Abstract:Background The relative rarity and anatomical position of retrorectal tumors may lead to difficulty in diagnosis and surgical management. Methods This was a retrospective review of 62 patients who had resection of retrorectal tumors between 2002 and 2010. Results All patients in this study were treated by excision of the retrorectal tumors. Surgical approach included transsacral approach (52 cases), transabdominal approach (eight cases), and combined approach (two cases). A total of 48 benign lesions (77.4%) and 14 malignant lesions (22.6%) were confirmed by histological examination. The 48 benign cases included dermoid cysts (17 cases), simple cysts (eight cases), teratomas (eight cases), neurofibromas (eight cases), fibrolipomas (four cases), neurilemmomas (two cases), and synovioma (one case). The 14 malignant cases included lymphomas (four cases), malignant teratomas (three cases), fibrosarcomas (two cases), interstitialomas (four cases) and malignant mesothelioma (one case). Complications occurred in 14.5% of patients and included intraoperative bleeding (three cases), rectal injury (three cases), and presacral infection (three cases). Conclusion Primary retrorectal tumors are very rare. Successful treatment of these tumors requires extensive knowledge of pelvic anatomy and expertise in pelvic surgery.

Abstract:Background and Objectives Retroperitoneal sarcomas (RPSs) are difficult to manage, rare malignant tumors. This single-center, retrospective study aimed to analyze the treatment algorithm and outcomes of aggressive surgical treatment in patients with primary and recurrent RPS. Methods Data of 242 consecutive patients with RPS who underwent surgical treatment at the Peking University Cancer Hospital Sarcoma Center between January 2010 and February 2021 were collected and analyzed. Indications for surgery were based on the treatment algorithm. Results A total of 145 patients with primary RPS and 97 with recurrent RPS were included. The recurrent cohort comprised more patients with multifocal tumors than the primary cohort (64.9% vs. 15.2%). R0/R1 resection was achieved in 94.5% and 81.4% of the primary and recurrent RPS cases, respectively. Major complication rates in the primary and recurrent cohorts were 17.9% and 30.9%, respectively. During a median follow-up of 51 months, the estimated 5-year overall survival, local recurrence, and distant metastasis rates for patients with primary and recurrent RPS were 61.0% versus 37.1%, 47.4% versus 71.3%, and 18.4% versus 17.6%, respectively. Conclusions Aggressive surgical treatment achieved good local control and long-term survival in patients with primary RPS, whereas the prognosis in patients with recurrence were significantly worse.

Abstract:Retroperitoneal liposarcoma is a rare tumor with an incidence of 2.5 per million individuals. Early diagnosis is difficult as there is an absence of specific clinical presentations. The present case study reports a patient diagnosed with retroperitoneal liposarcoma who was treated by complete surgical resection and relapsed 3 months following the surgery. In addition, the clinical data of 14 patients with retroperitoneal liposarcoma were reviewed and analyzed. The mean age of the 14 patients at presentation was 54.1 (range, 36-73 years) and 5/14 patients experienced recurrence, ranging between 1 and 10 times. Of the 12 cases that reported histological subtypes, 7 were well-differentiated liposarcoma, 2 were dedifferentiated liposarcoma, 2 were myxoid liposarcoma and 1 was mixed subtype. All the patients underwent complete resection and 5 received combined multiple organs resection (3 nephrectomy, 1 sigmoid colon and 1 multiple visceral organs). However, no patients received chemotherapy or radiotherapy. In conclusion, retroperitoneal liposarcoma is a rare disease with a high rate of recurrence. Complete resection is the predominant treatment and combined resection of adjacent organs is occasionally necessary.

Treatment of Locally Recurrent Soft Tissue Sarcomas of the Retroperitoneum: Report of 30 Cases

