Abstract:Background Total two-stage exchange is commonly used in clinical practice as a treatment for infected total hip arthroplasty (THA); however, this approach involves considerable limitations, including significant bone loss and severe trauma. This retrospective cohort study was conducted to evaluate clinical outcomes following the use of partial two-stage exchange (PTE) for infected THA. Methods We performed a retrospective analysis of 28 patients with infected THA who were treated by PTE between September 2000 and June 2019. Eligibility for PTE was limited to patients with a well-fixed femoral stem prosthesis. In the first stage of the operation, the femoral stem prosthesis was preserved; subsequently, the acetabular prosthesis, liner, and head were replaced with an antibiotic-loaded spacer. The new prosthesis was then implanted into patients and monitored for at least 3 months to ensure freedom from infection. Results Patients were followed for an average of 4 years (range, 2–11 years), with an overall success rate of 85.7% (24/28). The mean Harris hip score at the final follow-up was 76.2 ± 11.7 points. Conclusions The findings of this study suggest that PTE could be an acceptable option for a subset of patients with infected THA, offering a satisfactory infection control rate and clinical outcomes comparable to those of total two-stage exchange, but with less harm.
Partial Two?stage Exchange for Periprosthetic Joint Infection after Total Hip Arthroplasty

Abstract:Objective To evaluate the early stage effects of partial two?stage exchange (PTE) for infection after total hip arthroplasty (THA) in order to improve surgical treatment outcomes. Methods Twelve patients (7 males and 5 females) of infect?ed THA treated by PTE between September 2000 and February 2016 were included with an average of 59 years old (range, 40-74 years old). The femoral stem prosthesis was preserved when we ensured that it was not infected and with well fixation during opera?tion. The acetabular prosthesis was totally replaced. Furthermore, the secondary surgery was performed on patients who were free from infection for at least 3 months. At the follow?up duration, the pelvic X?ray was examined to access the presence of loosening of the prosthesis. The inflammatory index was recorded. Hip function was assessed by the Harris hip score, and the visual analogue score (VAS) and patient's subjective satisfaction were recorded by the questionnaire. Results All of the 12 patients were fol?lowed up for a mean of 33.3 months (range, 24-48 months). We achieved a 92% (11/12) success rate. One patient resuffered infec?tion, which was controlled after the second debridement. The preoperative Harris score of 12 patients was 23-57, with an average of 40.83±10.62. All patients had significant improvement in hip function after PTE. The Harris score reached 59.58±4.34 at one month after surgery, and reached 64.58±9.08 after three months and 86.75±4.58 at the last follow?up. There was significantly dif?ferent when compared with the preoperative Harris score. At the last follow?up, the length difference between the lower limbs was less than 1cm in all patients. Two patients did not complain of pain, while other 10 patients got a VAS score of less than 2 points. The patient's subjective satisfaction rating was 100%. Conclusion PTE could be a valuable alternative treatment for infection af?ter THA. The infection control rate and clinical outcomes are comparable to those of two?stage exchange.
Partial Component-Retained 2-Stage Reconstruction in the Treatment of Infected Hip Arthroplasty

Abstract:Background: It is considered the gold standard treatment for infected hip arthroplasty to remove and reimplant the corresponding whole set of implant components before and after infection control, but it usually causes substantial bone loss to remove the well-fixed cup or stem, which may increase the difficulty in reconstruction. We would like to determine whether infected hip arthroplasty can be treated without removal of a well-fixed cup or stem. Methods: Patients with infected hip arthroplasty and a radiographically well-fixed, cementless cup or stem were selected. During the first surgical stage, we retained the stem or cup if these cannot be removed using a stem or cup extractor. We performed the reimplantation surgery after control of infection. Results: From January 2008 to December 2016, 26 patients underwent partial component-retained 2-stage reconstruction. All the patients were free of infection with a mean follow-up time of 43.85 months. Conclusion: Partial component-retained 2-stage reconstruction may be a treatment option for infected total hip arthroplasty with a well-fixed component in patients. (C) 2019 Elsevier Inc. Management of the infected total hip or knee arthroplasty with two-staged revisions

