The diagnostic performances were evaluated by receiver operating characteristic (ROC) analysis.ResultsKmax, Dmin, and ADCmin had the highest area under the curve (AUC) (0.875, 0.830, and 0.847, respectively), sensitivity (85.5%, 74.2%, and 77.4%, respectively), and accuracy (85.0%, 79.2%, and 81.7%, respectively) in their individual histogram parameter groups, and Kmax was found to outperform Dmin and ADCmin. ADC histogram parameters (from ADCmin to ADCsd) were significantly lower than D histogram parameters in all groups.Data ConclusionKmax, Dmin, and ADCmin were found to be better metrics than the corresponding average values for differentiating benign from malignant tumors. Abstract:Diffusion kurtosis imaging (DKI) is a diffusion-weighted imaging method that describes non-Gaussian signal behavior using a relatively simple mathematical model. A parameter, kurtosis K, describes the deviation of the diffusion signal decay from a gaussian pattern. The deviation reflects the complexity of the tissue microstructure affecting water diffusion. Several studies have investigated the diagnostic performance of DKI in distinguishing malignant from benign breast lesions. DKI has been reported to correlate with subtypes and with several molecular and other factors related to the treatment and prognosis of breast cancer. Some technical considerations remain to be resolved for the clinical application of DKI in the breast. DKI, which increases the sensitivity to complex tissue microstructure compared to standard DWI, has been applied in the breast, allowing to increase clinical performance in distinguishing malignant from benign lesions and in predicting prognosis or treatment response in breast cancer.
Whole-volume diffusion kurtosis magnetic resonance (MR) imaging histogram analysis of non-small cell lung cancer: correlation with histopathology and degree of tumor differentiation

Abstract:Aims: To evaluate the role of diffusion kurtosis imaging (DKI) histogram analysis in the characterization of non-small cell lung cancer (NSCLC) and to correlate DKI parameters with tumor cellularity. Materials and methods: Sixty-four patients with pathologically diagnosed NSCLCs were evaluated by DKI on a 3-T scanner. Regions of interest (ROIs) were drawn on the map of b1000 manually. All NSCLCs were histologically graded according to the degree of tumor differentiation. Tumor cellularity was measured by the nuclear-to-cytoplasm (N/C) ratio and the number of tumor cell nuclei (NTCN), the expression of Ki-67 was detected using the streptavidin-peroxidase method. Histogram analysis was performed using voxel-based on raw data from each ROI. Results: NSCLCs were classified as grades 1, 2, and 3 according to differentiation degree. Histogram parameters of apparent diffusion coefficient (ADC) and DKI could discriminate between different grades of tumors (p<0.001). Receiver operating characteristic (ROC) curve analysis showed that Kapp 75th exhibited the best performance with an AUC of 0.936 and sensitivity/specificity of 95.74%/80% (p<0.001) in distinguishing grade 1 from grade 2, ADC mean exhibited the best performance with an AUC of 0.923 and sensitivity/specificity of 92.33%/86.67% (p<0.001) in distinguishing grade 2 from 3. N/C ratio and Ki-67 changed significantly with grade (p<0.01). Negative correlations were found between the ADC mean and the N/C ratio, Ki-67, Dapp mean and N/C ratio, whereas Kapp mean and N/C ratio, Ki-67 were positively correlated. Conclusions: DKI histogram analysis could quantitatively characterize NSCLC with different grades by probing non-Gaussian diffusion properties related to changes in the tumor microenvironment or tissue complexities in the tumor.
Texture analysis of diffusion kurtosis imaging for differentiating malignant from benign sinonasal lesions: added value to conventional imaging features

Abstract:Objectives: To evaluate the performance of texture anal-ysis (TA) of diffusion kurtosis imaging (DKI) in differen-tiating malignant from benign sinonasal lesions, and its added value to the conventional imaging features.Methods: Fifty -eight patients with malignant and 40 patients with benign sinonasal lesions were retro-spectively enrolled. Conventional CT and MRI features were reviewed. Texture parameters were obtained and compared between two groups. Multivariate logistic regression analysis was used to identify the most valu-able variables. Receiver operating characteristic curves were performed to assess the differentiating perfor-mance of independent variables and their combination.Results: There were significant differences in tumor necrosis, bone erosion and soft tissue invasion between the two groups (all p < 0.05). There were significant differences in the 10th and entropy of Apparent diffu-sion coefficient map, the mean, 10th and entropy of D map, the mean and 90th of K map between the two groups (all p < 0.002). The bone erosion, entropy of D, and mean of K were independent variables associated with malignant tumors. Receiver operating character-istic analyses indicated that the combination of three features possessed better differentiating performance than bone erosion alone (p = 0.003).Conclusion: TA of DKI could supply incremental value to conventional imaging features for pre-operative differ-ential diagnosis between benign and malignant sinon-asal lesions. Advances in knowledge: The present study is the first to combine conventional imaging features and the TA of DKI in the differential diagnosis between benign and malignant sinonasal lesions. Our findings suggest that TA of DKI could supply incremental value to conven-tional imaging features.