Abstract:To assess the results in treating locally recurrent soft tissue sarcomas of the retroperitoneum, we reviewed the clinical, therapeutic, and pathologic characteristics of 30 patients who underwent surgical exploration. Complete resection was achieved in 18 patients (60%), 6 with en bloc resection of adjacent organs involved by the tumors. Following the first operation for recurrent lesions, the median survival was 33 months for those undergoing complete resection and 14 months for those undergoing partial resection or biopsy (P = 0.04). Patients with a disease-free interval (DFI) of > 24 months were associated with better prognosis than those with DFI of < or = 24 months (median survival: 40 months vs. 17 months, P = 0.03). Patients with tumor size of < or = 20 cm had longer survival than those with tumor size of > 20 cm (median survival: 39 months vs. 23 months, P = 0.04). Among 14 patients who received adjuvant radiation therapy, four patients whose tumors had been considered unresectable at first exploration for recurrence underwent complete resection after preoperative radiation therapy. An aggressive surgery aimed at complete resection in treating recurrent retroperitoneal sarcomas is strongly recommended. We suggest that in large tumors preoperative radiation therapy should be used to reduce the tumor volume and therefore a higher rate of complete resection may be achieved.

Abstract:Retroperitoneal sarcomas (RPS) are rare cancers that often reach massive size before detection. The mainstay of treatment for RPS is surgical resection, and complete resection is the only chance for potential cure. The management of RPS can be challenging and in individual cases, radiation and systemic therapy may be beneficial in both primary and recurrent disease. Further research through multi-institutional collaboration, ideally on a global level, is needed to better understand RPS and optimize management of this disease.

Abstract:Objective To investigate the experiences of retroperitoneal liposarcoma(RPLS) management,and clinicopathological and surgical factors associated with postoperative recurrence.Methods Clinicopathological characteristics and surgical management were reviewed retrospectively.Multivariate analysis of factors with postoperative recurrence were performed.Results One hundred and ten operations were performed on all 84 cases with RPLS,of which 72 undertaken gross complete resection(65.5%),66 undertaken the resection involved organs(60.0%).Postoperative recurrence rates were 64.3% and median recurrence-free time were 10 months.Multivariate analysis showed that tumor differentiation and gross complete resection were independent factors affecting recurrence-free time in RPLS.Group of the resection involved organs gained a higher gross complete resection rate than simple tumor resection group(72.7% vs 54.5%,P<0.05).Conclusions RPLS has high postoperative recurrence rate.Tumor differentiation and gross complete resection are independent factors affecting recurrence-free time in RPLS.The resection involved organs is often required to increase the complete resection rate.

Abstract:Thirty patients with locally recurrent retroperitoneal soft tissue sarcomas, whose primary tumors had been previously resected, were surgically treated. Complete resection was achieved in 18 patients (60%), of which 6 underwent en bloc resection of adjacent organs involved by the tumor. The overall 3- and 5-year survival rate for all patients was 39% and 14%, respectively. Survival at 3 and 5 years was 49% and 24% respectively for those undergoing complete resection, compared to 25% and 0% respectively for those undergoing partial resection or biopsy only (P < 0.005). An aggressive surgery aimed at complete resection. in treating recurrent retroperitoneal soft tissue sarcomas is strongly recommended. Prognostic value and clinical significance of factors such as disease-free interval, tumor size, and histologic type were discussed.

Abstract:AIMS:Factors that predict the prognosis of patients with recurrent or metastatic retroperitoneal leiomyosarcomas after treatment are uncertain.MATERIALS & METHODS:Clinical and pathological data on 22 patients with retroperitoneal leiomyosarcomas who were treated by a multidisciplinary abdominal soft-tissue tumor group were analyzed retrospectively to identify prognostic factors for their progression-free survival (PFS).RESULTS & CONCLUSION:The median PFS in the 22 patients was 9 months. Microvessel density in tumor tissues, which was evaluated via CD34 expression in 16 cases, and the presence of metastasis on admission were both found to be correlated with PFS in a Kaplan-Meier analysis, but not in a multivariate analysis. The multivariate analysis did, however, show that complete resection of tumors and the FNCLCC (Fédération Nationale des Centres de Lutte Contre Le Cancer) disease grade were independent factors predicting PFS.