Abstract:Objective To present our experience for the management of the infected hip and knee arthroplasty with two-staged revision.Methods Between June 2008 and Aug 2010 the replacement of the infected arthroplasty in two stages was performed in 14 patients.The mean follow-up time after revision was 16.6 months.Results One of 14 patients had a recurrence of the infection.One case had femoral fracture.The revision increased the Harris Hip Score from 48.9 points preoperative to 82.9(44～95) points postoperative,and the postoperative AKS averaged 84.25 points and increased about 41.75 points more than preoperative.Conclusions Two-stage reconstruction of the infected hip and knee is useful and effective because of higher rate of eradication of the infection and good postoperative functional results in treating infected THA or TKA.
Two-stage Cementless Total Hip Arthroplasty for Chronic Infection after the Internal Fixation of the Proximal Femoral Fracture

Abstract:Objective To retrospectively analyze the short-term clinical effects of the two-stage cementless total hip arthroplasty in the treatment of chronic infection after the internal fixation of the adult proximal femoral fracture.Methods From June 2006 to June 2011,9 patients who had suffered a chronic infection after an internal fixation of the proximal femoral fracture,including 8 males and 1 female,aged from 31 to 74 years (average,52.6 years),were treated with two-stage cementless total hip arthroplasty.There were 3 cases of femoral neck fracture,and 6 cases of intertrochanteric fracture.In the first stage,after surgical debridement and thorough removal of all the implants,an antibiotic-loaded cement spacer was implanted.All patients postoperatively underwent intravenous and oral antibiotics,and the erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) were tested.When the values of ESR and CRP become normal,the second stage operation using cementless components was performed.Results The presence of infection was confirmed by a positive culture of joint fluid obtained intraoperatively in two cases;one case was staphylococcus aureus and another one was staphylococcus epidermidis.The second stage arthroplasty was performed 6-14 months (average,9 months) after the first stage operation.All 9 patients were followed up for an average of 23.2 months (range,6 to 50 months).No recurrent infection,component loosening,bone osteolysis and cement spacer or component dislocation occurred.The Harris score improved from 74.6 (range,64 to 86) before the first stage operation to 90.9 (range,86 to 97) at the final follow-up.Conclusion The two-stage cementless total hip arthroplasty is an effective strategy for the treatment of chronic infection after the internal fixation of the adult proximal femoral fracture.
Treatment of Infection after Hip Arthroplasty Using Antibiotic-Impregnated Cement Spacer by Two-Stage Exchange Arthroplasty

Abstract:Objective To assess the effects of antibiotic-impregnated cement spacer on the management of infection after hip arthroplasty.Methods The replacemennt of the infected total hip arthroplasty in two stages was performed in 14 patients.In the first stage operation,the following steps were performed: complete debridement,removal of infected prothesis,addition of 1 g of Vancomycin to 20 g bone cement forming Vancomycin-impregnated cement spacer.After(3.6±1.2) months,antibiotic-loaded prosthesis was implanted in the second stage.The hip functions before and after operation were evaluated by Harris score.Results After the first procedure,between 7~29 months,all the wounds were closed smoothly and the mean follow-up after revision was(16±5.9) months.None of 14 patients had a recurrence of the infection.There were no cases of dislocation,deep-vein thrombosis,heterotopic ossification during the follow-up.The mean postoperative Harris Hip Score averaged 81±6.8(72~91) points and increased about 42±1.7 points than preoperative score averaged 39±8.1(22~51).In this group,2 cases were excellent,10 cases were good,2 cases were fair.Conclusions The effective treatment of infection after hip arthroplasty are complete debridement,using antimicrobial agents-loaded cement spacer with two-stage revision.
Prosthesis Retention Therapy for Postoperative Infection After Hip or Knee Arthroplasty