Abstract:Abstract Background The present study aims to investigate the role of histogram analysis of intravoxel incoherent motion (IVIM) in the differential diagnosis of benign and malignant breast lesions. Methods The magnetic resonance imaging and clinical data of 55 patients (63 lesions) were retrospectively analyzed. The multi-b-valued diffusion-weighted imaging image was processed using the MADC software to obtain the gray-scaled maps of apparent diffusion coefficient (ADC)-slow, ADC-fast and f. The MaZda software was used to extract the histogram metrics of these maps. Combined with the conventional sequence images, the region of interest (ROI) was manually drawn along the edge of the lesion at the maximum level of the gray-scale image, and the difference of the data was analyzed between the benign and malignant breast lesions. Results There were 29 patients with 37 benign lesions, which included 23 fibroadenomas, 6 adenosis, 1 breast cysts, 4 intraductal papillomas, and 3 inflammations of breast. Furthermore, 26 malignant lesions in 26 patients, which included 20 non-specific invasive ductal carcinomas, 5 intraductal carcinomas and 1 patient with squamous cell carcinoma. The ADC-slow (mean and the 50th percentile) and f (minimum, mean, kurtosis, the 10th percentile and 50th percentile) of these malignant breast lesions were significantly lower than those of benign lesions ( P < 0.05), while ADC-fast (kurtosis) and f (variance, skewness) of these malignant breast lesions were significantly higher than those of benign lesions ( P < 0.05). Conclusion The histogram analysis of ADC-slow (mean and the 50th percentile), ADC-fast (kurtosis) and f (minimum, mean, kurtosis, the 10th percentile and 50th percentile. Variance, skewness) can provide a more objective and accurate basis for the differential diagnosis of benign and malignant breast lesions.

Abstract:PURPOSETo assess diagnostic accuracy with diffusion kurtosis imaging (DKI) in patients with breast lesions and to evaluate the potential association between DKI-derived parameters and breast cancer clinical-pathologic factors.MATERIALS AND METHODSInstitutional review board approval and written informed consent were obtained. Data from 97 patients (mean age ± standard deviation, 45.7 years ± 13.1; range, 19-70 years) with 98 lesions (57 malignant and 41 benign) who were treated between January 2014 and April 2014 were retrospectively analyzed. DKI (with b values of 0-2800 sec/mm(2)) and conventional diffusion-weighted imaging data were acquired. Kurtosis and diffusion coefficients from DKI and apparent diffusion coefficients from diffusion-weighted imaging were measured by two radiologists. Student t test, Wilcoxon signed-rank test, Jonckheere-Terpstra test, receiver operating characteristic curves, and Spearman correlation were used for statistical analysis.RESULTSKurtosis coefficients were significantly higher in the malignant lesions than in the benign lesions (1.05 ± 0.22 vs 0.65 ± 0.11, respectively; P < .0001). Diffusivity and apparent diffusion coefficients in the malignant lesions were significantly lower than those in the benign lesions (1.13 ± 0.27 vs 1.97 ± 0.33 and 1.02 ± 0.18 vs 1.48 ± 0.33, respectively; P < .0001). Significantly higher specificity for differentiation of malignant from benign lesions was shown with the use of kurtosis and diffusivity coefficients than with the use of apparent diffusion coefficients (83% [34 of 41] and 83% [34 of 41] vs 76% [31 of 41], respectively; P < .0001) with equal sensitivity (95% [54 of 57]). In patients with invasive breast cancer, kurtosis was positively correlated with tumor histologic grade (r = 0.75) and expression of the Ki-67 protein (r = 0.55). Diffusivity was negatively correlated with tumor histologic grades (r = -0.44) and Ki-67 expression (r = -0.46).CONCLUSIONDKI showed higher specificity than did conventional diffusion-weighted imaging for assessment of benign and malignant breast lesions. Patients with grade 3 breast cancer or tumors with high expression of Ki-67 were associated with higher kurtosis and lower diffusivity coefficients; however, this association must be confirmed in prospective studies.
Two Diffusion Kurtosis Imaging Post-Processing Methods for Differentiating Glioma Grades, IDH Mutation Statuses, and Heterogeneity

