Abstract:Pancreatic cancer is an invasive malignant tumor with a high mortality rate. As the preferred imaging method for pancreatic imaging, multi-slice CT angiography can evaluate the presence or absence, extent, and degree of invasion of peripancreatic major vessels to provide reliable evidence for tumor staging and surgical resectability evaluation. As an important supplementary method of CT, magnetic resonance imaging has an important guiding value in disease diagnosis, liver metastasis, and prognostic evaluation. Vascular invasion of pancreatic cancer is the main indicator for preoperative evaluation of respectability. Lymph node metastasis, distant metastasis, neural invasion of pancreatic cancer, and hepatic artery abnormalities are influencing factors for patients′ prognosis. It is pointed out that preoperative imaging evaluation for patients with pancreatic cancer has guiding significance for the development of treatment regimens, selection of surgical procedures, and prognostic evaluation.

Abstract:Objective To investigate the application value of multi-slice spiral CT urography(MSCTU) in the diagnosis of upper urinary tract obstructive disorder.Methods 20 patients with upper urinary tract obstructive disorder treated from July 2008 to June 2012 were performed MSCTU scan.The primary data were performed the three dimensional reconstructions for the comparison with the results of X-ray,B-type ultrasound and IVU.Results 5 cases were developed at 1 h,1 case was postponed to develop at 2 h,3 cases had undeveloped one side and other 11 cases were normally developed.Conclusion MSCTU has the high sensitivity and high specificity,and can clearly display the upper urinary tract lesions,congenital variations and deformities.

Abstract:BACKGROUND:To compare the differences in the diagnosis of retroperitoneal tumors among multi-slice spiral computed tomography (MSCT), magnetic resonance imaging (MRI), and ultrasound (US).METHODS:Sixty cases of retroperitoneal tumors admitted in our hospital from January 2016 to January 2019 were collected and related data were analyzed. After admission, patients were examined by MSCT, MRI, and US, and the pathological results of the patients were used as the controls. The differences in the diagnosis of retroperitoneal tumors were compared with the results of MSCT, MRI, and US.RESULTS:Thirteen cases of benign tumors were diagnosed by MSCT, 47 cases were malignant, and 1 case was false benign, with diagnosis accuracy, sensitivity and specificity of 98.33%, 97.92% and 92.30%, respectively. Thirteen cases of benign tumors were diagnosed by MRI, 47 cases of malignant tumors, and 1 case was false benign, with diagnosis accuracy, sensitivity and specificity of 98.33%, 97.92%, and 92.30%, respectively. Fourteen cases of benign tumor were diagnosed by US, 46 cases were malignant, and 2 cases was false benign, with diagnosis accuracy, sensitivity and specificity of 96.67%, 97.92%, and 85.71%, respectively. There were no statistically significant differences in the accuracy, sensitivity, and specificity of MSCT, MRI, and US in the diagnosis of retroperitoneal tumors (P>0.05).CONCLUSIONS:MSCT, MRI, and US tests are highly accurate, sensitive, and specific in the diagnosis of retroperitoneal tumors.

Abstract:With the development of computer medical imaging technology, three-dimensional technology has been gradually applied in medicine. We try to investigate the application value of 3D technology in the preoperative evaluation of retroperitoneal tumors. The clinical data of 70 retroperitoneal tumor cases admitted to the clinical center were retrospectively analyzed. The preoperative abdominal CT images were converted to 3D images using XMQB liver 2.0 software. These 3D images were imported into XMQB-I human organ model 3D printing equipment, and physical models facilitate preoperative evaluation and surgical strategy. The reestablished models were compared with the actual conditions during the operation. 3D imaging was completed in all the 70 cases, containing 24 cases in the left abdominal cavity, 26 cases in the right abdominal cavity, 5 cases in the pelvic cavity, and 15 cases in the multi-sites. Also, 3D printing was successfully carried out in 20 cases. The 3D imaging and 3D printing models could stereoscopically, clearly and objectively display the spatial relationships of the tumor with surrounding organs and vessels. The operation was successfully performed on all the 70 cases without death during the preoperative period. 3D technology could accurately evaluate the retroperitoneal tumors at different positions before the operation, showing a certain value in making the preoperative strategy.