Abstract:Objective To probe indications and cautions of prosthesis retention therapy for postoperative infection after hip or knee arthroplasty.Methods A retrospective study was conducted by collecting data from 7 patients between 2004 and 2009 for postarthroplastic infection and treated with prosthesis retention subsequent to routine debridement and antibiotics treatment.Out of the 7 patients,3 received hip arthroplasty while the others underwent knee arthroplasty.By describing the courses from diagnosis and preoperative assessment to implementation of proposed interventions,the factors responsible for success and failure were analyzed.Results Five cases gained favorable outcomes and lived without reinfection at a mean follow-up for 5~72 months,24.8 months on average;while the other 2 cases received two-stage re-implantation for failure of retention treatment were with no evidence of reinfcetion at a mean follow-up for 4.75 years on average.Conclusion Prosthesis retention combined with debridenment and antibiotics has certain practical values in treating patients with hip or knee postarthroplastic infection.However,outcomes of this strategy mainly rely on mastery of indications,therapeutic time and specific cautions.
Comparable Results of Single and Two-Stage Exchange for Select Periprosthetic Hip and Knee Infection

Abstract:Introduction: Although two-stage exchange has been the standard of care for periprosthetic joint infection (PJI) in the United States, single-stage exchange is emerging as an option in select patients. The purpose of this study was to compare outcomes of patients undergoing single-stage and two-stage exchange using strict surgical indications. Methods: We reviewed a consecutive series of 196 patients with diagnosed PJI undergoing revision total knee and hip arthroplasty from 2017 to 2021. Patients were excluded if they had PJI history, plastic surgery coverage, or extensive bone loss requiring endoprosthesis. We compared the number of patients PJI-free at 1-year follow-up using MusculoSkeletal Infection Society criteria and patients requiring re-revision between the single-stage and two-stage groups. Results: In total, 126 patients met inclusion criteria. Of 61 knee patients (48.4%), 22 underwent single-stage (36%) and 39 underwent two-stage (63.9%). Of 65 hip patients (51.6%), 38 underwent single-stage (58.5%) and 27 underwent two-stage (41.5%). At a mean follow-up of 1.95 ± 0.88 years, a higher rate of knee patients were classified as having treatment success in the single-stage group (77.3% versus 69.2%, P = 0.501), however with comparable septic failure rates (18.1% single-stage versus 17.9% two-stage; P = 0.982). At a mean follow-up of 1.81 ± 0.9 years, a higher rate of hip patients were classified as having treatment success in the single-stage group (94.7% versus 81.5%, P = 0.089), and more patients had septic failures in the two-stage group (18.5% versus 5.3%; P = 0.089). No differences were observed in the microorganism profile. More total complications (P = 0.021) and mortalities were found in the single-stage knee cohort than in the two-stage cohort (22.7% versus 2.6%; P = 0.011). Conclusion: Single-stage arthroplasty is a viable alternative to standard two-stage exchange in patients with PJI without a history of infection and with no bone or soft-tissue compromise. Additional studies with longer term follow-up are needed to evaluate its efficacy.
[Cementless Two-Staged Total Hip Arthroplasty for Chronic Periprosthetic Infection].

Abstract:OBJECTIVE:To determine the clinical outcomes of two-staged cementless revision arthroplasty for the treatment of deep periprosthetic infection after total hip arthroplasty.METHODS:Twenty-three patients with deep periprosthetic infection treated with a standard protocol of two-staged cementless revision hip arthroplasty were enrolled in this study. There were 9 male patients and 14 female patients with an average age of 64 years (range, 52-78 years). In all cases, antibiotics-loaded cement spacers were implanted after removal of all the prosthetic components and thorough debridements had been done. All patients had a minimum of 2 weeks of intravenous antibiotics followed by 4 weeks of oral antibiotics after implant removal. After a mean interval of 6.7 months (3-28 months), revision arthroplasties were carried out with cementless femoral components followed by 2 weeks of intravenous antibiotics and 4 weeks of oral antibiotics.RESULTS:The mean follow-up period was (4.3±3.5) years. There were 2 cases of recurrent infections in this study. Intraoperative periprosthetic fractures were observed in 3 patients. One patient had dislocation of the implanted spacer during the interval period and 2 patients had hip dislocation after reimplantation. Mild subsidence of femoral component occurred in 1 patient. There were no cases of loosening of femoral components and cementless acetabular components in patients without infection recurrence. The Harris hip score increased from a preoperative mean of 36±13 to 85±13 at 12 months after reimplantation.CONCLUSIONS:Using cementless prostheses in two-staged revisions of hip periprosthetic infections can provide low rate of infection recurrence and good implant stability, but cautions must be taken when treating patients with infection caused by multidrug-resistant organisms.
Extensively Coated Non‐modular Stem Used in Two‐stage Revision for Infected Total Hip Arthroplasty: Mid‐term to Long‐term Follow‐up

