC structures resulted in increased loading on the posterior cruciate ligament and anterior cruciate ligament. Among PLC structures, the PT is the most influential on cruciate ligament forces under dynamic loading conditions.
Anatomical Single Bundle Anterior Cruciate Ligament Reconstruction with Rounded Rectangle Tibial Tunnel and Oval Femoral Tunnel: a Prospective Comparative Study Versus Conventional Surgery

Abstract:Conventional anatomical single bundle anterior cruciate ligament (ACL) reconstruction technique with round tunnels could not simulate morphology of native insertion, while studies about ACL reconstruction technique with modified tunnels based on morphology of anatomical insertion are rare. The purpose of this study was to demonstrate an ACL reconstruction technique with rounded rectangle tibial tunnel and oval femoral tunnel and compare clinical outcomes with conventional technique. A prospective comparative study was performed in 80 consecutive subjects who underwent ACL reconstruction with the conventional round tunnels (RT-Group, n=40) or modified tunnels (MT-Group, n=40). For the modified surgery, the tunnel was modified with a bone file based on the anatomical direction and area of the remnant insertion fibers. Graft maturity were evaluated by MR images at 12 months post-operatively and patients were examined for functional scores, physical examinations at 2-year follow-up. The primary variable was the pivot-shift test. No serious complications were experienced in either group. Seventy patients (87.5%) were examined at 2-year follow-up, significant improvements were seen in both groups compared with the preoperative values in terms of all clinical assessments. Tegner scores, pivot-shift test results and SNQ value in the MT-Group were significantly better than RT-Group (P=0.04, P=0.03 and P=0.001, respectively). There were no significant differences in Lysholm scores, IKDC scores, KT-2000 measurements and Lachman tests. We successfully developed the ACL reconstruction technique with rounded rectangle tibial tunnel and oval femoral tunnel, which was superior to conventional technique in terms of postoperative Tegner scores, pivot-shift tests and early graft maturity.

Abstract:The posterior cruciate ligament (PCL) reconstruction is a fairly recent innovation, mostly performed either by two-root technique which makes two bundles of PCL, anterolateral and posteromedial bundles, or inlay technique. To perform either technique, a better understanding of the anatomical structure of the two bundles of PCL is very important, particularly the femoral insertion. This anatomical insertion has been inferred to be the anatomic point in PCL reconstruction.

Abstract:OBJECTIVE:To conduct a primary clinical analysis on 411 cases of arthroscopic 4 bone-tunnel double-bundle anterior cruciate ligament (ACL) reconstruction done by one operator on the basis of anatomic measurements of cadaver knees.METHODS:Twenty-two adult male Chinese cadaver knees of 25-45 years old were dissected. The sizes, distributions and positions of femoral and tibia footprints of anterior medial bundle (AMB) and posterior lateral bundle (PLB) of ACL and their fiber's orientations were measured and recorded. The anatomy and measurement results were used by one operator in 413 double-bundle ACL reconstruction knees of 411 cases with arthroscopic 4 bone-tunnel technique. Among them, 297 males and 114 females, there were 222 left knees and 191 right knees. The average age was 27.61 +/- 7.23 years old.RESULTS:The anatomy results showed the locations and sizes of AMB and PLB footprints in both femoral and tibia sides were different for different individuals. It was suggested to locate AMB femoral tunnel 7.95 +/- 1.40 mm inferior to 12:00 o'clock position on the posterior edge of the lateral wall of intercondylar fossa with 1 mm thick of the posterior wall of finished AMB femoral tunnel. With the knee flexed to 90 degrees, the PLB tunnel was 5.05 +/- 0.76 mm superior to the lower cartilage edge of the lateral wall of intercondylar fossa, 8.60 +/- 1.52 mm away from the anterior cartilage edge and 8.65 +/- 1.54 mm from the posterior edge. The anatomic orientations of tibia and femoral tunnels were also measured. The study results were used in arthroscopic double-bundle ACL reconstructions of 413 knees. In double-bundle ACL reconstruction operations, the grafts were used in the way from 2-strip double-bundle to 9-strip double-bundle. For most cases, Endo-Button was used for femoral fixation and bio-absorbable interference screw and staple were used for tibia fixation. For the patients adopting autologous semitendinosus and gracilis tendons (STG), the diameter of PLB bone tunnels was 5-6 mm in 95.9% cases, while that of AMB bone tunnel 5-7 mm in 72.4% cases. After a 28-month follow-up in 75 cases, the results showed that double-bundle ACL reconstruction could better reconstruct the stability of knees.CONCLUSION:In 4-bone-tunnel double-bundle arthroscopic ACL reconstruction, both bone tunnel positions and their orientations should be determined according to the anatomic measurement results. Since the PLB diameters of 95.9% cases were 5-6mm, considering the operative outcome of ACL revision, the double bundle ACL reconstruction technique was safe. A better knee stability could be reconstructed by this technique.