Abstract:Objective To investigate the diagnostic value of multi-slice helical CT(MSCT) and the post processing techniques in colorectal cancer.Methods 42 patients with colorectal cancer underwent MSCT scanning.Multi-mode image reconstructions including multi-planar reformation(MPR),shaded surface display(SSD),Raysum and CT virtual colonoscopy(CTVC) were performed in the images post processing work station.The image data were compared with electronic colonoscopy(EC) and pathology.Results CTVC,MPR,SSD and Raysum could clearly show the location of masses,and help to observe the shape and character of masses.But there were different value among these post-processing techniques in showing the external contour of colon,the external and internal circumference of colon,and detail data of lesions such as shape,extent and infiltrating etc.Conclusion MSCT is a non-invasive,feasible,safety and accuracy method in detecting colorectal cancer,and playing an very helpful role in the clinical practice.

Abstract:Objective:To evaluate Multi-slice spiral computed tomography(MSCT)technique in diagnosing biliary obstructive diseases.Methods:Sixty-two cases confirmed with surgical pathology,endoscopic retrograde cholangiopancreatography(ERCP)and clinical follow-up,together with their MSCT post processed images were collected and analyzed and then were furthermore compared with correlative data. Results:Among 21 cases of malignancy included cholangiocarcinoma 6,gallbladder carcinoma with invasion or metastasis to biliary tract 6,bladder associated with biliary carcinoma 1,pancreatic head carcinoma involved biliary tract 2,and ampulla vater carcinoma 6.The other 41 benign cases included biliary tract calculi 32,cholangitis 8 consisted of association with calculi 4 and duodenal villus adenoma 1.The total accurate localization and qualitative rates for benign and malignant biliary obstructive diseases with MSCT were 98.39% and 83.87%,respectively.Those for benign and malignant ones were 97.56% vs.100% and 78.05%vs.95.24%,respectively.After comparison of evaluating the routine transverse enhanced and post processing image,it is evidently the later technique providing strengthened diagnostic confidence with supplementary information and correction of diagnosis.Conclusion:MSCT is effective for detecting and diagnosing biliary obstructive diseases especially the malignant ones with direct displaying the extent and grade of the lesions;and furthermore providing the prediction of evidence for clinical treatment and prognosis.

Clinical Analysis of 46 Cases of Primary Retroperitoneal Neoplasm

Abstract:Objective To investigate the diagnose and treatment of primary retroperitoneal neoplasm.Methods The clinical data of 46 patients treated for primary retroperitoneal neoplasm from 1990 to 2000 were analyzed retrospectively.Results Fifteen patients had benign tumor,the otheres had malignant tumor.Those patients who had malignant tumor had only suffered for 3 6 months ,the diameter of tumor was 13cm.The complete tumor resection rate was 51 6%, and 5-years surrival rate was 41%.While those patients with benign tumor had suffered for 16 months ,the diameter of tumor was 7cm,the rate of complete tumor resection was 91%.Conclusion It is very useful to assess the patients by BUS and CT before operation.Completely tumor resection is important for treatment.

Abstract:Pancreatic cancer is a highly malignant tumor and surgical resection is the only curative treatment option. In order to improve the prognosis of patients with pancreatic cancer, it is very important to diagnose and evaluate pancreatic cancer accurately and early. Imaging examinations play an important role in tumor detection, staging and surgical resectability evaluation, which can provide reliable evidences for diagnosis and treatment of pancreatic cancer. At present, the imaging techniques commonly used for diagnosis of pancreatic cancer include conventional ultrasound, endoscopic ultrasonography, PET-CT, multi-detector row CT (MDCT), magnetic resonance imaging(MRI),in which MDCT and MRI are the most widely used. In this article, the application and research progress of imaging in accurate diagnosis and evaluation of pancreatic cancer are reviewed.
Diagnosis and treatment of primary retroperitoneal tumors: an analysis of 30 cases