Abstract:Objective: To determine the rate of curing the infection and mid-to long-term outcomes of using extensively coated non-modular stems in two-stage revision for infected total hip arthroplasty (THA).Methods: The clinical data of 33 patients (33 hips) in whom extensively coated non-modular stems had been used in two-stage revision THA for deep infection were retrospectively analyzed. All operations received two-stage reimplantation, which included resection arthroplasty, thorough debridement, insertion of a hand-molded antibiotic-impregnated cement spacer with stainless steel reinforcement, a course of intravenous antibiotics, and delayed reimplantation. Microorganism-specific antibiotics had been chosen according to the results of microbiological studies performed postoperatively. All patients received i.v. antimicrobial therapy for 4 weeks and oral antibiotics to which their organisms were sensitive for a further 6 weeks. Harris hip score (HHS) and plain X-ray films were used to perform clinical and radiological evaluations.Results: During follow-up for a minimum of 5 years, no reinfection or loosening were found. Cultures of samples taken during the second stage were all negative for infection. The mean HHS improved from 42 preoperative to 89 at the final follow-up. All granular bones had fused well with the host bones by 12 months after the surgery.Conclusion: Using extensively coated non-modular stems combined with intramedullary allografts in two-stage revision for treating infected THAs can achieve satisfactory outcomes.
Clinical Outcome of Extended Trochanteric Osteotomy in Two-Stage Revision of Periprosthetic Hip Infection

Abstract:Objective To investigate the efficacy of using an extended trochanteric osteotomy as part of a two-stage exchange procedure for periprosthetic hip infection.Methods We retrospectively analyzed 16 patients (16 hips) including 9 males and 7 females who had extended trochanteric osteotomy as part of two-stage revision procedure for periprosthetic hip infection from January 2008 to January 2012,the average age was 65.2±6.4 years at the time of operation.The two-stage revision protocol included taking out of all implants using extended trochanteric osteotomy,meticulous debridement,restoration of the osteotomies and fixed with cerclage wiring,implantation of antibiotic bone cement spacer in first stage.After sequential therapy of antibiotics through peripheral vein and enteral prescription,the prosthesis was implanted in the second stage.Harris hip score was used to evaluate the hip function.X-ray radiograph was conducted to evaluate the healing of osteotomy,eradication of infection,prosthesis stability,displacement and looseness.Results All patients were followed-up for 24-90 months (53.4±7.2 months on the average).The interval between the first and second surgery was 8.6±2.1 months witha range from 5-18 months.All osteotomies healed uneventfully,with the average heal time 13.2± 1.3 weeks (range from 8 to 16 weeks).The infection was eradicated in 15 hips (93.8％) at the second surgery.One hip with staphylococcus epidermidis infection was confirmed by bacterial culture of synovial fluid in second surgery.Infection symptom disappeared after sequential antibiotic therapy.At the last follow-up,all patients had successful eradication of infection,with Harris score improved from 38.6 ± 5.4 preoperatively to 86.4 ± 3.2.Thirteen patients achieved bone ingrowths,while the other 3 patients had fibrous ingrowths.Two cases had intraoperative fracture,2 had postoperative spacer dislocation.Conclusion Extended trochanteric osteotomy as part of a two-stage exchange arthroplasty can be performed safely and effectively in periprosthetic hip infection,with reliable osteotomy healing.Furthermore,it does not influence the effects of infection treatment and component stability.
Total Hip Arthroplasty for Patients with Osteoarthritis Secondary to Hip Pyogenic Infection.