ObjectivesThe constant study evolution of the structures and biomechanical functioning of the posterolateral complex (PLC) of the knee show that this structure plays an extremely important role in joint stability, especially when associated with injury to the anterior cruciate ligament (ACL). Failure to detect the PLC lesion causes posterolateral rotational instability in the knee, generating an overload on the reconstructed ACL, increasing its potential for failure. Surgical treatment is the gold standard for patients with combined injuries of the PLC and ACL. However, despite the numerous reconstruction techniques described, the best technique is still controversial. This study presents a less invasive technique with fewer bone tunnels, lower cost and less surgical complexity, using four bone tunnels with a single tunnel for the ACL and lateral collateral ligament (LCL) in the lateral femoral condyle and only 04 interference screws for the anatomical reconstruction of the ACL / PLC and restoration of stability of the injured knee.Surgical techniquePatient in supine position. A graft is prepared with 2 semitendinosus tendons (1 from each knee) and 1 gracilis tendon (from the knee with ligament injury). The tendons must be at least 24 cm long. The graft is prepared in quadruple for the ACL (gracilis tendon duplicated and 2 simple semitendinosus tendons) and double for the PLC (continuation of the 2 simple semitendinosus tendons).An incision is made on the lateral aspect of the knee over the lateral epicondyle, towards the head of the fibula, approximately 12cm long. The femoral insertion site of the lateral collateral ligament (LCL) is identified slightly proximal and posterior to the femoral epicondyle. The unique femoral tunnel for the ACL and LCL is performed using the outside-in technique. The guidewire must be inserted in the origin of the LCL anatomical point at the external lateral cortex towards the origin of the ACL anatomical on the medial wall of the lateral fêmur condyle. The fibular head tunnel is performed, positioned from anterolateral to posteromedial, approximately 0.5 cm below the tip of the fibular head. The fourth and last tunnel is performed l at the popliteal tendon femoral insertion. It should be directed anteriorly and proximally to avoid confluence with the ACL tunnel. The graft is passed through the tibial tunnel towards the femoral tunnel. Both semitendinosus legs exit the LCL insertion at the lateral aspect of the femur. Graft fixation at the femoral tunnel lateral cortex is performed with an interference screw. The graft is tensioned at its tibial exit with knee flexed at 20° and fixed with an interference screw, reconstructing the ACL. For PLC reconstruction, the two semitendinosus legs are passed under the iliotibial tract through the fibular head tunnel from anterolateral to posteromedial. The graft is tensioned with knee at 30° of flexion and valgus and fixed with an interference screw at the fibular tunnel, reconstructing the LCL. The remaining portion of the graft is passed below the iliotibial tract and LCL, being inserted into the femoral tunnel corresponding to the insertion of the popliteal tendon. The graft is then tensioned with knee flexed at 60° and valgus and fixed with an interference screw.ConclusionWe believe that the described technique for ACL reconstruction associated with PLC injury is able to restore knee stability through a minimally invasive, reproducible and less technically challenging approach, with a lower learning curve than those currently available.