Abstract:Objective To evaluate the diagnosis and treatment of the primary retroperitoneal tumor based on experience with 30 cases. Methods The clinical manifestations,image examinations and operations of 30 cases with primary retroperitoneal tumor that were diagnosed and treated in our department from January 2007 to July 2009 were Retrospectively analyzed and discussed. Results Of the 30 cases,8 were male and 22 were female,the age range was 15~82 years,average 44.8 years.Nine of the cases were asymptomatic,7 had abdominal pain or fullness,4 had back pain,5 had abdominal mass,4 had lower extremity pain or numbness,and 1 case had body weight loss.All the 30 cases were treated with surgical operations,14 received laparoscopic surgery and the rest laparotomy.The average operation time was 205 minutes.There were no major complications in any of the patients.Twenty-seven cases were followed-up for an average of 18 months. Conclusion The image examinations are the main methods to detect the primary retroperitoneal tumors and the operations to remove the tumor are the main treatment.For benign or less malignant neoplasmas,laparoscopic surgery can be attempted.
Primary malignant tumours and malignant transformation of cysts in the retrorectal space: MRI diagnosis and treatment outcomes

Abstract:Background There are no clear guidelines for the diagnosis and treatment of malignant retrorectal tumours. The purpose of this study was to increase preoperative diagnostic knowledge and to describe the outcomes of treatment for these patients. Methods This retrospective study was conducted on patients who underwent complete retrorectal tumour resection between May 2006 and July 2018, and had confirmed post-operative pathology reports. Demographic and clinical data (including imaging, perioperative, pathological, and prognostic data) were collected and analysed. Results Malignant lesions were identified in 15 (9 [60%], female) patients. The median age of the patients was 59 years (range, 34-72 years). Primary malignant tumours were identified in seven patients with solid tumours, in which gastrointestinal stromal tumours accounted for 71.4% (five of seven) and the remainder were chordoma or mucinous adenocarcinoma. Malignant transformation of cysts occurred in another eight patients with heterogeneous tumours, while histopathological features were present in 75% (six of eight) of patients with mucinous adenocarcinoma, and the remainder were squamous-cell carcinoma or neuroendocrine tumour (Grade 2). The malignant characteristics of the solid portions observed using magnetic resonance imaging (MRI) were as follows: the cyst wall of the tumour was irregularly thickened; the surface was convex or lobed; the solid tumour had no capsule, or the capsule was destroyed; and the surface had a gyrus-like morphology. At a median follow-up time of 52 months (range, 13-100 months), the overall recurrence-free survival rate was 40.0% and the survival rate was 46.7%. Conclusion Some MRI features can be used to distinguish malignant retrorectal tumours from benign retrorectal tumours. The survival rate of patients with malignant retrorectal tumours is poor.
CT features in differential diagnosis of primary retroperitoneal tumors_