Abstract:Background Pyogenic hip arthritis occurs most often in young patients. Delayed treatment causes significant anatomical deformation of bony and soft tissue structures leading to premature onset of secondary osteoarthritis. Total hip arthroplasty (THA) in patients who had osteoarthritis secondary to hip pyogenic infection has been associated with high complication rates. Methods We analyzed 19 THAs performed from April 2003 to July 2008 in adults with osteoarthritis secondary to hip pyogenic infection (average age 40.7 years; range 34-52 years). There were 7 males and 12 females, the average age of infection was 10.6 years (range 7-13 years) and the average quiescent period of infection was 29.5 years (range 22-41 years). The count of white blood cell (WBC), erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) were examined routinely before surgery. The duration of followup was 34 months (range 6-52 months). Results There was no evidence of infection according to laboratory values, intraoperative exploration and bacterial culture of joint fluids and synovial tissues. Split fracture above the lesser trochanter occurred in two hips and healed without adverse sequelae after fixation with wires. One patient with sciatic nerve palsy was successfully treated by positioning the hip in extension and the knee in flexion with complete resolution of motor symptoms one month later. There were no cases of dislocation, deep vein thrombosis, or postoperative reinfection. The symptoms and hip joint activities were significantly improved. The median Harris hip score improved from 47.3 preoperatively to 89.7 and the median range of motion from 53 degrees to 125 degrees. Conclusions It is safe and efficient to perform THA in patients who had osteoarthritis secondary to pyogenic hip arthritis when the infection is quiescent. The key points of successful surgery are exclusion of active infection preoperatively, quiescent period of infection more than ten years and adequate intraoperative soft tissue releases.
Treatment of Comminuted Trochanteric Fractures and Non-Union of Trochanteric Osteotomy in Revision Total Hip Arthroplasty.

Abstract:OBJECTIVE:To explore the clinical efficacy of the treatment of comminuted trochanteric fractures and trochanteric osteotomy non-union in revision total hip arthroplasty with tension-band fixation.METHODS:A retrospective review of 295 revision total hip operations performed between 1992 and 1998 was undertaken. Twenty hips of 19 patients with comminuted fractures or nonunion of the greater trochanter were stabilized with tension band technique. Multiple 2.0 mm k-wires and tension-band wires were placed through the intact cortex distally and the abductor tendon proximally in the pattern "8".RESULTS:The average follow-up was 30 months. The Harris Hip Score improved on average from 45 preoperatively to 89 at follow-up. Sixteen hips with intra-operative trochanteric fracture through osteolytic bone and four hips with symptomatic trochanteric nonunion were approached with tension band fixation. Perioperative loss of fixation in one patient required a repeated surgery. The same fixation at the second operation achieved an uneventful healing. Two patients had a 2 cm proximal migration of one K-wire without loss of bony fixation. The trochanteric fractures healed with no further proximal wire migration. One patient had loss of fixation with trochanteric escape at 6 weeks post-operatively. The patient has abductor weakness with Trendleburg limp but without pain. On average, radiographic examination showed that healing occurred at 16.6 weeks postoperatively. Six patients developed grade 1 heterotopic ossification and two patients grade 3. All were asymptomatic. None of the 19 patients experienced a dislocation during the follow-up.CONCLUSIONS:Tension-band fixation for greater trochanter can enhance the success rate of revision total hip arthroplasty without a deficient abductor mechanism.
Treatment of Postprosthesis Infection by Revision Total Hip Arthroplasty

Abstract:Objective: To evaluate efficacy of revision arthroplasty in dealing with postprosthetic infection after total hip arthroplasty (THA). Methods: Seven cases of postprosthetic infection underwent revision arthroplasty from 1998 to 2002 in the hospital, 3 out of which were two-stage revision. The follow-up duration averaged to 14 months. Results: The patients achieved 37. 6 in average Harris score and no recurrence was found in follow-up. Conclusion: One-stage or two-stage total hip revision is useful in dealing with postprothetic infection on the basis of thorough debridement and administration of sensitive antibodies.
Complications in the treatment of prosthetic joint infection