Abstract:Background: The factors that influence functions of knees after anterior cruciate ligament reconstruction (ACLR) still remains uncertain. The functional restoration of knees after ACLR can be reflected on gait kinematics restoration. The purpose of this study was to evaluate the gait kinematics and clinical outcomes of knees after anatomical and non-anatomical single-bundle ACLR during level walking. Methods: Thirty-four patients with unilateral primary single-bundle ACLR and 18 healthy people were recruited. Patients were divided into anatomical reconstruction group (AR group; n=13) and non-anatomical reconstruction group (Non-AR group; n=21) according to Bernard Quadrant method. The ACL graft orientations on coronal and sagittal planes were measured on 3D models from medical images. The 6 degrees of freedom (DOF) kinematics of knees and range of motion (ROM) of 6 DOF kinematics were measured with a portable optical tracking system. The comparison of 6 DOF kinematics and ROM of 6 DOF kinematics were performed between the ACLR knees and contralateral knees. The following assessments were also performed including clinical examination, KT-2000 arthrometer measurement, International Knee Documentation Committee (IKDC) and Lysholm scores. Results: All patients reached a minimum follow-up of 6 months (10 +/- 4 months). For AR group and Non-AR group, no statistically significant differences were observed in gait kinematics between the ACLR knees and contralateral knees. No statistically significant differences between the ACLR knees and contralateral knees were observed in terms of ROM of 6 DOF kinematics in AR group. However, in Non-AR group, the ACLR knees exhibited significant ROM of anterior-posterior translation by approximately 0.5 cm than contralateral knees (P=0.0080). No statistically significant differences between the two groups were observed regarding IKDC subjective score, Lysholm score and KT-2000 arthrometer test. Conclusions: The anatomical ACLR can restore close to normal gait kinematics and ROM of 6 DOF kinematics compared with non-anatomical ACLR. The ACL graft after anatomical ACLR simulated native ACL fibers to function in terms of graft orientation.
Anterolateral Structure Reconstruction Similarly Improves the Stability and Causes Less Overconstraint in Anterior Cruciate Ligament-Reconstructed Knees Compared with Modified Lemaire Lateral Extra-articular Tenodesis: A Biomechanical Study

Abstract:Purpose: To compare the kinematics of anterolateral structure (ALS) reconstruction (ALSR) and lateral extra-articular tenodesis (LET) in ACL-ALS-deficient knees with anterior cruciate ligament (ACL) reconstruction. Methods: Ten fresh-frozen cadaveric knees with the following conditions were tested: (1) intact, (2) ACL-ALS deficiency, (3) ACL reconstruction (ACLR), (4) ACLR combined with ALSR (ACL-ALSR) or LET (ACLR+LET). Anterior translation and tibial internal rotation were measured with 90-N anterior load and 5 N.m internal torque at 0 degrees, 30 degrees, 60 degrees, and 90 degrees. The anterolateral translation and internal rotation were also measured during a simulated pivot-shift test at 0 degrees, 15 degrees, 30 degrees, and 45 degrees. The knee kinematic changes in all reconstructions were compared with each other, with intact knees as the baseline. Results: Isolated ACLR failed to restore native knee kinematics in ACL-ALS-deficient knees. Both ACL-ALSR and ACLR+LET procedures decreased the anterior instability of the ACLR. However, ACLR+LET caused overconstraints in internal rotation at 30 degrees (-3.73 degrees +/- 2.60 degrees, P = .023), 60 degrees (-4.96 degrees +/- 2.22 degrees, P = .001) and 90 degrees (-6.14 degrees +/- 1.60 degrees, P < .001). ACL-ALSR also over-constrained the knee at 60 degrees (-3.65 degrees +/- 1.90 degrees, P < .001) and 90 degrees (-3.18 degrees +/- 2.53 degrees, P < .001). For a simulated pivot-shift test, both combined procedures significantly reduced the ACLR instability, with anterolateral translation and internal rotation being overconstrained in ACLR+LET at 30 degrees (-3.32mm +/- 3.89mm, P = .005; -2.58 degrees +/- 1.61 degrees, P < .001) and 45 degrees (-3.02mm +/- 3.95 mm, P = .012; -3.44 degrees +/- 2.86 degrees, P < .001). However, the ACL-ALSR overconstrained only the anterolateral translation at 30 degrees (-1.51mm +/- 2.39mm, P = .046) and internal rotation at 45 degrees (-2.09 degrees +/- 1.70 degrees, P < .001). There were no significant differences between the two combined procedures at most testing degrees in each testing state, except for the internal rotation at 30 degrees (P = .007) and 90 degrees (P = .032) in internal rotation torque. Conclusion: ACL reconstruction alone did not restore intact knee kinematics in knees with concurrent ACL tears and severe ALS injury (ACL-ALS-deficient status). Both ACL-ALSR and ACLR+LET procedures restored knee stability at some flexion degrees, with less overconstraints in internal rotation resulting from ACL-ALSR.
Anatomical Study of the Anterolateral and Posteromedial Bundles of the Posterior Cruciate Ligament for Double-Bundle Reconstruction Using the Quadruple Bone-Tunnel Technique