Abstract:Retroperitoneal tumors (RPTs) encompass both benign and malignant entities, constituting ~0.1% to 0.2% of all malignant tumors, of which 70% to 80% manifest malignancy. Predominantly, retroperitoneal sarcomas (RPS) represent the most prevalent subtype among RPT. With over 70 histologic forms identified, liposarcomas and leiomyosarcomas emerge as the primary constituents of RPS. Accurate diagnosis of RPTs necessitates preoperative core-needle biopsy and comprehensive imaging assessment. The current staging protocol for RPS relies on the eighth edition of the American Joint Committee on Cancer/TNM classification. Surgical excision remains the established gold standard for treating RPS. Therapeutic approaches vary according to the underlying pathophysiology. Although chemotherapy and radiotherapy exhibit efficacy in managing metastatic and recurrent unresectable RPS, their role in primary RPS remains unresolved, necessitating further clinical trials for validation. Concurrently, ongoing research explores the potential of targeted therapies and immunotherapy. This literature review aims to provide a comprehensive overview of existing research, delineating diagnostic pathways and optimal therapeutic strategies for RPT.

Abstract:Surgical resection is the cornerstone of curative treatment for retroperitoneal sarcomas (RPS), aiming for complete excision, yet the complexity of RPS with its proximity to vital structures continues to lead to high local recurrence rates after surgery alone. Thus, the role of radiotherapy (RT) continues to be refined to improve local control, which remains an important goal to prevent RPS recurrence. The recently completed global randomized trial to evaluate the role of surgery with and without preoperative RT – STRASS1, did not demonstrate a significant overall benefit for neoadjuvant RT based on the pre-specified definition of abdominal recurrence-free survival, however, sensitivity analysis using a standard definition of local recurrence and analysis of outcomes by compliance to the RT protocol suggests histology-specific benefit in well- and some de-differentiated liposarcomas. Ultimately, multidisciplinary collaboration and personalized approaches that consider histological sarcoma types and patient-specific factors are imperative for optimizing the therapeutic strategy in the management of RPS.

Current Surgical Strategy of Locally Recurrent Rectal Cancer

Abstract:The incidence of locally recurrent rectal cancer (LRRC) is less than 10% following treatment for primary rectal cancer, while LRRC usually has serious symptoms and a very low survival rate when lack of treatment. For further efficacy improvement of LRRC operation, recent research topics include preoperative precise evaluation , correct decision, reasonable individual treatment by multidisciplinary collaboration. Surgery remains the only option for potential cure. Studies show R0 resection is the most important prognostic factor. Peking University First Hospital proposes a new classification of LRRC: Pelvic cavity is divided into right pelvic wall, left pelvic wall and posterior sacrococcyx. LRRC is classified as F0 to F3 according to invasion quantity in pelvic wall or sacrococcyx. This classification is more suitable for clinical practice. Though determining the LRRC location by CT, MRI and PET, scientific classification evaluation and correct choice of proper patients using F classification to fix the extent of pelvic wall can effectively elevate the R0 resection rate, and at the same time, can avoid ineffectual high-risk re-operation. This article also investigates the efficacy of comprehensive treatment based on preoperative neoadjuvant therapy, intra-operative radiotherapy and re-radiotherapy, in order to provide the guidance for the institution of reasonable and scientific treatment strategy in clinic.

Abstract:Retroperitoneal sarcoma (RPS) represents a rare and heterogeneous group of malignancies, posing significant challenges in evaluation and management. Surgery, the cornerstone of RPS treatment, critically depends on complete resection for a favorable prognosis. The extent of resection is a crucial determinant of local control and survival. This review delves into the evolution of multidisciplinary management of localized RPS, highlighting the imperative to adapt surgical strategies to tumor histology, location, and patient functional status. We explore the principles of compartmental surgery—an extended first-line approach that involves resecting adjacent viscera for wide negative margins—and its effectiveness across different histological subtypes of RPS and more limited resections for other types. Particular emphasis is placed on the heterogeneity of the disease, as various histological subtypes exhibit distinct biological behaviors. This necessitates a shift away from a one-size-fits-all treatment approach. The review analyzes the role of different surgical strategies, focusing on histological type and location. Additionally, the potential benefits of (neo)adjuvant treatments, such as radiotherapy and chemotherapy, are examined, recognizing their specific histological indications and limitations. This comprehensive review consolidates recent data on surgical strategies and complementary therapies, advocating for a personalized approach tailored to histology. As understanding of the molecular and genetic underpinnings of RPS continues to evolve, so will strategies for its effective management, underscoring the need for global collaboration among specialists in this field to enhance our collective knowledge and treatment methodologies.