Abstract:Background: To compare the application of two DKI post-processing methods that DKE software and DKI histogram analysis in glioma grading, IDH mutation typing, and evaluation of tumor heterogeneity. Methods: Patients who underwent surgery and were pathologically diagnosed with glioma after MR DKI scan. DKE software was used to calculate diffusion parameters, including fractional anisotropy, mean kurtosis (MK), radial kurtosis, and axial kurtosis. Histogram parameters were calculated, including minimum, maximum, mean, standard deviation, percentile values (25th, 50th, 75th, 95th), kurtosis, and skewness of Kapp and Dapp. The ROIs of the two post-processing methods were consistently and manually selected in continuous solid tumor regions. According to the result of Kolmogorov-Smirnov (K-S) test, Independent-samples T test or Mann - Whitney - Wilcoxon test was used to distinguish glioma grads. The parameters with the best percentile were identified by analysis of the area under the curve (AUC) of the receiver operating characteristic (ROC) analysis. Results: Seventy-three patients with glioma were observed, including 21 with low-grade gliomas (WHO II) and 52 with high-grade gliomas (WHO III, n=13; WHO IV, n=39), 38 of whom had IDH mutation status. There were significant differences between the high- and low-grade glioma groups regarding the maximum, mean, standard deviation, C75, and C95 of the Kapp values and the minimum, mean, C25, C50, C75, C95, and skewness of the Dapp values. The MK values were significantly different among the WHO II, III, and IV grades. MK, mean Kapp, and C75 and C95 of the Kapp could be used to predict IDH mutations in patients with glioma. Conclusions: Several quantitative DKI parameters obtained from the DKE software and histogram analysis could be used for glioma grading and predicting IDH mutations. However, DKI histogram analysis was useful for glioma heterogeneity.
Comparative Study of Diffusion Kurtosis Imaging Model and Diffusion Weighted Imaging Model in Diagnosisofbreastcancer

Abstract:Objective To compare the value of diffusion kurtosis imaging (DKI) model with single-index DWI model parameters in the differential diagnosis of benign and malignant breast lesions,and to explore the correlation between the parameters and molecular subtypes and prognostic factors of breast cancer.Methods A retrospective analysis was performed with inclusion of 64 cases of breast diseases from January 2016 to May 2017 in Shanghai First People's Hospital.The patients were pathologically confirmed and typed, 30 cases are malignant tumors and 34 cases are benign lesions. DKI and DWI were performed within 2 weeks before the pathological examination. Invasive ductal carcinoma of grade Ⅰ, Ⅱ and Ⅲ were revealed in 1, 7 and 13 cases respectively. Luminal A breast cancer was found in 10 cases, Luminal B breast cancer was diagnosed in 11 cases, HER-2 positive breast cancer was 4 cases and triple negative breast cancer was 5 cases. The expressions of estrogen receptor (ER), Progesterone receptor (PR), and HER-2 positive were found in 20, 14 and 15 cases respectively. Ki67 was highly expressed in 24 cases and low expression in 6 cases. All patients underwent both plain and enhanced mammography scanning. The kurtosis (MK), mean diffusivity (MD) and ADC value were measured. Prognosis analysis was performed according to the maximum diameter (>2 cm, ≤2 cm), vascular or neurological invasion (positive, negative), lymph node metastasis (positive, negative), ER (positive, negative), PR (positive, negative), HER-2 (positive, negative),Ki67 (positive, negative), pathological grade (grade Ⅰ+Ⅱ,Ⅲ). Two independent samples t test was used to compare DKI and DWI parameters between benign and malignant lesions. ROC analysis was performed for assessing the values of parameters in discriminating benign and malignant breast lesions. Mann-Whitney U and Kruskal-Wallis H tests were used for the comparison of various prognostic factors or molecular subtypes.Spearman rank correlation analysis was used to explore the correlation of different prognostic factors and DKI and DWI parameters. Results The MK value of malignant group was higher than that of benign group,and the MD value and ADC value were lower than that of benign group (P<0.05). The area under the ROC for MK, MD and ADC were 0.897, 0.827 and 0.776, respectively. The area under the ROC was improved to 0.935 when three parameters were combined. The MK of ER positive group was higher than that of negative group (P<0.05). There was no significant difference of parameters among the other prognostic groups (all P>0.05). There was a low positive correlation between ER and MK (r= 0.417, P= 0.022). There was no correlation between the other prognostic factors and parameters (r=－0.086 to 0.313, all P>0.05). There was no significant difference in the MD, MK and ADC values among the four different subtypes of breast cancer (all P>0.05). Conclusions MK, MD and ADC values can be used to discriminate benign and malignant breast tumors, among which MK value has the best diagnostic performance. There is a certain correlation between DKI model parameters and prognostic factors.