Abstract:Aims: Two-stage exchange arthroplasty is the most common definitive treatment for prosthetic joint infection (PJI) in the USA. Complications that occur during treatment are often not considered. The purpose of this study was to analyze complications in patients undergoing two-stage exchange for infected total knee arthroplasty (TKA) and determine when they occur. Methods: We analyzed all patients that underwent two-stage exchange arthroplasty for treatment of PJI of the knee from January 2010 to December 2018 at a single institution. We categorized complications as medical versus surgical. The intervals for complications were divided into: interstage; early post-reimplantation (three months); and late post-reimplantation (three months to minimum one year). Minimum follow-up was one year. In total, 134 patients underwent a first stage of a two-stage exchange. There were 69 males and 65 females with an mean age at first stage surgery of 67 years (37 to 89). Success was based on the new Musculoskeletal Infection Society (MSIS) definition of success reporting. Results: Overall, 70 (52%) patients experienced a complication during the planned two-stage treatment, 36 patients (27%) experienced a medical complication and 47 (41%) patients experienced a surgical complication. There was an 18% mortality rate (24/134) at a mean of 3.7 years (0.09 to 8.3). During the inter-stage period, 28% (37/134) of patients experienced a total of 50 complications at a median of 47 days (interquartile range (IQR) 18 to 139). Of these 50 complications, 22 were medical and 28 required surgery. During this inter-stage period, four patients died (3%) and an additional five patients (4%) failed to progress to the second stage. While 93% of patients (125/134) were reimplanted, only 56% (77/134) of the patients were successfully treated without antibiotic suppression (36%, 28/77) or with antibiotic suppression (19%, 15/77) at one year. Conclusion: Reported rates of success of two stage exchanges for PJI have not traditionally considered complications in the definition of success. In our series, significant numbers of patients experienced complications, more often after reimplantation, highlighting the morbidity of this method of treatment. Optimal reimplantation timing in two-stage exchange for periprosthetic joint infection: an observative cohort study in Asian population

Abstract:Abstract Background The optimal timing for reimplantation for periprosthetic joint infection (PJI) has not been established and varies from a few weeks to several months. The aim of this study was to assess the commendable time between implant removal and reimplantation in patients who underwent two-stage exchange arthroplasty for PJI. Methods We retrospectively reviewed 361 patients who were treated with two-stage exchange arthroplasty for hip and knee chronic PJI at our institution between January 2000 and December 2018. Patient characteristics, comorbidities, surgical variables, microbiology data, and time to reimplantation were recorded. All patients were followed for a minimum of one year. Treatment failure was defined by Delphi criteria. Logistic regression analyses were used to calculate survival rates and adjusted odds ratios (ORs) of treatment failure. Results In final analysis, 27 (7.5%) had treatment failure. Factors related to treatment failure including interim spacer exchange (OR, 3.13; confidence interval (CI), 1.04–9.09, p = 0.036), higher ESR level at reimplantation (OR, 1.85; CI, 1.05–3.57; p = 0.04), and time to reimplantation (OR, 1.00; CI, 1.003–1.005, p = 0.04). Performing revision arthroplasty surgery from 16 to 20 weeks had highest successful rate. The reimplantation over 24 weeks had a lower successful rate. However, no statistical significance in comparing each interval group. Conclusion Our study emphasized the importance of timely reimplantation in achieving successful outcomes. Factors such as ESR levels, spacer exchange, and the duration of time to reimplantation influenced the likelihood of treatment failure in two-stage exchange arthroplasty for hip and knee PJI.

Abstract:Objective To investigate the clinical effect of peri-prosthetic infection treatment of two-stage exchange without the use of the spacers.Methods Twenty-one patients with hip peri-prosthetic infection following hip replacement,who underwent the spacer-free two-stage revision in the Second Affiliated Hospital of Xi'an Jiao Tong University from August 2009 to December 2017,were analyzed.These patients included six males and 15 females,aged from 28 to 76 years.In the first stage,these patients were treated by surgical removal of all components without spacers.When the clinical evaluation and serum inflammatory markers suggested the infection had been resolved, the second stage of debridement and new prosthesis replacement was performed.The effects were evaluated with Harris hip score and limb shortening via paired t-test and Wilcoxon rank sum test.Results All the patients were followed up for(4.0 ±2.8)years on average.The hip Harris score(86.4 ±4.4)at the last follow-up was significantly higher than the score(48.4 ± 4.9)before two-stage revision(t=25.509,P<0.01).The preoperative length of the affected limb was 4.3 (3.85,4.90)cm shorter than that of the healthy side, while it was 0.1cm(-0.2,0.35)cm(negative value represented the increased length of the affected side compared with the healthy side)after the two-stage revision,and the difference was significant(Z=-4.016,P <0.01).One patient was infected after the operation.Conclusion Two-stagerevision without spacer has a high infection control rate in the treatment of periprosthetic hip infection,and does not result in significant shortening of the limb due to peripheral tissue contracture of the hip;the patients could get better hip function.