Abstract:ObjectiveTo study the diagnosis and treatment of primary retroperitoneal tumor ( PRT) and the key for preventing and treating of recurrent retroperitoneal tumor (PRT).MethodsThe clinical data of 42 patients with PRT operated on from 1990 to 2000 were retrospectively analyzed.ResultTumors were benign in 19 patients and malignant in 23. Of the 19 patients with benign tumor, 15 underwent complete resection with incidental organ resection in 3 cases, palliative resection in 1 case, and complete resection of recurrent tumor in one. Of the 23 malignant tumors, complete resection was performed in 9 cases, complete resection with incidental organs resection in 11 cases, biopsy only in 3 cases. There was no operative death.ConclusionPreoperative imaging results are gists on which the resectability is judged, and complete resection is the only hope of cure. Surgical resection is the therapy of choice for postoperative recurrent PRT.

Abstract:Objective:To summarize our experience on the diagnosis and surgical treatment of primary retroperitoneal tumor(PRPT).Methods:The clinical data of 315 patients with primary retroperitoneal tumor(from Jan.2000 to Jun.2008) were retrospectively analyzed,and the clinical experience on diagnosis and treatment was summarized.Results:The ratio of benign to malignant tumors in our group was 0.55:1.Totally 294 patients received operation,including 161 cases of radical resection,69 cases of combined resection,and 64 partial palliative resection.Twenty-one patients received no operation.The prognoses of patients who received radical resection for malignant tumors were significantly improved compared with those of patients who received partial palliative resection(P0.05).Conclusion:Imaging diagnosis can effectively identify PRPT.Complete resection is the key for treatment of PRPT,and reoperation is an effective treatment for recurrent PRPT.

Abstract:Retroperitoneal tumors,especially highly malignant retroperitoneal soft tissue sarcomas and recurrent tumors,often involve adjacent viscera by infiltrative pattern or by expansive pattern.Extended surgery with resection of adjacent viscera is recommended to control local recurrence and improve survival.Resectability should be carefully evaluated preoperatively by computed tomography scan and at surgical exploration.The frontline aggressive surgical approach and the way of en bloc resection have many advantages such as clear anatomic approach,high success rate of R0 resection,less concomitant damage of organ,and less bleeding in the operation.

Abstract:Purpose: The percentage of retroperitoneal sarcomas (RPS) among all soft tissue sarcomas ranges from 10 to 15%. Surgery remains the gold standard for RPS. In this study, we analyzed the impact of surgical treatment for primary RPS on recurrence and overall mortality at a Chinese institution and identified and evaluated prognostic variables. Methods: Data from patients with RPS who underwent surgical treatment were retrospectively analyzed. The patients were treated at a single center from January 2000 to June 2018. Retrospectively collected demographic, clinicopathological, and surgical factors were examined. Overall survival (OS) and disease-free survival (DSF) were used as the primary endpoints. Predicted 5-year survival rates, encompassing both DFS and OS, were derived from the Sarculator prognostic nomogram. Results: A total of 110 patients met the inclusion criteria. The median follow-up time after surgery for patients with primary RPS was 5.3 years. During this period, 59 patients died. The 5-year OS and DFS estimates were 63.5% and 35.3%, respectively. In a multivariate analysis, poor OS following surgical treatment of primary RPS was independently correlated with FNCLCC grade (p < 0.001) and surgical margin status (p = 0.016). FNCLCC grade (p = 0.001) and surgical margin status (p = 0.002) were also independently associated with poor DFS. The C-indices for 5-year OS and DFS survival utilizing the Sarculator prognostic nomogram were 0.71 and 0.73 respectively. Conclusion: The overall mortality rate of patients with RPS was considered acceptable. OS and DFS prognostic markers were established for primary RPS. Tumor grade and intraregional margins are other factors that affect survival and recurrence.

Abstract:Objective To explore the experience in treatment of primary retroperitoneal tumors (PRPT).Methods The clinical data of 62 cases with primary relroperitoneal tumors from 2001 to 2007were studied retrospectively.Results The abdominal mass was the main symptom in PRPT.There were 29 cases of benign tumors and 33 cases of malignanat tunlors respectively.Complete resections were performed in 47 patients(76%),partial resection was conducted for 10 patients.Resection could not be performed in 5 patients so only biopsy was conducted,12 patients accepted combined organ resections,2 patients died.Conclusion Adequate preoperative preparation and excellent surgical technique are the keys to compele tumour resection.