Abstract:Objective: To investigate diagnostic performance of diffusion kurtosis imaging with histogram analysis for stratifying pathologic Gleason grade of prostate cancer (PCa). Materials and methods: This retrospective study was approved by the institutional review board, and written informed consent was waived. A total of 110 patients pathologically confirmed as having PCa (diameter>0.5 cm) underwent preoperative diffusion-weighted magnetic resonance imaging (b value of 0-2,100 s/mm(2)) at 3T. Data were postprocessed by monoexponential and diffusion kurtosis models for quantitation of apparent diffusion coefficients (ADCs), apparent diffusion for Gaussian distribution (D-app), and apparent kurtosis coefficient (K-app). The measurement was based on an entire-tumor histogram analysis approach. The ability of imaging indices for differentiating low-grade (LG) PCa (Gleason score [GS] <= 6) from intermediate-/high-grade (HG: GS>6) PCa was analyzed by receiver operating characteristic regression. Results: There were 49 LG tumors and 77 HG tumors at pathologic findings. HG-PCa had significantly lower ADCs, lower diffusion kurtosis diffusivity (D-app), and higher kurtosis coefficient (K-app) in mean, median, 10th, and 90th percentile, with higher D-app in skewness and kurtosis than LG-PCa (P< 0.05). The 10th ADCs, the 10th D-app, and the 90th K-app showed relatively higher area under receiver operating characteristic curve (Az), Youden index, and positive likelihood ratio in stratifying aggressiveness of PCa against other indices. The 90th K-app showed relatively higher correlation (rho>0.6) with ordinal GS of PCa; significantly higher Az, sensitivity, and specificity (0.889, 74.1%, and 93.9%, respectively) than the 10th D-app did (0.765, 61.0%, and 79.6%, respectively; P<0.05); and higher Az and specificity than the 10th ADCs did (0.738 and 71.4%, respectively; P<0.05) in differentiating LG-PCa from HG-PCa. Conclusions: It demonstrated a good reliability of histogram diffusion kurtosis imaging for stratifying pathologic GS of PCa. Use of Diffusion Kurtosis Imaging and Quantitative Dynamic Contrast-Enhanced MRI for the Differentiation of Breast Tumors

Abstract:BackgroundPurposeBreast MRI is a sensitive imaging technique to assess breast cancer but its effectiveness still remains to be improved. To evaluate the diagnostic performance of diffusion-weighted imaging (DWI), diffusion kurtosis imaging (DKI), and quantitative dynamic contrast-enhanced (DCE)-MRI in differentiating malignant from benign breast lesions independently or jointly and to explore whether correlations exist among these parameters. Study TypePopulationRetrospective. In all, 106 patients with breast lesions (47 malignant, 59 benign). SequenceAssessmentDKI sequence with seven b values and quantitative DCE sequence on 3.0T MRI. Diffusion parameters (mean diffusivity [MD], mean diffusivity [MK], and apparent diffusion coefficient [ADC]) from DKI and DWI and perfusion parameters from DCE (K-trans, k(ep), v(e), and v(p)) were calculated by two experienced radiologists after postprocessing. Disagreement between the two observers was resolved by consensus. Statistical TestsResultsThe parameters in benign and malignant lesions were compared by Student's t-test. The diagnostic performances of DKI and quantitative DCE, either alone or in combination, were evaluated by receiver operating characteristic (ROC) analysis. The Spearman correlation test was used to evaluate correlations among the diffusion parameters and perfusion parameters. MK, MD, ADC, K-trans, and k(ep) values were significantly different between breast cancer and benign lesions (P < 0.05). MK from DKI demonstrated the highest AUC of 0.849, which is significantly higher than ADC derived from conventional DWI (z = 3.345, P = 0.0008). The specificity of DCE-MRI-derived parameters was improved when combining diffusion parameters, such as ADC and MK. The highest diagnostic specificity (93.2%) was obtained when k(ep) and ADC were combined. k(ep) was correlated moderately positively with MK (r = 0.516) and moderately negatively with MD (r = -0.527). K-trans was weakly positively correlated with MK with an r of 0.398 and weakly negatively correlated with MD with an r of -0.450. Data ConclusionDKI is more valuable than conventional DWI in distinguishing between benign and malignant breast lesions. DKI exhibits promise as a quantitative technique to augment quantitative DCE-MRI. Histogram analysis comparison of readout-segmented and single-shot echo-planar imaging for differentiating luminal from non-luminal breast cancer