Abstract:Background Several techniques have been described for posterior cruciate ligament (PCL) reconstruction. However, double-bundle PCL reconstruction using the quadruple bone-tunnel technique has been seldom reported. The current study investigated this technique, focusing on the anatomy of the femoral and tibial insertions of the anterolateral (AL) and posteromedial (PM) bundles of the PCL.Methods Twenty-two fresh, healthy adult cadaveric knees were dissected and measured. The PCL was divided into the AL bundle and PM bundle at the insertion footprint. The insertion footprints of the AL and PM bundles, their location, size, and the clock positions were measured and described.Results On the femur, the clock position of the footprint of the AL bundle was 11:21 +/- 0:23 (left) or 0:39 +/- 0:23 (right), and the PM bundle was 9:50 +/- 0:18 (left) or 2:10 +/- 0:18 (right), with the knee flexed at 90 degrees. The distances from the center of the femoral insertions of the AL and PM bundles to the anterior cartilage margins of the medial femoral condyle were (7.79 +/- 1.22) mm and (8.36 +/- 1.63) mm, respectively. On the tibia, the vertical distances from the center of the tibial insertions of the AL and PM bundles to the tibial articular surface were (3.25 +/- 1.20) mm and (6.91 +/- 1.57) mm, respectively.Conclusions These results have led to a better definition of the anatomy of the AL and PM bundle footprint of the PCL. The technique of double-bundle PCL reconstruction using quadruple bone-tunnel is feasible. Application of these data during PCL reconstruction using the quadruple bone-tunnel technique may help optimize knee stability. Chin Med J 2012;125(22):3972-3976

Abstract:Introduction Multiligament knee injury with posteromedial laxity is serious and usually requires surgery. Reconstruction is preferable to repair. The main aim of the present study was to report clinical results and laximetry for an original posteromedial corner (PMC) allograft reconstruction technique known as The Versailles Technique. The secondary aim was to determine prognostic factors for surgery. The study hypothesis was that anatomic PMC reconstruction by tendon allograft provides satisfactory medium-term clinical and laximetric results. Methods A retrospective study assessed postoperative clinical and laximetric results after PMC allograft reconstruction at a minimum 12 months' follow-up. Laxity was assessed on comparative bilateral stress X-rays, and functional results on the International Knee Documentation Committee (IKDC) score, the Lysholm score and the Knee injury and Osteoarthritis Outcome Score (KOOS). Results Twenty-six patients were included between 2013 and 2019. Mean follow-up was 27.4 ± 9 months. Mean subjective IKDC score was 69.21 ± 17.36, mean Lysholm score 77.78 ± 14.98 and mean KOOS 66.44 ± 18.52. Objective IKDC results were 77% grade A, 22% grade B, and 0% grade C or D. Mean medial differential laxity in forced varus was 0.83 ± 1.26 mm. Mean subjective IKDC scores were poorer in Schenck KD-III than KD-I ( p = 0.03). Functional results were comparable with acute and with chronic laxity. Age correlated inversely with median KOOS ( p = 0.009). There was no correlation between postoperative radiologic laxity in forced varus and functional results. Discussion Versailles anatomic PMC allograft reconstruction for acute or chronic posteromedial knee laxity showed medium-term efficacy in restoring good objective and subjective stability. Abstract:Injuries to the posterolateral corner of the knee continue to be a complex problem for orthopedic surgeons. Early recognition and treatment are important factors in the patient's long-term outcome. To properly treat these patients, the surgeon must have a clear understanding of the anatomic relationships amongst the structures in the posterolateral knee. This knowledge combined with a thorough physical examination and imaging studies, allows the surgeon to make the correct diagnosis and devise an appropriate treatment plan. This article will discuss the anatomy, diagnosis, and treatment options to improve the surgeon's understanding of posterolateral knee injuries. The senior author's technique for anatomic reconstruction of the posterolateral corner of the knee and the rehabilitation protocol are described.