Abstract:Objective To determine medium-term curative effect of extensively coated non-modular femoral stems in twostage infection revision after joint replacement.Methods We retrospectively analyzed 33 patients (33 hips) who had a two-stage infection revision after total hip replacement arthroplasty from March 2005 to December 2006.Among of them,there were 20 men (20 hips) and 13 women (13 hips).Their mean age was 66.1±7.2 years with a range from 52 to 80.The two-stage revision protocol consisted of joint prosthesis taken out in the first stage,thorough debridement,hand-molded antibiotic bone cement spacer,and two-stage joint revision after anti-infective therapy.Anti-infective therapy included intravenous antimicrobial therapy for 4 weeks and oral antibiotics for 6 weeks.Harris hip score (HHS) was used to evaluate functions of hip joint.X-ray films were used to perform clinical and radiological evaluations of prosthesis stability,displacement and looseness.Reinfection criteria were the same as the criteria used to diagnose the joint infection.Results Follow-up visit was carried out for all patients for 5-9 years (6.1±1.7 years on the average).No infection or prosthesis looseness happened after two-stage revision.Microculture of samples taken during two-stage revision or pathological examination was negative.Harris hip joint score rose to 89.1 ±8.5 in the last follow-up visit from preoperative 42.3±6.5.30 patients achieved stable bone ingrowths and the remaining three had stable fibrous ingrowths.Impacted bone graft at acetabulum side for 11 patients and all impacted bone graft in marrow cavity were fused well with the host bones.Allogeneic cortical bone plate grafts were used in 10 hips on the femoral side.Nine of these cortical bone plates fused with the host bones after postoperative 1 year,whereas one fused after 2 years.Conclusion Using extensively coated non-modular femoral stems combined with intramedullary allograft in two-stage revision for treating infection can achieve satisfactory medium-term survival rate of prosthesis and infection clearance rate.

Abstract:Background: The effect of extended trochanteric osteotomy (ETO) with immediate cerclage wire fixation on the scene of chronic periprosthetic joint infection on infection eradication and the osteotomy healing remains unclear. Methods: One hundred seventeen patients who underwent two-stage revision total hip arthroplasty for chronic periprosthetic joint infection were enrolled in the study. The minimum follow-up was 2 years. Of these, 48 patients had underwent ETO and immediate cerclage wire fixation during the first-stage surgery, while 69 did not undergo ETO or any other osteotomy. In addition, 18 patients had underwent ETO in the second-stage reimplantation surgery. Repeated debridement, curative rate of infection, and complications were compared between the 2 groups. Meanwhile, osteotomy healing between the first and second osteotomy were also compared. Results: The repeated debridement rate and curative rate of infection were 2.1% and 95.8% in the ETO group, compared with 13.0% and 82.6% in the non-ETO group; there were significant difference between the 2 groups in these parameters (P=.037 and .030, respectively). Meanwhile, osteotomy healing rate was 93.8% in first-stage surgery with infection scene and 100% in second-stage surgery without infection; no significant difference was detected in osteotomy healing rate and time to healing (P=.278 and .803, respectively). The rate of complication was similar in both groups. Conclusions: ETO combined with immediate cerclage wire fixation does not appear to reduce the rate of infection eradication. Extraction Techniques for Well-fixed, Infected Hip Arthroplasty Implants

Abstract:Removal of well-fixed total hip arthroplasty components is a fundamental necessity to cure chronic prosthetic hip joint infection. However, the benefits of infection eradication must be weighed against the potential morbidity associated with implant removal. Preoperative identification of implant type, knowledge of its design characteristics, availability of specialized instrumentation and knowledge of specific surgical techniques are key elements of success. With proper preoperative planning and surgical technique, removal of well-fixed total hip arthroplasty components can be accomplished safely and can provide improvements in quality of life for patients suffering from the symptoms associated with prosthetic hip joint infection.