Abstract:To compare diffusion-kurtosis imaging (DKI) and diffusion-weighted imaging (DWI) parameters of single-shot echo-planar imaging (ss-EPI) and readout-segmented echo-planar imaging (rs-EPI) in the differentiation of luminal vs. non-luminal breast cancer using histogram analysis. One hundred and sixty women with 111 luminal and 49 non-luminal breast lesions were enrolled in this study. All patients underwent ss-EPI and rs-EPI sequences on a 3.0T scanner. Histogram metrics were derived from mean kurtosis (MK), mean diffusion (MD) and the apparent diffusion coefficient (ADC) maps of two DWI sequences respectively. Student's t test or Mann–Whitney U test was performed for differentiating luminal subtype from non-luminal subtype. The ROC curves were plotted for evaluating the diagnostic performances of significant histogram metrics in differentiating luminal from non-luminal BC. The histogram metrics MK mean , MK 50th , MK 75th of luminal BC were significantly higher than those of non-luminal BC for both two DWI sequences (all P<0.05). Histogram metrics from rs-EPI sequence had better diagnostic performance in differentiating luminal from non-Luminal breast cancer compared to those from ss-EPI sequence. MK 75th derived from rs-EPI sequence was the most valuable single metric (AUC, 0.891; sensitivity, 78.4%; specificity, 87.8%) for differentiating luminal from non-luminal BC among all the histogram metrics. Histogram metrics of MK derived from rs-EPI yielded better diagnostic performance for distinguishing luminal from non-luminal BC than that from ss-EPI. MK 75th was the most valuable metric among all the histogram metrics.

MR Diffusion Kurtosis Imaging for Cancer Diagnosis: A Meta-Analysis of the Diagnostic Accuracy of Quantitative Kurtosis Value and Diffusion Coefficient.

Abstract:PURPOSE:To perform a meta-analysis for assessing the accuracy of diffusion kurtosis imaging (DKI)-derived quantitative parameters (kurtosis values, K; and corrected diffusion coefficients non-Gaussian bias, D) in separating malignant cancers from benign lesions.METHODS:Relevant studies were searched in PubMed and Cochrane Library databases and were analyzed by Meta-DiSc software.RESULTS:Fourteen eligible studies involving 1847 lesions in 1107 patients (895 were benign and 952 were malignant) were included. Pooled analysis showed the sensitivity, specificity, positive likelihood ratio (LR), and negative LR were respectively 0.83 (95% CI, 0.79-0.85), 0.83 (95% CI, 0.80-0.86), 4.61 (95% CI, 2.98-7.14), and 0.22 (95% CI, 0.18-0.28) for K, with the overall area under curve (AUC) of 0.89. The sensitivity, specificity, positive LR, and negative LR were 0.85 (95% CI, 0.80-0.88), 0.85 (95% CI, 0.79-0.89), 6.39 (95% CI, 3.14-12.99), and 0.18 (95% CI, 0.14-0.23) for D, with the overall AUC of 0.92. The sensitivity, specificity, positive LR, and negative LR for apparent diffusion coefficient (ADC) derived from standard diffusion-weighted imaging (DWI) were 0.82 (95% CI, 0.79-0.84), 0.85 (95% CI, 0.82-0.88), 4.75 (95% CI, 3.38-6.68), and 0.24 (95% CI, 0.19-0.29), with the overall AUC of 0.89. The superiority of D to K and ADC was also confirmed by the subgroup analysis of prostate cancer.CONCLUSION:Our findings suggest that DKI should be added to the routine imaging protocol for screening cancer, with the highest diagnostic accuracy of diffusion coefficients.
Characterization of rectal cancer: Whole-tumour histogram analysis of diffusion kurtosis imaging at 3 T

Abstract:Abstract Purpose: This study aimed to explore the performance of whole-tumour histogram analysis of diffusion kurtosis imaging (DKI) in the characterization of rectal cancer.Methods: Forty-two rectal cancer patients were retrospectively enrolled. Histogram analysis of D, K and the conventional apparent diffusion coefficient (ADC) were calculated. Student’s t test or the Mann–Whitney U test and receiver operating characteristic (ROC) curves were used for statistical analysis.Results: The ADCSD and ADCkurtosis were significantly higher in N (+) patients than in N0 patients (p<0.05). The ADCSD, DSD and Kkurtosis were significantly higher in PNI-positive patients than in PNI-negative patients (p<0.05). Dskewness was significantly higher in KRAS mutation-type patients than in KRAS wild-type patients (p<0.05). The area under the curve (AUC) of the multiparametric ADCSD and ADCkurtosis values for the detection of metastatic lymph nodes was 0.816 (p<0.001); the sensitivity and specificity were 81.0% and 76.2%, respectively. The AUC of the multiparametric ADCSD, DSD, and Kkurtosis values for the detection of PNI was 0.830 (p<0.001); the sensitivity and specificity were 80% and 88.9%, respectively. Dskewness can be used to determine the KRAS status; the cut-off value, AUC, sensitivity and specificity were 0.465, 0.732 (p=0.010), 78.3%, and 73.7%, respectively. Conclusion: Whole-tumour histogram analysis of DKI could be used to identify both pathological and genetic characteristics of rectal cancer.