Anatomic Single-Graft Anterior Cruciate Ligament Reconstruction Restores Rotational Stability: A Robotic Study in Cadaveric Knees

Abstract:Purpose: First, we aimed to investigate the ability of a single bone-patellar tendon-bone graft placed in the anatomic center of the femoral and tibial attachment sites to restore normal tibiofemoral compartment translations and tibial rotation. Second, we aimed to investigate what combination of anterior load and internal rotation torque applied during a pivot-shift test produces maximal anterior tibiofemoral subluxations. Methods: We used a 6-df robotic simulator to test 10 fresh-frozen cadaveric specimens under anterior cruciate ligament (ACL)-intact, ACL-sectioned, and ACL-reconstructed conditions measuring anterior translations of the medial, central, and lateral tibiofemoral compartments and degrees of tibial rotation. Specimens were loaded under Lachman, anterior limit, and internal rotation conditions, as well as 3 different pivot-shift conditions. Results: On ACL sectioning, compartment translations in the Lachman and 3 pivot-shift tests increased significantly and were restored to ACL-intact values after single-graft ACL reconstruction. In the pivot-shift tests, the single graft restored lateral and medial compartment translations (e.g., group 3, within 1.3 ± 0.6 mm and 0.8 ± 0.6 mm, respectively, of the ACL-intact state and internal rotation within 0.7° ± 1.2°). Anterior subluxation of the medial compartment during pivot-shift loading was reduced when internal rotation torque was increased from 1 to 5 Nm (P < .0001). Conclusions: A single-graft ACL reconstruction performed at the central femoral and tibial ACL attachment sites restored anterior-posterior translation and tibial rotation motion limits. In addition, rotational knee stability as defined by tibiofemoral compartment translations was restored under all simulated pivot-shift testing conditions. Clinical relevance: This study provides in vitro evidence to support the clinical use of single-graft ACL reconstructions in restoring tibiofemoral compartment translations. It also shows the advantage of describing ACL insufficiency in terms of medial and lateral compartment subluxations as compared with the common approach of describing changes in central tibial translations and rotations.
Effects of Anterolateral Structure Augmentation on the In Vivo Kinematics of Anterior Cruciate Ligament–Reconstructed Knees

Abstract:Background: Double-bundle anterior cruciate ligament (ACL) reconstruction (ACLR) is a well-known treatment that restores the stability of ACL-deficient knees. However, some isolated ACL-reconstructed knees ultimately show rotatory laxity and develop osteoarthritis. Whether combined ACLR with anterolateral structure (ALS) augmentation (ALSA) can provide better improvement in the in vivo knee rotational kinematics remains unknown. Hypothesis: When compared with isolated double-bundle ACLR, combined double-bundle ACLR with ALSA can improve knee in vivo rotational kinematics and provide better restoration of knee kinematics. Study Design: Controlled laboratory study. Methods: Sixteen patients with unilateral ACL injury were randomly divided into 2 groups to receive either combined double-bundle ACLR and ALSA (ALSA group) or isolated double-bundle ACLR (ACLR group). All patients performed a single-leg lunge using the operative and nonoperative/contralateral legs under dual-fluoroscopic imaging system surveillance during a hospital visit at a minimum 1 year (12-13 months) of follow-up to assess the 6 degrees of freedom knee kinematics. Functional evaluation using the Lysholm and Marx rating scales and clinical examinations were also performed. Results: From full extension to approximately 90° of knee flexion at 5° intervals, the mean ± SD internal rotation of the reconstructed knees in the ALSA group (1.5°± 0.9°) was significantly smaller than that of the contralateral knees (8.2°± 1.9°; P = .008). The ALSA group knees also showed significantly ( P = .045) more medial translation than the contralateral knees. In the ACLR group, the mean internal rotation of the reconstructed knee (6.0°± 2.1°) was significantly smaller than that of the contralateral knees (8.9°± 0.6°; P < .001). At full extension, the tibia was significantly more externally rotated than that of the contralateral legs (0.5°± 7.4° vs 7.6°± 3.4°, P = .049). Conclusion: When compared with isolated double-bundle ACLR, double-bundle ACLR augmented with ALS reconstruction resulted in anterolateral rotatory overconstraint during the lunge motion. Clinical Relevance: Additional ALSA of double-bundle ACL-reconstructed knees overconstrained rotatory stability. Therefore, the use of ALSA for ACL-reconstructed knees should be considered with caution for patients with ACL deficiency and anterolateral rotatory instability. Longer-term follow-up to evaluate long-term outcomes and altered kinematics over time is recommended.