Abstract:To investigate the value of certain CT features in differentiating primary retroperitoneal tumors. Methods:Seventy-nine cases with primary retroperitoneal tumors who underwent MSCT examinations proven by surgery and pathology were studied.CT features were evaluated and compared between benign and malignant groups and different tumors,including the size,growth patterns,specific lesion components,as well as the patterns of enhancement.Results:Six-ty-seven cases (84.8%)were correctly localized.Thirty-two benign (76.2%)and 35 malignant (94.6%)lesions were cor-rectly diagnosed respectively.The signs denoting malignant neoplasms included the larger size,infiltrative growth,moderate and intensive enhancement,necrosis and cystic degeneration,and calcification.The sensitivity,specificity,positive predicitive value (PPV),and negative predicitive value (NPV)of serpiginous growth pattern in diagnozing lymphangioma and neuro-genic tumors were 75.0%,98.4%,92.3% and 93.9% respectively.The prensence of fat was a quite good predicitive sign of liposarcoma and teratoma,with sensitivity,specificity,PPV,and NPV of 85.0%,98.3%,94.4% and 95.1% respectively in this study.The sensitivity,specificity,PPV,and NPV of water density within the mass in diagnosing lymphangioma and ter-atoma were 80.0%,93.2%,80.0%,93.2% respectively.Complete cystic lesions were highly suggestive of lymphangioma, with the sensitivity,specificity,PPV,and NPV of 100%,98.6%,90.0% and 100%.Calcification is relatively sensitive for neurogenic tumors and teratomas,with the sensitivity,specificity,PPV and NPV of 51.2%,87.5%,80.0%,64.8% respec-tively.Extreme hypervascularity was sensitive to the diagnosis of paraganglioma,with the sensitivity,specificity,PPV and NPV of 77.8%,98.6%,87.5% and 97.2% respectively.Conclusion:Carefully analyzing CT features can be helpful to dif-ferentiate between benign and maligant retroperitoneal tumors.Certain characteristics can lead to a correct diagnosis of cer-tain tumors.

Abstract:Objective To explore the experience in treatment of primary retroperitoneal tumors (PRPT).Methods The clinical data of 62 cases with primary relroperitoneal tumors from 2001 to 2007were studied retrospectively.Results The abdominal mass was the main symptom in PRPT.There were 29 cases of benign tumors and 33 cases of malignanat tunlors respectively.Complete resections were performed in 47 patients(76%),partial resection was conducted for 10 patients.Resection could not be performed in 5 patients so only biopsy was conducted,12 patients accepted combined organ resections,2 patients died.Conclusion Adequate preoperative preparation and excellent surgical technique are the keys to compele tumour resection. Abstract:Objective:To summarize our experience on the diagnosis and surgical treatment of primary retroperitoneal tumor(PRPT).Methods:The clinical data of 315 patients with primary retroperitoneal tumor(from Jan.2000 to Jun.2008) were retrospectively analyzed,and the clinical experience on diagnosis and treatment was summarized.Results:The ratio of benign to malignant tumors in our group was 0.55:1.Totally 294 patients received operation,including 161 cases of radical resection,69 cases of combined resection,and 64 partial palliative resection.Twenty-one patients received no operation.The prognoses of patients who received radical resection for malignant tumors were significantly improved compared with those of patients who received partial palliative resection(P0.05).Conclusion:Imaging diagnosis can effectively identify PRPT.Complete resection is the key for treatment of PRPT,and reoperation is an effective treatment for recurrent PRPT.
[Comparison and Clinical Significance of Different Imageological Methods in the Detection of Transitional Carcinoma of Upper Urinary Tract: Analysis of 234 Cases].

Abstract:OBJECTIVE:To determine the diagnostic value of multislice CT urography (MSCTU) in patients with transitional cell carcinoma (TCC) of upper urinary tract by comparing other imageology methods used.METHODS:Two hundred and thirty four cases of transitional cell carcinoma of upper urinary tract, in which 82 cases were diagnosed pathologically with pelvic carcinoma and 152 cases with ureteral carcinoma, between June 2004 and September 2006 in our institute were enrolled in a retrospective study. Most of them underwent urological ultrasound, intravenous urogram (IVU), retrograde pyelography and MSCTU. We compared the positive rate (PR) and diagnostic rate (DR) of these methods used by chi-square test.RESULTS:Among the 234 cases, 215 patients underwent urologic ultrasound, in which 152 cases were detected to be abnormal, with the PR of 70.7%; Meanwhile, 58 cased were diagnosed by this examination, with the DR of 27.0%. IVU was performed in 193 patients and 132 cases were found to be abnormal, and the PR was 68.4%, 65 cases were diagnosed by IVU and the DR was 33.7%. And 132 patients underwent retrograde pyelography, by which 115 cases of lesion were detected, with the PR of 87.1%; In the meantime, 93 cases were diagnosed, with the DR of 70.5%. MSCTU was performed in 226 cases and 220 cases were found to be abnormal, and the PR was 97.3%; 214 cases were diagnosed by MSCTU, with the DR of 94.7%. The DR of detecting TCC of retrograde pyelography had statistically significant difference with that of ultrasound and IVU (P<0.001). As compared with retrograde pyelography, MSCTU had statistically significant superiority (P<0.001).CONCLUSION:To shorten the diagnosis time and mitigate the sufferings, patients with hematuria supposed to be TCC of upper urinary tract should be recommended to undergo MSCTU first.
The Preoperative Evaluation of Breast Cancer by Multi-Slice Helical CT Reconstruction