Abstract:Objective:To evaluate the application value of diffusion kurtosis imaging (DKI) based MD, MK parameters in grading invasive breast carcinoma and compare their diagnostic potential.Materials and Methods:Collecting 53 patients with invasive breast carcinoma diagnosed by pathological examination in this study. One male patient and 52 female patients were included. All patients underwent breast magnetic resonance imaging, including traditional magnetic resonance imaging and diffusion kurtosis imaging. ADC, MK and MD were calculated by using post-processing software, Matlab 2011b. Compared with histological grade, the classification of invasive breast carcinoma and its correlation with prognostic factors were statistically analyzed.Results:ADC, MD and MK values have no significant difference in well, moderately and poorly differentiated invasive breast carcinoma (P>0.05). Only ADC and MD were significantly different in ER expression (P<0.05). ADC, MD and MK values showed no significant difference in PR, HER-2 and Ki-67 expression (P>0.05,P=0.055 with MK in Ki-67).Conclusions:DKI has limited value in the evaluation on the classification of invasive breast carcinoma. However it provides useful information in the assessment of tumor proliferation activity.

Abstract:BACKGROUND: The associations between diffusion kurtosis imaging (DKI)-derived parameters and clinical prognostic factors of breast cancer have not been fully evaluated; this knowledge may have implications for outcome prediction and treatment strategies.PURPOSE: To determine associations between quantitative diffusion parameters derived from DKI and diffusion-weighted imaging (DWI) and the prognostic factors and molecular subtypes of breast cancer.STUDY TYPE: Retrospective.POPULATION: A total of 383 women (mean age, 53.8 years; range, 31-82 years) with breast cancer who underwent preoperative breast MRI including DKI and DWI.FIELD STRENGTH/SEQUENCE: A 3.0 T; DKI using a spin-echo echo-planar imaging (EPI) sequence (b values: 200, 500, 1000, 1500, and 2000 sec/mm2 ), DWI using a readout-segmented EPI sequence (b values: 0 and 1000 sec/mm2 ) and dynamic contrast-enhanced breast MRI.ASSESSMENT: Two radiologists (J.Y.K. and H.S.K. with 9 years and 1 year of experience in MRI, respectively) independently measured kurtosis, diffusivity, and apparent diffusion coefficient (ADC) values of breast cancer by manually placing a regions of interest within the lesion. Diffusion measures were compared according to nodal status, grade, and molecular subtypes.STATISTICAL TESTS: Kruskal-Wallis test, Mann-Whitney U test with Bonferroni correction, receiver operating characteristic (ROC) analysis, and multivariate logistic regression analysis. (Statistical significance level of P < 0.05).RESULTS: All diffusion measures showed significant differences according to axillary nodal status and histological grade. Kurtosis showed significant differences among molecular subtypes. The luminal subtype (median 1.163) showed a higher kurtosis value compared to the HER2-positive (median 0.962) or triple-negative subtypes (median 1.072). ROC analysis for differentiating HER2-positive from luminal subtypes revealed that kurtosis yielded the highest area under the curve of 0.781. In multivariate analyses, kurtosis remained a significant factor associated with differentiation between HER2-positive and luminal (odds ratio [OR] = 0.993), triple-negative and luminal (OR = 0.995), and HER2-positive and triple-negative subtypes (OR = 0.994).DATA CONCLUSION: Quantitative diffusion parameters derived from DKI and DWI are associated with prognostic factors for breast cancer. Moreover, DKI-derived kurtosis can help distinguish between the molecular subtypes of breast cancer.EVIDENCE LEVEL: 4 TECHNICAL EFFICACY: 3.

Abstract:PURPOSE:To evaluate the predictive performance of multiparameter and histogram features derived from amide proton transfer-weighted imaging (APTWI), intravoxel incoherent motion (IVIM) and diffusion kurtosis imaging (DKI) for histopathological types of breast tumors.METHODS:Region of interest (ROI) was delineated by outlining the largest slice of the tumor on the false-color images of the DKI, IVIM and APTWI parameters, and extracted the histogram features. Receiver operating characteristic (ROC) curve was used to evaluate the performance of parameters in predicting benign and malignant breast lesions, molecular prognostic biomarkers, lymph node status, and subtypes of breast lesions. The Spearman correlation coefficient was used to determine the correlations between each parameter and clinical-pathological factors.RESULTS:All 52 breast lesions were enrolled in this prospective study, including 8 benign lesions and 44 breast cancers. To diagnose malignant and benign breast lesions, the value of APT (min) performed best, with the AUC reaching 0.983. According to the different imaging methods, the APTWI performed best. To predict the positive status of ER, PR, Ki67, the value of Dapp (uniformity), Dapp (uniformity), f (entropy) performed best, with the AUC values reaching 0.743, 0.770, 0.848, respectively. For the identification of Luminal B, HER2-enriched, and TNBC breast cancers, Kapp (max), f (kurtosis), and Dapp (uniformity) performed best, with AUC values reaching 0.679, 0.826, 0.771, respectively.CONCLUSION:This study found the APTWI, IVIM and DKI parameters could diagnose breast cancer. The histogram features of DKI and IVIM, based on tumor heterogeneity, may help to predict breast cancer subtypes.
Histogram Analysis of Diffusion Kurtosis Magnetic Resonance Imaging for Diagnosis of Hepatic Fibrosis

Abstract:Objective: To investigate the diagnostic value of diffusion kurtosis imaging (DKI) histogram analysis in hepatic fibrosis staging. Materials and Methods: Thirty-six rats were divided into carbon tetrachloride-induced fibrosis groups (6 rats per group for 2, 4, 6, and 8 weeks) and a control group (n = 12). MRI was performed using a 3T scanner. Histograms of DKI were obtained for corrected apparent diffusion (D), kurtosis (K) and apparent diffusion coefficient (ADC). Mean, median, skewness, kurtosis and 25th and 75th percentiles were generated and compared according to the fibrosis stage and inflammatory activity. Results: A total of 35 rats were included, and 12, 5, 5, 6, and 7 rats were diagnosed as F0-F4. The mean, median, 25th and 75th percentiles, kurtosis of D map, median, 25th percentile, skewness of K map, and 75th percentile of ADC map demonstrated significant correlation with fibrosis stage (r = -0.767 to 0.339, p < 0.001 to p = 0.039). The fibrosis score was the independent variable associated with histogram parameters compared with inflammatory activity grade (p < 0.001 to p = 0.041), except the median of K map (p = 0.185). Areas under the receiver operating characteristic curve of D were larger than K and ADC maps in fibrosis staging, although no significant differences existed in pairwise comparisons (p = 0.0512 to p = 0.847). Conclusion: Corrected apparent diffusion of DKI histogram analysis provides added value and better diagnostic performance to detect various liver fibrosis stages compared with ADC.
Diagnostic Value of Diffusion Kurtosis Imaging Combined with Quantitative Dynamic Contrast-Enhanced MRI in Breast Lesions

Abstract:Objective To evaluate the diagnostic efficacy of MRI diffusion kurtosis imaging (DKI) and quantitative dynamic contrast enhancement MRI (DCE-MRI) in benign and malignant breast lesions, and to explore the differential diagnosis ability for different pathological types and molecular subtype lesions. Methods Sixty four females were retrospectively enrolled in the study of MRI diffusion kurtosis imaging and quantitative dynamic contrast enhancement between November 20 and May 2017. All of them were confirmed to have benign or malignant lesions after surgical resection or puncture. All patients underwent axial T1WI, DKI and DCE-MRI examinations. The mean kurtosis (MK) and mean diffusivity (MD) values were calculated by the DKI model, and the hemodynamic parameters were obtained by quantitative dynamic contrast enhancement, including volume transfer constant (Ktrans), rate constant (Kep), extravascular extracellular space distribute volume per unit tissue volume (Ve) and blood volume fraction (Vp). Pathological analysis was performed to monitor the expression of estrogen receptor (ER), progesterone receptor (PR), human epidermal growth factor receptor 2 (HER-2) and nuclear proliferation index Ki67. The breast cancer was divided into Luminal A type, Luminal B type, HER-2 positive and triple-negative 4 subtypes. The differences of DKI parameters and DCE-MRI parameters between benign and malignant breast lesions were compared using two independent samples t test (normal distribution and homogeneity of variance) or Mann-Whitney U test (skewed distribution or variance). ROC analysis was used to evaluate the value of DKI and DCE-MRI parameters in differential diagnosis of benign and malignant breast lesions with pathological results as the gold standard. The Mann-Whitney U test and Kruskal-Wallis H test were used to compare the differences of DKI and DCE-MRI parameters among different prognostic factors and molecular subtypes of breast cancer. Spearman rank correlation analysis was used to evaluate the correlation between DKI and DCE-MRI parameters and different prognostic factors. Results Sixty-four cases were single lesions, with breast cancer in 23 cases and 41 cases of benign lesions. In breast cancer, there were 9 cases of Luminal A type, 7 cases of Luminal B type, 3 cases of HER-2 positive type and 4 cases of triple negative type. The positive numbers of ER, PR and HER-2 were 14, 11 and 10 cases respectively. Nineteen cases showed high expression of Ki67, while 4 cases showed low expression. There were significant differences in MK, MD, Ktrans and Kep between benign and malignant lesions (P<0.05). However, there was no significant difference between Ve and Vp (P>0.05). The area under the ROC for the differential diagnosis of benign and malignant breast lesions were 0.897, 0.808, 0.844 and 0.842, respectively. The combined multi-parameter differential diagnosis improved the efficacy. Combined with the above four parameters, the area under the ROC was 0.950. The diagnosis Sensitivity, specificity and accuracy were 0.870, 0.951 and 0.922 respectively. The Ktrans and Vp values of patients with ER positive and ER negative, as well as Ve value of deferent lymph node status, were significantly different (P<0.05), but there was no significant difference between the other prognostic factors (P>0.05). There was a moderate positive correlation between ER and Ktrans and Vp values. There was a low positive correlation between lymph node status and Ve value (r= 0.6, 0.5 and 0.4, respectively, P<0.05). No correlation was found among other parameters and prognostic factors (P>0.05). There were no significant differences in DKI and DCE-MRI parameters among different subtypes of breast cancer patients (all P>0.05). Conclusion DKI combined with DCE-MRI can improve the differential diagnosis of breast lesions and some DCE-MRI parameters are related to prognostic factors.