Abstract:ObjectiveTo evaluate the accuracy of multi-slice helical CT three reconstruction methods in preoperative evaluating and surgical planning of breast cancer.Methods40 cases breast cancer verified pathologically were examined by MSCT scanning with MPR, MIP and VR reconstructions before surgical treatment.Compared with pathological result, the imaging signs of breast tumors,surrounding structures and axillary lymph nodes were analyzed.ResultsAmong the 40 cases breast cancer,there were 23 cases manifested as irregular masses,17 cases manifested as round tumors with spiculated and segmental border,35 cases enhanced inhomogeneously, 5 cases showed nipple adherence,7 cases presented skin incrassation,3 cases displayed chest wall invasion,and 17 cases detected preoperatively the lymph node metastasis.(Conclusion)Based on the axial thin scanning,the three reconstruction methods of MSCT can accurately evaluate the extent and location of breast tumors, surrounding structures and axillary lymph nodes status,and it was very valuable in accurate preoperative diagnosis and surgical treatment.

Abstract:Background: Retroperitoneal fibrosis (RPF) and lymphoma presenting as retroperitoneal mass may closely resemble each other and misdiagnosis may occur. This study investigated the differential imaging features of RPF and lymphoma which presented as a retroperitoneal soft tissue using multidetector-row computed tomography (MDCT).Methods: The 42 consecutive patients were included in this retrospective review, including 19 RPF patients (45.2%; including 13 males and 6 females; mean age: 56.7 +/- 6.2 years) and 23 patients with lymphoma (54.8%; including 14 males and 9 females; mean age: 57.4 +/- 12.3 years). An array of qualitative computed tomography (CT) features of lesions in 42 consecutive patients with newly diagnosed untreated RPF and lymphoma were retrospectively analyzed. The quantitative size of the lesion at the para-aortic region and attenuation in the precontrast, arterial, and portal phases were calculated in regions of interest and compared between the patients with newly diagnosed untreated RPF and with lymphoma. Receiver operating characteristic curve analysis was used to assess the potential diagnostic value of each quantitative parameter. Inter-reader concordance was also calculated.Results: Mean ages between patients with RPF and lymphoma were not significantly different (56.7 +/- 6.2 years vs. 57.4 +/- 12.3 years P = 0.595). Compared to those in patients with lymphoma, homogeneous enhancement (65.2% vs. 94.7%, P = 0.027) and pelvic extension (52.2% vs. 89.5%, P = 0.017) were significantly more common while the involvement of additional nodes (78.3% vs. 5.3%, P < 0.001), suprarenal extension (60.9% vs. 15.8%, P = 0.004), and aortic displacement (43.5% vs. 5.3%, P = 0.006) were significantly less common in patients with RPF. Lesion size at the para-aorta was significantly greater in patients with lymphoma, compared with RPF patients (3.9 +/- 1.2 cm vs. 1.8 +/- 0.6 cm; P < 0.001). The attenuation values in three phases were not significantly different between patients with RPF and lymphoma. Inter-reader concordance for subjective features ranged from very good to excellent (range: 85.7-100.0%).Conclusions: This study showed that MDCT can help differentiate between untreated RPF and lymphoma on the basis of qualitative CT features and lesion sizes. Differentiating RPF from lymphoma on the basis of attenuation values in the precontrast, arterial, and portal phases was difficult to accomplish.