Abstract:Objective To evaluate the feasibility of diffusion kurtosis imaging (DKI)in the diagnosis and aggressiveness assessment of prostate cancer.Methods The MR data with multi-b-value diffusion-weighted imaging (DWI)of 30 male patients with prostate cancer were retrospectively analyzed.D maps and K maps were generated from the DKI model and ADC maps were obtained using the conventional monoexponential model.Differences in the D,K and ADC values between prostate cancer and benign peripheral zone (PZ)tissues,among tumors with different Gleason scores were analyzed.P<0.05 was considered statistical significance.Results ADC and D values were significantly lower in the prostate cancers than in the benign PZs (P <0.001).K values were significantly greater in the prostate cancers than in the benign PZs (P <0.001).ADC,D and K values were different significantly among the tumors with Glea-son scores of 6,7 and ≥8 (P <0.001,P =0.01 5 and P <0.001,respectively).Conclusion The DKI model can better describe the signal intensity attenuation with b values increasing,which is helpful for diagnosing and grading prostate cancer.K value may be used to quantitatively evaluate the complicated microstructure of prostate cancer.

Abstract:Background: The aim of this study was to prospectively evaluate the diagnostic efficacy of diffusion kurtosis imaging (DKI) in predicting microvascular invasion (MVI) and histologic grade of hepatocellular carcinoma (HCC) with comparison to the conventional diffusion-weighted imaging (DWI). Methods: This prospective study was approved by the Institutional Review Board, and written informed consent was obtained from all patients. From September 2015 to January 2017, 74 consecutive HCC patients were enrolled in this study. Preoperative magnetic resonance imaging including DKI protocol was performed, and patients were followed up for at least one year after surgery. Diffusion parameters including the mean corrected apparent diffusion coefficient (MD), mean apparent kurtosis coefficient (MK), and apparent diffusion coefficient (ADC) were calculated. Differences of diffusion parameters among different histopathological groups were compared. For parameters that were significantly different between pathological groups, receiver operating characteristics (ROC) curve analyses were performed to evaluate the diagnostic efficiency for identifying MVI and predicting high-grade HCC. Univariate and multivariate logistic regression analyses were used to evaluate the relative value of clinical and laboratory variables and diffusion parameters as risk factors for early recurrence (<= 1 year). Results: Among all the studied diffusion parameters, only MK differed significantly between the MVI-positive and MVI-negative group (0.91 +/- 0.10 vs. 0.82 +/- 0.09, P<0.001), and showed moderate diagnostic efficacy (AUC = 0.77) for identifying MVI. High-grade HCCs showed significantly higher MK values (0.93 +/- 0.10 vs. 0.82 +/- 0.09, P<0.001), along with MD (1.34 +/- 0.18 vs. 1.54 +/- 0.22, P<0.001) and ADC values (1.17 +/- 0.15 vs. 1.30 +/- 0.16, P=0.001) than low-grade HCCs. For differentiating high-grade from low-grade HCCs, MK demonstrated a higher area under the ROC curve (AUC) and significantly higher specificity than MD and ADC (AUC = 0.81 vs. 0.76 and 0.74; specificity = 82.2% vs. 60.0% and 60.0%, P=0.02). In addition, higher MK (OR = 5.700, P=0.002) and Barcelona Clinic Liver Cancer (BCLC) stage C (OR = 6.329, P=0.005) were independent risk factors for early HCC recurrence. Conclusions: DKI-derived MK values outperformed conventional ADC values for predicting MVI and histologic grade of HCC, and are associated with increased risk of early tumor recurrence.
